Fredericksburg Health Inspections: Sushi Kola, Tito's Diner

The Rappahannock Area Health District recorded violations at several Fredericksburg facilities during recent inspections.

FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities during recent inspections from Sept. 28 to Oct. 4.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Meadow Event Park, 13111 Dawn Blvd Doswell, VA 23047
    • Date inspected: Sept. 29
    • Total violations: 1
      • Observed fish thawing in standing water. Fish was placed under running water.
  • Honey Baked Ham & Cafe, 2007 Plank Rd Fredericksburg, VA 22401
    • Date inspected: Oct. 3
    • Total violations: 5
      • Observed several racks in the dry storage area starting to rust.
      • Observed the back hand sink is no longer adjoined to the wall and needs recaulking.
      • Observed several metal shelves in the kitchen holding clean equipment are soiled to the sight and touch.
      • Observed the ac/ceiling vents of the kitchen area are in need of cleaning.
      • Observed the floors of the kitchen area need repainting or resealing so that the surfaces are smooth and easy to clean.
  • Sushi Kola, 1811 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Oct. 3
    • Total violations: 13
      • Observed the interior surface of the ice machine soiled.
      • Observed 2 Windex spray bottles containing Pine-Sol all purpose cleaner. During the inspection, the PIC discarded spray bottles. (Corrected)
      • Observed 40 frozen packages of imitation crab sticks thawing on the prep table and 4 packages of frozen raw shrimp in a plastic container of water(70F) on the floor under the prep table. During the inspection, the PIC cut open the frozen package of imitation crab sticks and placed in the walk-in cooler and the frozen packages of raw shrimp were placed in the prep sink under cold running water. (Corrected)
      • Observed several wet wiping cloths on the server station counter. During the inspection, the wet wiping cloths were placed in sanitizer buckets. (Corrected)
      • The following were observed: 1. Two scoops and 1 spatula in standing water (107F) next to the rice cooker. During the inspection, a food employee discarded the water, rewashed the utensils, and left clean utensils in a clean container with a 4 hour label. 2. One knife being stored between the prep unit and prep table. During the inspection, the knife was relocated to the dishwasher area. EHS informed the food employees on how to store of in-use utensils/equipment. (Corrected)
      • Observed 10 containers of seaweed salad being reused to store cubed beef in the walk-in cooler. During the inspection, the food employees removed the beef from the containers. (Corrected)
      • The following are in need of repair: 1. Ice build-up in the walk-in freezer. 2. Ice build-up in the large chest freezer. 3. Cracked shield cover over the handwashing sink in the warewashing area. 4. Two lights out in the warewashing area. 5. One light out between the 3 compartment sink and handwashing sink. 6. One light out over the dry food storage area.
      • The caulking at the handwashing sink (along beverage counter), 3 compartment sink, and dishwasher are in of cleaning or recaulking.
      • The following non-food contact surfaces are in need of cleaning: 1. Ice machine filters. 2. The exterior of the fryer prep unit. 3. The vents of the flat top grill.
      • Observed 4 ceiling tiles in the kitchen that were not smooth and easy to clean.
      • The ceiling and floor (also walk-in cooler/freezer) throughout the establishment is in need of cleaning.
      • Observed grease dripping from the light covers in the hood system. EHS discussed cleaning more often to prevent build-up of grease on surfaces of hood system.
      • The following were observed: 1. Three personal beverages (red cups, insulated mugs) being stored between utensils, napkins, and water pitchers. 2. One open energy drink on the shelf with clean utensils. 3. A stuffed pink pig (toy) on top of the mayo containers. During the inspection, the PIC had the food employees relocate the beverages and toy. (Corrected)
  • Tito's Diner, 1695 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Oct. 4
    • Total violations: 10
      • Observed a food employee fill up a pan with water from the hand sink.
      • Observed an employee handling and touching the food contact surface of clean utensils when wrapping silverware.
      • Observed the upper portion of the ice machine was soiled. During the inspection the ice machine was cleaned.
      • Observed two wrapped turkey breast thawing in a container of water at room temperature. Turkey breasts were frozen solid and were relocated to the walk in cooler to properly thaw. EHS discussed approved thawing methods with the person in charge
      • Observed food employees wearing wrist jewelry.
      • Observed a heavy build-up of ice at the back of the walk in freezer.
      • Observed several racks used to store food and equipment are rusted.
      • Observed metal racks of the walk in cooler and racks of the dry storage room that require cleaning.
      • Observed the following in need of cleaning: floor drains, floors under equipment, floors of walk in freezer, ac/ceiling vents of dish area and womens bathroom.
