Fredericksburg Health Inspections: Dead Bugs, Mold On Ice Machine

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Aug. 9 and Aug. 15.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Benny's Sub Shop, 16430 Consumer Row King George, VA 22485
    • Date inspected: Aug. 9
    • Total violations: 14
      • Observed food employee wash their hands with only water and no soap for less than 10 seconds. Person in charge instructed employee to wash their hand in the hand sink.
      • Observed food employee wash his hands at the dump sink.
      • Observed to-go plastic containers with no handles in a rice cooker and dry rice storage container. Containers were removed during inspection.
      • Observed the interior of the microwave heavily soiled. Microwave was cleaned during inspection.
      • Observed cold holding foods at improper temperatures: salmon (47 degrees F) and imitation crab (46 degrees F) in the sushi prep unit. Sliced tomato (44 degrees F) and raw beef (44 degrees F) in the cookline prep unit. Egg rolls (46 degrees F), and breaded chicken (49 degrees F) at the reach in cooler of the cookline prep unit. Person in charge relocated the items to the walk in cooler and lowered the temperature of the units.
      • Observed no proper system of date marking throughout the kitchen.
      • Observed raw chicken and shrimp thawing in still water in the three compartment sink.
      • Observed two containers with flour without any labels.
      • Observed two food employees without hair restraints.
      • Person in charge could not provide test strips to test sanitizer.
      • Observed the interior and exterior of equipment throughout the kitchen in need of cleaning.
      • Observed floors, walls, and ceilings throughout the kitchen in need of cleaning.
      • Observed several employee personal belongings (wallet, phones) throughout the kitchen.
      • Observed hood vents over the grill in need of cleaning.
  • Taco Bell/KFC, 5400 James Madison Parkway Dahlgren, VA 22485
    • Date inspected: Aug. 9
    • Total violations: 8
      • Observed the handwashing sink near the drive thru filled with two empty fry containers and miscellaneous trash in the sink basin. Items were relocated.
      • Observed dust entering kitchen from dining room, where construction is taking place. A plastic barrier was installed during inspection.
      • Observed food employee without a beard guard.
      • Observed several plastic containers wet while stacked on the shelves.
      • Observed the interior and exterior of equipment throughout the kitchen in need of cleaning.
      • Observed that the up splash on the soda machine at the drive thru was heavily soiled and in need of cleaning.
      • Observed the ceilings, walls, and floors throughout the kitchen in need of cleaning. Including ceiling vents and floors underneath the equipment.
      • Observed hood vents above the fryers and grills in need of cleaning.
  • Ledo Pizza, 9849 Patriot Hwy Fredericksburg, VA 22407
    • Date inspected: Aug. 10
    • Total violations: 8
      • Observed the water at the handwashing sink by the pizza prep station temperature is 81 degrees.
      • Observed in a prep cooler (1) cut lettuce at 48 degrees and mozzarella cheese at 46 degrees, foods were placed in the prep cooler at 11:00 am the day of the inspection. Foods were relocated to the walk-in cooler during the inspection.
      • Observed rusted shelving throughout the facility.
      • Observed the following equipment in poor repair: 1. doors (on the top) of the pizza prep cooler are loose 2. 1 sandwich/salad prep cooler not maintaining required temperatures 46 -50 degrees.
      • Observed the following nonfood contact surfaces in need of cleaning: 1. floor fan in the kitchen 2. wire shelving 3. inside surfaces of the fryer cabinet 4. wheels of equipment at the cook line.
      • Observed insufficient air gap at the three compartment sink and at the mop sink. The pipe at the three compartment sink and the hose at the mop sink was sitting below the rim line. (Corrected)During the inspection the person in charge cut the hose above the rim line at the mop sink.
      • Observed the following facilities in need of cleaning: 1. AC air vents throughout 2. Caulking around the three compartment sink 3. Floor drains
      • Observed the lighting inside of the walk-in cooler was 1.4 - 6.0 foot candles.
  • Sonic Drive In, 10045 PATRIOT HWY Fredericksburg, VA 22407
    • Date inspected: Aug. 10
    • Total violations: 12
      • Observed a food employee place soap on their hands and lather without rinsing hands with warm water first. Discussed and demonstrated correct handwashing.
