Fredericksburg Health Inspections: Dead Pests, Expired Food
The Rappahannock Area Health District recorded several violations at restaurants around Fredericksburg during recent inspections.
FREDERICKSBURG, VA — Several facilities around the Fredericksburg area recorded violations during health department inspections between Dec. 7 and Dec. 13.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Billikens Smokehouse, 623 Caroline Street Fredericksburg, VA 22401
- Date inspected: Dec. 7
- Total violations: 7
- Observed the outdoor bar (not in operation during this time) and the upstairs bar (not currently permitted), do not have hand washing sinks available for employees to be able to wash their hands.
- Observed 2 pans of macaroni and cheese (148 degrees) inside a hot holding unit that were not reheated to the required temperature 165 degrees within two hours. Pans of mac and cheese had been in the hot holding cabinet for 20 minutes. EHS instructed person in charge to reheat the pans of mac and cheese. Mac and cheese was reheated before the 2 hour time frame to 207 degrees F.
- Observed in the walk in cooler, 2 pans of green beans made on date of 11/30, 2 pans of barbeque beans made on date of 11/30 and one pan of collard greens made on date of 11/30. Foods should have been used or discarded by 12/6.
- Observed 2 spray bottles with chemicals without a label. During the inspection the person in charge labeled the spray bottles.
- Observed a metal rack holding clean equipment in the kitchen and metal wire racks inside the walk in cooler that require cleaning.
- Observed the air gap at the dish machine is less than one inch above the flood level rim.
- Observed the stair floors leading to the basement and the floors of the basement that need resealing/repainting so that the surfaces are smooth and easy to clean.
- Staybridge Suites, 2996 Richmond Hwy Stafford, VA 22554
- Date inspected: Dec. 7
- Total violations: 20
- Observed an employee walk through the kitchen to get to the laundry room. EHS addressed this with the employee. Observed that the kitchen person is not aware of: taking delivery and final reheating temperatures, the correct procedure for cleaning of the dishes, and using time as a public health control for the breakfast bar foods.
- Observed that the facility does not have a certified food protection manager.
- The facility does not have procedures for bodily fluid cleaning.
- Per discussion with the kitchen staff, he is washing his hands in the 3-compartment sink.
- Observed the kitchen person clean the tables then continue with food prep without washing his hands. EHS discussed this with the kitchen person.
- Observed that there is no soap at the hand sink.
- Observed that there are no paper towels at the hand sink. The person in charge supplied napkins.
- Observed that the inside of the ice machine require cleaning. Per discussion with the kitchen person, the juice machine is not being cleaned daily.
- Observed dispensing utensils in a drawer and a stainless steel pan (corrected) that were dirty to sight and touch.
- Observed that the low temperature dish machine is not sanitizing the equipment. EHS advised the kitchen person to use the 3-compartment sink.
- Observed that the commercially-made sausage and salsa are not dated for disposition.
- Observed that the shredded cheese, in which time as a public health control is being used for, does not have a time annotated when it was taken out of temperature control.
- Observed that the bulk cereals and condiments for the yogurt are not labeled.
- Observed dead pests inside a cabinet in the kitchen.
- Observed stainless steel pans stored with the food-contact surface facing upwards. The person in charge inverted the pans.
- Observed that the 3-compartment sink is not sealed to the wall.
- Observed several damaged cabinets in the kitchen and in the bar area - one cabinet door came off the hinge when it was opened.
- Observed that the interior of the beer cooler and the overflow tubes require cleaning.
- Observed that the air vent covers require cleaning, Observed that the ceiling above the entrance way to the kitchen require cleaning.
- Observed a blanket stored in the kitchen.
- Marian Manor Home For Adults, 177 Old Potomac Church Road Stafford, VA 22555
- Date inspected: Dec. 8
- Total violations: 3
- Observed containers of pudding and cut fruit that were uncovered in the reach in cooler.
- Observed that the low temperature dish machine is not sanitizing the equipment.
- Observed the cook working without a hair restraint.
- Bowling Green Health & Rehabilitation Center, 120 Anderson Avenue Bowling Green, VA 22427
- Date inspected: Dec. 8
- Total violations: 3
- Observed a bottle of degreaser hooked up to a pre-mix dispenser at the 3 compartment sink in place of the sanitizer. De-greaser was replaced with a bottle of sanitizer.
- Observed rust on the interior surfaces of the microwave in the kitchen.
- An irreversible registering thermometer is not provided to monitor the final rinse temperature of the dish machine.
- Olde Towne Steak & Seafood, 1611 Princess Anne Street Fredericksburg, VA 22401
- Date inspected: Dec. 8
- Total violations: 12
- Observed the facility does not have a person that is a Certified Food Protection Manager.
- Observe the facility could not provide an employee health policy. During the inspection EHS provided a copy that employees started reading and signing before the inspection was finished.
- Observed the person in charge could not provide a written procedure for clean up of vomit and diarrhea. During the inspection EHS provided a copy of a written plan.
- Observed the interior lip making direct contact with ice was observed soiled.
- Observed two gallons of 2% milk stored in a reach cooler with expiration dates of 11/13 and 11/24. During the inspection the gallons of milk were discarded.
- Observed the facility does have a thermometer but it is not a thin probe thermometer. EHS advised PIC to acquire a thin probe thermometer.
- Observed a food employee working without hair restraints.
- Observed the ice scoop was stored with the handle making direct contact with the ice.
- Observed the kitchen hand sink, 3 compartment sink and dish area sink need to be recaulked.
- Observed metal shelving holding equipment and foods in the walk in cooler that are rusting.
- Observed sides of cook equipment require cleaning. A heavy build up of grease and food debris was observed.
