Fredericksburg Health Inspections: Roma Italian, TGI Friday's, Outback
Health inspectors visited several schools and restaurants around the Fredericksburg area during a recent round of inspections.
FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities during recent inspections from Sept. 14 to Sept. 20.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Jersey Mike's Subs, 1495 Stafford Market Place Suite #119 Stafford, VA 22556
- Date inspected: Sept. 14
- Total violations: 6
- Observed food employee handle chemicals then donned gloves and proceeded to engage in food preparation without first washing hands. Employee was asked to wash his hands.
- The concentration of the quaternary ammonium sanitizer solution at the 3 compartment sink and the sanitizer buckets measured 0 PPM, according to the manufacture instructions the concentration should be between 150-400 PPM. PIC refilled the sanitizer buckets and the basin (200 PPM).
- Observed that the ice chute and dispensing tubes in the soda machine located in the lobby require cleaning. EHS instructed PIC to cease from using the machine until the unit is cleaned. Unit was cleaned during the inspection.
- Observed several flying insects throughout the facility. PIC contacted the pest control company (PCC) to come in and treat the insects. PCC come in monthly but will come in more often until the issue is resolved.
- Observed several wet wiping cloths on the counter. PIC placed the wet cloths in the sanitizer bucket.
- Observed the floor under all the equipment in need of cleaning.

- Observed the microwaves of the facility require cleaning.
- Observed a food employee wearing wrist jewelry.
- Observed numerous boxes of single service use article boxes stored on the floor.
- Observed the following non food contact surfaces in need of cleaning: 1. Metal racks holding equipment and boxes at the back kitchen area. 2. Inside racks and surfaces of reach in cooling units.
- Observed the following in need of cleaning: 1.walls behind 3 compartment sink area 2. floors of the facility 3. floor drains.
- Starbucks Coffee #7918, 1495 Stafford Market Place Stafford, VA 22556
- Date inspected: Sept. 14
- Total violations: 4
- Observed a food employee sweep the floor then handled clean equipment and engaged in drinks perpetration without first washing their hands. Employee was instructed to wash their hands and move the equipment to be washed, rinsed, and sanitized.
- Observed several flying insects in the back and front of the house.
- Observed clean dishes and equipment stored on the high temperature homestyle dishmachine located directly near the handwashing sink. The items were being exposed during the hand washing process. EHS instructed PIC to re-wash and re-locate the items and to install a splash guard.
- Observed a hose attached to a Y connector at the mop sink with an air gap that was less than 1 inch above the flood level rim. PIC removed the Y connector. Observed the coffee machines directly connected to the water line- EHS and PIC were not able to locate the backflow prevention device. PIC will check with the plumber and ensure a proper back flow prevention device is installed.

- Observed rodent-like feces on canned food items in dry storage. Observed packaging of food items compromised by rodent-like pests in dry storage area of food facility. Food discarded.
- Back exit door observed with gap and does not protect against the entry of pests into the facility.
- Observed rodent-like feces in dry storage area of food facility. Spoke with Person in Charge regarding increasing pest control inspections until issue is resolved.
- Observed toilet room door propped open during inspection.
- Hood baffles observed to be missing from hood ventilation system above cooking equipment.
- Wildcat Cafe, 2135 Mountain View Dr. Stafford, VA 22556
- Date inspected: Sept. 14
- Total violations: 4
- Observed accumulation of debris on the food contact surface of the char broiler. Instructed the person in charge to clean prior to next use.
- Observed 4-5 unwrapped disposable paper towels stored on a shelf above the 3 vat sink which may be exposed to splash or other contamination. During the inspection the person in charge removed the paper towels and placed them in a covered storage bin.
- Observed accumulation of debris on the following nonfood contact surfaces: two black freezer tubs inside the 4 door reach in freezer, prewash spray nozzle by the dishwasher, base and interior door of the oven and the back of the convection oven.
- Observed ceiling tiles throughout the kitchen are not secured to the frames, observed gaps and some ceiling tiles are protruding from the frame.

