Fredericksburg Health Inspections: Ricco's Pizza, Waffle House

The Rappahannock Area Health District recorded violations at facilities around Fredericksburg and Stafford during recent inspections.

FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities during recent inspections from Aug. 17 to Aug. 23.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Central Station, 1917 Princess Anne Street Fredericksburg, VA 22401
    • Date inspected: Aug. 17
    • Total violations: 7
      • There were no paper towels located at the hand sink in the front or rear hand sinks in the kitchen. Paper towels were provided during inspection.
      • The sign that notifies food employees to wash their hands is not clearly visible in either male or female restrooms.
      • The interior of the microwave oven and the deep fryer baskets were observed soiled. Oven and baskets were cleaned during inspection.
      • There are floor mounted homestyle reach in coolers and freezers in the kitchen that are not elevated off the floor or sealed to the floor. A cleaning hazard exists.
      • Observed the following: 1- A heavy buildup of frost inside one of the reach in freezers, 2-The lower door hinges of the prep unit are broken.
      • The floors in the kitchen, especially under the equipment is in need of cleaning.
      • The following physical items in the kitchen were noted in need of repair: 1-observed several missing/broken floor tiles, 2-observed several damaged ceiling tiles,

      • Observed that the floor under the fryers and the wall behind the fryers is soiled.
  • University of Mary Washington - University Center (Catering Kitchen 1st Floor), 1301 College Ave. Fredericksburg, VA 22401

