Fredericksburg Health Inspections: Taqueria Flavio, Quality Inn, Mi Pueblo Mexican

The Rappahannock Area Health District recorded violations around the Fredericksburg area during recent restaurant inspections.

FREDERICKSBURG, VA — Inspectors recorded violations at several Fredericksburg restaurants during inspections from July 20 to July 26.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

    • Date inspected: July 21
    • Total violations: 3
      • Observed damaged cans of pineapples and oranges.
      • Observed spray bottles of cleaning solution and sanitizer stored with foods. The PIC relocated the items.
      • Observed 44 foot candles of light in the kitchen.
  • Home2 Suites, 3051 Richmond Highway Stafford, VA 22554
    • Date inspected: July 21
    • Total violations: 10
      • The facility does not have documentation to show that employee health training is being accomplished.
      • Facility does not have written procedures for the clean up of bodily fluids.
      • Observed cream cheese at 59'F, half & half at 76'F, and waffle batter at 76'F. The cream cheese was discarded, the half & half was placed in the reach in cooler.
      • Observed hand sanitizer stored next to the food. The PIC relocated the chemical.
      • Observed that the refrigeration units do not have thermometers.
      • Observed that the employee is not wearing a hair restraint.
      • Observed that the doors to the dish machine require cleaning.
      • The facility does not have a way to test the temperature of the high temperature dish machine.
      • Observed that the mop is sitting in water - not allowing it to air dry.
      • Observed three (3) lights out in the kitchen.

      • Front handwashing sink observed at 87F, back handwashing sink observed at 94F.
      • Observed spray bottles on chemicals and soap stored next to and above food items (shelf-stable milk) in cabinets. Person in Charge relocated items to prevent contamination.
      • Observed sanitizer bucket stored on floor in prep area of food facility.
      • Observed an accumulation of debris around outdoor compactor.
  • Chipotle Mexican Grill #1109, 5420 Southpoint Plaza Way Fredericksburg, VA 22407
    • Date inspected: July 21
    • Total violations: 4
      • Observed spices stored in front prep area uncovered. Person in Charge covered food items to prevent contamination.
      • Observed cleaned and sanitized utensils stored in front prep area with an accumulation of debris on utensils. Person in Charge brought utensils back to warewash area to be cleaned and sanitized.
      • Observed chemical spray bottle and sanitizer bucket stored on shelf with food equipment. Person in Charge relocated food equipment to prevent contamination.
      • Observed clean food equipment stored on shelf in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Person in Charge separated food containers to allow equipment to air dry.
  • Mi Pueblo Mexican Restaurant, 10647 Courthouse Road Fredericksburg, VA 22407
    • Date inspected: July 21
    • Total violations: 18
      • The facility does not have a person that is a certified food protection manager.
      • Observed a metal scrub pad in the handwashing sink. During the inspection the metal scrub pad was removed.
      • Observed a moldy head of lettuce stored in the reach in cooler. During the inspection the lettuce was tossed for safety.
      • Observed the following foods uncovered and unprotected: 1. One head of cabbage in the reach in cooler 2. Two containers of chicken in the freezer 3. In the dry storage two containers of salt and sugar. During the inspection foods were covered.
      • Observed raw animal foods stored above ready to eat foods in the reach in cooler. During the inspection the raw animal products were placed on the bottom shelf of the reach in cooler.
      • Observed the interior surfaces of the microwave were heavily soiled.
      • Observed the following items stored in a cold holding unit that were not cooled properly. Per discussion with the person in charge, foods were made th day prior. Person in charge discarded the foods for safety. 1. One container of rice, condensation was observed around the wrapping covering the rice. Temperatuer of the rice was 60-63 degrees F. 2. One container of red tomato sauce (45-49 degrees F).
      • Observed the following items stored in a cold holding unit that was not maintaining foods at required temperatures: 1. One container of shredded lettuce (66 degrees F) 2. One container of shredded cheese (66 degrees F) 3. One block of cheese (61 degrees F) During the inspection person in charge discarded the foods for safety.
      • Observed the facility does not have a date marking system in place. when foods made in the facility and held for longer than 24 hours need to be discarded by
      • Person in charge was unable to provide a thin probe thermometer.
      • Observed small insects in the kitchen area.
      • Observed a food employee working without a hair restraint.
      • Observed ice build up in the deep freezer and a cold holding unit prep table is not maintaining adequate cold holding temperatures (60 degrees F).
      • The following non-food contact surfaces were observed in need of cleaning: 1. Food cart 2. Stove top and grill areas 3. Top and sides of cold holding/prep shelves 4. Drawers and bottom of prep table 5. Inside and outside of reach in cooler and prep unit. 6. Outside of the ice machine.
      • Observed many tools stored on the floor under the 3 compartment sink and in a drawer with knives and utensils used for cooking. Tools in the drawer were relocated during the inspection.
      • EHS recommends a deep and thorough cleaning of walls, floors and ceilings of the entire kitchen, specially walls and floors around the dish washing area (walls and floors behind 3 compartment sink and dish machine) and wall besides and behind stove.
      • Observed the following: 1. Cracked and damaged floor tiles throughout the kitchen and loose ceiling tiles above dish machine. Person in charge materials have been purchased to replace floor and ceiling tiles. 2. The partition behind the prep area is chipping and peeling.
      • Observed numerous items of personal belongings (mentos, tylenol, cell phone, headphones, a scarf) above a prep cooling unit. During the inspection personal belongings were relocated.

