Fredericksburg Health Inspections: Botta Bing Bagel, Sam's Pizza
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Nov. 1 and Nov. 6.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Benny Vitali's, 722 Caroline Street Fredericksburg, VA 22401
- Date inspected: Nov. 1
- Total violations: 1
- Observed the back door is not self-closing, and gaps around the conduits near the 3-compartment sink and the dough prep area.
- Subway (Ladysmith), 17485 U.S. 1 Ruther Glen, VA 22546
- Date inspected: Nov. 1
- Total violations: 10
- Observed 3 condiment shakers stored in the hand sink. Shakers were removed during inspection.
- The cutting boards along the makes line are heavily stained. Also observed 3 tomato slicers hanging from the 3 compartment sink that were soiled. All items were taken to the 3 compartment sink for cleaning.
- Observed sliced chicken (47'F), steak (45), bacon (47,F) and salami (45'F) in the walk in cooler that was not being maintained at proper temperature. Person in charge did not know how long the food had been in the cooler. Temperature of cooler was adjusted and the foods were voluntarily discarded.
- The sanitizer being dispensed at the 3 compartment sink measured well over 500 ppm. Water was added to the sink until sanitizer reached safe level. A service call will be made for the unit.
- Observed employee preparing food with hair restraints [sic].
- The cutting boards along the make line are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
- The hand sink and the 3 compartment sink need to be re-sealed to the wall.
- The upsplash of the customer soda dispenser and the counter underneath the machine were observed soiled.
- The ceiling tiles located in both restrooms is absorbent and not easily cleaned.
- The floors throughout the facility, especially under equipment is in need of a thorough cleaning.

- Date inspected: Nov. 1
- Total violations: 2
- Observed the interior ceiling of ice machine soiled.
- Observed ice build up on the ceiling, frozen boxed foods, shelving, and floors of the walk in freezer. Boxed frozen foods were relocated. PIC stated a repair ticket has been issued and are waiting for the unit to be serviced.

