Fredericksburg Health Inspections: Roaches, Flies, Moldy Cilantro

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between May 10 and May 16.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Family Food Palace (VA G72 138), 916 Churchill Dr Fredericksburg, VA 22407
    • Date inspected: May 10
    • Total violations: 5
      • The person in charge could not provide a certified food protection manager certificate.
      • The person in charge could not provide documentation for clean-up of bodily fluids.
      • The hand sink is missing the hand washing sign.
      • Observed the following encrusted items in need of cleaning: 1.Utensil and pans 2.Flat top grill and surrounding areas 3.inside of fryer.
      • Observed that the floors, walls, and ceilings in mobile unit in need of detailed cleaning.
  • Sizzling Kabob & Grill, Inc, 367 Warrenton Road # 110-111 Fredericksburg, VA 22405
    • Date inspected: May 10
    • Total violations: 21
      • Absence of person in charge upon arrival to conduct the inspection. The person in charge arrived within 25 minutes. Instructed the owner a designated person in charge is required to be onsite during operating hours.
      • Absence of active managerial control observed during the inspection. The person in charge is not monitoring temperatures of TCS foods during cold holding, applying a date marking system for ready-to-eat foods held longer than 24 hours in the food establishment and monitoring employee handwashing.
      • Observed a food employee wash their hands with water only. Instructed the food employee to wash their hands with soap.
      • Observed a bag of moldy cilantro and cheese with debris of unknown origin in the top of the prep cooler. The person in charge voluntarily discarded the food during the inspection. Observed considerable debris in the base of the prep cooler (top) which may be the source of the cheese debris.
      • Observed multiple soiled pans on top of the range. The person in charge reported the facility was closed the day prior; equipment is being left soiled over 24 hours in the establishment.
      • Observed the following food contact equipment soiled to the sight and touch: 2 tongs on the handrail of the range, plates and bowls in the front service area, bin of skewers and knives by the hand sink. All equipment with the exception of the bin of skewers and knives were removed to the 3 vat sink for cleaning. Instructed the operator to remove the skewers/knives for cleaning during the inspection but this was not completed.
      • Observed the following TCS foods cold holding at improper temperatures inside the prep cooler: Cooked potatoes (43 degrees F), cooked beef (44 degrees F), cooked chicken (45 degrees F), paneer cheese block (43 degrees F) and heavy cream (45 degrees F). The person in charge reported TCS foods were held overnight in the unit and were voluntarily discarded for safety. Instructed the person in charge to discontinue using the cooler to cold hold TCS foods until evaluated by a technician. The person in charge reported he did not know how to turn down the temperature of the unit. Also observed naan dough, which is prepared with milk, holding on a table at 65 degrees F. The person in charge reported the pan was pulled out at 11:00 a.m. During the inspection the pan of dough was relocated to a refrigeration unit.
      • Absence of date marking for TCS foods prepared between Sunday and Monday (5/7/23 and 5/8/2023). The facility has use by stickers but they are not being completed. A food employee placed dates on the food containers during the inspection.
      • Absence of date marking system in the food establishment for TCS ready-to-eat foods prepared and held over 24 hours. Date marking handout is posted in the kitchen. Discussed requirements during the inspection.
      • Observed a spray bottle of straight bleach (exceeding 200 ppm when tested with a testing kit) which was not labeled to identify the contents. The person in charge labeled the bottle during the inspection.
      • Observed absence of hair restraints by food employees.
      • Observed stacks of plates and bowls with the food contact surface facing up in the front service area which may be contaminated by guests. Instructed the person in charge to remove the items which was not completed during the inspection.
      • Observed single use spoons and forks laying in a tray in the front service area with the food contact surface exposed. Also observed debris on the utensils. The person in charge discarded the utensils during the inspection. Discussed storage options with the operator to prevent contamination.
      • Observed cauliflower stored in plastic shopping bags inside the small deep freezer which will be cooked. During the inspection the food was relocated to a food storage container.
      • Observed two heavily scratched and scored cutting boards.
