Fredericksburg Health Inspections: Primavera Pizzeria, Red Lobster
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Oct. 11 and Oct. 17.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Tito's Diner, 1695 Carl D. Silver Parkway Fredericksburg, VA 22401
- Date inspected: Oct. 11
- Total violations: 7
- Observed a food employee with gloved hands touching their shirt and then proceeding to handle raw food. Employee was told to wash her hands.
- The interior ceiling of the ice machine is soiled.
- The stainless steel containers on the shelves in the dry storage area are wet while stacked after being cleaned and sanitized.
- Observed many cutting boards heavily scratched.
- The following are soiled: shelving throughout the kitchen and fryer cabinets.
- The walls behind equipment and floors underneath the equipment throughout the kitchen are in need of cleaning.
- The hood vents above the fryers are in need of cleaning.
- Red Lobster #0572, 3109 Spotsylvania Mall Drive Fredericksburg, VA 22407
- Date inspected: Oct. 11
- Total violations: 16
- Person in charge was unable to provide written procedures for clean-up of vomiting and diarrheal events. EHS gave person in charge a sample copy of procedures.
- Observed no paper towels at the handwashing sinks in the bar, ware washing, and salad prep areas.
- Observed the following utensils stored soiled: 1. five tongs; 2. two round fryer baskets; 3. three blue ceramic trays; 4. three white and one yellow cutting board stained and scored. Items were taken to the warewashing area for cleaning.
- Observed the charbroil grill and the inside of the microwave ovens soiled.
- Observed the interior of the ice machine, not in contact with the ice, soiled.
- Observed the hot water sanitizing dishwasher only reached a dish surface temperature of 144 degrees F, using a Dish-Temp plate-simulating thermometer, 160 degrees F is needed for hot water sanitization. Discussed with the person in charge, using an alternate method, the 3-compartment sink with Sink & Surface sanitizer, to manually wash/rinse/sanitize dishes and utensils, until the dish machine is fixed. The dish machine was not in use yet, asked the person in charge to run it, it was run 5 times.
- Observed one container of coleslaw in the 1-door reach-in cooler across from the grill holding at 56 degrees F and a partial carton of 1% milk in the 2-door reach-in server cooler holding at 54 degrees F. Both items were in the unit overnight. The person in charge voluntarily discarded the items.
- Observed multiple food employees not wearing hair restraints, while in food prep areas and while preparing salads.
- Observed several stainless steel and plastic containers, stacked while still wet, and stored on a wire shelving unit.
- Observed a food worker placing their hand on the food contact surface of the ice scoop while transferring the ice from the ice machine into a large ice bucket.
- Observed the salad prep cooler with icing in the back, and the temperature dial missing from the broil box #2 (2 of 2) reach-in cooler.
- Person in charge was unable to provide a temperature measuring device for their hot water sanitizing dish machine.
- Observed the following non-food contact surfaces of equipment soiled: 1. the exterior of the dish machine; 2. the exterior of the microwaves; 3. the gaskets around the doors of all of the coolers and freezers; 4. the trays that servers put the plates on to take to the customers; 5. the exterior of coolers and freezers; and 6. shelving throughout the facility.
- Observed the following in need of a backflow device: 1. a 4-way connector attached to the mop sink faucet, connected to the 4-way device was a hose reel with a long hose attached; and 2. a spigot near the ware washing area, with a long hose attached.
- Observed floors, walls, and ceilings soiled.
- Observed the A/C vents and vent hood filters soiled.
- Bob Evans Restaurant, 1020 Stafford Market Pl Stafford, VA 22556
- Date inspected: Oct. 12
- Total violations: 11
- Observed food employees not wash hands after donning new gloves on.
- Observed several clean dishes and utensils with debri on the drying rack and cookline. Items were removed for cleaning and sanitization.
- The microwaves throughout the kitchen are soiled. In addition, all baking trays are heavily encrusted and contain carbon build up.
- Observed two food employees without hair restraints.
- Observed food employee with gloved hands handling raw food, then touch the POS system and then proceed back to cooking raw food without changing gloves.
