Ashburn Restaurant Inspections: Flies Found In Facilities

The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.

ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between May 17 and May 23.

Almost all COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • Dunkin Donuts, 47010 Community Plaza #180 Sterling, VA 20165
    • Date inspected: May 17
    • Total violations: 4
      • Observed the following TCS food items in a state of cold holding stored above 41F in the Delfield 4 Drawer Prep Top: "Bacon Bites (containing cheese)" 45F and 45F, Cooked Sausage at 47F, "Taco Mix" at 50F, and Cheddar Cheese at 48F.
      • Observed the 4 Drawer Delfield Prep Top to not have an ambient air thermometer.
      • Observed Ice Cream Scoops stored in stagnant water. (water recirculation for scoop holder was off.
      • Observed the Zurn GT2700-50 Grease Trap to have a hole in it causing it to leak and cause standing water in the kitchen.

    • Date inspected: May 17
    • Total violations: 3
      • Observed Hand Sink by the 3 VAT sink used to store sanitizer bucket, utensils, and steel wool.
      • Observed the following items in a state of cold holding above 41F: From the Sandwich Prep Top Station: Cooked Beef Steak at 57F and Swiss Cheese at 49F.
      • Quaternary ammonia test strips for testing sanitizer at 3 compartment sink not present.
  • Los Toltecos Bar & Grill, 43150 Broadlands Center Plaza #194 Ashburn, VA 20147
    • Date inspected: May 18
    • Total violations: 2
      • Proper drinking procedure not followed in that employee using an uncovered beverage container to drink coffee in food prep area in kitchen while prepping food in the kitchen. Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
      • Date marking not provided to food items such as shrimp, mashed potatoes, refried beans, ground beef and pork on the shelf in the walk in refrigerator. Said food were cooked a day prior and had been cooling in said walk in refrigerator and will be held for up to 4 days.

      • No disposable towels were provided a the hand washing facility in the front drinks prep area.
      • Three live fruit flies observed in rear office/dishwashing/storage area.

      • Observed tomato sauce and goat meat cold holding at improper temperature (F) inside the walk in cooler. Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.

    • Total violations: 3
      • No disposable towels were provided a the hand washing lavatory in the front food prep/service area.
      • Several live fruit flies observed in front food prep/service area.
      • Maximum temperature measuring device (thermometer/thermolabel) not provided for high temperature dishwasher.

    • Total violations: 2
      • Observed food employee rinsing hands between tasks at 3 compartment sink. Hands shall be properly washed using soap, scrubbing for a minimum of 20 seconds, and shall be done so utilizing the hand sink.
      • Observed food handler without hair restraint. Hair restraints shall be worn to protect food and food contact.

    • Total violations: 5
      • Two of the hands sinks observed to be blocked by the carts. Hand sinks should allow employees to conveniently access them.
      • Slicer blade observed with accumulation of food residues. Food contact surfaces should be washed/rinsed and sanitized after each use.
      • Sliced cabbage cold holding at improper temperature on the kitchen counter. Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
      • Raw shrimp and beef stored on the counter to thaw. Thaw potentially hazardous foods by either of the following methods: 1. Under refrigeration that maintains food temperature at 41°F 2. Completely submerged under running water at a temperature of 70°F or below with sufficient water velocity to agitate and float off loose particles in an overflow 3. As part of the cooking process.
      • Multiple wet towels observed being stored on the kitchen counter. Wet soiled towels should be stored in the sanitizer solution.
  • Bracket Room, 1 Saarinen Cir. Dulles, VA 20166
    • Date inspected: May 22
    • Total violations: 5
      • Food improperly storing an open drink on the line. The cup was stored without a lid on top of the open top prep cooler on the line.
      • The bar hand sink hand soap dispenser is broken and soap is not dispensing at all. Provide hand soap at all hand sinks at all times.
      • An open package of ready to eat ham observed improperly stored in contact directly with a container of raw chicken in the walk in cooler.
      • Both the low temperature dish machines in the facility measured at 0ppm sanitizer when tested. Wares are not being sanitized after cleaning and before use. All wares shall be sanitized after cleaning and before use.
      • The facility changed their menu last week. The new menu does not have a consumer advisory printed on it for menu items served undercooked (sunny side up eggs, cooked upon request beef burgers)
  • District Chop House Main Terminal, 1 Saarinen Cir. Dulles, VA 20166
    • Date inspected: May 22
    • Total violations: 4
      • Front of the house employee observed using tongs with their right hand to place fries from a bowl onto a customer's plate however they used their left bare hand to arrange the fries on the plate. Ready to eat (RTE) foods can not be handled with bare hands. Corrected- on the spot training on bare hand contact with ready to eat foods conducted with the employee.
      • The low temp dish machine is not sanitizing. Wares not sanitized after cleaning and before use. When tested the sanitizer is testing at 0ppm. After further observations of the machine EHS let the person in charge know that the rinse aid line is hooked up to the sanitizer dispenser, it appears that the rinse aid is being dispensed when the sanitizer should be dispensed. All wares shall be sanitized after cleaning and before use.
      • Reduced oxygen packaged fish not removed from its packaging before thawing in the refrigerator. Fish observed still in packaging in the low boy cooler Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment: Prior to its thawing under refrigeration.
      • Facility does not have the right test strips for testing the sanitizer at the three compartment sink. The facility uses Ecolab sink and surface sanitizer and they only have quat ammonium test strips.

    • Date inspected: May 22
    • Total violations: 3
      • The hand sink at the bar does not have paper towels for proper hand drying. The dispenser is empty. Provide paper towels for proper hand drying at all hand sinks at all times.
      • The bare dish machine is not sanitizing wares after cleaning them. When tested, the sanitizer concentration registered at 0ppm. All wares shall be sanitized after cleaning and before use.
      • Two knives observed improperly stored between use in a sanitizer bucket with sanitizer on the line.
  • Matchbox One Loudoun, 44720 Thorndike St Ashburn, VA 20147
    • Date inspected: May 23
    • Total violations: 4
      • The handwashing station being used for purposes other than washing hands in that the handwashing sink in front food prep area at the first floor and at the food prep area in second floor observed with dirty knives being placed in it.
      • Handwashing facility not adequately supplied in that handsink at the front food prep area in main floor kitchen observed without soap.
      • Chemical sanitizer at the bar is not sanitizing adequately.
      • Several food items such as pork ribs, cheese, rice, grilled chicken raw chicken, raw shrimp, raw beef, crab cakes, scallops, mussels, tilapia registered between 44°F-54°F in front food prep area. Lower section of three refrigeration units registered operating between 44°F to 58°F.

      • Ice dispenser of the soda machine observed with accumulation of black residues. Clean the surface at a frequency necessary to preclude accumulation of soil residues.
