Fredericksburg Health Inspections: Riverside Center, Panaderia Emanuel

The Rappahannock Area Health District recorded violations at several restaurants around Fredericksburg during recent inspections.

FREDERICKSBURG, VA — Inspectors recorded violations at several Fredericksburg restaurants during inspections from July 13 to July 19.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Starbucks Coffee #9677, 2001 Plank Road Fredericksburg, VA 22401
    • Date inspected: June 13
    • Total violations: 6
      • Person in charge was unable to provide a verifiable way to show employees had been trained on foodborne illnesses and symptoms associated with foodborne illnesses. EHS provided a copy that employees read signed.
      • Per discussion with the person in charge the steam wand of the espresso machines and the blenders do not currently undergo cleaning frequency of every 4 hours.
      • Observed the following: 1. The interior of the ice machine is heavily soiled with pink and black growth. 2. Food debris was observed around the oven/countertop area.
      • Observed ice at the back of the walk in freezer near the condenser.
      • Observed the following non food contact surfaces require cleaning: 1.The wire shelves above the 3 compartment sink holding clean equipment 2. The upsplash of the coffee brewers
      • Observed a water leak in the faucet of the mop sink.
  • Panda Express #974, 1680 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: July 13
    • Total violations: 10
      • Person in charge could not provide a signed copy of the employee health policy. EHS provided a copy, person in charge went over the policy with employees and had them sign before inspection was through.
      • Observed a food employee scrub soapy hands under running water. EHS spoke with food employee about proper handwashing technique. Food employee rewashed thier hands.
      • Observed the following cold holding at improper temperatures in a cold holding unit that was registering (53-56 degrees) 1. One bag of kale (49 degrees F) 2. Two containers of cut cabbage (51 degrees F) TCS items in the unit had been in the unit less than 1 hour and were relocated to the walk in cooler.
      • Observed a lighter being stored next to open food containers in a cold holding unit. Person in charge relocated the lighter.
      • Observed several metal containers stored stacked while still wet.
      • Observed the following: 1. One cold holding unit not holding required temperature (53-56 degrees F). 2. Ice build up in the back of the condenser of the walk in freezer.
      • Observed the metal wire racks in the kitchen are rusted.
      • Observed the caulking of the 3 compartment sink and the back kitchen handwashing sink need recaulking.
      • Observed metal wire racks next to 3 compartment sink that were soiled to the sight and touch.
      • Observed the lighting of the walk in cooler is not sufficient (4 foot candles).
  • Paragon Village 12, 51 Towne Centre Blvd Fredericksburg, VA 22407
    • Date inspected: July 13
    • Total violations: 6
      • Observed an electric powerwasher blocking access to the handwashing sink at the prep area. During the inspection, the PIC relocated the powerwasher. (Corrected)
      • Observed the interior of the ice machines soiled.
      • Observed 3 spray bottle containing DC33 on a shelf that was not labeled. During inspection, a food employee labeled spray bottles. (Corrected)
      • The following are in need of repair: 1. The handwashing sink next to the prep area is leaking. 2. The shelf in the prep unit has standing water.
      • The hood system over the fryers was observed soiled.
      • The floors in the concession stands and upstairs bar were observed heavily soiled.