      • Observed shell eggs (47, 50 degrees F) cold holding at improper temperatures on the top of the prep cooler. Observed eggs stored in a small and overfilled container. The person in charge reported the eggs were pulled from the walk in cooler prior to the commencement of the inspection. The person in charge instructed a food employee to pull the top layer of shell eggs and relocate to the walk in cooler. Cool eggs to 45 degrees F or less by 11:30 a.m. Discontinue overfilling the food containers and recommend storing eggs in a deep container.

Instructed the person in charge to ensure employees are adequately trained. This was discussed during the routine inspection debrief on 7/12/2022.
      • Observed a sealed chemical bottle stored on top of the warewashing handsink and paper debris inside the basin. Instructed the person in charge to clean the handsink. Ensure handsinks are used for handwashing tasks only.
      • Observed sliced cheese (43, 44 degrees F) cold holding at improper temperatures inside the prep line reach in cooler. The person in charge could not verify how long the food has been in the unit and voluntarily discarded for safety. Instructed the person in charge to discontinue cold holding of TCS foods until evaluated by a technician.

      • The following nonfood-contact surfaces were observed to be soiled and are in need of cleaning: Single-use cup tray next to drive-thru window; Ice machine vents; floor drain near broiler and next to chicken prep station; French fry hot hold unit; French fry utensil storage area; Floor in walk-in freezer; Bucket holding whisks and food container lids on shelves.
      • Observed an accumulation of debris and residue on floor under drive-thru drink station. Observed an accumulation of static dust on ceiling and vent above prep area.

      • Observed the following TCS foods hot holding at improper temperatures on the online orders hot holding unit: Pinto beans (127, 128 degrees F), steak (106 degrees F) and chicken (106 degrees F). The person in charge could not verify hot long the foods have been in the unit or when the foods were cooked or reheated and voluntarily discarded for safety. Observed the stainless steel pans are not in contact with the electric heating element. Discussed monitoring hot holding temperatures every 2 hours.
      • Observed cut lettuce (44, 45 degrees F) cold holding at improper temperatures inside the online orders cold holding well. The person in charge could not verify when the lettuce was placed in the unit and voluntarily discarded for safety.

