Fredericksburg Health Inspections: Hot Chickn Kitchn, Mission BBQ

The Rappahannock Area Health District visited several facilities around the Fredericksburg area during recent restaurant inspections.

FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities during recent inspections. Inspectors conducted the most recently published round of visits between April 27 and May 3.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Boonedocks Seafood (VA 119-091 TM), 19249 Partlow Rd Beaverdam, VA 23013
    • Date inspected: April 28
    • Total violations: 1
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
  • Children of America Doc Stone, LLC, 75 Dunn Drive Stafford, VA 22554
    • Date inspected: April 28
    • Total violations: 4
      • Observed that the can opener requires cleaning. The person in charge placed the can opener in the 3-compartment sink.
      • The reach in freezer is in disrepair. Most of the items in the freezer were not frozen.
      • Observed that the can opener holder and all of the storge shelves requires cleaning.
      • Observed that the mop sink requires cleaning.
  • Hampton Inn and Suites, 1080 Hospitality Lane Fredericksburg, VA 22401
    • Date inspected: April 28
    • Total violations: 2
      • Observed several boxes of single-use articles (10 oz cups, utensils) being stored on the floor in the electrical room over the fire suppression system.
      • Observed a cell phone and a coffee cup on the prep counter and a purse and coat stored on the dry food storage rack.
  • Fas Mart #19, 620 Cambridge Street Fredericksburg, VA 22405
    • Date inspected: April 28
    • Total violations: 4
      • Observed uncovered container of raw tilapia fish stored in the walk in cooler that was not protected from cross contamination due to being stored under the vents, soiled shelves, and a soiled cardboard box. PIC discarded the fish.
      • Observed an uncovered container of raw tilapia fish stored near pre-cooked ready to eat eggs. PIC discarded the fish.
      • Observed the following: several containers, lids, cooking utensils, and bowls were observed soiled to sight and touch throughout the facility The person in charge was asked to wash, rinse, and sanitized the items.
      • Observed a hose attached to a Y connector at the mop sink without an air gap or a back flow prevention device.
  • Hot Chikn Kitchn, 50 N. Stafford Complex Center 102 Stafford, VA 22556
    • Date inspected: April 28
    • Total violations: 6
    • Note: This was a pre-opening inspection. The health department said the following issues would need to be addressed prior to opening.
      • Remove the plastic from the equipment like the waffle maker and heat lamps
      • Clean the air vent covers throughout the kitchen
      • Replace the blown lights in the hood system
      • Ensure that all the equipment is clean
      • Provide a thin probe thermometer
      • Remove all the stickers off of the new equipment
  • McDonald’s (Central Park), 1701 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: April 28
    • Total violations: 3
      • Observed one microwave with a loose door handle and one microwave with missing screws.
      • Observed mop sink basin not smooth and easy to clean.
      • Improper handwashing procedures observed. Employee washed hands under running water for five seconds. (Corrected)
  • The Anna Cabana, 4227 Boggs Dr Bumpass, VA 23024
    • Date inspected: April 28
    • Total violations: 15
    • Note: This was a pre-opening inspection. Officials said the following issues would need to be addressed before opening.
      • Need Certified Food Protection Manager
      • Thin probe metal stemmed food thermometer
      • New QT-10 quat test kit (available kit expired 11/2021)
      • Remove all expired foods from dry storage. Remove all TCS foods from refrigeration units due to power outage that occurred in January
      • Provide self-closing mechanisms on all outer doors in kitchen & dining room
      • Clean & sanitize all food contact surfaces, dishware, small wares, ice machine
      • Protect clean items while in storage such as plates, silverware, beer pitchers, food storage containers by covering or inverting
      • Clean non-food contact surfaces of equipment such as interior refrigeration units, fryer, stove, ovens, surfaces below flat top grill, hood filters
      • Clean wall next to fryer and upper wall behind ice machine. Clean ceiling vent cover behind ice machine
      • Clean inside microwave. Clean air fryer or remove from facility.• Replace worn and stained cutting boards
      • Provide paper towels at hand sink in beverage area
      • Repair 2 door OMCAN refrigerator (part is on order)
      • Lower temperature of Magali 2 door Bev Cooler (currently 43°F)
      • Ensure employees are provided employee health policy training & document when training took place
  • Stevenson Ridge, 6912 Meeting Street Spotsylvania, VA 22553
    • Date inspected: April 28
    • Total violations: 3
      • The lid of the chest freezer in the kitchen is covered in rust.