    • Total violations: 2
      • Observed the following time/temperature control for safety TCS foods cold holding at improper temperatures in the walk-in-cooler (WIC): Rice (43'F), raw chicken (44'F), beef soup (42'F). PIC will lower the thermostat in the unit and will contact a technician to come evaluate the unit. Items are either being discarded, placed in the reach in freezer or using ice depending on the time it was placed in the unit until the technician arrives. EHS advised PIC to remove non TCS foods from the WIC and discussed purchasing an additional unit as the unit is overfilled.
      • Observed the walk-in-cooler in need of an evaluation by a technician (45'F frequent use- foods temperatures were above 41'F).
  • The Agency Kitchen & Bar, 51 Towne Centre Blvd Fredericksburg, VA 22407
    • Date inspected: July 13
    • Total violations: 12
      • No handwashing sign at handwashing sink in upstairs bar. During inspection, the PIC created and posted a sign. (Corrected)
      • The sanitizer at the 3 compartment sink is not being pulled through the tubing. During the inspection, the PIC replaced the bottle and primed the system. (Corrected)
      • Observed the interior of the ice machine soiled.
      • The following were cooled improperly: 1. One large plastic container of homemade ranch (46 F) in the reach in cooler that was prepared on 07/12/2022. 2. One large container of cooked chicken wings (54 F - 56 F) in the wing prep unit that were prepared on 07/11/2022. During the inspection, a food employee discarded the food. (Corrected)
      • Observed 1 spray bottle containing DC33 at the dishwashing counter that was not labeled. During inspection, a food employee labeled spray bottle. (Corrected)
      • Observed several flying insects throughout the kitchen and upstairs bar.
      • The following are in need or repair/replace: 1. The handwashing sink at entrance into kitchen does not have running cold water. 2. One of 2 faucets at the 3 compartment sink is broken. 3. The pizza oven and pizza dough prep unit are missing side panels. 4. Bottom shelf of pizza prep unit has standing water.
      • The 3 compartment sink in the upstairs bar is not sealed to the wall.
      • Observed no sanitizer test kit for the 3 compartment sink in upstairs bar. During the inspection, the PIC had a food employee deliver a test kit to the employee working in upstairs bar. (Corrected)
      • The soda gun holder in the upstairs bar was observed heavily soiled.
      • The floor tile in the dishwashing area need to be re-grouted.
      • Observed the ceiling and floors (heavily) soiled throughout the kitchen.

      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Observed food employee washing hands for less than 20 seconds. Discussed handwashing procedures with food employee while they rewashed hands.
      • Observed food employees without beard restraints while preparing food in kitchen.
      • Observed the following: walk-in freezer door damaged with ice build-up on the inside. walk-in cooler door damaged.
      • Observed that there are not any testing kits in the facility. Person in Charge ordered testing strips online from amazon.
      • Observed the following non-food contact surfaces soiled: Reach-in coolers, Reach-in freezer, Pizza Oven Top.
      • Observed that walk-in cooler has a hole in the wall and a leak coming from ceiling.
  • La Delizia Pizza, 10041 Patriot Hwy Fredericksburg, VA 22407

    • Total violations: 4
      • Observed food employee washing hands at 3-compartment sink. Food employee was informed and proceeded to wash hands at handwashing sink.
      • Observed garlic in oil sitting out of refrigeration, holding at 50F, for less than 4 hours. Person in Charge relocated food to refrigeration to cool.
      • Observed personal medication stored on prep table in back prep area of food facility. Person in Charge relocated medicine to prevent contamination.