      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Facility does not have written procedures for the clean-up of a vomiting or diarheal event. Person in Charge was provided with procedures.
      • Observed food employee filling water pitcher at wait station handwashing sink. Food employee was informed of proper procedures.
      • Handwashing sink in wait station area of food facility did not have a sign or poster notifying employees to wash hands. Person in Charge was provided with sign.
      • Observed food employee handle raw meat product with gloves, then proceed to handle ready-to-eat bread without washing hands and changing gloves. Food employee was informed of proper procedures and discarded bread.
      • Sanitizer dispenser was not working at dish machine. Person in Charge fixed sanitizer dispenser.
      • Observed residue on shield inside ice machine.
      • Observed food held in walk-in refrigerator (deli meat, cheese, lasagna, tuna salad) held for greater than 24 hours not being date marked as required.
      • Observed spray bottle on shelf below deli slice and in wait station not labeled with common name of chemicals. Food Employee labeled spray bottles.
      • Observed food employees in kitchen area of food facility not wearing proper hair restraints.
      • Observed plates stored next to pizza oven, above microwave, and in private dining room, stored uncovered.
      • Observed cardboard lining shelf below deli slicer prep table and lining side of refrigerator next to dough prep table.
      • Observed food containers stored on shelf in warewash area that were cracked and are no longer in good repair.
      • Observed wall damage next to hot water heater and next to prep table in back storage area of food facility.

      • Person in Charge could not provide proof of training for employee health.
      • Observed food employee with gloved hands touch soiled equipment and not wash hands before continuing to prepare food.
      • Observed hand sink without paper towels to dry hands. Person in Charge replaced paper towels.
      • Observed food employee with gloved hands touch soiled equipment, wipe hands on pants, and continue to prepare food.
      • Observed microwaves soiled and damaged.

      • Food employee was observed cleaning their hands at the prep sink. Employee was instructed to wash their hands only at the handwashing sink.
      • Food employee was observed handling soiled equipment then handled clean equipment without first washing their hands.
      • Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
      • The handwashing sink located in the back of the house is not maintained so that it is accessible at all times for employee use.
      • Observed imitation crabmeat being advertised as crabmeat.
      • Observed cooked pork stored near raw beef in the prep unit by the cook line- items were relocated.
      • Food employee was observed washing their hands while having gloves on then proceeded to engage in food preparation. EHS discussed proper hand washing requirements.
      • The following TCS foods were observed cold holding at improper temperatures in the prep unit by the hot holding station: Cooked Ceviche (54'F), cut lettuce (53'F), cut tomatoes (54'F). Items were discarded. Person in charge (PIC) contacted a technician to evaluate the unit.
      • Observed clean utensils stored in a crate on the floor near the dishmachine.
      • The refrigeration unit near the hot holding station is not operating properly.
      • Observed molcajete being used to serve TCS food that is not smooth and easily cleanable.
      • The ceiling vents above the prep table and the shelving units by the dishmachine were clean dishes are stored are in need of cleaning.
      • Observed a cellular phone, a speaker, keys, covered tumblers, bag, and a sweater stored near food and equipment.

      • Observed the following areas require cleaning: 1. Floors under dish machine area 2. Walls behind 3 compartment and dish machine area 3. Ceiling and AC vents above prep table and above the reach in refrigerator