    • Total violations: 8
      • Observed two dented 6 lb. cans of kidney beans on the can racks. During the inspection, the food employee discarded cans. (Corrected)
      • Observed a plastic ice scoop and paddle sitting on the top of the ice machine. During the inspection, the PIC relocated the scoop and paddle to the dishwashing area. (Corrected)
      • The sanitizer at the 3 compartment sink in the dishwashing room is not being pulled through the tubing. During the inspection, the food employee went to the 4th floor for sanitizer. The 1st floor will use the 4th floor sanitizer until the sanitizer dispensing system can be repaired. (Corrected)
      • Observed a rack with boxes of plastic and paper plates, knives, plate covers, and cups that were not protected from contamination. The rack was located near the exit door to the dumpster. During the inspection, the PIC had a food employee discard all exposed articles. (Corrected)
      • Observed 3 damaged rubber spatulas hanging from hooks over the prep tables. During the inspection, the PIC discarded spatulas. (Corrected)
      • The following are in need of repair: 1. Garbage disposal is not working. 2. Ice build-up in the large walk-in freezer.
      • The following are in need of cleaning: 1. Water collecting on the floor under the dishwasher drainboards. 2. A few soiled ceiling tiles in the dishwashing room. 3. The caulking along the dishwasher drainboards was observed soiled. 4. The caulking at the large prep sink was observed soiled.
      • The following are in need of repair: 1. One missing ceiling tile in the dry storage room. 2. One water damaged ceiling tile in the cold prep area.
  • Subway (Lee's Hill), 10635 Spotsylvania Avenue Fredericksburg, VA 22408
    • Date inspected: Aug. 18
    • Total violations: 3
      • Observed in use Quaternary Ammonium sanitizing solution at 500 ppm and 94 degrees F (solution was tested at 78 degrees F). The solution was not actively being used to sanitized equipment during the inspection. Worked with the person in charge to dilute the sanitizing solution to 400 ppm. The person in charge reported a technician was not contacted, the employees changed out a part on the dispenser. Instructed the person in charge to contact a technician for service and manually titrate and dilute the sanitizing solutions as needed to achieve a solution concentration of 200-400 ppm, using a testing kit to monitor the solutions.
      • Observed the following TCS foods cold holding at improper temperatures inside prep cooler #1: Sliced deli turkey (44 degrees F), precooked sliced chicken (47 degrees F), sliced deli roast beef (47 degrees F), precooked chicken breast (50 degrees F), sliced ham (53 degrees F), and shredded cheese (47 degrees F). The person in charge reported the sliced turkey and ham were prepped and pulled from the walk in cooler at 11: 00 a.m. The precooked chicken, roast beef, and shredded cheese were pulled from the walk in cooler at 8:30 a.m. Temperatures were observed at 12:37 p.m. The person in charge voluntarily discarded the TCS foods in the unit which were pulled at 8:30 a.m. Instructed the person in charge to sell, discard or cool the sliced turkey and ham to 41 degrees F or less by 3:00 p.m. Contact technician for service/repair. Discussed placing small portions, using ice baths and monitor internal temperatures of TCS foods every 3 hours.
      • The person in charge could not verify documentation of employee health training. The owner reported employees were verbally trained on the employee health policy but documentation was not available for verification.
  • Burger King #1222, 5430 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Aug. 18
    • Total violations: 6
      • Facility did not have written procedures for the clean-up of vomiting or diarheal events. Facility provided with clean-up procedures document.
      • Observed butter stored out of refrigeration at 75F for an unknown amount of time (Person in Charge estimated 3-4 hours). Food was not being marked with discard time. Through discussion with the Person in Charge, they stated the food is moved to refrigeration after sitting out of temperature. Person in Charge was informed that temperature control for safety food shall be marked with the discard time and should be discarded after being held out of temperature for 4 hours.
      • The following nonfood-contact surfaces were observed to be soiled and are in need of cleaning: Split freezer in prep area, walk-in freezer floor, upsplash area of soda dispenser, upsplash area of coffee maker, single-use cup & lid storage areas by drive-thru prep area.
      • Observed an accumulation of static dust on ceiling area of prep area.
      • Cove base tiling in back dry storage area of food facility was observed to be damaged and is in need of repair.
      • Observed an accumulation of grease and residue on hood ventilation system. Observed an accumulation of static dust on ceiling vent above prep area of food facility.
  • Jerks of the Caribbean, 3631 Plank Road Fredericksburg, VA 22407
    • Date inspected: Aug. 18
    • Total violations: 6
