Fredericksburg Health Inspections: Dead Pests Inside A Drain

The Rappahannock Area Health District recorded violations at several facilities in Fredericksburg area during recent inspections.

FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent inspections from Nov. 2 to Nov. 7.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Home2 Suites, 9901 Patriot Hwy Fredericksburg, VA 22407
    • Date inspected: Nov. 2
    • Total violations: 1
      • The lower shelf of the prep table where the coffee maker is located is not elevated at least 6 inches above the floor. The table is not easily movable. A cleaning Hazard exists.
  • Comfort Suites Interstate, 541 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: Nov. 2
    • Total violations: 7
      • The food establishment does not have a certified food protection manager. Provided certified food manager course information to the operator.
      • The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Guidance documents were emailed with training inspection documentation on 1/13/2022. Provided sample procedures using Chlorine bleach to the operator.
      • Observed four to five unopened packages of commercially processed hard boiled eggs (43-45 degrees F) in the door of Whirlpool refrigerator #1 at improper temperatures. The person in charge reported the eggs were held in the refrigerator overnight. During the inspection the person in charge voluntarily discarded the eggs. Discussed placing eggs on the shelf instead of the inside door .
      • Observed containers of commercially prepared hard boiled eggs, egg patties and pork sausage, which have been removed from the manufacturer packaging, held in refrigeration equipment over 24 hours without a date label. The person in charge reported the foods were removed from the original packaging and placed in a food storage container on 11/1/2022. During the inspection the operator placed a date label on the containers.
      • Observed bread and bagels, placed in plastic Ziploc- type bags and available for customer self-service at the breakfast area which were not labeled to identify the ingredients or allergens.
      • Observed coffee stirrers in cup for customer self service which were not wrapped or otherwise protected from contamination. Observed single use disposable cup is being used to store and dispense the stirrers.
      • Observed the following in poor repair: Burned out ceiling lights by the storage room, ceiling tiles which are not tightly secured to the frame and damaged wall seals behind the 3 compartment sink.
  • Target, 1090 Stafford Market Place Stafford, VA 22554
    • Date inspected: Nov. 2
    • Total violations: 7
      • The facility cannot provide written documentation on the procedures for cleaning up bodily fluid incidents
      • Observed employee chewing gum in the prep area. The person in charge had the employee discarded the gum.
      • Observed that the water at the front hand sink measured 89'F.
      • Observed the employee working in the prep area without a proper beard restraint. The employee put on a beard restraint.
      • Observed bare wood under the service counter.
      • Observed that the pipe under the dump sink is repaired with duct tape.
      • Observed that the air vends over the service line and the floor on the service line are in need of detailed cleaning.

      • The following are in need of repair: 1. The bottom cover of the 2 door reach in cooler was observed taped to cooler. 2. Four broken cabinet doors along prep counter.
      • Observed cinnamon sugar and baking soda being store in blue Rubbermaid container on the bottom shelve of counter. During the inspection, the PIC discarded food that touched container. (Corrected)
      • The following are in need of repair: 1. Paint peeling at entrance door and employee restroom door. 2. Drywall missing along floor coving outside of employee restroom door. 3. Floor electric outlet is missing cover.
  • Auntie Anne's Pretzels, 210 Spotsylvania Mall Drive Space 210 Fredericksburg, VA 22407
    • Date inspected: Nov. 2
    • Total violations: 1
      • Observed 2 food employees working without hair restraints. During the inspection, the food employees applied hair restraints. (Corrected)
  • Baba Ganoush Mediterranean Grill, 1115 Emancipation Highway Fredericksburg, VA 22401