      • Observed cardboard is being used to hold hood vents in place (all vents).
      • Observed significant accumulation of encrusted and other debris on nonfood contact surfaces throughout the facility including cooking and refrigeration equipment (interior and exterior including gaskets), and exterior of equipment and food storage containers. Strongly recommend a deep a thorough cleaning.
      • The floors, walls, ceilings and plumbing fixtures throughout the facility are in need of cleaning. Strongly recommend a deep and thorough cleaning.
      • Through discussion with the person in charge and observation, the facility have various equipment which are broken or not being used. Also observed accumulation of empty food containers in the back of the house.
      • Observed the following in poor repair: Loose floor tiles by the 2 door upright cooler in the lobby, loose coving tiles under the 3 vat sink, ceiling tiles in sections are not tightly sealed to the frame, and ceiling tiles in the front service area are peeling.
      • Observed absorbent ceiling tiles installed in the kitchen, mop closet and men's toilet room. Ceiling tiles are not smooth or easily cleanable.
  • Kentucky Fried Chicken #0161, 302 Hanson Avenue Fredericksburg, VA 22401
    • Date inspected: May 10
    • Total violations: 8
      • Person in charge was unable to provide the signed employee health policy. During the inspection the staff on shift read and signed the policy.
      • Observed a food employee using a cup to dispense ice at the drive thru area.
      • Observed the interior sides of the ice machine soiled.
      • Observed a stack of sheet trays stored stacked while still wet.
      • The following equipment was observed in poor repair: 1. ice build up on the walls, ceilings and floors of walk in freezer. 2. leak at the soda machine/basin in the lobby, water(liquid) was observed leaking onto the floor of the lobby area 3. light shield shattered in front of the hand washing sink 4. one of the light switches for the raw chicken walk in is missing and or shattered.
      • The following non food contact surfaces were observed in need of cleaning: 1. rolling carts holding foods 2. racks inside the walk in cooler 3. back of cook/hot line equipment 4. air curtains between hallway and back room.
      • Observed the following in need of cleaning: 1. floor drains 2. wall above the hot holding line 3. ac/ceiling vents dusty 4. the ceiling, fans and walls of the walk in cooler.
      • Observed the following facilities in poor repair: 1. loose coving tiles in the walk in cooler 2. loose ceiling tiles in the dry storage room 3. loose and missing ceiling tiles in the hallway leading to the back door 4. all of the ac and ceiling vents appear to have mold like growth and black staining.
  • Primo Pizza, 10849 Tidewater Trail Fredericksburg, VA 22408
    • Date inspected: May 11
    • Total violations: 2
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • The floor tiles around the 3 compartment sink need to be re-grouted.
  • Basilico New York Deli & Bakery, 2577 Cowan Blvd Fredericksburg, VA 22401
    • Date inspected: May 11
    • Total violations: 9
      • Handwashing sink in kitchen had no paper towels available. Food employee corrected and provided paper towels during inspection.
      • Observed the chemical sanitizing dish machine was not dispensing any sanitizer while running. During the inspection the machine was primed and was dispensing the required amount of sanitizer.
      • Observed the following food items out of temperature due to units not working properly: 1. one container of cooked shrimp 53 degrees F 2. two containers of coleslaw 44-46 degrees F 3. two containers of feta 45-47 degrees F 4. two containers of hard boiled eggs 43-45 degrees F 5. one container of mayo pasta salad 44 degrees F 6. one container of Italian pasta salad 43 degrees F 7. one container of potato salad 44 degrees F 8. one container of shredded cheese 47 degrees F 9. one container of bean and corn salsa 44 degrees F 10. two containers of cut tomatoes 43-44 degrees F 11. one container of tomato sauce 46 degrees F 12. one container of Italian meets 44 degrees F 13. two container of chicken soup 46-47 degrees F 14. one bag of spinach 46 degrees F 15. one bag of salad greens 46 degrees F 16. one large plastic container of tomato sauce 66 degrees F 17. one container of white sauce 46 degrees F 18. one container of cheese sauce 47-48 degrees F 19. one container of beef broth 51 degrees F 20. one container of pizza sauce 48 degrees F 21. one pan of sausage links 46 degrees F During the inspection the foods were discarded and a technician came in to fix the unit.