- The following equipment are in poor repair: 1) reach in cooler on cookline. 2) spray nozzle in the three compartment sink. 3) ice build up in reach in cooler on the cookline.
- The interior/exterior of equipment and shelving throughout the kitchen are in need of cleaning.
- A hose with a spray nozzle is connected to the mop sink that is being used in the dishwashing area while the actual spray nozzle is out of use. The hose is not food grade safe. The PIC removed the spray nozzle from the area and hung it up with in the mop sink while turned off. PIC stated they will use the 3CS until the spray nozzle is repaired.
- Observed the outside lid of the grease receptacle heavily encrusted and soiled.
- The floors underneath equipment, walls behind equipment, ceiling tiles and vents throughout the kitchen are need of cleaning.
- The hood vents above the cook tops are in need of cleaning.
- Primavera Pizzeria, 600 William Street Fredericksburg, VA 22401
- Date inspected: Oct. 12
- Total violations: 15
- The person in charge was unable to provide a valid certified food protection manager certificate. The person in charge was able to demonstrate that they are knowledgeable.
- Observed the panini press and wire pizza trays heavy carbon build-up.
- Observed the following foods cold holding above 41 degrees F: 1. large plastic pan of cooked linguini noodles 45-50 degrees F in the walk-in cooler (condensation was observed on the bag covering the pan); 2. partial small plastic pan of cooked chicken wings in the 2-door reach-in cooler 43-45 degrees F. Items were made the day prior per the person in charge. Items were voluntarily discarded by the person in charge. Per discussion with the person in charge no one is checking the temperatures of cooling foods to ensure that they are getting to correct temperature in the alloted time. Also, items are being left in large plastic pans to cool.
- Observed multiple items in the reach-in coolers and walk-in cooler that were not date marked. The person in charge had a food employee date mark items.
- Observed 2 spray bottles of chemicals not marked with the common name. The person in charge marked the bottles with the common name of the chemical.
- Observed the following outer openings not protected: 1. large gap between the bottom of the window and the windowsill (a hand can touch the outer brick wall from inside of the kitchen) by the hot holding area; 2. the back door near the warewashing area is not close fitting (light can be seen around the door); 3. the back door near the dry storage area is not self-closing; 4. there is a large gap in the wall beside the three compartment sink (foam sealant was used on part of the wood underneath but the gap is not enclosed); 5. gap between the conduits and ceiling above the coca cola reach-in cooler.
- Observed a large number of live flying insects in the kitchen and bar areas, and several live crawling insects in the kitchen. Discussed with the person in charge the need to call pest control today in order to get them out to start controlling the issue.
- Observed several food workers preparing food without hair or beard restraints.
- Observed the following being stored improperly: 1. eight spoons hung on the wall, right above the trash can, with the food contact surface in contact with the wall itself; 2. multiple rolls of paper towels no longer in the box, with no plastic covering them; and 3. one pizza cutter sitting on a prep table, in the pizza prep area.
- Observed the non-food contact surfaces equipment throughout the facility, to include the gaskets on the coolers and freezers, the pizza oven conveyor rack, and drink machine up splash soiled.
- Observed the following in need of repair: 1. missing and broken floor tiles throughout the facility; 2. missing coving throughout the facility; 3. replace gasket of two door reach-in cooler; 4. duct from pizza oven hood vent to wall is extremely bent.
- Observed the floors, walls, and ceilings throughout the facility soiled.
- Observed the following not smooth and easily cleanable: 1. plywood under window beside the 3-compartment sink; 2. exposed studs on the end of the separating wall between the ware washing area and hot holding area; 3. plywood on top of wire shelving above reach-in prep cooler on left; 4. porous acoustic tiles around the vent hood for the pizza oven; 5. spackle on the ceiling above the handwashing sink in the warewashing area; 6. rusty shelving near three compartment sink and pizza oven.
- Observed no light shields for the fluorescent light fixtures in the kitchen. Discussed either getting replacements or LED bulbs with the person in charge.
- Observed employee articles in multiple locations around the facility. Discussed having a designated area for personal belongings and a separate shelf for any employee refrigerated items.