      • Observed the floors of the back storage area require resealing or repainting so that they are smooth and easy to clean.
  • Sedona Taphouse, 591 William St. Fredericksburg, VA 22401
    • Date inspected: Oct. 4
    • Total violations: 6
      • Observed mussel shellstock tags after 09/01/2022 were not dated with sold or served date.
      • Observed one container of chicken salad (47-51°F) in the prep unit that was prepared on 10/2/2022 at improper temperature. During inspection, the chicken salad was discarded.
      • Observed several food employees wearing wrist jewelry (watch, bracelets) while preparing food. During inspection, the food employees removed jewelry.
      • Observed the following utensils being stored in water: 1. One ice cream scoop in 81°F water next to queso. 2. Several spatulas in 67°F water next to the gas range. During inspection, the utensils were removed from water, cleaned, and placed in a clean food container for storage.
      • The following equipment need sealed to adjoining wall: 3 compartment sink and prep table next to prep sink.
      • The following are in need of repair: 1. Broken ice machine lid. 2. The prep unit door and standing water on bottom shelf (located at end of prep line). 3. The prep unit door located next to handwashing sink.
  • Cook Out Restaurant, 10108 Jefferson Davis Hwy #300 Fredericksburg, VA 22407
    • Date inspected: Oct. 4
    • Total violations: 6
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • The handwashing station at the front of the kitchen is being used for purposes other than washing hands. Observed an employee cleaning parts of the shake mixer in the sink. Employee was told to use the three compartment sink to wash equipment and use the hand sink for hand washing only.
      • The 3 compartment sink is not sealed to the wall.
      • The following non-food contact surfaces are in need of cleaning: 1-The lid on the ice machine, 2-the shelf under the char grill, 3-the shelf under the drink dispenser in the dining room, 4-the top of the tea maker.
      • There is no cover to the feminine napkin refuse container in either of the ladies room stalls.
      • The following physical facilities were observed in need of cleaning: 1-The ventilation hoods over the cook's line, 2-the Ansul system under the hood on the cook's line, 3-the floor in the walk in cooler, 4-the light covers in the kitchen.
  • Mattaponi Springs Golf Club, 22490 Penola Rd Ruther Glen, VA 22546
    • Date inspected: Oct. 4
    • Total violations: 14
      • The facility does not have a certified food manger. The person in charge provided a certified food manger certificate for an employee which expired in 2021.
      • The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Sample handouts were provided during the routine inspection on 9/8/21. Provide sample written procedures during the inspection.
      • Observed a wet towel and two green scrubbing sponges stored in the basin of the bar hand sink. The wet towel was removed for cleaning and the green scrubbing sponges were removed to the kitchen.
      • Observed uncut tomatoes in contact with soiled dry towels. The person in charge voluntary discarded the tomatoes and removed the linens for cleaning.
      • Observed numerous utensils at the bar used in contact with bar beverages soiled. The equipment was removed to the kitchen for cleaning during the inspection.
      • Observed interior surfaces of the microwave oven soiled and observed four fryer baskets with accumulation of oily debris. the person in charge reported the fryer baskets have not been used within the past 24 hours. The fryer baskets were removed for cleaning.
      • Observed food contact surfaces of the meat slicer soiled, observed one plate and pair of tongs, knife held in a knife block and cutting board attached the the wall near the kitchen hand sink soiled. The equipment with the exception of the meat slicer and cutting board were removed for cleaning. Clean and sanitize the cutting board and meat slicer prior to next use. Recommend disassembly of the cutting board attached to the wall to prevent contamination and easy of cleaning.
      • Observed containers of the following TCS foods not labeled to identify the date of preparation or discard: Previously reheated hot dogs, opened and sliced bulk deli ham, ham sandwiches and pulled pork in the 1 door upright cooler. The person in charge reported the foods were prepared on 9/30/22 and placed a date label on them.
      • The person in charge could not provide a thin probed food thermometer. The facility has two bimetallic stem thermometers with the sensor noted halfway up the shaft. Instructed the person in charge to purchase a thin probed food thermometer.
      • Observed numerous wet and soiled cloths stored in the bar 3 vat sink. The cloths were removed to be cleaned.
      • The basins of the kitchen and bar handwashing sinks have accumulation of rust.
      • Observed the Quaternary Ammonium test kit expired 8/2021. Instructed the person in charge to purchase a new test kit.
      • Accumulation of grime and debris was noted on the shelf under the cooking equipment, metal plating in the back of the fryers and upsplash of the coffee brewer.
      • Observed the ceiling tiles in the male and female toilet rooms are not smooth and easily cleanable and are absorbent. Observed heavily soiled ceiling tile in the female toilet room.