      • Facility was unable to provide written procedures for the clean-up of a vomiting or diarheal event. Facility provided with procedure document.
      • Observed sanitizer spray bottle with 0ppm of chlorine solution. Chlorine was added to spray bottle and read at 100ppm.
      • Observed in-use utensils stored in container of water holding at 83F. Observed in-use spatula stored under tray on counter, not an easily cleanable surface.
      • Observed two rice scoops stored on shelving in warewash area that were chipping and are no longer in good repair.
      • Observed mop sink with hose attachment without a backflow prevention device. Person in Charge detached hose until able to provide mop sink with proper backflow prevention device.
      • Observed aprons stored on shelving next to food items (spice containers) and food equipment.
  • University of Mary Washington - University Center (4th Floor Main Kitchen), 1301 College Ave. Fredericksburg, VA 22401
    • Date inspected: Aug. 18
    • Total violations: 3
      • Observed a food employee apply soap and scrub under running water and another wash their hands in less than 10 seconds. During the inspection, the food employees rewashed hands before returning to work. (Corrected)
      • The following were at improper temperatures: 1. One container of raw chicken breasts (45F) in the reach in cooler at the Serrano Mesa Mexicana station. The food employee stated the chicken had been left in the reach in cooler overnight. 2. One container of pico (44F - 45F) in the walk-in cooler that was prepared 08/17/2022. During the inspection, the PIC had the food employees discarded the food. (Corrected)
      • The following are in need of repair: 1. The door handles on the reach-in freezer in the hallway. 2. The Chef Fare reach in cooler has ice build-up on the bottom shelf.
  • Jepson Alumni Executive Center, 1119 Hanover Street Fredericksburg, VA 22401
    • Date inspected: Aug. 18
    • Total violations: 3
      • No hand soap in employee restroom. During the inspection, the PIC provided hand soap. (Corrected)
      • The ceiling tile throughout the kitchen is porous.
      • Observed 8 water damaged ceiling tile in front of the tea dispenser and dish caddies.
  • Ricco's Pizza, 50 North Stafford Complex Center Suite #104 Stafford, VA 22556
    • Date inspected: Aug. 18
    • Total violations: 23
      • The person in charge could not explain how to properly cool foods like pasta and lasagne and cold holding and cooling temperatures.
      • The facility does not have written procedures for cleaning of bodily fluid accidents.
      • Observed employee drinks stored on the prep tables and inside of the pizza prep unit. The person in charge discarded all of the drinks.
      • Observed the employee spray his hands in the 3-compartment sink. EHS discussed this with the employee.
      • Observed the employee wipe his hands several times on his dirty apron after washing his hands. The employee then continued to prepare foods. EHS discussed this with the employee.
      • Observed the employee wash his hands then turn the hand sink off with his washed hands. EHS demonstrated the proper way to wash hands.
      • Observed several cartons of eggs stored over an open container of lettuce in the walk in cooler. The person in charge relocated the eggs.
      • Observed uncovered foods in the reach in freezer.
      • Observed that the frying pans and the dispensing utensils require cleaning. The person in charge placed the items in the dish machine area.
      • Observed that the interior and door to the microwave require cleaning.
      • Observed that the pizza cutters (corected), slicer, fryer baskets, soda nozzles (corrected), and interior of the ice machine required cleaning.
      • Observed cooked sausage at 44'F in pizza prep unit and ground beef at 49'F in the pizza prep unit. Both items were placed in the walk in cooler.
      • Observed alfredo sauce, in-house made sauces, baked lasagne, and sliced meats that were not dated for disposition.
      • Observed several chemical spray bottles that were not labeled. The person in charge labeled the bottles.
      • Observed that one employee was not wearing a hair restraint (corrected) and two employees are not wearing beard restraints.
      • Observed that the ceramic saucers are stored with the food-contact surfaces facing upwards. The person in charge inverted the saucers.
      • Observed plastic and stainless steel containers stored wet.
      • Observed the shipping plastic on the entrance door to the kitchen.
      • Observed that the doors to the dish machine require cleaning.
      • Observed that the cabinet under the fryers, sprayer nozzle, outside of the dish machine, and in between the floor-mounted equipment require cleaning.
      • Observed that the dumpster has a lot of debris around it.
      • Observed that the wall by the pizza prep assembly area and the rear prep area is in disrepair.
      • Observed that the floors, walls, and ceiling in the kitchen require cleaning to include the air vent covers in the restrooms. Observed cob webs around the rear entrance door.