      • The non-food contact surfaces of the floor mixer and the floor fan in the kitchen were observed soiled.
      • The floors underneath the equipment in the kitchen and the wall behind the dish machine was observed soiled.
  • Kobe Japanese Steak & Seafood, 261 Garrisonville Road Suite 101 Stafford, VA 22554
    • Date inspected: April 29
    • Total violations: 1
      • The disclosure and reminder for the consumer advisory do not meet the food regulations requirements. The consumer advisory lacks the reminder, it only has the disclosure- Observed no asterisk (*) by the sushi and the cook to order steak.
  • Mission BBQ Fredericksburg VA, 3515 Plank Road Fredericksburg, VA 22407
    • Date inspected: April 29
    • Total violations: 13
      • The PIC could not provide an employee health policy for the food employees working in the kitchen. EHS provided a sample copy of the employee health policy. During inspection, the PIC had the food employees sign the policy. (Corrected)
      • Observed 2 food employees wash their hands under running water. The food employees rewashed hands using the proper procedure. (Corrected)
      • Observed a sanitizing bucket in the handwashing sink. During the inspection, a food employee relocated the sanitizing bucket. (Corrected)
      • Observed several dented cans on the can rack. During the inspection, the PIC discarded the cans. (Corrected)
      • The following were observed soiled: 1. Measuring cup and a spatula hanging over the prep table. 2. The fryer punch over the 3 compartment prep sink. During inspection, the equipment was relocated to the dishwashing area for cleaning and sanitizing. (Corrected)
      • Observed 5 large plastic containers of mac and cheese that were prepared on 04/29/2022 at 11:00 am (time of inspection: 3:30pm) and were at improper temperatures (ranging from 44 F - 49F). During the inspection, the PIC discarded the mac and cheese. (Corrected)
      • Observed a spray bottle of soap hanging near the dishwashing area that was not labeled. During the inspection, the PIC discarded soap. (Corrected)
      • Observed several food employees working without hair and beard restraints.
      • Observed several plastic and metal food containers on the drying rack that were stacked while wet after cleaning and sanitizing.
      • The following single use articles were observed not protected from contamination and/or at least 6 inches above the floor: 1. One large bag containing 1 gallon containers for sauce stored outside (inside the fenced area) next to the smoker. 2. Several boxes of cups, 1 box of utensils, and catering boxes on the floor next to the 2 smokers and under the dry storage racks.
      • Observed 7 chipped spatulas hanging over the prep table. During the inspection, the PIC discarded the spatulas. (Corrected)
      • Observed paint peeling along the side wall of swinging door into prep kitchen and the exterior back door.
      • Observed personal items (jacket, backpack, beverages) being stored at the handwashing sink (beverage), on top of the hot holding unit (beverage), food storage rack (backpack), and throughout the establishment.
  • Mission BBQ Stafford, 267 Garrisonville Road, Suite #110 Stafford, VA 22554
    • Date inspected: April 29
    • Total violations: 12
      • Observed the following: 1) an employee using the hand washing sink to fill a spray bottle. Observation was discussed with the person in charge (PIC), 2) a large spatula stored in the hand washing sink basin in the front of the house. PIC relocated the item.
      • Observed smoked salmon hot holding at improper temperature (109-111'F)- PIC reheated salmon to 165'F.
      • he consumer advisory provided for the undercooked salmon and steak does not meet the requirements. The permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder. PIC was instructed to add an asterisk by the salmon and steak. (*)
      • Observed 2 trays of thawed out salmon stored in the walk-in-cooler. The product has to be removed from the package prior to thawing as per the instructions on the package. PIC discarded the salmon.
      • Observed several packages of brownies prepared at the facility that were stored by the cash registers for self dispensing. The brownies did not include name of food, ingredients, preparation date, or allergies warning.
      • Observed several bulk food storage containers which were not labeled to identify the contents on the shelves next to the walk in cooler and in the front of the house. PIC labeled the containers during the inspection.
      • Observed multiple employees conducted food preparation process wearing wrist jewelry. Jewelry was removed during the inspection.