      • Observed spray bottle with chemical cleaner without being labeled with common name of chemical. Food employee labeled spray bottle.
  • Hazelwild Day Care Center, 5325 HARRISON ROAD Fredericksburg, VA 22407
    • Date inspected: July 13
    • Total violations: 1
      • Observed chemicals (Chafing gel, sanitizer, disinfectant, & cleaner) stored on shelf with single-use lids and single-use plastic bags. Items were relocated to prevent contamination.
  • Fairfield Inn & Suites, 2784 Jefferson Davis Highway Stafford, VA 22556
    • Date inspected: July 13
    • Total violations: 12
      • The only breakfast attendant preparing food has not completed the employee health policy but was able to communicate symptoms and illnesses requiring reporting. The employee signed the employee health policy during the inspection and was advised of health reporting requirements by the person in charge.
      • Observed a food employee relocate their handbag and then proceed to handle clean linens. Instructed the employee to wash their hands.
      • Observed a food employee lather their hands less than 10 seconds under running water. Hands were not effectively cleaned. Instructed the food employee on correct handwashing and the employee washed their hands correctly.
      • Through discussion with the person in charge two pairs of scissors, which are used to open packages of TCS foods, are rinsed and are not being washed and sanitized. Instructed the person in charge to clean and sanitize scissors at least once every 4 hours when they come into contact with TCS foods.
      • Observed sausage gravy (112 degrees F) hot holding at improper temperatures. The person in charge reported the sausage has been in the hot holding unit since 8:00 a.m., temperature was taken at 9:10 a.m. The person in charge reported the sausage gravy would be discarded at 9:30 a.m. (end of breakfast service). Observed hot holding unit on a low heat setting.
      • Observed the following TCS foods cold holding at improper temperatures in the small reach in cooler in the breakfast area: Single use container of milk (42 degrees F), single use container of yogurt (45 degrees F) and single use container of cream cheese (44 degrees F). The person in charge reported the TCS foods have been in the unit overnight and voluntarily discarded the foods for safety. Instructed the person in charge to contact a technician for repair, place small quantities on ice and monitor temperatures during breakfast service (3 hours) until repairs have been completed.
      • Observed one can of flying insect insecticide in a milk crate under the handsink in the kitchen. The person in charge could not communicate where the insecticide is used and relocated the can to the office.
      • The person in charge provided a thin probed food thermometer which was bent and damaged. Instructed the person in charge to purchase a new thin probed food thermometer.
      • Observed containers used to store bagels and muffins and cereal decanters available for consumer self-service in the breakfast area are not labeled to identify ingredients and potential allergens.
      • Observed multiple boxes of single use articles (coffee, beverage containers and lids) stored on the floor in the kitchen and single use utensils stored with the food contact surface facing up on open shelving in the kitchen.
      • The person in charge provided Quaternary Ammonium test kit which expired in 2021. New test kits were ordered 7/11/22.
      • Observed coffee brewer connected the the water in absence of adequate backflow preventer. Observed a flow control valve connected to the water line. Provided backflow prevention handout during the inspection. Contact plumber to install adequate backflow prevention.
  • Tila's Cocina on Board (UUF-8211 VA), 7638 Courthouse Road Spotsylvania, VA 22551

