Ashburn Restaurant Inspections: Dead Bug Found In Bread
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Aug. 16 and Aug. 22.
COVID-19 restrictions around the state have relaxed, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Total violations: 4
- A container of cooked potatoes (50F), and a container of cut lettuce (49) in the reach in cooler to the immediate left as you enter the kitchen, and a container of cut tomatoes (50F), and a gallon of milk (50F) in the flip top cooler on the line were found improperly cold holding. Time/temperature control for safety foods (TCS) that are ready to eat (RTE) must be maintained at 41F or less to prevent the growth of harmful bacteria. All improperly cold holding foods were voluntarily discarded.
- The chlorine sanitizer at the three basin sink, as well as in 2 wiping buckets tested at 200+ppm concentration. Chemical sanitizers shall meet the requirements of 40 CFR 180.940. Chemical sanitizers may not exceed specific concentration levels, as they may be construed as a chemical contaminant. Person in charge changed the 3 basin sink and both wiping buckets so that they registered 50ppm chlorine.
- A pair of in-use tongs was observed stored in a container of stagnant water to the left of the grill. Utensils that are in-use, and stored in between uses must be stored on a clean dry surface, in water that is 135F or more, or in continuously running water with enough velocity to wash away food particles. Employee removed the container and replaced both container and tongs with clean, dry ones.
- EHS observed a severe leak from the ceiling, near the prep area, and a bucket used to catch the wastewater. PIC stated that the leak is from condensation of the HVAC system. Condensate drainage liquids shall be drained from the point of discharge to disposal according to law. EHS directed that prep area be moved away from the leak and no food pass under this area until the leak is repaired. Leak constitutes an environmental contamination risk, as well as a slip risk to employees. Repair leak ASAP.

- Documentation (written or electronic) of an Employee Health Reporting Policy was unable to be provided when requested. Verifiable means that all food employees have been trained in the signs and symptoms of food borne illness and how to report them must be available upon request. EHS gave copies of Form 1B in English and Spanish. Have each employee read and sign this document and keep within the facility and available for review. If this document is electronic, ensure that it is accessible for review at all times.

- Two plastic drinking cups with drink inside of them was observed stored on the line. Employees are allowed to have drinks while engaged in food processes so long as they do not pose a risk of contamination. Drinks should be enclosed (lid) and not stored in a way that may contribute to contamination. Employee discarded both drinks.
- Hand washing soap dispensers at the dish machine and the bar area both observed with soap in them, but the dispenser is not working/dispensing soap. Hand sinks that are utilized by employees must be provided with accessible hand cleanser at all times. Provide accessible hand soap to all hand sinks utilized by food employees.
- Several containers of cooked time/temperature control for safety foods (TCS) that are ready to eat (RTE) were found within the walk-in cooler with no date marking. When keeping TCS/RTE foods longer than 24 hours, a date that the food was prepared/opened or a discard date not to exceed 7 days from the day prepared/opened shall be labeled on the food/container. Date mark these items.
- A personal cell phone was found stored on a prep counter, next to an in-use cutting board on the line. Food shall be protected from contamination by separating employee items from foods, food contact surfaces, equipment, utensils etc. Employee removed the cell phone to a shelf designated for employee items.
- No test strips to test the concentration of Quat sanitizer utilized at the three basin sink was able to be produced during the inspection. Test strips or other testing device that tests the concentration of sanitizer and monitor its effectiveness must be provided. Procure test strips for Quaternary Ammonium sanitizer.

- Observed no provision for drying hands after washing at the audibon and carriage bar hand sinks.
- Observed ambient air temperature in small 2 door reach in cooler in the rear food prep area at 52F. Cheese held in this refrigeration unit observed at 54F, and 58F. Cold TCS, time/temperature control for safety, foods shall be held at or below 41F.
- The following containers of food observed exceeding their discard by date: chipotle sauce, calabrian sauce, tasty paste. Observed one container of sauce unlabeled and one container of potato salad dressing mislabeled with preparation date and discard by date of 9/11 to 9/15.
- The lighting intensity in the basement dry storage area is insufficient. Most ceiling light fixtures throughout the dry storage area are not working. At least 10ft candles of light is required above the floor during periods of cleaning.

- Rice is being soaked in large vats with no temperature control. Complete a Written procedure for time as the public health control.
- Facility is making their own yogurt on site as well as purchasing yogurt.

- No testing strips to test the concentration of the chlorine sanitizer for the low temperature dish machine are available. Testing strips or other device must be provided to test and monitor the concentration of sanitizer used on food contact surfaces. Person in charge stated that testing strips have been ordered, but have not come in yet. Provide strips and order so that there are back ups available.

- The handwashing facility located in the kitchen is blocked with rice cooker, preventing access by employees for easy handwashing.

- The following food item (s) for which time rather than temperature is being used as a control was not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control: Sushi Rice.
- Mayo container was stored on the kitchen floor. Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks.
- Wet wiping cloths were observed stored on the kitchen counter.
- The kitchen floor, equipment door/glass are not cleaned as often as necessary to keep them clean.

- A spray bottle containing an unknown cleaner was found with no identifying information. When taking toxic materials/cleaners from bulk supplies, the common name of the material must be labeled on the working container. Employee labeled the spray bottle.
- A large ladle was found with moderate heat damage to its surfaces. Food contact surfaces of utensils, containers, equipment etc. shall be easily cleanable and free of chips, breaks, cracks or any other imperfections that may impeded cleaning and sanitizing. Employee voluntarily discarded the ladle.