      • Observed several wiping cloths throughout the facility that were held in sanitizer buckets (50 PPM) that did not meet the required concentration the required concentration according to the manufacturer is (200-400 PPM). EHS made a sanitizer bucket and discussed the requirements with the person in charge. All sanitizer buckets were remade and checked.
      • Observed unwrapped single service knives, spoons, and forks stored with the lip-contact surface in different directions on the counter for self-service. Corrected during the inspection.
      • Observed the following in need of cleaning: 1) fryer cabinets, 2) shelving units by the dish machine, 3) non food contact surfaces of soda machines, 4) non food contact surfaces of hot holding units.
      • Observed the following in need of repair: 1) re-caulk all sinks in the front and back of the house to include 3 compartment and mop sinks, 2) repair broken tiles, 3) seal exterior door located in the kitchen, 4) leak in the mop sink, 5) re-grout tiles under 3 compartment sink, 6) re-adjust ceiling tiles, 7) sprayer in 3 compartment sink.
      • Observed jackets, covered drinks, bags, keys stored by food and single service articles. PIC moved items.
  • Royal Farms #237, 1221 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: May 2
    • Total violations: 2
      • Observed a sanitizer bucket in the hand washing sink in the front of the house- PIC relocated the bucket.
      • Quaternary Ammonium sanitizing solution in the 3 compartment sink (0 PPM) was not at an acceptable concentration. Corrected during the inspection.
  • Subway, 15 South Gateway Drive Suite #117 Fredericksburg, VA 22406
    • Date inspected: May 2
    • Total violations: 2
      • Employee was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy sample and had employee read and sign during the inspection.
      • The exterior door which is located in the kitchen is not self-closing. This may allow the entry of insects or rodents.
  • Primavera Pizzeria & Grill, 570 Celebrate Virginia Parkway #145 Fredericksburg, VA 22406
    • Date inspected: May 2
    • Total violations: 10
      • Absence of active managerial control observed during the inspection. No person in charge available during the inspection. Employees are not aware of the required food cooking temperatures. Adequately train employees and monitor temperature of TCS foods during cold holding
      • No certified food protection manager available.
      • No PIC available, Employee was unable to provide documentation of training relating to the employee health policy for this facility.
      • Observed an employee cutting onions, the employee had a plate of food on the prep table near the cutting board. Observation was discussed with the employee. Food was discarded.
      • A food employee was preparing food then handled soiled dishes and stepped out of the kitchen then went back to preparing food without first washing her hands. Discussion with discussed with the employee.
      • Several containers and cooking utensils were observed soiled to the touch and sight. Employee removed the items to be washed, rinsed, and sanitized.
      • Quaternary Ammonium sanitizer solution used to clean food contact surfaces was measured at greater than 500 parts per million. Solution was diluted to approximately 200 parts per million.
      • Observed 2 bottles of all purpose cleaners stored next to food and equipment. bottles were relocated.
      • Rear door of the facility was propped open by a brick upon the start of the inspection. Employee closed the door.
      • Observed disposable plastic ramekins stored inside of food containers and being used for dispensing.
  • Holy Cross Academy, 250 Stafford Lakes Parkway Fredericksburg, VA 22406
    • Date inspected: May 2
    • Total violations: 1
      • The food establishment could not provide written procedures for clean-up of vomiting and diarrheal events. Written procedure handouts were provided during routine inspection conducted on 11/19/2021. Discussed requirements with the person in charge.
  • Smith Station Elementary, 7320 Smith Station Road Fredericksburg, VA 22407
    • Date inspected: May 2
    • Total violations: 5
      • Observed a dog walk into the person in charge's (PIC) office through the kitchen. PIC stated the dog was the IT employee's service dog. Informed PIC and front office of guidelines regarding service dogs in kitchen. IT employee attached leash to dog and walked dog out of kitchen. (Corrected)
      • Observed a gloved food employee eating an apple in the kitchen. Food employee discarded apple and removed gloves and washed hands before returning to work. (Corrected)
      • Observed gloved food employee adjust face mask and attempt to return to prepping food. EHS informed food employee of when to wash hands. Food employee removed gloves and washed hands before returning to work. (Corrected)
      • Observed food employee wash hands in less than 10 seconds. EHS informed food employee of the proper handwashing procedure. Food employee rewashed before returning to work. (Corrected)
      • No temperature measuring device for high temp dishwasher.