      • Observed food employees working while wearing wrist jewelry.
      • Observed a container of cooking utensils stored in container of water in for use in between uses. EHS discussed proper storage of utensils during use.
      • Observed during the inspection two cooling units were not working properly. Walk in cooler was holding temperature of 45-46 degrees F and the salad prep cooler was holding temperatures of 47 degrees F. Before the inspection was over person in charge had contacted a technician. Also observed the gasket of the sandwich prep cooler needs placing.
      • Observed shelving throughout the facility needs cleaning.
      • Observed the following facilities in need of cleaning: 1. floor drains 2. walls around food prep tables and ware washing area 3. floors behind equipment in the kitchen 4. ac/ceiling vents 5. ceiling tiles need dusting or wiping
      • Observed a container of keys and employees foods and beverage stored on the top portion of a prep table.
  • McDonald's Restaurant #12770, 10607 Hilltop Plaza Way Spotsylvania, VA 22553

    • Total violations: 1
      • The interior of the ice machine was observed soiled. Machine was cleaned during inspection.
  • Quality Inn, 6409 Dan Bell Lane Thornburg, VA 22565

      • There were no paper towels at the hand sink in the kitchen. Towels were provided during inspection.
  • Woodmont Genesis Health Care, 11 Dairy Lane Fredericksburg, VA 22405
    • Date inspected: May 12
    • Total violations: 22
      • Upon arrival the facility did not have a designated person in charge. A food employee contacted the director via telephone who designated an employee to be the person in charge.
      • The food establishment does not have at least one employee in a management or supervisory role who is a certified food protection manager.
      • The person in charge could not provide documentation employees have been trained on the employee health policy. Food employees were knowledgeable of symptoms and illnesses requiring reporting. Sample policy provided to the operator.
      • Food employees could not provide written procedures for clean-up of vomiting and diarrheal events. Sample procedures were provided during routine inspection conducted on 4/22/22 and provided to the operator.
      • Observed a food employee place soap on their hands and lather under running water at the warewashing area hand sink. Hands were not effectively washed due to hand soap being flushed down the drain. Instructed the employee on correct handwashing and the employee washed their hands.
      • Observed hot water at 70 degrees F at the warewashing area hand sink. Contact plumber to evaluate hot water.
      • Observed opened box containing unsealed package of raw bacon stored over a box of tomatoes in the walk in cooler. During the inspection the bacon was relocated to prevent contamination.
      • Observed the following food contact equipment soiled: Meat slicer, whisk for the stand mixer, one spatula, and one pizza cutter. All equipment were removed to the 3 compartment sink for cleaning with the exception of the meat slicer. Instructed the person in charge to clean the meat slicer prior to next use.
      • Observed single use containers of yogurt (56.5 degrees F) and strawberry shake (56.3 degrees F) cold holding at improper temperatures in a container of ice. Observed containers not being held in direct contact with ice. A food employee reported the items were pulled from refrigeration at 7:30 a.m. (temperature observed at 8:35 a.m.). Foods were removed to the walk-in cooler to cool to 41 degrees F or less by 11:30 a.m. Ensure adequate quantities of ice are used to maintain temperatures 41 degrees F or less.
      • Observed the counter mounted can opener and plastic lip inside the ice machine soiled. The can opener was removed for cleaning, recommended disassembly for deep cleaning.
      • Observed unpasteurized shell eggs sitting on a counter in the cookline area at 56.3 degrees F. A food employee could not communicate how long the eggs were sitting on the counter and discarded them.
      • Observed single use containers of yogurt (56.5 degrees F) and strawberry shake (56.3 degrees F) cold holding at improper temperatures in a container of ice. Observed food containers were not being held in direct contact with ice. A food employee reported the items were pulled from refrigeration at 7:30 a.m. (temperature observed at 8:35 a.m.). Foods were removed to the walk-in cooler to cool to 41 degrees F or less by 11:30 a.m. Ensure adequate quantities of ice are used to maintain temperatures 41 degrees F or less.