      • Observed interior of the ice machine soiled.
      • Through discussion with food employees the following food contact surfaces and not being cleaned at the required frequency: 1- Interior of ice cream/shake machines in contact with shake base (contains milk) are cleaned three times a week and 2- Prep table at the cook line which is in contact with bread for sandwiches (observed during the inspection) is being sanitized only, absence of wash step. Discussed cleaning requirements with the person in charge.
      • Observed the following TCS foods cold holding at improper temperatures: 1- Sliced cheese stored in well above the deep freezer (49 degrees F). Observed the container uncovered and stored near the flattop which may be affecting the cold holding capacity. A food employee reported the cheese was pulled from the walk-in cooler at 10:00 a.m. and 2- Full container of whipped cream stored on the top of prep cooler #2 (51 degrees F). The person in charge reported the whipped cream was placed in the unit at 9:00 a.m. and was relocated to the walk-in cooler. Cool to 41 degrees F, sell or discard by 2:00 p.m. (cheese) and 1:00 p.m. (whipping cream). Discussed holding whipping cream on the bottom due to small space on the top of unit and prevent overstocking of sliced cheese near the cook line.
      • Observed employees without proper hair and beard restraints.
      • Observed plastic and stainless-steel container on warewashing shelving which were stacked while wet after chemical sanitizing.
      • Observed the following equipment in poor repair: Ice build up inside the deep freezer and 2 door reach in freezer, gaskets inside the Electro Freezer are separating, and rusted shelving in the facility.
      • Observed accumulation of debris on upsplash of beverage machines, tubing under the flattop, shelving throughout the facility and interior of refrigeration equipment. Recommend thorough cleaning of the facility.
      • Observed broken door for the recycling dumpster, observed the door on the ground. Contact recycling for repair or replacement.
      • Observed the following in poor repair: missing/chipped floor tiles by the flattop with unsealed concrete exposed, ceiling tiles are not secured to the frames, and seal toilets in the female toilet room to the floor.
      • Observed the floors, walls and ceiling including the air vents in need of cleaning.
      • Observed hood vents above the flattop grill soiled.
  • The Grounds Coffeehouse, 50 N Stafford Complex Center Suite #107 Stafford, VA 22556
    • Date inspected: Aug. 10
    • Total violations: 2
      • Observed the following cold holding at improper temperature overnight at the cold drinks reach in cooler #2 in the front of the house : 1) one jug of non fat milk (44 degrees F), 2) two jugs of whole milk (43-45 degrees F), 3) four cartons of half and half that contains dairy (44-46 degrees F), 4) three cartons of almond milk (43-46 degrees F), 5) three cartons of oat milk (46-47 degrees F). Items were discarded.
      • Observed a long water hose attached to a Y connector at the mop sink without the proper back flow prevention device.
  • Kid's Station Powered by KinderCare at Work, 1100 Sam Perry Blvd Fredericksburg, VA 22401
    • Date inspected: Aug. 10
    • Total violations: 8
      • Per discussion with the person in charge and looking at the signed employee health policy it was found that not all employees that enter the kitchen and handle foods have signed the employee health policy.
      • Observed the following soiled: 1. several glass baking dishes in cabinet; 2. can opener. Items were relocated to warewashing area.
      • Observed one bag of commercially made chicken nuggets cold holding at 47 degrees F. In discussion with the person in charge chicken were made the day prior and cooling of the chicken nuggets did not get monitored. During the inspection chicken nuggets were discarded.
      • Observed a container of commercially made beans date marked 7/27. Beans should have been discarded by 8/2/23. During the inspection person in charge discarded the beans.
      • Observed the presence of flying insects in the kitchen and dry storage areas. A box of cantaloupes was moved, and a large number of the flying insects came out. Per discussion with the person in charge the fruit shipment from their supplier comes in with the flying insects. EHS discussed the importance of inspecting shipments for pests, refusing if pests are present, and speaking to pest control company to reduce the number currently present.