      • The person in charge is not monitoring employee handwashing.
      • No certified food protection manager (CFPM) certificate.
      • The PIC could not provide an employee health policy for the food employees working in the kitchen. EHS provided a sample copy of the employee health policy.
      • Observed a food employee take personal cup of tea out of the microwave, take a drink, and then place on the naan prep table. EHS discussed the use a designated area for drinking. (Corrected)
      • Observed a gloved food employee handling raw meat, remove his gloves and then proceed to apply new gloves. EHS informed food employee when to wash hands. During the inspection, the food employee washed hands before applying new gloves. (Corrected)
      • Observed a food employee wash their hands in less than 5 seconds. EHS demonstrated the proper handwashing procedure. During the inspection, the food employee rewashed their hands before returning to work. (Corrected)
      • Observed a food employee wash their hands at the 3 compartment sink. EHS informed food employee where to wash hands. During the inspection, the food employee washed hands at handwashing sink. (Corrected)
      • Observed 1 large container and 1 small container of raw chicken over raw shrimp and raw lamb in the meat reach in cooler. During the inspection, the PIC relocated the chicken to the bottom shelf. (Corrected)
      • Observed the interior of the microwave heavily soiled.
      • Observed no date marking on any of the prepared TCS foods in the establishment.
      • Observed 1 spray bottle containing water and 2 spray bottles containing chlorine that were not labeled. During the inspection, the food employee labeled the spray bottles. (Corrected)
      • The PIC could not produce an appropriate food temperature measuring device to verify cooking temperatures.
      • Observed cans of tomato paste and crushed tomatoes being stored in a broken reach in cooler in the back hallway of the establishment that are accessible to anyone walking in the back hallway. During the inspection, the food employees moved the food into the establishment. (Corrected)
      • Observed a food employee portioning cubed raw meat without hair restraints (head and beard).
      • The following in-use utensils were being stored: 1. Spatula for cooked rice directly on cart. 2. Several cooking spoons in a metal container of water (83 F).
      • Observed single use/service articles being stored in an unlocked shed in the back hallway of the establishment.
      • The following non-food contact surfaces are in need of cleaning: 1. The stock pot burner. 2. The gas range. 3. The interior of all reach in coolers and freezers.
      • Observed the spray nozzle at the 3 compartment sink hanging below the flood plain.
      • No covered trash can in the women's restroom.
      • Observed broken equipment (reach in coolers, fryer, floor mixer) and litter being store in the back hallway of the establishment.
      • The following are in need of repair: 1. The wall along the 3 compartment sink is crumbling. 2. Missing floor tile at the back exterior door into back hallway.
  • Fatty's Taphouse Seafood & Grille, 225 Garrisonville Rd Stafford, VA 22554
    • Date inspected: Nov. 2
    • Total violations: 1
      • Observed the consumer advisory does not contain an adequate disclosure to describe the raw animal foods which may be prepared raw and/or undercooked, upon a customer's request or order. The person in charge reported a meeting is scheduled to review menu changes on 11/16/2022.

      • Observed food employees working while wearing wrist jewelry.
      • Observed one of the hand sinks in the kitchen need recaulking.

    • Total violations: 6
      • Observed during inspection school personnel (not kitchen employes) entering the kitchen area. Through discussion with the person in charge this is a common issue.
      • Observed a container of dish machine detergent stored on top of boxes of plastic compartment trays. Person in charge stated compartment trays are not in use. The container of dish detergent was relocated.
      • Observed numerous dead pests inside a floor drain in the kitchen area.
      • Person in charge was unable to provide a test kit for the chlorine chemical sanitizer being used at the 3 compartment sink.
      • Observed the ceiling tiles of the dry storage room are porous and are not smooth and easily cleanable.
      • Observed ceiling tiles and panels around the dish machine that require cleaning and several ceiling tiles in the kitchen that are dusty around the edges.

    • Total violations: 3
      • Observed several serving trays, 2 knives, and a plastic container that was soiledl to sight and touch. The person in charge moved the items to the dish washing area.
      • Observed that the pressure gauge is reading 29.
      • Observed that the air vent cover over the dish machine and the hood system is in need of cleaning.

    • Date inspected: Nov. 2

      • No certified food protection manager (CFPM) certificate.
      • Observed a food employee use her bare hands to dice/slice tomatoes on the prep unit cutting board. During the inspection, the food employee discarded the tomatoes and washed her hands and applied gloves before continuing to cut tomatoes. (Corrected)
      • The eggs on the menu (under breakfast sandwiches and sides) do not have an asterick directing to consumer advisory footnote.
      • Observed food employee preparing food without hair restraints (head and beard).
      • Observed duct tape and screws on the doors of the 3 door horizontal cooler.