      • One single use carton of whole milk was not dated for disposition by the manufacturer. The person in charge discarded the milk carton during the inspection.
      • Observed presence of small flying insects in the kitchen and dishwasher area. Contact pest control for inspection and service.
      • Observed multiple boxes of canned goods stored on the floor in the dry storage room.
      • Observed numerous cutting boards are scratched and scored. Cutting boards are no longer cleanable.
      • Observed accumulation of debris on the following nonfood contact surfaces: Upsplash of the coffee brewers, interior and exterior of the range, exterior of refrigeration units, top of dishwasher, meat slicer, and carts. Recommend thorough cleaning of all nonfood contact surfaces of equipment.
      • Observed build-up of encrusted residue on the nonfood contact surfaces of baking sheets stored on wire shelving.
      • Observed the following plumbing in poor repair: Leak at 2 compartment sink faucet, loose faucet at the warewashing hand sink.
      • Absence of backflow preventer for juice/coffee machines which are connected to the water line. Also observed a long hose, without a spray nozzle, attached to the mop sink faucet via a Y-connector in absence of any backflow preventer. An atmospheric vacuum breaker is not installed on the faucet. Observed the hose opening mounted with an adequate air gap above the flood rim of the mop sink. The person in charge reported a sprayer is attached to the hose which is used to clean the floor. Install a dual check valve with atmospheric vent backflow preventer to prevent backflow and backsiphonage. Discontinue use of the hose until backflow preventer is installed.
      • The following areas were noted in need of cleaning: Floor drains under the dishwasher and 3 vat sink, interior of the mop sink, dishwasher drainboard caulking, and walls behind cooking equipment.
      • Observed the floor tile group deteriorated by the cooking area and the room enclosing the dishwasher.
  • Golden China Restaurant, 5430 Southpoint Plaza Way Fredericksburg, VA 22407
    • Date inspected: May 12
    • Total violations: 6
      • Observed a cup without a handle, being use as a scoop in the yellow rice. Person in charge discarded cup and found a spoon to use.
      • Observed several flats of raw shell eggs stored over peeled raw onions in the walk in cooler. Eggs were relocated during inspection.
      • The interior of the microwave oven was observed soiled. Oven was cleaned during inspection.
      • The utility sink near the back door in the kitchen needs to be re-sealed to the wall.
      • The following non-food contact surfaces are in need of cleaning: 1-The top shelf of the expo station, 2-the outer surfaces of all bulk storage bins, 3-all of the shelving throughout the kitchen, 4-outer surfaces of the ice machine, 5-outer surfaces of the microwave.
      • The following physical facility items were observed covered with grime and debris: 1-the floors under all of the equipment in the kitchen, 2-the walls throughout the kitchen, especially behind the equipment on the cook's line.
  • My Little Italy's Pizza, 2010 Princess Anne St Fredericksburg, VA 22401
    • Date inspected: May 12
    • Total violations: 1
      • Observed the following equipment in disrepair: 1. 7 blown out light bulbs throughout the facility 2. kitchen hand sink is leaking water at the faucet 3. 1 poor fitting ceiling tile in the employee bathroom 4. pizza cooler is not maintaining required temperatures. EHS recorded temperatures of 42-44 degrees F.
  • Courthouse Cafe, 8955 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: May 12
    • Total violations: 4
      • Observed a sanitizing bucket located in the only hand sink in the kitchen. Bucket was removed during inspection.
      • The chlorine based sanitizing agent being applied to food contact surfaces does not meet the requirements of 21 CFR 178.1010. Chlorine sanitizer measured well over 400 ppm. Sanitizer was diluted to 100 ppm during inspection.
      • Observed employees working in the kitchen preparing food without head coverings.
      • Observed that the ventilation hood and filters over the cook's line were soiled with grime and grease.