- Date inspected: Oct. 13
- Total violations: 5
- Observed the top interior of the ice machine soiled.
- Observed several containers of salad held at improper temperatures (42-48 degrees F) in the pull out drawer on the hamburger line at around 2:15pm. The salads were placed in the unit at 8am. The salads were voluntarily discarded and the temperature of the unit was lowered. In addition, observed several containers of salad in the pull out drawer on the cookline held at 41-43 degrees F. Items were placed in unit at 8am. Items were relocated to the walk in cooler and the temperature of the unit was lowered.
- Observed several food employees without hair restraints.
- Observed several plastic containers on a drying rack above the 3CS wet while stacked after being sanitized. Items were removed to the 3CS.
- Observed the exterior of equipment throughout the kitchen in need of cleaning.
- Patriot Subs, 1131 Emancipation Hwy Fredericksburg, VA 22401
- Date inspected: Oct. 16
- Total violations: 5
- Observed the lid of a pot sitting inside the basin of the kitchen hand sink. Lid was moved to the dishwashing area.
- Observed the interior of the microwaves require cleaning.
- Observed stacks of clean food containers that were stored stacked while still wet.
- Observed the cutting boards of the prep stations are heavily scratched and stained.
- Observed ice build up in all reach in freezers of the facility.
- Jersey Mike's Subs, 1495 Stafford Market Place Suite #119 Stafford, VA 22556
- Date inspected: Oct. 16
- Total violations: 1
- Observed the ice chute and dispensing tubes in the soda machine in the lobby require cleaning. PIC contacted the company that supplies their soda to come in and service the machine.
- El Patron Mexican Restaurant, 9112 Old Battlefield Blvd 112 Spotsylvania Courthouse, VA 22553
- Date inspected: Oct. 16
- Total violations: 2
- Observed the following TCS foods cold holding at improper temperatures in the Pepsi reach-in cooler over night: 1.Salsa 43-46 degrees 2. Strawberry cream cheese 45 degrees 3. Caned whipped cream 45 degrees Person in charge voluntary discarded all TCS foods.
- Facility could not provide a test kit for sink & surface sanitizer and chlorine for the dishwasher.
- Anthony Burns Elementary School, 60 Gallery Road Stafford, VA 22554
- Date inspected: Oct.16
- Total violations: 1
- Observed the corners of several plastic container lids cracked and chipped. The lids were voluntarily discarded during the inspection.
- Taco Bell #3921, 255 Garrisonville Road Stafford, VA 22554
- Date inspected: Oct. 16
- Total violations: 4
- The chemical sanitizer in the 3 compartment sink and wiping cloth bucket read 0ppm. The dispensing pipe was not properly connected to the chemical sanitizer bag. After the person in charge connected the piping correctly, the concentration of the sanitizer in the 3 compartment sink read 200 ppm. All wiping cloth buckets throughout the kitchen were replaced.
- Observed several plastic containers on shelves in ware wash area wet while stacked after being clean and sanitized. Items were removed for sanitization.
- Observed the following in need of cleaning: 1) exterior of equipment throughout the kitchen. 2) shelving racks of the hot holding box units. 3) upsplash of soda machine in drive thru area.
- The following are in need of cleaning: 1) walls behind equipment. 2) ceiling tiles. 3) ceiling vents.
- Domino's Pizza, 840 Warrenton Road Fredericksburg, VA 22406
- Date inspected: Oct. 17
- Total violations: 4
- Facility could not provide written clean up procedures for bodily fluids. Written procedures were given during inspection.
- Observed no paper towels at the hand washing sink. PIC placed paper towels at the sink.
- Observed the following in poor repair: 1) water dripping from a pipe in the walk in cooler (located in front of pizza oven). 2) rusted floors in the walk in cooler. 3) broken door of the reach in cooler.
- Observed the caulking of the hand wash sink and 3 compartment sink in need of cleaning.

- Date inspected: Oct. 17
- Total violations: 2
- Observed several flying insects near the back door of the kitchen.
- Observed food employee use a single use paper towel to dry several stainless steel containers.

- The wall behind the dish machine and the HVAC vents above the service line were observed soiled.