      • The person in charge was unable to provide verification that employees had received training concerning employee health. A sample Employee Health Policy was left with the person in charge.
      • Observed employee enter kitchen and removed a pan of eggs from the oven and take them to the serving line without washing their hands. Employee was educated on when to wash hands.
      • No disposable towels were provided at the hand washing lavatory in the kitchen. Towels were provided during inspection.
      • The equipment being cleaned in the high temperature sanitizing dish machine were not observed being sanitized. The temperature on the EHS's irreversible maximum temperature registering thermometer measured 112'F. The three compartment sink was set up to sanitize equipment.
      • The person in charge was unable to produce a food thermometer to verify the temperatures of food.
      • Observed serving platters stored uncovered on top of the kitchen cupboards. Dishes were inverted during inspection.
      • Observed coffee filters stored uncovered near the coffee maker in the kitchen. Filters were inverted during inspection.
      • The interior of the cabinet under the sink in the kitchen is water-damaged and is no longer cleanable.
      • The employee was unable to produce a non-reversible, maximum reading thermometer to measure the temperature of the rinse cycle in the high temperature sanitizing dish machine.
      • The non-food contact surfaces of the interior of the reach in cooler and the upsplash of the coffee maker in the kitchen were observed soiled.
      • The ceiling above the serving line is not smooth and easily cleanable.
      • Light bulb(s) in the kitchen are not covered by a protective shielding.
  • Berea Health & Rehab Center, 55 Brimley Drive Fredericksburg, VA 22406
    • Date inspected: Nov. 3
    • Total violations: 10
      • Observed a food employee use a wiping cloth on a prep surface using their bare hands and then proceed to handle a container of cinnamon rolls. Instructed the employee to wash their hands.
      • Observed a food employee use the same single use disposable gloves to handle soiled equipment at the dishwasher and handle cleaned and sanitized equipment from the dish rack. Instructed the food employee to remove the gloves and wash her hands.
      • The following food contact equipment was soiled to the sight: 1- stainless steel bowl stored on shelf under the prep area, and 4-5 dispensing utensils stored in a container under the steamer. The person in charge removed the container to clean the utensils. The soiled equipment was removed to the dishwasher area for cleaning.
      • Observed interior plastic lid of the ice machine soiled on the bottom near the ice. Instructed the person in charge to burn or remove the ice from under the lid and clean the ice machine.
      • Observed hand sanitizer pump stored on a shelf over baked bread which was cooling. The person in charge removed the hand sanitizer.
      • Observed plastic reusable beverage lids stored in a container with a pair of soiled scissors. The container was removed to the dishwasher area to wash and sanitize the beverage lids.
      • Observed single use disposable forks, knives and spoons stored in a container at the west end dining room with the food contact surface facing upwards and unprotected from contamination.
      • Observed three unsealed wooden spoons which were in a state of poor repair from chipped surfaces and two pastry brushes (paint-brush style) with tight bristles which were not easily cleanable. During the inspection the person in charge discarded the wooden spoons. Discussed replacing the pastry brushes.
      • Observed accumulation of debris on the following nonfood contact surfaces: Stainless steel backsplash on the flattop, bins used to hold various dispensing utensils under the steamer, base of the 1 door upright cooler, and exterior surfaces of the prewash spray nozzle.
      • Observed three blown out shielded lights above the hood, one ceiling tile not secured to the frame in the dry storage room and two ceiling tiles near the 1 door upright cooler with gaps.

      • Shelf located to the left of the fryers above the prep table was observed with an accumulation of food residue and debris and is in need of cleaning.
      • Mop sink spray nozzle hose observed without a proper backflow prevention device. Water was turned off during inspection.
      • Flooring under fryers observed with an accumulation of grease and is in need of cleaning.
      • Ceiling vents located above 2-compartment sink were observed with an accumulation of static dust and are in need of cleaning.
  • Courthouse Road Elementary School, 9911 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Nov. 4
    • Total violations: 4
      • Observed a spray bottle of sanitizer disinfectact stored near packaged snacks on the bottom shelf of the serve line. During the inspection the spray bottle was relocated.
      • Observed the lactic acid sanitizer solution at the 3 compartment sink dispenser was at a toxic level 1130ppm. No sanitizer buckets were made prior to inspection nor was the sanitizer in use at the time at the 3 compartment sink. EHS advised the person in charge to closely monitor and test the solution when employees use wiping clock buckets or manually sanitize dishes at the 3 compartment sink. Person in charge will manually dilute the solution until a technician comes to fix the dispenser or use the high temperature sanitizing dish machine.
      • Person in charge was unable to provide a thermometer or thermolabels to ensure the dish machine is reaching the required temperatures for proper sanitization.
      • Observed the floors of the walk in cooler are rusted.