      • Observed the facility operating with expired food manager certificate.
      • Observed the following non-food contact surfaces soiled: Inside baking ovens Inside fryers Outside ice machine Outside dishwasher.
      • Observed the floors underneath the fryers and flat top are soiled.

      • The prepared ready-to-eat (RTE) sausages in the refrigeration unit is not labeled with a ""consume by"" date. Sausages were date marked during inspection.

      • Walk-in freezer door does not close properly due to ice build-up. Observed food containers throughout facility that were broken and cracked.

      • The person in charge (PIC) was operating without a working handwashing sink and 3 basin dishwashing station.
      • No water at handwashing sink. PIC stated pump was not working. During the inspection, the next door vendor allowed them to use their handwashing station.
      • Observed clean whole lemons and limes being stored in the original box. After discussion with PIC, it was determined the lemons and limes are washed in the kitchen and then placed back into the original box.

      • The Person in Charge was unable to provide documentation of training relating to the Employee Health Policy for this facility Facility provided with Employee Health Policy that was reviewed and signed by all available employees.
      • Observed scoops stored in food in walk-in refrigerator that do not have handles to prevent hand contact with food items.
      • Observed sanitizer bucket stored in back handwashing sink. Sanitizer bucket removed from handwashing sink.
      • Observed food employees putting soiled food equipment into sanitizer solution without a prior wash and rinse. Person in Charge was informed and equipment was moved to wash basin.
      • Facility does not have a consumer advisory for undercooked food (ceviche) being served at the facility. Person in charge shall rewrite the menu with the consumer advisory included.
      • Observed several wet wiping cloths stored out of sanitizer on back prep table.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Observed cutting boards that are deeply scored and discolored and are in need of resurfacing or replacement.
      • Observed knives stored in back prep area that are chipped and are no longer in good repair.
      • Fan guards in walk-in refrigerator observed with an accumulation of static dust and are in need of cleaning.
      • Lighting in front cook area observed at 25 foot candles, rather than 50 foot candles as required.
  • Danny's Pizza & Sub, 9837 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Aug. 22
    • Total violations: 5
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Observed raw shell eggs stored over bell peppers in walk-in refrigerator. Person in Charge relocated food to prevent contamination.
      • Spray bottle of chlorine solution used on food contact surfaces observed at 0ppm. New chlorine solution was made.
      • Facility unable to provide chlorine test strips to test sanitizer solution. Provided test strips for Person in Charge and informed them to order more.
      • Mop sink observed without a proper backflow prevention device. Hose was removed until backflow device is installed.
  • Super Rico, 2386 Plank Road Fredericksburg, VA 22401
    • Date inspected: Aug. 22
    • Total violations: 13
      • Observed the men and womens bathroom are mising the handwashing signs.
      • Person in charge was unable to provide a parasite destruction for the raw fish (ceviche) offered in the facility on the weekends.
      • Through discussion with the person in charge the raw chicken prep table currently only follows the sanitize step after preparation and does not go under the wash and rinse step. EHS had a thorough discussion with the person in charge and food employees on following the wash rinse and sanitize step for the raw chicken prep table.
      • Observed one pan of cooked spinach at 48-52 degrees F. In discussion with the person in charge spinach was cooked the day prior and cooling methods were not followed properly for this item. During the inspection the pan of spinach was discarded.
      • Observed the following cold holding at improper temperatures due to a beverage cooler they were stored in, not in good working order. 1. 2 slices of tres leches cake (46 degrees F) 2. 16 slices of flan (46 degrees F) Foods were discarded during the inspection.
      • Observed the chicken hot holding unit is not in working order, facility is currently using a insulated cooler to hold grilled chickens.EHS advised using time as public health control until the hot holding unit gets fixed. EHS provided time as public health control paperwork. Paperwork was signed by operator and EHS.
      • Observed the facility does not have a consumer advisory statement for the raw fish (ceviche) on their menu.
      • Observed one squirt bottle of water and one spray bottle of cleaning chemicals without a name label. During the inspection the person in charge instructed employees to label the squirt and spray bottle.
      • Observed several food employees wearing wrist jewelry. During the inspection food employees removed their jewelry.
      • Observed the two kitchen handsinks and the front service area handsink need caulking, they are no longer adjoined to the wall.
      • Observed the following: 1. a beverage cooler where desserts also get stored in not cold holding adequately. Unit measured 46 degrees F. EHS advised PIC to not store desserts in unit until a technician comes to repair unit. 2. the chicken hot holding unit is not in working order and not in use at this time.
      • Observed the following non food contact surfaces that require cleaning: 1. Inside and outside of prep coolers. 2. Outside surfaces of the rice cookers and rice warmers 3. Metal and plastic racks holding equipment and boxes throughout the back kitchen area.
      • Observed the following: 1. the floors behind and underneath equipment require cleaning 2. the ceiling ac/fan vents need dusting 3. walls behind food prep areas need cleaning
  • Robiolina Restaurant, 356 Garrisonville Road, Suite # 101 Stafford, VA 22554
    • Date inspected: Aug. 22
    • Total violations: 3
      • Facility does not have a certified food protection manager.
      • Observed that the facility is not maintaining their shell stock tags for 90 days.
      • Observed that there is no date marking system in the facility.
  • Waffle House #930, 10357 Patriot Hwy Fredericksburg, VA 22408
    • Date inspected: Aug. 22
    • Total violations: 9
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS initially provided bodily fluids clean up on August 1st, 2022. EHS provided another handout.
      • Observed a cigarette on the water heater in the back of the house. PIC discarded the cigarette.
      • Employee was observed handling soiled dishes then proceeded to engage in food preparation without first washing their hands.. Employee was asked to wash their hands.