- The person in charge could not provide an irreversible registering thermometer for the high temperature sanitizing dish machine.

- Sanitizer solution at dish machine was observed empty and had food equipment being washed inside. Sanitizer solution was replaced and tested to proper ppm.
- Walk-in refrigerator door observed to not be sealing properly and is in need of repair.
- Cardinal Village, 4621 Spotsylvania Pkwy Fredericksburg, VA 22408
- Date inspected: Sept. 15
- Total violations: 8
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- There was no sanitizer being dispensed into the 3 compartment sink due to the sanitizer bottle being empty. Bottle was replaced with a full one during inspection.
- Observed several sauce squeeze bottles near the prep area that were not labeled. Bottles were labeled during inspection.
- Observed cantaloupe not being washed before being cut, combined with other ingredients, cooked, served, and/or offered for human consumption in the ready-to-eat form. Person cutting melon was instructed to wash melon before cutting.
- Both the chlorine and quaternary test kits in the facility were expired.
- The following non-food contact surfaces were observed soiled with grime and debris: 1-the top of the dish machine, 2-inside of the deep fryer cabinets, 3-the top of the fryer hot holding unit.
- Observed cell phone, hand held radio, coats, dirty apron stored next to clean cutting boards, on top of prep table and touching clean paper towels. Personal items were relocated during inspection.
- Observed that the HVAC ceiling intake vents were soiled.

- Observed 2 dented cans stored in dry storage area of food facility. Dented cans were separated to be returned or discarded.
- Water pressure at service line handwashing sink observed without adequate pressure. Other hand washing sinks available until handwashing sink can be repaired.
- Mop sink hose observed without backflow prevention. Hose disconnected until proper backflow device can be installed.

- Shelving in walk-in refrigerator observed to be rusted and are no longer smooth and easily cleanable.

- Observed 0ppm was found at the 3 compartment sink of the bar area. During the inspection sanitizer solution was re-made.
- Observed the high temperature of the dish machine was not adequately working. After several attempts highest registered temperature was 145.6 degrees F. EHS advised to person in charge to manually sanitize dishes at the 3 compartment sink until a technican comes to fix the machine.
- Observed the following: 1. interior surfaces making direct contact with the ice in both of the ice machines require cleaning. 2. blade of the can opener was observed soiled. Can opener was moved to warewashing area.
- The following foods were found cold holding at improper temperatures due to equipment in disrepair: At the fry station (today only facility will use time as a public health control and will discard foods by 3pm). 1. one container of breaded shrimp 47-51 degrees F 2. one container of chicken tenders 50-51 degrees F In cooling drawers: (foods were left overnight and were discarded for safety: 3. one container of chicken breast 44-46 degrees F 4. one container of chicken skewers 44-46 degrees F 5. one container of portioned rice 50 degrees F 6. one container of portioned pico 53 degrees F 7. one container of portioned cooked onions 47-50 degrees F 8. one container of shredded mozzarella 60 degrees F 9. one container of portioned salsa 57 degrees F 10. onec ontainer of portioned cooked veggies 50 degrees F. In cooling drawers: (foods had been placed in unit less than 2 hours and were relocated to another cooling unit) 11. one container of mozzarella sticks 46 degrees F 12. one container of onion rings 47 degrees F 13. one container of sliced cheese 47 degrees F 14. one container of chicken wings 46 degrees F 15. one container of boneless wings 44-46 degree F 16. one container of chicken flet 42-44 degrees F.
- Observed the following: 1. 1 container of glass cleaner stored above the ice machine 2. 1 spray bottle of cleaning chemical, one container of stainless steel spray and 1 container of lotion stored above the wrapped silverware. Items were relocated during the inspection.
- Observed several stacks of dishes stored facing upwards at the cooks line.
- The following equipment was found in disrepair and requires fixing: 1. Fry station unit not holding required temperatures 2. Saute drawers not holding required temperatures
- The following non food contact surfaces were observed in need of cleaning: 1. outside surfaces of oven 2. inside cabinet of fryers 3. racks holding clean equipment in the warewashing area 4. upsplash of the soda machine 5. top part of the dish machine.
- Observed numerous sheet trays with the non food contact surface with a heavy build up of grime and debris.
- Observed walls, floor throughout the facility and ceiling tiles require cleaning. Floors of the bar also require a deep cleaning.
- Observed the floors of the beer walk in cooler are not smooth and easy to clean.