    • Date inspected: Nov. 4
    • Total violations: 7
      • No written agreement/statement from the supplier/aquaculturist stipulating that the fish were raised (open water in net pens; or land-based operations such as farm and tanks) and pellet fed.
      • Observed the following soiled to touch and sight: 1. One plastic food container on the dry dish rack. 2. The tomato/onion dicer. During the inspection, the items were relocated to the 3 compartment sink for cleaning and sanitizing. (Corrected)
      • Observed 8 portioned bags of linguine (between 47 F - 55 F) in the saute prep unit that were at improper temperatures. During the inspection, the food employee discarded linguine. (Corrected)
      • Observed several flying insects at the dry food storage racks near the sweet potatoes/yams.
      • Observed several plastic containers on the drying rack that were stacked while wet after cleaning and sanitizing.
      • Observed several pizza boards cracked and all the pizza paddles chipped. During the inspection, the person in charge removed from service. (Corrected)
      • The plumbing under the sanitizing basin at the 3 compartment sink is leaking.
  • Ichiban Ramen & Sushi, 1632 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: Nov. 7
    • Total violations: 10
      • The person in charge could not provide an employee health policy for the food employees working in the kitchen. EHS provided a sample copy of the employee health policy.
      • No written agreement/statement from the supplier/aquaculturist stipulating that the fish were raised (open water in net pens; or land-based operations such as farm and/tanks) and pellet feed.
      • The following were observed at improper cold holding temperatures in the glass door reach in cooler: 1. One container of pre-packaged Takoyaki (fried octopus ball) between 58 F - 59 F. 2. One container of pre-packaged vegetable spring rolls at 46 F. 3. One container of pre-packaged chicken gyoza between 44 F - 46 F. During the inspection, the food employees discarded food. (Corrected)
      • Observed 1 thawed ROP frozen tuna in the sushi cooler that was not removed from the packaging as stated on the label, "Must remove from vacuum package before thawing." During the inspection, the PIC discarded the package. (Corrected)
      • The White Tuna (Shiro Maguro) on the menu is Escolar.
      • The following in-use utensils were observed incorrectly stored: 1. Rice scoop in a container of water (74 F). 2. Several tongs for hot holding pork and chicken hanging on cart handle. 3. Tongs for steam buns on the non-food contact shelf. During the inspection, EHS informed food employees of proper in-use utensil storage. The food employees relocated the utensils to the 3 compartment sink for cleaning and sanitizing. (Corrected)
      • Observed tempura crunch, sesame seeds, and raw pork being stored in peeled garlic (single-use) containers. During the inspection, the PIC discarded the containers. (Corrected)
      • Observed the non-food contact surfaces of the deep frying pans over the gas range encrusted with grease and food debris. During the inspection, the food employees relocated the pans to the 3 compartment sink for cleaning.       • The following are in need of repair: 1. Light out in the corner of dry food storage and reach in freezer. 2. One ceiling tile with water damage over the dry food storage rack.

    • Date inspected: Nov. 7
    • Total violations: 17
      • Observed that the facility does not have a certified food protection manager.
      • Facility does not have written procedures on cleaning of bodily fluids.
      • Observed the employee wash his hands then turn off the faucet with the washed hands. Observed the employee wash his hands for less then 20 seconds. EHS discussed this with the employee.
      • Observed that the inside of the microwave require cleaning.
      • Observed that the low temperature dish machine is not dispensing any sanitizer. The person in charge worked on the machine.
      • Observed the following non-food contact surfaces dirty to sight and touch: table top mixer, can opener (corrected), meat slicer, and the interior of the ice machine (corrected).
      • Observed chicken breast at 45'F, mashed potatoes at 44'F, mozzrella cheese at 42'F, and hamburger at 43'F in the sandwich prep units. The person in charge discarded the products and turned the units down.
      • Observed first aid supplies stored with food products. The person in charge relocated the first aid supplies.
      • Observed small flies in the ice machine room and in the bar area.
      • Observed wiping cloths in the bar stored in sanitizer that measured 25 ppm. The bartender added more bleach in the bucket.
      • Observed that both sandwich prep units are reading a high temperature. The person in charge lower the temperature.
      • Observed that the foil used as shelf liner, cardboard boxes used for storage, and the plastic on the dish machine are not smooth and easily cleanable.
      • Observed that the following non-food contact surfaces require cleaning: bread storage racks, tables under the grills, interior of the sandwich prep units, inside of the equipment storage bins, cabinets under the fryers, storage shelves in the reach in cooler, sprayer nozzle in the dish machine area, bottoms of several plates and saucers on the cook's line, reach in freezer, and the soda gun holders in the bar.
      • The following physical facilities are in need of detailed cleaning: all of the floors, walls, and ceiling to include: hood filters, all of the air vent covers to include the walk in refrigeration units, floor drain in the bar, and drainage pipes in the bar.
      • Observed 6 lights out in the ceiling over the kitchen and one light out in the hood system.
      • Observed 40 foot candles of light and 30 foot candles of light over the grill.
      • Observed cigarettes stored in the kitchen. The person in charge discarded the package.