      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Paper towels were not available at back handwashing sink and downstairs bar handwashing sink. Person in Charge provided paper towels.
      • Observed raw shrimp and fish stored behind uncovered pickles in cook line prep unit. Person in Charge relocated pickles to prevent contamination.
      • Observed residue build-up on non-TCS food contact surfaces (can opener blade & soda nozzles at downstairs bar). Items were brought to warewashing area.
      • Observed unlabeled spray bottle of chemicals in downstairs bar area. Person in Charge labeled bottle with common name of chemicals.
      • Observed raw shrimp thawing in ice bath in 2-compartment sink, not an approved thawing method.
      • Observed raw shrimp stored in direct contact with undrained ice in food containers at 2-compartment sink. Shrimp was removed from ice.
      • Observed plated above prep line and at 1-door prep unit stored uncovered.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Mop sink spray hose attachment does not have a backflow prevention device provided. Water was turned off.
      • Employee restroom is not provided with a self-closing door.
      • Observed wall damage behind and next to 3-compartment sink.
  • Orofino Restaurant, 1006 Caroline Street Fredericksburg, VA 22401
    • Date inspected: July 26
    • Total violations: 10
      • Person in charge was unable to provide signed copies of the employee health policy. Through discussion with food employees they were informed and had knowledge of the foodborne illnesses and symptoms associated with foodborne illnesses. EHS provided a copy of the policy and employees signed before inspection was over.
      • Observed handwashing signs were missing in the front men and women's bathroom. Signs were provided during the inspection.
      • Observed a food employee handle dirty and clean dish with the same gloves. EHS had a discussion with food employees about proper glove usage.
      • Observed the following: 1. The blade of the can opener was heavily soiled. 2.Several cutting boards that are stained and no longer seem cleanable.
      • Observed the following cold holding at improper temperatures due to not cooling properly: 1. One container of alfredo sauce (63 degrees F). 2. One container of soup (48 degrees F). During the inspection foods were discarded for safety.
      • Observed numerous stacks of clean dishes stored on a shelf with the food contact surface facing upwards.
      • Person in charge was unable to provide a test kit for the chemical sanitizing dish machine.
      • Observed the metal wire racks throughout the kitchen holding dried foods and clean equipment were soiled to the sight and touch.
      • Observed the floor of the kitchen is damaged/torn in front of the cook line.
      • Observed less than 3 foot candles of lighting in the walk in cooler.
  • NY Grill Kabob House, 537 Warrenton Rd Fredericksburg, VA 22406
    • Date inspected: July 26
    • Total violations: 3
      • Observed covered container of raw chicken stored over packaged cucumbers, raw fish stored over covered containers of cooked spinach and chickpeas and raw sliced gyro meat stored over covered containers of tzatziki and pizza sauce in the walk in cooler. Worked with food staff to relocate the raw animal foods to prevent contamination.
      • Observed soiled food contact equipment to the sight and touch on wire shelving in the warewashing area. Instructed the person in charge to remove all equipment for cleaning and sanitizing.
      • Observed containers of tzatziki sauce, pizza sauce and cooked spinach in the walk in cooler which were not dated to identify the prep or discard date. The person in charge reported the food were prepared on 7/24/22 and placed a date label on the containers. Also observed opened container of feta cheese which was opened on 7/24/2022. A food employee placed a date label on the package during the inspection.