- Observed food employee washing their hands for less than 10 seconds. Hands were not properly cleaned. EHS instructed employee to re-wash their hands properly.

- Mop sink hose observed without a proper backflow device. Hose was cut to provide an air gap between the hose and the flood rim of the mop sink.

- Observed a food employee touch the sides of their headset near the mouth wearing single use disposable gloves and then proceed to handle a flour tortilla with the same gloves. Instructed the food employee to remove gloves and wash their hands. Discussed proper glove use with the person in charge. A food employee voluntarily discarded the flour tortilla for safety.
- Observed stacks of stainless steel food containers stacked while wet after chemical sanitizing on the warewashing shelving.
- Outback Steakhouse, 308 Worth Avenue #101 Stafford, VA 22556
- Date inspected: Sept. 16
- Total violations: 7
- Observed food employee exiting the restroom located in the kitchen and donning gloves to start engaging in food preparation without first washing their hands- employee was not aware that they needed to re-wash their hands in the kitchen hand washing sink. Hands were re-washed.
- Observed food employee use the container of breading mix for onions (135'F) and raw shrimp (145'F). The items do not require the same cooking temperatures and the shrimp is one of the common food allergens. Breading mix was discarded. Two different containers were provided and labeled.
- Observed the following TCS foods cold holding at improper temperatures in the 2 door reach in cooler: Containers of shredded lettuce (44, 46, 49 degrees F) and scrambled eggs/sausage mix for breakfast burritos (48 degrees F) and reach in cooler by prep line: Containers of sliced cheese (49, 52 degrees F). The person in charge reported the TCS foods in the 2 door reach in cooler have been in the unit overnight and the sliced cheese has been in the reach in cooler by the prep line for an unknown period of time. The person in charge removed the TCS foods from the refrigeration units to the office to be logged as waste to be discarded for safety. Discontinue use of the refrigeration units to hold TCS foods until evaluated by a technician.
- Observed a bottle of all purpose cleaner stored near drinks in the bar area. Bottle was relocated.
- Observed several flying insects in the back of the house.
- Observed wet stacking of clear plastic food containers. PIC instructed an employee to re-wash, rinse, and sanitize the items.
- Observed water collecting in the walk-in-cooler.
- Anthony Burns Elementary School, 60 Gallery Road Stafford, VA 22554
- Date inspected: Sept. 16
- Total violations: 2
- Observed the interior surfaces of the ice machine soiled.
- Observed multiple plastic covers cracked and chipped with rough edges which are not smooth or easily cleanable. Also observed cracks in the deep freezer.
- Roma Italian Restaurant, 114 W Broaddus Ave Bowling Green, VA 22427
- Date inspected: Sept. 16
- Total violations: 10
- Soap was not provided at the hand sink in the kitchen. Soap was provided during inspection.
- Chlorine sanitizer is not being dispensed into the dish machine. A sink was set up to sanitize equipment after being cleaned in the dish machine.
- Half and half (44'F) and cut lettuce (45'F) were cold holding at improper temperature. Items were voluntarily discarded during inspection and the temperature of the cooler was lowered.
- The rear outer door in the kitchen is not self-closing.
- Food stored on the floor or food stored less than 6" above the floor. Observed a soda bag in box in the dry storage room, several cases of soups, desserts and steaks stored on the floor in the walk in freezer and 3 cases of tomatoes and a bag of potatoes stored on the floor in the walk in cooler.