  • Golden Corral, 23866 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: May 3
    • Total violations: 13
      • The equipment being cleaned in the dish machine were not observed sanitized. No sanitizer was being dispensed into the machine. A service call was made during the inspection.
      • Observed the following food contact surfaces soiled: 7 knives, 3 potato peelers, food cutting scissors, and 2 ladles stored over the 3 compartment sink on the clean equipment rack. All items were taken to the dishroom to be cleaned.
      • The following food items were not cold holding at proper temperature in the bakery prep unit: shredded cheese 47'F, pepperoni 47'F, tomato slices 47'F. All items had been stored in the unit overnight. Items were voluntarily discarded during inspection.
      • Observed the dough press in the bakery was stored under a flying insect zapper. Dough press was relocated during inspection.
      • In-use utensils improperly stored between use. Utensils were observed stored in water measuring 78'F. Utensils were removed from water and cleaned.
      • Observed several stainless steel and plastic line insert pans stacked while wet.
      • The following equipment was not sealed to adjoining equipment or walls: 1-the 3 compartment sink, 2-the dish machine staging tables, 3-all of the hand sinks in the kitchen.
      • The following equipment was observed in a state of disrepair: 1-the clean equipment shelving (collapsing), 2-the convection oven doors (a piece cardboard being used of keep the doors closed), 3-the black metal cart in the kitchen (rusty), 4-the shelving in the walk in cooler (rusty).
      • The following non-food contact surfaces were observed soiled with grime and debris: 1-the outer surfaces of the line insert pans on the clean pot rack, 2-the clean pot rack shelves, 3-the outer surfaces of the dish machine, 4-the clean plate transport carts, 5-the shelving throughout the kitchen, 6-the hot water dispenser, 7-the dunnage racks throughout the kitchen, 8-the sides of the cooking equipment on the cook's line, 9-the interior of the deep fryer cabinets.
      • Observed the following plumbing issues: 1-the faucet at the bakery hand sink is leaking, 2-the faucet on the hand sink near the 3 compartment sink is leaking, 3-The handsink near the walk in cooler is not working, there is no hot water at the meat room hand sink.
      • Observed a hand sink coming loose from the wall near the walk in cooler.
      • The following physical facilities were observed in need of cleaning: the floors throughout the kitchen, especially under the equipment, 2-the ceiling tiles around the ceiling vents (dusty), 3-the walls throughout the kitchen, 4-the floor in the meat walk in cooler, 5-the floor drain, 6-the mop sink and surrounding area. IN GENERAL, THE KITCHEN FLOORS WALLS AND CEILINGS ARE IN NEED OF A THOROUGH CLEANING.
      • Observed personal items such as keys and cell phone stored next to food items throughout the kitchen. Items were re-located during inspection.
  • Applebee’s Neighborhood Grill & Bar, 1000 Stafford Market Place Stafford, VA 22556
    • Date inspected: May 3
    • Total violations: 11
      • Observed that there were no paper towels at both hand sinks. The person in charge put paper towels at the hand sink.
      • Observed containers of seasonings and foods in the reach in freezer that are not covered. The person in charge (PIC) covered the items.
      • Observed dispensing utensils, ceramic plates, and the interior of the portioning cups that are soiled to site and touch. The PIC placed the items in the dish machine area.
      • Observed that the inside of 2 of the microwaves that require cleaning.
      • Observed the fly strip laying over the ceramic plates on the service line. The person in charge discarded to fly strip.
      • Observed an employee cutting cilantro without wearing a hair restraint. The person in charge provided a hair restraint for the employee.
      • Observed several broken plastic containers and ceramic dishes.
      • Observed gaps in both hood systems.
      • The following non-food contact surfaces require cleaning: hood filters, tops and cabinets under the fryers, interior of all reach in refrigeration units, and the beverage tubes.
      • Observed that the floor gout is missing throughout the kitchen, light in the balance near the rear door is misplaced, wall tile in the men's restroom is missing, and the door to the walk in freezer is in disrepair.
      • Observed that the floors and walls require cleaning to include the air vent covers in the walk in cooler, air vent covers over the to-go station in the kitchen, and under all floor-mounted equipment to include the bar.