      • Observed thin probed food thermometer and two three plastic food containers soiled. The equipment was relocated to the 3 compartment sink for cleaning.

    • Total violations: 23
      • The person in charge could not provide documentation of employee health training but was knowledgeable of symptoms and illnesses requiring reporting. Provided sample employee health policies in English and Spanish during the inspection to train staff.
      • Absence of handwashing signage at the server area handsink. Handwashing sign was provided to the operator.
      • Observed covered container of raw salmon stored on wire shelving over cooked beef inside the walk in cooler. The person in charge relocated the salmon during the inspection.
      • Observed counter mounted can opener soiled. The can opener was relocated to the dishwasher for cleaning. The person in charge reported a new can opener has been ordered. Observed considerable build-up on the can opener blade.
      • Observed two soiled fryer baskets stored on top of the charbroiler and observed debris on the food contact surface of the charbroiler. The fryer baskets were removed for cleaning.
      • Observed opened container of ice cream mix for beverages at the bar (43 degrees F) cold holding at improper temperatures. The person in charge could not verify how long the product has been in the cooler and voluntarily discarded for safety. Discussed lowering the temperature and contacting a technician for service. This was the only TCS beverage in the unit. Discontinue holding TCS food/beverages in the unit until cold holding capacity is evaluated.
      • Observed the following TCS foods held in refrigeration which should have been discarded: Opened package of deli turkey (package dated 6/27/22), Packaged goat cheese with a expiration date of 7/6/2022, two covered containers of cooked roast beef (preparation date 7/6/2022). The deli turkey should have been discarded on 7/3/2022 and the roast beef should have been discarded on 7/12/2022. During the inspection the person in charge voluntarily discarded the foods for safety.
      • Observed the following TCS foods in the walk in cooler which were not dated to identify the opening or preparation date: Opener container of whipping cream, single service container of tater tot casserole, single service cuts of cheesecake and container of beef tips. The person in charge reported the foods were prepared/opened the day prior. During the inspection the person in charge placed date labels on the the containers. Discussed date marking/disposition requirements. Also observed opened package of ice cream mixture which is used to prepare beverages which was not dated to identify the date of opening. The person in charge could not verify the date of opening. The container was discarded due to a cold holding violation.
      • Observed gap and light emitting from the bottom of the exterior double doors along the hall in which catering equipment is stored.
      • Observed several stacks of small plates and bowls stored on top of a prep table near the walk in cooler, with the food contact surface facing upward, without overhead protection. During the inspection a food employee placed plastic wrap over the plates. Also observed numerous stacks of plates stored on shelving by the cooking area, with the food contact surface facing upward, which are not fully covered by the shelving,. The plates were protruding beyond the shelving. Discussed pushing the plates back to ensure they are covered under the shelving or wrapping the top plate.
      • Observed stainless steel containers stored on the warewashing shelving stored wet after chemical sanitizing.
      • Observed the following equipment are not sealed to the adjacent walls or equipment: Prep area handsink, dishwasher drainboards, stainless steel plate attached to the faucet at the mop sink.
      • Observed the wire shelving in the dry storage and walk in cooler have become rusted and are no longer easily cleanable.
      • The person in charge could not provide a Quaternary Ammonium test kit. Purchase test kit to monitor Quaternary Ammonium sanitizer concentration.
      • Observed accumulation of debris on the following nonfood contact surfaces: Prewash spray nozzle, top of dishwasher, dry storage shelving, lids of bulk containers, range and interior of ovens, stainless steel backsplash at the range, interior of convection ovens, right hand side of the range, upsplash of stainless steel shelving in the dishwasher, build up of debris on the corner of sheet pans and prep area and upsplash of coffee brewer in the catering hallway. During the inspection a food employee cleaned the lids of bulk containers.
      • Observed long hose with a spray nozzle attached to a water line by the dishwasher area without backflow prevention. The spray nozzle was not on during the inspection. Provided backflow prevention handout to install a dual check valve with atmospheric vent. Instructed the person in charge to remove the spray nozzle.
      • Observed the self-closing mechanism for the employee toilet room door inside the kitchen is broken. The door is not self closing.
      • Observed considerable debris in the dumpster refuse enclosure.
      • Observed the recycling dumpster stored on grass. Discussed relocating the recycling container onto the asphalt parking lot.
      • Observed the following in poor repair: Cracked ceiling light cover by the dishwasher, damaged walls above and below the dishwasher drainboard, wall at the mop sink is caving in, observed numerous blown out ceiling lights throughout the food establishment, coving inside the produce walk in cooler has separated from the wall, exposed wall near the catering area ice machine.
      • Observed the following areas in need of cleaning: Walls at the dishwasher area, drain under the dishwasher, and ceiling tiles throughout.
      • Observed the wall in the dishwasher area located above and below the dishwasher drainboard in three areas, are damaged with unsealed cinderblock exposed. The surface is not smooth and easily cleanable.
      • Observed 2.8 foot candles of light inside the walk in freezer and 16.7 foot candles of light at the cook line. Observed burned out ceiling lights along the cooking area and dimmed shielded lights under the hoods.

      • Observed food employees not washing hands when switching tasks.
      • Observed food employees wearing wrist watches and bracelets in kitchen while preparing food.

      • Observed the following: Drawer cooler not working properly. (Corrected) Microwave door handles loose.
      • Observed the inside of fryer's soiled.
      • Observed that the ceiling tiles were soiled and not made of material that is smooth and easily cleanable.
      • Observed the following: Walk-in freezer and kitchen floors underneath fryers are soiled.