- The hand sink in the men's restroom and the hand sink in the kitchen are not sealed to the adjoining walls.
- The faucet unit on the kitchen hand sink is very loose.
- The following physical facility items are in need of repair: 1-missing light bulbs over pizza prep station, 2-light cover above ice machine is broken.
- The door frame of the door between the kitchen and the dining room is constructed of unsealed wood.
- The light intensity is below 10 foot candles in the walk in cooler. Measured 0 foot candles in the cooler.
- Biscuit Batch (VA BSCT B), 4828 Southpoint Pkwy Fredericksburg, VA 22407
- Date inspected: Sept. 18
- Total violations: 3
- Observed the following TCS foods cold holding at improper temperatures inside the 1 door upright cooler: Container of microgreens (66 degrees F), honey butter prepared in the permitted commissary (57 degrees F), and blackberry jam prepared in the permitted commissary (47 degrees F). The person in charge reported the TCS foods were transported in the refrigerator during transport. Through discussion with the person in charge the TCS foods were removed from refrigeration at the permitted commissary at 9:45 a.m. Sell, use or discard by 1:45 p.m. Discussed placing TCS foods in coolers wih ice during transportation to events.
- Observed a food employee handle a bottle of Quaternary Ammonium sanitizer under the 3 compartment sink and then proceed to handle oven mitts used to handle hot covers on the food warmer unit. Instructed the food employee to wash their hands.
- The person in charge could not provide verification of employee health training of employees. Blank employee health policies were provided for review by the person in charge. During the inspection the person in charge and one food employee working on the mobile unit signed the employee health policies. Food staff were knowledgeable of health reporting requirements. Ensure verification of employee health training are available for review during inspections.
- Taqueria y Pupuseria Don Jose (TYY-6973), 4409 Plank Road Fredericksburg, VA 22407
- Date inspected: Sept. 18
- Total violations: 3
- Observed a food employee wash their hands less than 10 seconds at the handsink. Instructed the person in charge on correct handwashing and the food employee rewashed their hands.
- Observed an empty bag in the only handsink on the mobile unit. The person in charge removed the bag during the inspection. Informed the person in charge handsinks are only to be used for handwashing.
- Observed the following TCS foods cold holding at improper temperatures: 1- Top of prep cooler not held in the well - Cheese filled pupusas (62 degrees F). The person in charge reported the pupusas were transported in a truck to the event and placed in the unit at 9:45 a.m. Instructed the person in charge to sell, discard or cool to 41 degrees F or less by 1:45 p.m. A food employee relocated the pupusas inside the top well. Ensure TCS foods are transported to events in a cooler with ice to maintain temperature. 2- Cooler - Shredded cabbage (45 degrees F), tomato salsa (47, 48 degrees F). Observed the TCS foods overfilled in the coolers and not held in adequate quantities of ice. The person in charge reported the foods were pulled 9:30 a.m. from the prep cooler and placed in the cooler. A food employee repositioned the foods in the ice to facilitate cooling. Instructed the person in charge to sell, discard or cool to 41 degrees F or less by 1:30 p.m. Foods were observed at 41 degrees F at 1:30 p.m.