      • Observed multiple hinges on the cabinet doors and drawer hardware very lose and in need of tightening or replacement. Per discussion with the person in charge a work order has already been submitted to repair the hinges and hardware.
      • Observed shelving inside of cabinets and the inside of drawers throughout the kitchen soiled.
      • Observed the vent hood filters with a heavy accumulation of dust. EHS advised the person in charge that the filters are removable for cleaning, as needed, in between the regularly scheduled maintenance from the company used.
  • Paragon Village 12, 51 Towne Centre Blvd Fredericksburg, VA 22407
    • Date inspected: Aug. 11
    • Total violations: 5
      • Observed in the front kitchen hand sink 2 green sponges sitting inside the basin and a pair of tongs were hanging from the faucet. During the inspection sponges and tongs were removed. During the inspection items were removed.
      • Observed the upper parts of the ice machine soiled with black mold like growth.
      • Observed in the prep area one spray bottle containing quaternary ammonium cleaner unlabeled. During the inspection an employee labeled the spray bottle.
      • Observed the following non food contact surfaces that require cleaning: 1. A lot of trash and food debris were observed inside the cabinet of the fron condiment customer service area. 2. gaskets and inside surfaces of the 2 door freezer. 3. outside surfaces of the popcorn maker 4. backsplash and up slash of the soda machines 5. handles and cabinets of the fryers 6. outside surfaces of the unpopped popcorn bins.
      • Observes walls, floors underneath equipment, ac/ceiling vents all require a deep cleaning. A lot of food debris and grime were observed on the walls of the kitchen/cooking area.
  • Hit the Spot Kitchen, 3318 Bourbon Street Fredericksburg, VA 22408
    • Date inspected: Aug. 11
    • Total violations: 3
      • Observed that the TCS (temperature controlled for safety foods) in the under counter reach in cooler including deli sliced ham (52'F), deli sliced turkey (51'F) and swiss cheese (48'F) were cold holding at improper temperatures. Items were voluntarily discarded during inspection. The unit had been mistakenly unplugged by a plumber the previous night.
      • Observed a heavy coating of ice and frost inside of both chest freezers.
      • Observed that the ventilation hood and filters over the cook's line are heavily soiled.
  • Home2 Suites, 3051 Richmond Highway Stafford, VA 22554
    • Date inspected: Aug. 11
    • Total violations: 4
      • Observed the following cold holding at improper temperature for less than four hours in the self serve area: 1) a container of pre-packaged cream cheese (45 degrees F), 2) waffle batter in a dispensing unit (76 degrees F). EHS recommended storing cream cheese over ice, EHS also provided TPHC documents for waffle mix.
      • Observed the only employee working in kitchen without a proper hair restraint.
      • Observed the following: 1) unwrapped single service knives and forks stored with the lip contact surfaces facing upward in the self service area, 2) the coffee stirrers are not individually wrapped by the coffee station in the self serve area.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher.
  • Golden Corral, 10320 Spotsylvania Avenue Fredericksburg, VA 22408
    • Date inspected: Aug. 14
    • Total violations: 23
      • Observed absence of active managerial control during the inspection. The person in charge is not monitoring temperatures of TCS foods during cold and hot holding and improper cooling and cooling methods observed.
      • Observed interior of the kitchen and beverage area ice machines soiled. Due to growth on the chute instructed the operator to discontinue production of ice on the kitchen ice machine. Observed soil residue on the chute in contact with ice at the beverage area ice machine. Instructed the operator to dispose of the ice. Through discussion with the person in charge the soda nozzles are soaked in sanitizer overnight and rinsed with water, absence of a wash step. Instructed the person in charge to wash, rinse and sanitize the soda nozzles (corrected during inspection).
      • Observed the following TCS foods improperly cooled: 1- Baked pasta with ground beef (45 degrees F) and cooked pasta (48 degrees F). Observed TCS foods in deep and covered containers (stainless steel for baked pasta with ground beef and plastic for cooked pasta) with condensation on the interior lid of the plastic container used to cool cooked pasta. The person in charge reported the TCS foods were prepared the evening prior. The foods were voluntarily discarded during the inspection.