      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods.
      • Observed a container of raw shell eggs (55'F surface). PIC stated the eggs were taken out of the WIC one hour prior to temperature check. Eggs were stored above the stove that were dated 8/17, today's date is 8/22. PIC stated the facility uses time as a public health control on daily basis, no written procedures were available. Eggs were labeled during the inspection.
      • Observed several crawling and flying insects in the kitchen. The person in charge was not able to provide pest control treatment invoices.
      • Observed the ceiling above exposed foods and drinks to be soiled.
      • PIC could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher. EHS had to run dish machine 5 times prior to obtaining a sufficient temperature. All items cleaned prior to reaching the proper temperature had to be re-washed.
      • EHS was not able to locate a back flow prevention device- PIC will check with the company and notify EHS. Observed the pre-wash spray hose to extend below the flood rim level of the sink basin at the pre wash station near the dishmachine in the front of the house.
  • Great American Buffet, 1780 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Aug. 22
    • Total violations: 1
      • Observed the high temperature dish machine is not reaching the required temperature of 160 degrees F for sufficient sanitizing. VDH thermometer recorded a temperature of 140-141 degrees F after several attempts. Person in charge instructed employees to manually sanitize dishes in the 3 compartment sink.
  • Paisano's Pizza, 1515 Stafford Market Place Stafford, VA 22556
    • Date inspected: Aug. 22
    • Total violations: 10
      • Employee was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy to be read and signed by employees.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS provided bodily fluids clean up sample.
      • Observed food employee mop the floor with gloved hands then changed gloves and handled ready to eat food without first washing their hands. Employee was asked to wash their hands and food was discarded.
      • Ensure that employees are washing their hands for at least 20 seconds and completely drying them before putting on new gloves.
      • Observed an employee hold a ready to eat sandwich with their bare hands then placed the sandwich in a to go container. Sandwich was discarded.
      • Per discussion with the person in charge the food contact surfaces for equipment and utensils time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the VA Food Regulations. The items sometimes get washed only once a day. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods. Items were moved to be washed, rinsed, and sanitized.
      • Observed the following TCS foods cold holding at improper temperature for approximately three hours: 2 bowl of fresh garlic mixed with oil, a container of pizza sauce (63'F), several pre-portioned marinara sauce (75'F) stored on the shelf in the to-go area. EHS moved the items to the freezer to quick chill them. Facility will use time as a public health control.
      • Facility is using time as public health control (TPHC) for 1) the whole pizza that is sold as individual slices, 2) the pizza sauce, and 3) the marinara sauce ( both sauces are made from tomato sauce that require ambient cooling) without prior approval from the health department. PIC was unable to provide written procedures. Foods were not labeled. PIC stated foods have been stored at room temperature for approximately two and a half hours- EHS instructed the EHS provided the required document and discussed proper labeling.
      • Observed uncovered disposable spoons stored in a container in the front of the house with the lip contact surface facing upward.
      • A backflow prevention device was not provided on the water hose at the mop sink.
  • Waffle House #2303, 45 Civilian Street Fredericksburg, VA 22406
    • Date inspected: Aug. 23
    • Total violations: 13
      • Person in charge (PIC) could not provide written procedures for cleaning up vomiting or diarrheal events. EHS emailed new regulations and clean up handouts to the PIC on January 14, 2022. EHS provided another clean up sample.
      • Observed food employee crack raw shell eggs with gloved hands then donned new gloves and proceeded to engage in food perpetration without first washing their hands. Employee was asked to wash their hands.
      • Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with PIC.
      • Observed several opened large iced tea filter bags stored directly on the shelving unit in the dry storage and on a container above pre-packaged jam in the front of the house. Items were relocated.
      • Observed the following in the walk in cooler (WIC) 1) a box of lettuce stored directly over raw shell eggs, 2) raw meat stored near butter and over ready to eat spread. PIC relocated the items.
      • The food establishment uses a hot water sanitizing dishmachine that require a utensil/equipment surface to reach a temperature of at least 160°F as measured by an irreversible registering temperature indicator. After running the dishmachine five times, the highest temperature reached was 158.2'F using a VDH irreversible registering temperature indicator. EHS instructed PIC to use the 3 compartment sink until the machine is evaluated by a technician.
      • Observed a bowl of sliced tomatoes (53'F), a bowl of diced tomatoes (57'F). a bowl of cut lettuce (57'F) stored over the stainless-steel containers in the prep unit by the cookline. Items were cut almost 4 hours prior to temperature check. Items were discarded.
      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods. PIC labeled the ready to eat TCS foods that have been stored in the cooling unit for over 24 hours.
      • Observed the following: 1) a bottle of glass cleaner stored next to syrup on the cart by the counter, 2) bleach near single service articles in the dry storage. The person in charge relocated the items.
      • Chlorine sanitizer solution in wiping cloth bucket, to later be used on food contact surfaces, was measured at greater than 200 parts per million chlorine. Solution was diluted to approximately 100 parts per million.
      • The exterior door which is located in the kitchen is not self-closing, which may allow the entry of insects or rodents.
      • PIC could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher. EHS ran the dishmachine five times and still did not reach the minimum required temperatrue allowed (160'F) for high temperature dishmachines. The maximum temperature reached was 158.1'F. PIC will use 3 compartment sink unlit the dishmachine is repaired. Technician was on-site repairing an equipment and will evaluate the dishmachine.
      • No back flow prevention device was observed on all of the equipment connected to the main water line.