      • Facility did not have an available signed Employee Health Policy. Person in Charge was provided with document and reviewed and signed Employee Health Policy with available employees.
      • Back handwashing sink was not provided with paper towels. Food employee refilled paper towel dispenser.
      • Sanitizer solution at 3-compartment sink observed at 50ppm, rather than 200-400 as specified on manufacturer specifications. Person in Charge changed out sanitizer solution.
      • Observed food contact surfaces of food equipment stored on shelving in warewash area to be soiled. Equipment brought to 3-compartment sink to be washed and sanitized.
      • Observed food contact surfaces of food equipment stored on shelving in warewash area to be soiled. Equipment brought to 3-compartment sink to be washed and sanitized.
      • 1-door prep unit & left prep unit observed without temperature measuring devices.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Observed boxed single-use containers stored on floor in front dry storage area of food facility.
      • Observed the following non-food contact surfaces to be soiled and in need of cleaning: Walk-in freezer floor, floor drain by bag-in-box soda, shelf above stove, and fan guard in 2-door refrigerator.
      • The following physical facilities were observed to be soiled and are in need of cleaning: Floor by mop sink, floor under ice machine, ceiling above front prep area.
      • Ceiling vents throughout facility were observed with an accumulation of static dust and are in need of cleaning.
      • Observed personal items (aprons & jacket) stored on shelving next to food items (cocktail sauce & oil) in front dry storage area of food facility. Observed personal item (purse) stored on prep table in front prep area of food facility.
  • Subway, 2213 Plank Road Fredericksburg, VA 22401
    • Date inspected: July 14
    • Total violations: 5
      • Observed the back door of the facility is missing the self closing mechanism.
      • Observed several rolls of paper towels stored unprotected. EHS advised to the person in charge to keep paper towels in the box they were received in to avoid contamination.
      • Observed walls and floors around prep area and the three compartment sink area require cleaning.
      • Observed the ceiling tiles are porous and not smooth, easy to clean.
      • Observed the caulking of the 3 compartment sink and a lightbulb of the walk in freezer need replacing.
  • Chuck E Cheese, 1280 Central Park Blvd Fredericksburg, VA 22401

    • Total violations: 5
      • Observed the metal wire racks besides the prep sink are sticky and soiled.
      • Observed the chemical sanitizing machine was not dispensing sanitizer (0ppm) because of an almost empty sanitizer container. Person in charge changed out the sanitizer and 50ppm was observed.
      • Observed the interior top and back section of the ice machine is extremely soiled with pink and black overgrowth.
      • Per discussion with the person in charge the food contact surface equipment and utensils (scoops and ladles, pizza cutter, pizza cutting board) do not currently undergo the required 4 hour frequency of wash, rinse and sanitize.
      • Person in charge was unable to provide an employee health policy. EHS provided a copy. Person in charge discussed the policy with employees and it was signed before the inspection was over.
  • Subway # 37501, 9817 Jefferson Davis Highway Spotsylvania, VA 22407
    • Date inspected: July 15
    • Total violations: 4
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Facility was unable to provide a signed Employee Health Policy.
      • Facility was unable to provide written procedures for responding to vomiting or diarrheal events in the facility.
      • Observed chemicals stored above onions in back storage area of food facility.
  • Chipotle Mexican Grill #2455, 28 S Gateway Road Suite 109 Fredericksburg, VA 22405
    • Date inspected: July 15
    • Total violations: 7
      • Through discussion with the person in charge the prep tables and food thermometers which come into contact with TCS foods are being sanitized only, a wash step is not being conducted. Instructed the person in charge food contact equipment are required to be washed, rinsed and sanitized. The person in charge agreed to comply. Demonstrated wash/rinse/sanitization of food thermometers during the inspection.
      • Observed the following TCS foods hot holding at improper temperatures: Front hot holding well: Pinto beans (113 degrees F). The person in charge reported the beans were reheated at 11:02 a.m. and temperature was observed at 11:50 a.m. The beans were placed on the range and were observed reheated to 191 degrees F by 12:03 p.m. Initial reheating temperature was verified by review of the temperature log. Online orders hot well: Pinto beans (119 degrees F) and black beans (113 degrees F). The person in charge reported the foods were reheated and placed in the unit at 11:15 a.m., temperatures were observed at 11:29 a.m. Initial reheating for hot holding temperatures were verified through review of the temperature log. The beans were removed to the range for reheating. Observed black beans reheated to 173 degrees F and pinto beans to 191 degrees F by12:09 p.m.
      • Observed the following TCS foods cold holding at improper temperatures inside the online orders cold holding well: Red salsa (53 degrees F), sour cream (50 degrees F) and cut lettuce (48 degrees F). The person in charge reported the TCS foods were pulled from other refrigeration units and placed in the unit at 9:30 a.m. The facility will use time as a public health control until the unit is serviced. During the inspection the person in charge set a timer for `1:30 p.m. in which the TCS foods will be discarded. Discussed placing a thermometer inside the cold holding well.
      • Observed two containers of various utensils at the cook line which were wet after cleaning and sanitizing. Instructed the person in charge to dry the utensils with a single use disposable paper towel prior to contact with food. Discussed air drying of equipment and utensils.
      • Observed accumulation of debris on the following nonfood contact surfaces: Handle of turbo chef, handles of the 2 door upright cooler, stainless steel shelf behind the flattop grill and range, wheels under movable equipment at the cook line, inside fryer doors, and stainless steel backsplash at the range.
      • Observed accumulation of oily and encrusted residue on top of the oil receptacle in the refuse area.
      • Observed 4-6 hood vents above the flattop grill were removed for cleaning and the food establishment was using the flattop grill to cook and reheat various foods. Observed the hood vents on the floor by the 3 compartment sink for cleaning. Instructed the person in charge hoods vents are required to be installed to prevent build up of smoke, noxious odors and and condensation. The hood vents were replaced during the inspection.
  • Days Inn by Wyndham, 24320 Rogers Clark Blvd Ruther Glen, VA 22546