      • Observed the following TCS foods hot holding at improper temperatures: 1- pork sausage (114 degrees F) and diced ham (105 degrees F). A food employee reported the TCS foods have been in the unit since 8:00 a.m. Instructed the operator to sell or discard by 12:00 p.m. Observed water in the steam well at 129 degrees F. Discussed increasing the temperature of the steam well to ensure adequate hot holding can be achieved. 2- Country fried steak (121 degrees F) - prepared at 7:45 a.m. Instructed the operator to sell or discard by 11:45 a.m.
      • Observed shell eggs (59.9, 53 degrees F) cold holding in a plastic container on ice inside a bus tub in the breakfast prep area. Observed ice only contacting the bottom of the container. A food employee reported the eggs were removed from the walk-in cooler at 6:45 a.m. Instructed the operator to cool to 45 degrees F, sell or discard by 10:45 a.m. A food employee placed more ice in the bus tub.
      • Observed the following TCS foods cold holding at improper temperattures: 1- Pastuerized liquid eggs (52 degrees F) in a plastic continer inside a bus tub on ice which was only contacting the bottom of the container. A food employee reported the eggs were pulled from refrigeration at 6:45 a.m. A food employee added more ice. Cool to 41 degrees F, sell or discard by 10:45 a.m. 2- Raw chicken in water at the chicken breading station (55 degrees F). A food employee reported the chicken was pulled from refrigeration at 8:15 a.m. and relocated the chicken to the meat walk in cooler. Cool chicken to 41 degrees F by 12:15 p.m. 3- Cut cantaloupe (53 degrees F) and cut honeydew melon (66 degrees F) at the breakfast cold well which were pulled from the walk-in cooler at 8:00 a.m. Observed containers overfilled above the fill line. Instructed the person in charge to cool to 41 degrees F, sell or discard by 12:00 p.m. 4- Cut tomato and cucumber salad (55 degrees F), diced ham (47 degrees F) and cut lettuce (57 degrees F) in the breakfast cold well observed overfilled above the fill line. The TCS foods were pulled from the walk-in cooler at 7:40 a.m. Cool to 41 degrees F, sell or discard by 11:40 a.m. 5- Two containers of tuna salad (44, 44 degrees F) held in the walk-in cooler by the door. Discussed storing TCS foods away from the door. The tuna salad was held overnight and was voluntarily discarded. 6- Raw pork (49 degrees F) stored in freezer well. The pork was being stored on top of other containers which may be affecting the cold holding capacity. The operator reported the pork was pulled from the walk-in cooler at 7:00 a.m. and was relocated back to the walk-in cooler to cool to 41 degrees F or less by 11:00 a.m. 7- Vanilla (47 degrees F) and chocolate (48 degrees F) ice cream base held in the ice cream machine since 9:00 a.m. The person in charge reported the ice cream bases were placed into the unit when the unit was turned off. The operator discarded the TCS food during the inspection. 8- Single use containers of half and half dairy creamer (78 degrees F) held in a container over ice (ice only contacting the bottom of the container). A food employee reported the creamer was removed from refrigeration at 9:00 a.m. and relocated the half and half to the walk in cooler to cool to 41 degrees F or less by 1:00 p.m.
      • Observed containers of cut cabbage, cut greens and cut lettuce (four total containers) which had a preparation date of 8/14/2023 (9:25 a.m.) labeled on the container. Through discussion with food staff the TCS foods were prepared the evening prior. Discussed correct date marking of TCS foods. A supervisor instructed the employees to discard the food during the inspection.
      • Improper cooling methods observed for pasta and baked pasta with ground meat in the facility. Observed foods cooling in a deep and covered container with condensation observed on the interior lid. The food employee could not communicate time and temperature cooling requirements.
      • Observed four boxes of raw pork ribs (33 degrees F) on bottom shelf of the prep table in the raw meat room. Food was relocated to the walk in cooler during the inspection.
      • Observed presence of flying insects in the food establishment. Instructed the person in charge to contact pest control for service. Also observed harboraging conditions including pooling water in the dishwasher area and significant accumulation of encrusted debris on nonfood contact equipment and floors under cooking equipment.
