Fredericksburg Health Inspections: 24 Violations At 1 Restaurant

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Sept. 13 and Sept. 19.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Sam's Pizza & Subs, 8925 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Sept. 13
    • Total violations: 4
      • The Person in Charge was unable to provide documentation of training relating to the Employee Health Policy for this facility. Facility provided with Employee Health Policy that was reviewed and signed by all available employees. EHS provided employee health policy to the person in charge.
      • Observed the interior of the microwave heavily soiled.
      • Observed deli slicer soiled . Through discussion with the person in charge the deli slicer was last used the day prior to inspection.
      • Observed several flying insects throughout the kitchen area. EHS advised the person in charge to contact there pest control services to further assist .
  • Chapo's Tacos (CHAPOS), 45 Dent Road Stafford, VA 22554
    • Date inspected: Sept. 13
    • Total violations: 2
      • Person in charge could not provide documentation of employee health training. Operator was knowledgeable of reporting requirements. Sample policy provided.
      • Observed employee touch certified food manager card wearing clot gloves and proceeded to don single use disposable gloves. During the inspection the operator removed the gloves and washed their hands.
  • Massaponax High School, 8201 Jefferson Davis Highway Fredericksburg, VA 22407
    • Date inspected: Sept. 13
    • Total violations: 6
      • Observed no paper towels near the sub sandwich serving line. Person in charge replaced the paper towels.
      • Observed 6 sheet trays of chicken sticks cold holding between 52-58 degrees F in the 2D reach-in cooler in the sub sandwich serving area. The chicken was labeled as being prepped on 09-12-23 1:45pm. The person in charge voluntarily discarded the chicken.
      • Observed a bag of sausage patties in the walk-in cooler that had a preparation date of 09-05-23 and discard date of 09-12-23. The person in charge voluntarily discarded sausage.
      • Observed 3 lights on the vent hood above the pizza ovens not working, and the walk-in cooler doesn't shut completely by itself.
      • Observed seals around reach-in coolers throughout the facility and the outside of the dishwashing machine soiled.
      • Observed A/C vents throughout the facility soiled.
  • Castiglia's Italian Restaurant & Pizzeria, 10705 Courthouse Rd #108 Fredericksburg, VA 22407
    • Date inspected: Sept. 14
    • Total violations: 5
      • Observed 1 pair of tongs soiled on the food contact surface, hanging on the clean dish rack above the three compartment sink . Person in charge removed the pair of tongs to be washed.
      • Observed a food employee preparing food and not wearing a hair or beard restraint.
      • Observed plastic containers above the three compartment sink on the clean dish rack and plastic cups in the bar area stacked wet. Person in charge removed containers and cups to be washed and air dry.
      • Observed both hand sinks in the kitchen and in the women's restroom are not sealed to the wall.
      • Observed the following non food contact surfaces in need of cleaning: 1. Upslash of soda machine 2. Shelving throughout the kitchen 3. Exterior of cooking equipment.
  • Sam's Pizza & Subs - Stafford Courthouse, 2142 Richmond Hwy. Suite #101-102 Stafford, VA 22554
    • Date inspected: Sept. 14
    • Total violations: 24
      • Person in charge could not provide cleaning procedures of bodily fluid clean-up. Documentation was provided.
      • The hot water temperature of the hand sink in the bar area is reading at 73 degrees F.
      • Observed bread maker in front of the hand washing sink. Bread maker was moved during the inspection.
      • Missing paper towels at the handwashing sink. PIC supplied sink with paper towels.
      • Missing handwashing signage at both the handwashing sinks.
      • Observed raw chicken over cooked chicken wings and chicken patties in the beverage cooler. Items were relocated.
      • Observed the following soiled: can opener, pizza screens, and cup with handle in flour container. All were removed for cleaning.
      • Observed the following held at improper temperature in the prep cooler: sliced turkey (44 degrees F), tuna (44 degrees F), sliced provolone cheese (43 degrees F), sliced cheddar cheese (43 degrees F). The thermometer in the prep unit read 44 degrees F at 11:30 AM. All foods were brought from the walk-in cooler at 10:30 AM and had till 2:30 PM to use the food. The unit's temperature was turned down and later read at 40 degrees F.
      • Observed food in the walk-in cooler without date marking.
      • Observed several employee medication bottles located on a shelve above a prep table.
      • Observed two spray bottles containing chemicals stored next to clean and sanitized dishes. Spray bottles were relocated.
      • Observed two spray bottles containing chemicals with no labels. One bottle was labeled Windex and the other one was labeled grill cleaner.
      • Observed packaged roast beef thawing in a bucket of water in the walk-in cooler. PIC removed water from bucket.
      • Observed several flying insects at the back of the house.
      • Observed outer openings in the back door of the kitchen and back door of the restroom area.
      • Observed containers of chicken wings and cut lettuce on the floors of the walk-in cooler.
      • Observed several food employees without hair restraints.
      • Observed several stainless-steel containers wet while stacked after being clean and sanitized. Items were relocated to the three compartment sink for sanitization.
      • Observed all prep tables and handwashing sink not sealed to the wall,
      • Observed the following in poor repair: interior door of the pizza prep cooler, the fan system on the top interior of the two-door freezer, and rusted shelves throughout the kitchen.
      • Person in charge could not provide test strips for checking the concentration of the sanitizer. PIC stated the test strips are ordered.
      • Observed the following in need of cleaning: walls behind equipment, floors underneath the equipment, ceilings, and mop sink basin.
      • Observed the hood vents over the grill and pizza oven in need of cleaning.
      • Observed the following: two employee cellular devices on a prep table and employee hat in the dry storage area.
  • Taqueria y Pupuseria Don Jose (TYY-6973), 5505 S Oaks Ave Fredericksburg, VA 22407
    • Date inspected: Sept. 17
    • Total violations: 2
      • Food employee donned gloves after handling Chlorine sanitizer. During the inspection the food employee washed their hands.
      • Observed overfilled pan of filled pupusas (cooked meat and cheese) holding between 43-48 degrees F in the prep cooler overnight. During the inspection the operator discarded the food.
  • Spice Thai Cuisine, 10029 Patriot Hwy Fredericksburg, VA 22407
    • Date inspected: Sept. 18
    • Total violations: 2
      • Facility could not provide documents on procedures for responding to vomiting and diarrheal events.EHS provided a sample to the person in charge.
      • Observed stainless steel utensils spoons , forks and knives in containers stored with the food contact surface facing up. Person in charge removed the utensils to be cleaned and be stored inverted.

      • Observed the three compartment sink, produce wash sink, and prep tables not sealed to the wall.

      • Observed the floors underneath the equipment and walls behind equipment in need of cleaning.

    • Date inspected: Sept. 18
    • Total violations: 1
      • Facility was unable to provide signed documents on employee health training.