- Chemical spray bottle observed without the proper labeling. All working containers of toxic materials should have a label with the common name of the material in it.
- In-use utensils improperly stored between use. Multiple knives were stored in between the kitchen table and a wooden piece attached to the table. Also, a plastic bowl without a handle is being used for the rice.
- Ice machine observed with accumulation of black residues.

- Observed employee drinking from container without a proper straw. After drinking from a beverage cup, the grill station food employee slid their gloved hand over the lip contact opening to close the container.
- No paper towels were observed at the kitchen hand sink. Paper towels shall be provided for drying hands after washing.
- Observed raw lamb and raw fish stored directly above container of diced onions in the walk-in-cooler. Raw animal proteins shall be stored in a manner as to not pose a cross contaminant risk to ready to eat foods.
- The following time/temperature control for safety (TCS) foods cold holding at improper temperatures: Cheese 44F, Milk 44F, diced Tomatoes 44, Lentils 44F. Chicken held on counter at 70F. TCS foods shall be held cold at or below 41F.
- Improper thawing methods used to thaw chicken. Chicken observed thawing in water on counter at 55F. Proper thawing methods shall be used to thaw frozen foods.
- Observed no ambient air thermometer provided in the walk-in-cooler. Provide thermometer in walk-in-cooler to monitor ambient air temperature inside of the unit.
- Observed several flies in kitchen and both exterior back doors cracked open. The entry of pests shall be minimized by keeping exterior doors closed completely.
- Observed containers of food stored on floor in the walk-in-freezer, walk-in-cooler, and cooking oil stored on floor near grill and back hall. Food shall be elevated at least 6" off floor to protect food and to allow for proper cleaning of floor surfaces.
- The following areas of the physical facilities observed in need of detail cleaning: Ceiling vent above wait station, refrigeration unit doors and handles, shelving throughout kitchen.
- Observed leak beneath 3-compartment sink rinse basin. The 3-compartment rinse basin is unusable. The plumbing shall be maintained in good repair, sink shall be usable at all times.

- A carton of raw eggs was observed stored over ready to eat (RTE) cut vegetables in the walk-in cooler. Foods shall be protected from cross contamination risks by separating raw animal proteins from RTE foods. Eggs were moved to the shelf below the cut vegetables.
- A spray bottle containing a green cleaning agent was observed with no labeling. Toxic materials/cleaners that are taken from bulk supplies must have the common name of the material labeled on the working container. Person in charge (PIC) labeled the spray bottle.
- Two reduced oxygen packaged (ROP) portions of tuna was found in the walk-in cooler that had not had their seal broken prior to thawing. When thawing fish that is in ROP, the vacuum seal must be broken in order to introduce oxygen so the growth of anaerobic bacteria (Clostridium botulinum) is eliminated. PIC had both packages of Tuna cut open.

- Improper hot holding in that fried rice registered at 87°F, observed being left on top of the food prep table in kitchen.
- TCS food cold holding at improper temperatures in that bean sprouts holding at 65°F on prep table.
- Cooked beef spring rolls cooked a day prior and to be held for 2 days observed not dated for disposition. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.
- Methods used for cooling chicken and beef spring roll were not adequate in that said food observed left on food prep table for long time before placing them in the refrigeration unit.

Correct by instructing staff to wash their hands prior to entering the kitchen prior to handling items. The Person in Charge corrected this by providing training to the appropriate staff and sanitizing the ice scoop.

- Two containers of white sauce noted as improperly cooled to prevent the growth of harmful bacteria. sauces were made on Friday and has been cooling for more than 6 hours. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours.
- Observed mashed potato hot holding at improper temperature (121F). Time/temperature control for safety (TCS) foods must be hot held at 135F or above to prevent growth of harmful bacteria.
- Observed diced tomato, corn, lobster cold holding at improper temperature (58F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- Ice machine observed with accumulation of black residues.

- Employee's drink observed on top of the ice cream. Store drinks on a separate counter to avoid possible food contamination.
- There are no paper towels at the hand sink to dry hands. There must be paper towels available at hand sink for proper hand drying at all times.
- Observed tuna cold holding at improper temperature (48F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- In-use utensils and/or dispensing utensils observed improperly stored. Ice cream scoops were stored in standing water.

- Upon reviewing the shellstock tags for the live mussels, EHS observed that they are not being kept with the shellstock, are not in chronological order and the date that the last shellstock of each batch is not being recorded, rather the date when the facility receives them. Shellstock tags/labels shall remain attached to the container/with the shellstock until the container/batch is empty and then that date recorded on the tag. This tag shall be kept in chronological order with others for a minimum of 90 days. Do not combine different containers/batches together. EHS discussed with manager this process.
- The low temperature dish machines (1 behind main bar, 1 server in cigar bar) are not dispensing adequate sanitizer (0-10ppm). Food contact surfaces shall be sanitized before use, after cleaning. Onsite maintenance began trouble shooting these issues during the inspection. Use the high temperature dish machine in the main kitchen until these 2 machines are capable of properly sanitizing.
- Several containers of cooked, ready to eat (RTE) foods were found with no date marking throughout the kitchen. When keeping time/temperature control for safety foods longer than 24 hours, a date marking the day that the food item was prepared/opened, or a discard date not to exceed 7 days shall be marked on the food/container. Person in charge had these items date marked.
- The establishment was unable to produce a irreversible thermometer for the high temperature dish machine within the kitchen. In hot water mechanical ware washing operations, an irreversible thermometer or other device shall be provided for measuring and monitoring the utensil surface temperature to ensure that sanitizing temperatures are consistently being reached. Provide and have readily available.
- Soil debris was observed inside the ice machine, beyond the deflector. Non food contact surfaces shall be cleaned at a frequency that precludes the accumulation of debris. Clean the inside of the ice machine.