      • Observed accumulation of dead flying insects on fly paper in the back of the house.
      • Observed food employees without proper hair and beard restraints.
      • Observed multiple tongs being stored in between wire shelving above the 3-compartment sink which were soiled to the touch. Tongs were removed for cleaning.
      • Observed stainless steel containers stacked while wet after sanitizing on shelving.
      • Observed the pressure gauge does not move during the hot water rinse cycle. Contact technician for repair or replacement.
      • Observed the following in poor repair: Rusted shelving units, damaged gaskets in cook and hold units, bent hood vents and gaps between hood vents, cracked cooking surface of pizza oven, and tape on handle of oven to hold in place.
      • Observed cutting boards throughout the facility were scratched and scored.
      • Accumulation of encrusted debris on nonfood contact surfaces of baking pans on shelving units.
      • Observed build up of debris on nonfood contact surfaces throughout including shelving, cooking and refrigeration equipment, and backsplash at beverage machines. significant encrusted debris was observed on wheels of cooking equipment.
      • Observed long hose with spray nozzle, which was being used during the inspection, connected to a Y-connector at the mop sink in absence of an adequate backflow preventer. Informed the operator a dual check valve with atmospheric vent is required between the hose and the Y-connector. The mop sink had an atmospheric vacuum breaker installed behind the faucet. Backflow prevention handout provided to the operator.
      • Observed the following in poor repair: Gaps in ceiling tiles at conduit cables, missing and peeling ceiling tiles, missing/cracked floor tiles, grouting eroded in floor tiles, and coving not sealed to the wall at the produce prep sink area.
      • Observed the floors, walls, ceilings, caulk at the dishwasher drainboard and handsinks and plumbing are in need of cleaning. Recommend thorough cleaning of the food establishment.
      • Observed debris on the hood vents above the chicken breading station.
  • Pizza Hut, 10512 Patriot Hwy, Fredericksburg, VA, USA Fredericksburg, VA 22408
    • Date inspected: Aug. 15
    • Total violations: 1
      • Observed a food employee discard a drink down the handwashing sink. EHS discussed with the employee during the inspection to only use the hand sinks for washing hands only.
  • Merge Cafe, 44 Mine Rd 7 Stafford, VA 22554
    • Date inspected: Aug. 15
    • Total violations: 15
      • Person in charge was unable to provide EHS with a signed employee health policy or other proof of foodborne illness training. Food employee was unable to name any of the foodborne illnesses. EHS discussed with the person incharge the need to train and provide proof of all employees.
      • Observed that there are no paper towels at the only hand sink in the kitchen. The person in charge placed paper towels at the hand sink.
      • Observed no hand washing sink near the handwashing area and near the boba station. A hand washing sink must be provided.
      • Observed cooked chicken and beef hot holding in the fornt of the house. Through discussion with the person in charge the owner allegedly owns another food establishment in the area and is preparing raw beef an pork; and transporting them to the food establishment. The food establishments are under different business entities and are not an approved food source unless inspection for wholesaling is obtained by Virginia Department of Agriculture and Consumer Services (VDACS).
      • Per discussion with the person in charge the food contact surfaces for equipment and utensils (blenders, stirrers, spoons) used with dairy which is a time/temperature control for safety food (TCS) are not being washed every four hours or when contamination occurs as per the food regulations. The items are being washed only once a day at closing time. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods. Items were taken to the warewashing area to be washed and sanitized.
      • A date marking system is not in proper use for the ready to eat time temperature control for safety (TCS) foods throughout the facility. EHS provided a sharpie for PIC and had them date mark TCS foods. Observed unlabeled milk, pre-portioned cooked meat, deli slice meat that was stored in the unit for more than 24 hours. PIC labeled the items during the inspection.
      • Per discussion with the person in charge, facility is using time as public health control for the tapioca pearls without proper procedures and records in place. EHS discussed Time as a public health control requirements.
      • Observed two unlabeled spray bottles that contained chemicals in the warewashing area. Bottles were labeled during the inspection.