    • Total violations: 2
      • Observed wet wiping cloth stored on the counter near the hand sink. Towel was placed in sanitizer during the inspection.
      • The following non-food contact surfaces were observed soiled: 1-the interior of the black homestyle reach in cooler/freezer (dirt and what appears to be mold), 2-the top of all reach in coolers/freezers, 3-the outer surfaces of the cereal dispensers.

    • Total violations: 4
      • Observed the following dented cans (6 lbs) in the dry storage area: one diced pear, one pineapple, one pizza sauce, two spaghetti sauce, and two mandarin oranges. During the inspection, the person in charge marked the cans and will be returning dented cans to food services. (Corrected)
      • Observed 8 stainless steel bowls of green beans (temperature range: 87 F - 110 F) in a hot holding cabinet (set to 142 F) that were reheated for hot holding 45 minutes prior to the inspection. EHS requested the person in charge properly reheat the green beans for hot holding. During the inspection, the green beans were reheated to 180 F. (Corrected)

      • No temperature measuring device for high temp dishwasher.

      • Person in charge was unable to provide a verifiable way that showed employees have been trained on foodborne illnesses and symptoms. EHS provided a copy, person in charge discussed with employees and policy was signed.

      • Observed food employee apply soap and scrub under running water. During the inspection, the PIC had the food employee rewash using the proper handwashing procedure. (Corrected)
      • Observed food employee wash their hands at the prep sink. During the inspection, the PIC had the food employee rewash at the handwashing sink. (Corrected)
      • No hand soap at hibachi handwashing sink. During the inspection, the PIC provided hand soap. (Corrected)
      • Observed soap at the sushi prep sink and tongs at the hibachi handwashing sink. During the inspection, a food employee removed the soap and the PIC removed the tongs. (Corrected)
      • Observed the following TCS foods were improperly cooled: 1. Sixteen rolls of tempura (43F - 46F) and ten chicken breast (45F - 48F) that were prepared the night before and being stored in the walk-in cooler. 2. One large container of fried rice (53F - 58F) that was prepared the night before and being stored in the sliding glass door cooler at the hibachi station

      • The sushi rice, fried chicken, fried imitation crab, and shrimp tempura for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.       • Observed no thin probe thermometer to check TCS food temperatures. During the inspection, the PIC sent an employee to purchase a thin probe thermometer. (Corrected)
      • The 4 spatulas used at the hibachi grill are being stored in a metal container of water (83F). During the inspection, EHS informed the PIC of the proper storage of in-use utensils. The PIC removed the container and spatulas to the dishwashing area. (Corrected)
      • Observed several containers of seaweed salad being reused to store julienned carrots and cucumber in the walk-in cooler and sushi stations.
      • Observed the following: 1. Gaps between the hood filters over the wok/fryer cook line. 2. Four lights out over the wok/fryer cool line.
      • There is no test kit for monitoring the concentration of the chemical sanitizing solution at the dishwasher and sanitizing buckets.
      • The exterior and interior of all the reach in coolers were observed soiled.
      • Observed personal items throughout the facility (drinks, phone, keys, food).

      • Observed a spray bottle of cleaning chemicals stored above boxes of bread. During the inspection person in charge relocated the bottle.

