Ashburn Restaurant Inspections: Chuy's, Pholicious, Moby Dick House Of Kabob
More than a dozen restaurants around the Ashburn area were cited with violations during recent inspections by the health department.
ASHBURN, VA ā During recent inspections from the Loudoun County Health Department, more than a dozen restaurants around Dulles, Sterling, and Ashburn were cited with inspections. The restaurants were visited by the health department between Dec. 15 and Dec. 21.
As coronavirus restrictions continue to fluctuate, health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Door of the low boy refrigerator in front of the grill in kitchen does not close properly.
- The thermometer of the high temperature dishwasher observed broken.
- The inner panel of the ice machine observed with heavy mildew build up in kitchen.
- Facility observed with strong pungent odor in rear food storage room in kitchen.

- EHS observed caterer improperly thawing frozen chicken breasts. The chicken breasts were in stagnant water in a metal bowl set out at room temperature in the kitchen.

- Employee used personal cellular device, changed gloves, and did not wash hands before returning to work.
- Employee at bar prepared a drink and added food garnish with bare hands.

- Ice scoops stored in ice bins with handle in the ice.
- Wet pans stored in dish drying rack stacked on top of each other, preventing air drying

- EHS observed a carafe of half and half coffee creamer improperly cold holding. The food employee explained that The carafe is brought out of the cooler when the cafe opens at 7 am and any remaining creamer at the end of the day when the cafe closes is discarded.
- The labels on the Packaged food in the grab and go cooler ( caesar salad, quinoa salad) are missing ingredient information. Sub-ingredients are missing. Labels only include the "common name" of ingredients such as- "mayonnaise, parmesean cheese, croutons etc). An allergen statement is also missing.

- Upon inspection, there is no person in charge and no one that has knowledge of food safety that oversees the daily continental breakfast.
- The facility does not have verifiable means of an Employee Health Reporting Policy.
- The high temp dishwasher is being used to clean and sanitize food contact surfaces but does not have hot water. Maintenance is aware that the machine is broken, has been for "two years".
- Hard-boiled eggs were found at improper temperatures. The facility is not using time as the public health control. Eggs are put into a container from cold holding, placed on the buffet, and kept at room temperature for 2-3 hours, then put back into the refrigerator per front desk.
- Cheddar Cheese slices that have been removed from their packaging, were observed stored with no date marking.
- There are no test strips to test/monitor the concentration of Quat sanitizer at the three basin sink.
- The facility does not have a thermometer for use with the high temp dishwasher to monitor temperatures to ensure sanitization is taking place.
- The high-temperature dishwasher was recorded at 77F.

- Handwashing facility in the kitchen observed without paper towel in rear kitchen.
- Temperature control for safety (TCS) food items such as ground goat, cooked mixed vegetables and cooked chicken prepared more than 24 hours prior observed without date making in refrigeration units.
- Three containers of Raid (pesticide) for ants and roaches, labeled for residential use were observed in the facility and are being applied by the operator to control pests.
- One dead roach under the dishwasher and two dead roaches in sink in bar area observed during inspection.
- Sink compartments of the three compartment sink as well as the dishwashing machine observed not large enough to accommodate immersion of the two largest.

- EHS observed an employee drink (open styrofoam cup) stored on the back prep table near the meat slicer and another employee drink (open styrofoam cup) stored on a shelf in the front kitchen with sandwich rolls.
- Several (lasagna, meat sauce-walkin, sliced ham, chicken salad, tuna salad) prepared ready to eat (RTE) TCS (time/temperature control for safety) foods are not date marked for disposition. The owner states that all prepared foods are made every two to three days.
- When tested, the food contact sanitizer (bleach) at the three compartment sink exceeded 200ppm.
- EHS observed several wet wiping cloths used for wiping counters and equipment sitting on counters throughout the facility.
- The facility is using drawstring garbage bags to cover food (trays of portioned pizza dough) in the walk in cooler.
- Physical facility (back kitchen) in need of a deep cleaning focusing on below the three compartment sink and drying racks as well has below the prep table that holds the meat slicer. EHS observed food debris build up at said areas.

- The paper towel dispenser at the handwashing sink located at the inside line is out of paper towels.
- A spray bottle marked "Vegetable spray" that had a clear liquid resembling water was found.

- Observed Milk at 45F and Half and Half at 48F at the Coffee Self Service Table. Staff indicated that milk and half and half are rotated and not kept for more than 4 hours. Items were not marked with the time they were put out.
- Observed Facility to be using Quat and a High Temp Dishwashing Machine without Test Strips to verify disinfectant concentration or heat verification.

- The surface face of the line coolers and their handles have a moderate build-up of debris/residue.

- At time of inspection there was no verifiable sign form that employees have been train of the signs, symptoms, and exposures of foodborne illnesses and who to report to.
- Observed hand sink in kitchen blocked with a metal tray and the hand sink by the 3 compartment sink limes in a container in sink.
- Observed cooked noodles out of temperature (stored on counter) there is no written procedures for time as a public health control.
- Observed chicken and pork setting on the counter after being cooked at a temperature of 74F.
- Observed raw shrimp thawing on the floor in a bucket of water.
- Observed boxed noodles stored on the floor of walk in cooler.