      • Observed three containers of pesticide stored in the kitchen by the reach in freezer. per conversation with the person in charge, he is spraying pesticides inside of the establishment. EHS discussed the restrictions with the PIC. Spray bottles were removed from the establishment.
      • No thin probe thermometer available onsite to monitor the temperature of the hot and cold holding items.
      • Observed a large bag of sugar and several boxes of the concentrated syrup stored directly on the floor.
      • Observed the containers of in-use utensils stored in water at a temperature of 81, 87, and 92 degrees F. Utensils were re-located to the dishroom to be washed and sanitized.
      • The only quaternary ammonium test kit available on site have expired over a year ago.
      • Observed that the following non-food contact equipment in need of cleaning: 1) in between all floor-mounted equipment, 2) outside of the ice machine, 3) inside of all refrigeration units, 4) all of the storage shelves, 5) tops of all of the equipment, and 6) the inside of the hot holding well.
      • Observed that the floors, walls, ceiling in the rear prep area are in need of detailed cleaning to include the air vent covers and the air vent covers in the restrooms.
  • Days Inn by Wyndham, 24320 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: Aug. 15
    • Total violations: 3
      • No paper towels were located at the hand sink. Paper towels were provided during inspection.
      • The interior surfaces of the ice machine were observed soiled.
      • The outer surfaces of the ice machine, the shelving throughout the kitchen, the cereal dispensers, and the interior of the non-functioning home-style reach in cooler were observed soiled.
  • Bubblebee, 1005 Princess Anne St 2nd Floor Fredericksburg, VA 22401
    • Date inspected: Aug. 15
    • Total violations: 9
      • Observed two employee beverages with no lids sitting on top of beverage prep station. During the inspection beverages were discarded.
      • Observed 2 containers of raw shell eggs stored above containers of various milks in an under counter prep cooler. During the inspection a food employee relocated raw shell eggs to the bottom shelf.
      • Observed a pot of honey boba hot holding at 111 degrees F. Person in charge stated boba had been made 30 minutes prior and the hot holding pot was turned off by accident. Person in charge reheated the boba to 165 before the inspection was over.
      • Observed abscence of date marking for the cakes (in house made) offered at the customer self service cooler. During the inspection the person in charge date marked the cakes.
      • Observed 3 boxes of frozen strawberries stored on the floor of the back room area (next to mop sink). During the inspection frozen strawberries were relocated to a deep freezer.
      • Observed food employees preparing drinks without hair restraints.
      • Observed wet wiping clothes stored on kitchen counters and prep table. EHS discussed with the person in charge, food employee relocated wiping cloths to a sanitizing solution.
      • Observed several reusable mesh/cloth tea and coffee filters hanging stored above and next to the hand sink. During the inspection filters were relocated.
      • Observed the following in need of cleaning: 1. floors underneath equipment 2. AC vent 3. walls around handwashing and 3 compartment sink.
  • Lin's Express, 10853 Tidewater Trail Fredericksburg, VA 22408
    • Date inspected: Aug. 15
    • Total violations: 3
      • No paper towel was provided at either hand sink. Paper towel was provided during inspection.
      • Unwrapped or uncovered food in the walk in cooler. Observed sprouts, cooked shrimp, shredded lettuce, snap peas and cooked chicken wings that were uncovered. Items were covered during inspection.
      • Observed that the sides of the deep fryers and the wok station were heavily covered with grease and debris.
  • Comfort Suites, 4615 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Aug. 15
    • Total violations: 5
      • Facility could not provide procedures for vomiting and diarrheal events. EHS provided a sample during the inspection.
      • Observed the men's and women's restroom has no handwashing signage posted. ESH provided signage during the inspection.
      • Observed the interior of the microwave on the buffet line heavily soiled.
      • Observed at the mop sink in the laundry room a hose connected to the sink laying in the basin without a backflow prevention device. During the inspection the person in charge cut the hose to be one inch above the rim line of the sink. EHS provided a backflow prevention handout during the inspection.