      • Observed food employees working without a hair restraint. Person in charge provided hair restraints during the inspection.
      • Observed clean containers and trays stored stacked while still wet.
      • Observed the following non food contact surfaces that require cleaning: 1. the outside surfaces of the white bins holding flour and spices 2. drawer holding equipment.
      • Observed litter and debris around the dumpster area.
      • Observed the following: 1.walls and floors throughout the facility, especially floors of the walk in freezer and cooler, require cleaning. 2. food debris was observed on the floor of the mop sink.

    • Date inspected: July 19
    • Total violations: 11
      • Observed the front handsink at 81 degrees F and very low flow and the left side toilet room handsink at 78 degrees F. Contact plumber to restore hot water and adjust the water pressure at the front handsink.
      • Through discussion with the person in charge, the steaming wands, which are in contact with TCS beverages (dairy and plant based milk) are removed for cleaning and sanitizing once daily. Instructed the person in charge steaming wands are required to be cleaned and sanitized once every four hours due to contact with TCS beverages. The person in charge agreed to comply.
      • Observed milk cold holding at improper temperatures inside the 2 door upright cooler ( 44, 45, 46, 43, 44 degrees F). Observed the cooler is being overstocked with milk. Milk was observed protruding over the shelf near the door. The person in charge reported the milk was received prior to opening (2:30 - 3:30 a.m.) and could not verify the receiving temperature. The person in charge voluntarily discarded the milk during the inspection. Discussed preventing overstocking, creating separation between milk containers to allow adequate air flow and discontinue storage of milk over the edge near the door which is the warmest part of the refrigeration equipment.
      • Observed Lubriderm hand lotion stored at the front area handsink. During the inspection the person in charge relocated the hand lotion to the office area to prevent contamination.
      • Observed absence of ambient air thermometer inside warming refrigerators #1 and #2. Instructed the person in charge to install a thermometer inside the refrigeration equipment.
      • Observed numerous wiping cloths held in containers without any measurable Quaternary Ammonium sanitizing solution when tested with a testing kit. During the inspection the person in charge discarded the sanitizing solutions and retitrated to 200 ppm and 80 degrees F (solution was tested at 76 degrees F).
      • Observed a container of various dispensing utensils stored wet after hot water sanitizing in a container on the warewashing shelving. During the inspection the person in charge removed the equipment to be washed and sanitized.
      • Observed accumulation of debris on the following nonfood contact surfaces: Interior of cabinets along the front service area, interior of warming coolers #1 and #2, and shelf under the convection ovens.
      • Observed trash and debris on the ground inside the dumpster enclosure.
      • Observed the flooring around the floor sinks in the front of the establishment are no longer smooth or easily cleanable. It appears the seal has eroded and the flooring is rough to the touch. Discussed sealing the floor to create a smooth and easily cleanable surface.
      • Observed the following areas in need of cleaning: walls and ceiling in the warewashing area and caulking along the warewashing handsink.

      • Observed three knives and multiple baking pans soiled.
      • Observed 34 cooked chicken tamales prepapred yesterday afteroon at 45 degrees. Person in Charge voluntarily discarded tamales.
      • Observed three containers of tamales being held past the date they were to be discarded. Person in Charge voluntarily discarded tamales.
      • Person in Charge does not have thin probe thermometer in facility.
      • Observed pests and mouse droppings in the kitchen.
      • Observed that all non-food contact surfaces are soiled.
      • Observed that the facility is soiled.
      • Observed three ceiling tiles and walk-in freezer in need of repair.

      • Observed a container of beef broth cold holding at 44-46 degrees F. In discussion with the person in charge the broth was not properly cooled down the day prior. The beef broth was discarded for safety.
      • Observed a container of salsa roja with a made on date of 6/23. Food should have been discarded 7 days from the date it was made. Salsa roja was discarded for safety.
      • Observed food employees working without hair restraints.
      • Observed clean plastic and stainless steel containers stored stacked while wet.
      • Observed the metal wire rack used to hold clean equipment is sticky and requires cleaning.
      • Observed the unisex bathroom does not have a covered trash can.
      • Observed two walls of the storage room (dried wall) are not painted so that they are smooth and easy to clean.