      • Facility does not have written procedures for the clean-up of vomiting or diarheal events. Facility was provided with procedure document.
      • Facility has a consumer advisory on the menu stating that shrimp may be undercooked per customer request. The person in charge could not provide parasite destruction for shrimp. The person in charge was instructed to discontinue offering shrimp undercooked unless parasite destruction can be ascertained. The person in charge agreed to comply.
      • Observed chicken stored on stove hot holding at 130F, rather than 135F or higher as required. Chicken was reheated to 190F.
      • Observed spray bottle of chemicals stored on shelf with food equipment and food. Spray bottle was relocated to prevent contamination.
      • Observed personal medication stored on shelf next to and above food items in kitchen area of food facility. Items were relocated to prevent contamination.
      • Observed several large containers throughout kitchen area that were not labeled with the common names of the food items.
      • Observed bags of maseca stored on the floor in the dry storage area of the food facility.
      • Observed cardboard-like material used in place of ceiling tiles in kitchen area of food facility, not a smooth and easily cleanable material.
  • Moe's Southwest Grill, 1495 Stafford Market Place Suite# 111 Stafford, VA 22556
    • Date inspected: Aug. 23
    • Total violations: 2
      • Observed a container of cut chicken (123-136'F) and a container of whole chicken (127-137'F) stored in the hot holding unit for less than two hours. The hot holding unit thermostat dial was accidently turned down. Chicken was reheated to 165'F.
      • Observed the prep sink and the 3 compartment sink not sealed to the wall.
  • Brother's Pizza, 3605 Plank Road Fredericksburg, VA 22407

    • Total violations: 6
      • Facility was unable to provide written procedures for the clean-up of a vomiting or diarheal event. Facility was provided with procedures.
      • Observed wiping cloth at handwashing sink to be used to dry hands, this is not an approved hand drying provision. Person in Charge provided paper towels at handwashing sink.
      • Observed dough trays with cracks and are not longer smooth and easily cleanable. Person in Charge removed trays to be used for food items.
      • Observed cardboard lining bottom of small 3-door refrigerator and 2-door refrigerator, not a smooth and easily cleanable surface.
      • Wire shelving throughout facility observed with an accumulation of static dust and are in need of cleaning.
      • Ceiling above back prep area observed with an accumulation of static dust and is in need of cleaning.