      • Observed the following pieces of equipment in the kitchen that through discussion with the person in charge is no longer in use. 1. Stove top 2. Oven 3. Ice machine

    • Total violations: 9
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Facility does not have written procedures for the clean-up of a vomiting or diarrheal event. The person in Charge was provided with procedures.
      • Observed in the walk-in cooler raw ground beef next to salami deli meat. Foods were relocated during the inspection.
      • Observed the deli slicer soiled, through discussion with a food employee the slicer had not been used since the previous day. The slicer was cleaned during the inspection.
      • Observed plastic containers of raw chicken and salad mix being stored on the floor in the walk-in cooler. During the inspections foods were relocated.
      • Observed food employees wearing watches while preparing food.
      • Observed a container of tuna salad in the prep cooler with a spoon stored inside the container with the handle touching the food. During the inspection the spoon was removed.
      • Observed numerous cutting broads throughout the facility heavily scored and stained.
      • Facility could not provide a quaternary ammonium testing kit. The chlorine testing kit for the dishwasher expired in 2019.

      • Observed the following cold holding at improper temperatures in the salad prep unit #2 for about 2 and a half hours: 1) cut lettuce (44 degrees F), 2) cooked rice (43 degrees F), 3) plain yogurt (43 degrees F). Items were relocated to the walk in freezer for a quick chill.

    • Date inspected: Aug. 15
    • Total violations: 15
      • Observed food employee don single use disposable gloves without washing their hands prior to engaging in food preparation tasks. Also observed the employee handle bag of raw shrimp, touching the opening of the bag, wearing single use disposable gloves, remove gloves and handle equipment without washing their hands. Employee was instructed to wash their hands.
      • Observed food employee lather soap on hands under running water. Hands were not effectively washed due to water flushing soap down the drain. Employee was instructed to wash hands properly.
      • Observed debris on the following food contact surfaces: eight fryer baskets, cutting boards, squeeze bottle, 2-3 stainless steel food container and vegetable slicer. All items were relocated to the dishwashing area to be cleaned.
      • Observed the top interior of ice machine soiled. Also observed debris on food contact surfaces of bar beverage gun. Beverage nozzle was removed for cleaning during the inspection.
      • Observed the following TCS foods held at improper temperatures: 1- Sliced cheese (48 degrees F) in the pull-out drawer on the cookline. Sliced cheese was put in the unit at 9:00 am. The cheese was relocated to the walk-in cooler to cool to 41 degress F or less by 1:00 p.m. 2- Raw chicken (45 degrees F) and raw chicken wings (45 degrees F) in the deep cooler (breading station cooler). The raw chicken was placed in the cooler at 9:30am. Person in charge will put bagged ice to cool foods to 41 degrees F or less by 1:30 p.m. Contact technician to evaluate cold holding capacity of the units. Discussed monitoring temperatures every 3 hours.
      • Observed the back door to the establishment is not tight sealing. Employees need to physically close the door.
      • Observed several flying insects throughout the kitchen. Also observed harboraging conditions in the establishment including pooling water and accumulation of debris.
      • Observed two food employees without hair and beard restraints.
      • Observed several plastic and stainless-steel containers stacked while wet after chemical sanitizing.
      • Observed the following in poor repair: gaskets of the reach in cooler, rust on underneath shelving at the wing station and shelving of the reach in freezer, and missing doors of equipment on the cookline.
      • Absence of alarm or visual means to alert staff of sanitizer or detergent levels. A line of sight is installed inside the dishwasher which cannot be observed during operation.
      • The interior and exterior of the equipment throughout the kitchen are in need of cleaning including shelving, reach in refrigeration, cooking equipment, lever for water at soda machines and top of dishwasher.
      • Observed heavy oily residue and debris on the ground and on the oil receptable in the dumpster enclosure. Person in charge is aware and stated the receptacle leaks. Advised the operator to contact the company for replacement.
      • The walls, floors, and ceilings throughout the kitchen are in need of cleaning including air vents.
      • Observed the following in poor repair: Seal gaps around conduit cables in the ceiling tiles, gaps in ceiling tiles near the 3 vat sink, and rust on ceiling tile frames.