  • Greek House Of Pizza, 215 Kings Highway Suite 104 Fredericksburg, VA 22405
    • Date inspected: May 3
    • Total violations: 2
      • Observed cold holding violations, no testing strips to test the sanitizer concentration, and no thermometer used to monitor food temperature. Person in charge (PIC) is not monitoring the temperatures of TCS food after the routine inspection conducted on April 20, 2022.
      • Observed the following Time/Temperature Control for Safety (TCS) Foods cold holding at improper temperatures at the small prep unit by pizza prep unit (44`F) : 1) Salami (47'F); and in the prep unit by across from the range burner stove (42'F) : 2) deli sliced ham (47'F), cooked noodles (49'F) 3) tzatziki sauce (47'F) 4) gyro (49'F). Items were placed in the unit less than 3 hours prior to temperature check. PIC moved the items in the refrigerator unit and bring very small portions to use during rush hours, will use time as public health, and place containers over ice until the units are fixed. EHS checked items in 30 minutes and temperatures had already dropped to 41'F.
  • Colonial Forge High School, 550 Courthouse Rd Stafford, VA 22554
    • Date inspected: May 3
    • Total violations: 2
      • Observed food contact equipment not adequately sanitized after cleaning in the high temperature dishwasher. After four cycles, observed surface temperature on sanitized equipment at 156°F. Instructed the person in charge to discontinue use of the dishwasher and contact a technician for repairs. During the inspection the person in charge contacted maintenance for service.
      • Observed opened container of commercially prepared hard boiled eggs which was opened and dated 4/12/2022. The eggs should have been discarded on 4/18/2022, During the inspection the person in charge voluntarily discarded the hard boiled eggs. Discussed date marking and disposition requirements for TCS foods.
  • Subway, 626 Warrenton Road Fredericksburg, VA 22405
    • Date inspected: May 3
    • Total violations: 11
    • Note: This was a pre-opening inspection. The inspector said the following issues needed to be addressed prior to opening.
      • Contact technician to evaluate cold holding capacity of the sandwich prep unit. Discussed monitoring of food temperature, placing small portions in the unit and using ice in food grade bags to facilitate adequate cold holding temperatures.
      • Replace burned out ceiling lights in the storage areas
      • Open ceiling in the bag in the box
      • Clean flooring under the bag in the box (soda syrups), appears to have a leak due to presence of congealed syrup on the floor.
      • 3 compartment sink is directly plumbed through the wall. Install an air brake or air gap under the basins of the three compartment sink to prevent backflow and bring the facility up to VDH Food Regulations. Contact the building department to inquire about permits or applications required for changes in plumbing.
      • Clean base of the bread oven.
      • Observed the following areas in poor repair: Numerous blown out ceiling lights- the person in change reported they were on order.
      • Damaged door leading the soda syrup rack
      • Holes in walls above the mop sink and inside the bag in a box room
      • Cracked floor tiles in the employee toilet room and prep areas
      • Replace ceiling in the employee toilet room. Current ceiling tiles are absorbent and not easily cleanable.
  • Tropical Smoothie Cafe, 9913 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: May 3
    • Total violations: 3
      • During follow-up inspection, observed food employee placing wiping towels into the handwashing sink basin. EHS informed food employee the handwashing sink is only for handwashing. (Corrected)
      • Observed 2 food employees prepping food while wearing watches. One of the 2 food employees removed the watch.
      • The exhaust fans in restrooms were observed soiled.
  • Subway, 29 Banks Ford Pkwy 101 Fredericksburg, VA 22406
    • Date inspected: May 3
    • Total violations: 6
      • Employee was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy and had employee read and sign during the inspection.
      • Observed an employee handle chemicals and soiled dishes with gloves then discarded the gloves and donned new gloves and was in the process of prepping ready to eat food. Observation was discussed with the employee.
      • Observed olives in the handwashing sink strainer. Observation was discussed with the employee, strainer was cleaned during the inspection.
      • Observed several knives and ladles soiled to the touch and sight. Employee removed them to be washed, rinsed, and sanitized.
      • Observed a container of commercially processed ready to eat meatballs hot holding at improper temperature (109-11'F). Meat balls weren't heated properly prior to placing in the hot holding unit. Employee heated meatballs to 165'F.
      • No chlorine testing kit strips available to test the concentration for the sanitizer used in the front of the house to sanitize the knives.