- Total violations: 4
- Observed inadequate ground cover under the tent where foods are being prepared. The food establishment is operating on grass surface and cardboard and rubber mats are only placed in specific sections under the canopy. Also observed the warewashing and handwashing station is not located under a canopy. Covers were placed on the 3 basin warewashing station during the inspection.
- Observed on site prepared horchata is being stored in milk (gallon) containers. Informed the person in charge to discontinue reuse of manufacturer containers of milk.
- Observed the following TCS foods cold holding at improper temperatures inside the deep freezer: Cooked pork in cheese prepared in the permitted food establishment (47 degrees F) and packaged cheese (47 degrees F). The person in charge reported the TCS foods were transported in coolers with ice and placed in the deep freezer. Observed the freezer temperature at 53 degrees F. The temperature was lowered during the inspection. Advised the person in charge to maintain TCS foods in the coolers with ice until the freezer temperatures is at least 41 degrees F or less for future temporary events. The person in charge reported the TCS foods were removed from the food establishment at 9:00 a.m. Instructed the person in charge to cool, sell or discard by 12:00 p.m. Also observed birra sauce, prepared from tomatoes, sitting on a shelf next to the flattop grill (60 degrees F). The person in charge reported the sauce was pulled from the cooler at 9:30 a.m. The sauce was relocated back to the cooler. Use, discard, or cool to 41 degrees F or less by 12:30 p.m.
- Observed a food employee contact a child with their hands and they proceed to touch packaged foods without washing their hands. Instructed the food employee to wash their hands.

- Observed the 3 hand sinks in the kitchen need re-caulking.
- Observed several ceiling tiles of the dry storage and hallway storage are porous and not smooth and easily cleanable.
- Observed the following areas that require cleaning: 1. walls behind equipment at the cook line 2. ceiling/ac vents of the dishroom 3. lightshields throughout kitchen area, debris was observed inside light shields.
- Spotsylvania Middle School, 8800 Courthouse Road Spotsylvania, VA 22553

- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Observed boxes of single-use equipment stored on the floor in the dry storage area of the food facility.
- The following physical facilities are in need of repair: 1. Observed lights out above back prep line; 2. Observed wall damage next to red hot hold cabinet; 3. Ceiling tile displaced above 2-door freezer.
- Ceiling vents throughout kitchen observed with an accumulation of static dust and are in need of cleaning.
- Spotsylvania Elementary School, 7415 Brock Road Spotsylvania, VA 22553

- Total violations: 3
- Observed temperature measuring device stored in sanitizer in prep area of the food facility. Thermometer was removed from sanitizer during inspection.
- Facility did not have available chlorine test strips for the sanitizer solution at the 3-compartment sink. Test strips were provided during inspection.
- Ceiling vents throughout kitchen were observed with an accumulation of static dust and mold-like residue and are in need of cleaning.
- Caroline Middle School, 13325 Devil’s Three Jump Road Milford, VA 22514
- Date inspected: Sept. 19
- Total violations: 2
- A food handler was observed drinking from an uncovered beverage container while in the kitchen. Drink was covered during inspection.
- Observed food employee handle cell phone, leave the kitchen the return to the kitchen without washing their hands. Person in charge directed employee to wash their hands after touching cell phone and returning to the kitchen.
- Stafford High School, 63 Stafford Indian Lane Fredericksburg, VA 22405
- Date inspected: Sept. 19
- Total violations: 2
- The facility is using time as a public health control for salads (made from cut lettuce and pre-cooked chicken) and string cheese in all four lines without any written procedures for the use of time as a public health control (TPHC) with potentially hazardous foods. EHS provided TPHC documents.
- Observed frost all around the door frame of the walk in freezer PIC stated an order has already been issued to repair the unit.

- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The following kitchen equipment is not functioning or not functioning correctly: 1-the walk in freezer condenser pipe is leaking and leaving an ice buildup inside of the freezer, 2-hot box #2 is not maintaining proper temperature, 3-the upper convection oven is not working, 4-the lower steamer is not working.
- John J. Wright Center, 7565 Courthouse Road Spotsylvania, VA 22553
- Date inspected: Sept. 20
- Total violations: 5
- No paper towels available at back handwashing sink. Paper towels provided during inspection.
- Observed food employees not wearing proper hair restraints as required.
- Observed boxes of single-use equipment stored on the floor in the dry storage area of the food facility. Dry storage observed with an absence of shelving needed to store equipment the required 6 inches off of the floor.
- Observed an accumulation of unused equipment stored in warewash room of food facility.
- Observed several lights out throughout kitchen area of food facility. Observed ceiling tile displaced near back exit door of food facility.