- Observed floors of the kitchen are chipping away with paint and are no longer smooth and easily cleanable.
- Tropical Smoothie Cafe, 1465 Stafford Market Place Suite 115 Stafford, VA 22556
- Date inspected: Dec. 9
- Total violations: 12
- Observed that the employees are not aware of the foodborne illnesses, allergies, and conditions of working healthy.
- Observed that the penni grill is encrusted with grime and debris.
- Observed cut kale at 64'F and baby spinach at 63'F sitting on a counter top. The person in charge placed the items in the sandwich prep unit.
- Observed commercially made potatoes and portioned corn cups not date for disposition.
- Observed that items like fat burners and other ingredients that are on the menu that claim to have nutritional value are not labeled as required.
- Observed foods that were delivered yesterday that were sitting on the floor.
- Observed that the cook has a long pony tail and the cashier who prepared foods are not wearing their hair restraints properly.
- Observed several wet wiping cloths stored on the counter. The cook placed the cloths in a sanitizer bucket.
- Observed that the 3-compartment sink is not sealed to the wall.
- Observed that the following non-food contact surfaces require cleaning: caulking on the front hand sink and prep table, can opener holder, inside of the cookie oven, exterior of the squeeze bottles, and the plastic containers with date labels on them,
- Observed 3 lights out in the kitchen.
- Observe that the following physical facilities require cleaning: the pipes under the 3-compartment sink, air vent covers in the walk in refrigeration units, air vent covers in the restrooms, and wall over the sandwich prep units.
- Primo Pizza, 7132 Salem Fields Blvd Fredericksburg, VA 22407
- Date inspected: Dec. 9
- Total violations: 2
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Flooring in walk-in freezer observed to be damaged and is in need of repair. Hood baffle on ventilation system above pizza oven observed to be damaged and is in need of replacement.
- Bright Beginnings Childcare Center, 85 Cleremont Drive Fredericksburg, VA 22405
- Date inspected: Dec. 9
- Total violations: 7
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS informed PIC of the update to the regulations on December 2, 2021. EHS provided a sample to the bodily fluids clean up procedures.
- Observed a food employee handle chemicals and touched personal belongings then handled clean single service plates from the food contact surface without first washing their hands and donning gloves. Employee was instructed to wash their hands- The plate which came in contact with their soiled hands was discarded.
- The highest temperature of the hot water was 99'F in the only hand washing sink in the kitchen.
- Observed 2 cartons of raw shell eggs placed near ready to eat deli turkey and salsa. Eggs were relocated.
- Several raw shell eggs are cooked in large batches in one pan for several students without a HACCP plan- as per discussion with the person in charge (PIC) eggs are cooked to 135'F, not 145'F as per the VA Food Regulations.
- The temperature of the hot water at the three compartment sink only reached 107'F.
- A P-trap was observed under the 3 compartment sink. Direct connection exists at the 3 compartment sink sewer line.

- Date inspected: Dec. 12
- Total violations: 10
- Upon arrival staff reported the designated person in charge was not on the premises. A staff member was designated as the person in charge during the inspection. Ensure a designated person in charge is available during all hours of operation.
- The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Sample written procedures were provided during the inspection.
- Observed a cup with a lid which had an opening for drinking (absence of a straw) stored on the prep table in the back of the house used to pan foods. Observed uncovered food on the prep table upon arrival. During the inspection the cup was removed.
- Observed a food employee touch their cell phone and then proceed to don single use disposable gloves to engage in food tasks. Instructed the employee to wash their hands.
- Observed the following food contact equipment soiled to the sight: 1- 6-8 silicone mats used to prepare bread and 2- vegetable slicer. The food contact equipment was removed to the 3 vat sink for cleaning.
- Observed spray bottle of Peroxide cleaner/disinfectant hanging on the shelving over the back prep table which had uncovered containers of sliced jalapeno peppers upon arrival. During the inspection the spray bottle was removed. to prevent contamination.
- Observed accumulation of debris on the following nonfood contact surfaces: Exterior surfaces of stack of stainless steel pans stored under the prep table in the back of the house and shelving under the hot holding well.
- Observed the following in poor repair: 1- holes/gaps in ceiling tiles near the bag in the box and walk in cooler, 2- ceiling tiles not secured to the frames in the back of the house and 3- Ceiling light cover by the bag in the box is hanging, not secured to the frame.
- Observed employee handbags stored on shelving above the back prep table. During the inspection the handbags were relocated.
- Nayeli Salvadorian & Mex Food, 20 Plantation Drive, #149 Fredericksburg, VA 22406
- Date inspected: Dec. 12
- Total violations: 2
- Observed a food employee handling ready to eat cilantro with their bare hands. Cilantro was discarded.
- Cooked to order raw shell eggs are offered on the menu without a disclosure and a reminder for the consumer advisory to inform consumers of the significantly increased risk of consuming raw or undercooked raw eggs.
- Umi Japanese Fine Dining, 1500 Emancipation Hwy Fredericksburg, VA 22401
- Date inspected: Dec. 12
- Total violations: 6
- Observed a food employee rinse a hand towel at the hand sink.
- Observed a food employee washing a bowl at the prep sink.
- Observed knives being stored near a hand sink. During the inspection the knives were relocated.
- Observed the following: 1.cardboard boxes and foil lining equipment in the kitchen. 2. metal shelves holding foods and equipment that are rusted
- Observed the following non food contact surfaces in the facility that require cleaning: 1. metal shelving in the walk in cooler 2. inside the fryer cabinets 3. inside and outside of cooling/freezing equipment 4. outside surfaces of flour/rice bins.
- Observed walls of the walk in cooler and floors under equipment that require cleaning.

- Employee restroom door is not self-closing.