- Total violations: 3
- Person in charge could not provide written documentation of training on employee health policy. Documentation given during inspection.
- Person in charge could not provide written procedures regarding cleanup of bodily fluids. Documentation given during inspection.
- Observed the exterior of equipment throughout the kitchen in need of cleaning.
- Botta Bing Bagel & Deli, 310 Garrisonville Road Stafford, VA 22554
- Date inspected: Nov. 2
- Total violations: 14
- Person in charge could not provide written documentation on the training of the employee health policy. Documentation given during inspection.
- Person in charge could not provide bodily fluid cleanup written procedures. Document given during inspection.
- Observed employee uncovered beverage container on a prep table and another uncovered beverage container next to single use articles on a kitchen counter. Items were relocated.
- Per discussion with the person in charge, the knives on the bagel line are only cleaned and sanitized once daily. The knives need to be cleaned and sanitized every 4 hours due to the exposure to TCS foods. PIC replaced knives.
- Observed no date marking system of TCS foods in cooling units throughout the kitchen.
- Per discussion with the person in charge, cooked to order raw shell eggs are offered by request. The menu does not contain asterisks (*) with a consumer advisory advising customers of the risk of having raw/undercooked foods.
- Observed a package of frozen raw chicken thawing on a kitchen counter. PIC relocated item to the reach in cooler.
- Observed exposed sack of potatoes on the floor in the dry storage area and observed flour in an uncovered plastic container on the floor underneath the bread mixer. Items were relocated.
- Observed two food employees without hair restraints.
- Observed whole lettuce wrapped in newspaper stored in a one-door hometyle refrigerator. Item was removed.
- Observed heavy ice buildup on the vent, ceiling, shelving, and packaged frozen foods in the walk-in freezer. PIC relocated the packaged frozen foods.
- Observed the following nonfood contact surfaces in need of cleaning: 1) interior and exterior of all equipment. 2) shelving throughout kitchen. 3) up splash of the coffee machine.
- Observed the floors underneath equipment, walls, ceilings tiles, and ceiling vents throughout the kitchen in need of cleaning.
- Observed employee food not separated and in a designated area in all reach in coolers. In addition, observed employee cellular device on the meat slicer and eyeglasses on a prep table. Items were removed during inspection.
- Sam's Pizza & Subs, 6072 Riverview Dr King George, VA 22485
- Date inspected: Nov. 2
- Total violations: 13
- There was no soap provided at the hand sink in the prep area. Soap was provided during inspection.
- No hand drying provisions were provided at the handwashing sink in the prep area. Paper towels were provided during inspection.
- Observed loosely wrapped lasagna stored in a plastic container in the walk in cooler soaked with water that had accumulated at the bottom of the container. Lasagna was voluntarily discarded during inspection.
- The blade of the can opener was observed soiled. Can opener was taken to the 3 compartment sink to be cleaned.
- The cutting boards throughout the facility were observed heavily stained. Boards were taken to the 3 compartment sink to be cleaned.
- The surfaces of the pizza oven including the interior and the conveyor were observed soiled with grime and debris.
- During discussions with the person in charge, it was learned that pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests. Person in charge was instructed that he was not allowed to apply these chemicals in the facility and to find a licensed pest control company.
- Observed employees working in the kitchen without hair restraints. Hair nets were provided during inspection.
- The cutting boards throughout the facility are heavily scored.
- The door seals on the dish machine need to be replaced.
- The following non-food contact surfaces were observed soiled: 1-the top of the dish machine, 2-the can opener, 3-the shelf holding the microwave, 4-the outer surfaces of the microwave, 5-all of the air circulation fans in the facility, 6-the wall heater above the rear door, 7-the shelving over the prep units, 8-the hoods and the fire suppression system over the cook's line , 9-outer surfaces of the display reach in freezer, 10-the inner and outer surfaces of the prep units, 11-the bottom shelf of the prep table.
- The walls near the 3 compartment sink are damaged and are in need of repair.
- The following physical facility items were observed soiled and in need of cleaning: 1-the walls around and near the mop sink, 2-the walls around and near the 3 compartment sink, 3-the floors under the equipment throughout the kitchen.
- Randolph's on the River, 136 Main Street Port Royal, VA 22535

- Total violations: 9
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Observed uncovered drink next to the coffee maker. Drink was removed during inspection.
- There was no soap at the hand sink at the bar. Soap was provided during inspection.
- The handwashing facility located at the bar is blocked, preventing access by employees for easy handwashing. Observed plastic containers, credit card receipt paper and other items blocking use of the sink. Items were moved during inspection.
- The surface of the cutting boards in the kitchen are heavily stained.
- Observed shipping plastic still covering a prep table.
- The following non-food surfaces were observed soiled: 1-the doors on the display reach in cooler, 2-the doors on all reach in coolers, 3-all of the circulation fans in the kitchen, 4-the de-humidifier in the kitchen, 5-the wire shelving in the kitchen.
- Observed personal items, cell phone, purse, keys next to the coffee maker in the kitchen.
- Observed light bulbs in the kitchen not shielded, coated, or otherwise shatter-resistant.

- Observed the interior ceiling of the ice machine soiled. PIC cleaned the surface during inspection.

- Observed employee wipe bare hands on apron in between cleaning the grill and cooking hamburger patties. Employee was instructed to wash hands.
- Observed several plastic containers wet while stacked on a shelve after being cleaned and sanitized. Items were taken to the 3-comp sink for cleaning.

- Observed wire shelving in the walk in cooler rusted.
- Observed the following in need of cleaning: 1. back of equipment in the cook line was heavily dusty 2. ac vents throughout the facility need dust debris cleared off 3. ceiling fan in the kitchen needs dusted off 4. debris was observed in a corner of the kitchen between two reach in coolers 5. floors of the walk in cooler
- Observed less than 3 foot candles inside of the walk in freezer.