  • Hangry Joe's, 9915 Southpoint Pkwy Fredericksburg, VA 22407
    • Date inspected: May 12
    • Total violations: 1
      • Adequate hot water is not available at the kitchen 3-compartment sink. After running the water for at least 15 minutes, water temperatures was only reaching 105°F. Water at adjacent food prep sink and kitchen hand sinks measured 117°F.
  • China King, 11 Hope Road Stafford, VA 22554
    • Date inspected: May 16
    • Total violations: 17
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS provided handout. Person in charge was notified of the update 4/13/22 and a handout was provided.
      • Observed a large pot of rice blocking the hand sink.
      • Imitation crab was observed in the walk-in freezer that is used to prepare platters offered on the menu as crab meat.
      • Observed the following food items cold holding in deep plastic containers at improper temperatures in walk-in cooler. Per discussion with person in charge the items were cooked 2 days prior to inspection. 1.cooked chicken - 44'f 2. fried rice - 44'f 3. spaghetti noodles - 44'f 4. cut cabbage - 44'f
      • Observed the following time/temperature control for safety (TCS) foods cold holding at improper temperature in the prep unit (45'F) by the chest freezer - the items were placed in the unit for over 4 hours. 1) one container of pork dumplings (44'f) 5; 2) boiled eggs (48'F); 3) hoisin sauce (47'F). The PIC discarded the items. TCS items will be placed on ice until the is Evo waited by technician.
      • Facility does not have a date marking system in place. Observed several containers of cooked TCS foods not date marked in all cold holding units.
      • Observed the following items at in proper temperatures in deep plastic containers cooked 2 days prior to inspection. Cooked Chicken (44'f), noodles (44'f), fried rice (44'f), per discussion with the person in charge the temperatures are not being monitored to prevent reaching the danger zone while out of the cooling units. EHS explained the proper cooling procedures.
      • Observed the following: 1) a tray of raw beef frozen on sheet pan defrosting at room temperature. 2) a container of sliced raw chicken in the 3-compartmen(37'F) stored in standing water.3) the prep table between the walk-in-freezer and walk-in-cooler a container of raw chicken frozen thawing at room temperature. (EHS explained the proper thawing methods with the PIC and food employees. The items were submerged under running water below 70'F.
      • Observed 2 live roaches and several what appeared to be fruit flies throughout the facility.
      • Observed the food items being stored on the floor.
      • Observed 2 boxes of carry out containers stored directly on the floor.
      • Observed a container of cooked chicken stored in the walk-in cooler that was in direct contact with a raw shell eggs carton which was used as a liner. Chicken was discarded.
      • Observed the following in need of repair. 1. the cold holding prep unit across from the grill in need of repair. (45'F) 2. Chest freezer has ice buildup and needs to be defrosted.
      • Chlorine test kit used by the faciality expired September 2022.
      • Accumulations of debris were noted on the following nonfood contact surfaces: 1) exterior and Interior of reach in refrigeration units, 2) shelving units, 3) rice cookers.4) fryer cabinets.
      • Observed the walls, ceiling tiles, and walls are in need of cleaning.
      • Observed employees' personal belongings stored near food and single service articles throughout the facility. PIC moved items.
  • Mission BBQ Stafford, 267 Garrisonville Road, Suite #110 Stafford, VA 22554
    • Date inspected: May 16
    • Total violations: 5
      • Observed the following: 1) two unlabeled spray bottles by the mop sink- PIC could not communicate what was in the bottles that contained sanitizer. 2) one unlabeled spray bottle in the front of the house that contained chemical.
      • Observed unwrapped single service knives stored with the lip-contact surface facing upward on the counter for self-service. Corrected during the inspection.
      • Observed the Quaternary Ammonium testing kit expired in 2020. Discussed purchasing a new test kit during the inspection.
      • Observed the following in need of cleaning: 1) fryer cabinets, 2) dish machine, 3) non food contact surfaces of ice machine, 4) shelving units in the back of the house.
      • Observed the following in need of repair: 1) re-caulk 3 compartment, 2) leak in the handwashing sink faucet by the perp area near WIF, 3) re-adjust ceiling tiles,