    • Date inspected: July 26
    • Total violations: 21
      • Absence of active managerial control observed during the inspection. Observed temperature violations, a lack of date marking, a lack of hand washing at required points during food preparation, and bare hand contact with ready to eat food, and soiled equipment.
      • No certified food protection manager (CFPM) available, based on the foods offered on the menu a CFPM is required.
      • The person in charge was unable to provide documentation of training relating to the employee health policy for this facility. Discussed requirements with the person in charge.
      • Food employee was observed handling perisomal belongings then proceeded to engage in food preparation without first washing their hands and donning gloves. Employee was asked to wash their hands.
      • A food employee was observed cleaning their hands at the three compartment sink. Employee was instructed to wash their hands only at the handwashing sink.
      • Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
      • Employee was observed handling raw dough to make tortillas, then flipping the tortillas and removing them off the grill at a ready to eat stage with their bare hands. Discussion was observed with the PIC. Employees will start using gloves and tongs.
      • Observed a pitcher and a sponge in the hand washing sink basin. Items were relocated.
      • Observed trays of raw shell eggs on the shelf in the reach in cooler stored near ready to eat vegetables. Eggs were removed.
      • Observed an employee wash utensils with soap and rinse with water, failing to properly sanitize the equipment. EHS informed the manager and the manager provided training to the employee during the inspection. items were moved to be properly sanitized.
      • The following TCS foods were observed at improper temperatures in the reach in cooler (RIC) and on the counter: 1) stored overnight RIC near the prep table cooked chicken (46'F), cooked rice (45'F), shredded cheese (45'F), cut lettuce (45' F), diced tomatoes (46' F). cooked beans (47'F); PIC discarded the items. 2) stored on the counter for almost 2 hours: cooked beef (68'F), cooked tongues (64'F), flour used for raw chicken is left at room temperature all day without being sifted (discarded); stored in the cooling unit under the prep table for less than 2 hours, raw chicken (44'F), raw beef (45'F)- items were placed in a RIC. The person in charge contacted a technician during the inspection to evaluate the units. Discussed using time as a public health control to hold the food for a period up to 4 hours or place ice in a food grade and monitor the temperature every 2 1/2 hours.
      • A date marking system is not in proper use for the prepared ready-to-eat TCS foods that is being kept for over 24 hours throughout the facility. EHS discussed the requirements of the VA Food Regulations.
      • Chlorine sanitizer solution in 3 compartment sink to later be used on food contact surfaces, was measured at greater than 200 parts per million chlorine. Solution was diluted to approximately 50 parts per million.
      • Observed a spray bottle of bug spray and roaches killing bait stored near and over vegetables and fruits on the shelving unit by the exterior door. PIC removed the items.
      • The exterior door which is located in the kitchen is not ight-fitting. This may allow the entry of insects or rodents.
      • Observed several cardboard of boxes of food stored on the floor or food stored less than 6" above the floor.
      • Observed food employees preparing food without proper hair restraints.
      • The nonfood contact surfaces of the interior of the reach in freezers and refrigerators are noted with an accumulation of grime and debris.
      • Discarded products with food residue are being stored outside of refuse containers, outside of the establishment.
      • Dumpster lids were left open.
      • Observed several missing floor tiles throughout the facility.

      • The facility does not written procedures for bodily fluid clean up.
      • Observed that the microwave requires cleaning.
      • Observed employees wearing watches while preparing drinks.

      • Observed food employee handling money, then change gloves and handle food. Food employee was informed to wash hands between tasks and before donning gloves. Food employee proceeded to wash hands and don new gloves.
      • Paper towel dispenser was observed to be empty at catering handwashing sink. Food employee supplied paper towels at handwashing sink.
      • Observed warewash, bakery, and front prep handwashing sink's were dispensing water between 80-90F, rather than 100F as required.
      • Observed salad and sandwich toppings in drive-thru and front prep and baked good in front bakery area, stored uncovered.
      • Inside of ice machine, drive-thru ice dispenser, and self-service ice dispenser were all observed with residue and are in need of cleaning.
      • Observed roasted turkey, holding at 50F in catering prep unit, for less than 2 hours. Food employee moved roasted turkey to 2-door refrigerator to cool.
      • Observed salad bowls in drive-thru area and in front prep area stored uncovered.
      • Drive-thru salad prep unit drawer observed to not be closing properly. Wall corner in bakery walk-in refrigerator observed to be damaged and is in need of repair.
      • Observed 2 unused prep units in catering prep area and 1 unused small 2-door refrigerator in conference room.
      • Observed lighting in catering prep area at 10 foot candles, rather than 50 foot candles as required.

      • The person in charge could not explain the reheating temperatures, how to sanitize the equipment, and hot holding temperatures. The person in charge was not available during the time of inspection.
      • The facility does not have a certified food protection manager.
      • The person in charge could not provide documentation of training in employee health. EHS provide a document that could be used.
      • The PIC could not provide procedures for cleaning up bodily fluid accident.
      • Observed that there no paper towels at the hand sink. The PIC supplied paper towels.
      • Observed that there is no cold water at the hand sink - making it hard to wash hands properly.
      • Observed that the hot holding trays are in need of cleaning. The EHS placed the trays in the 3-compartment sink.

      • Facility unable to provide certified food manager certificate.
      • Observed food employee with gloved hands touch soiled equipment and attempt to prepare food with same gloved hands. Food employee discarded gloves, washed their hands, and changed gloves for food prep.
      • Observed the following: x3 spatulas damaged. Person in Charge voluntarily discarded x5 ladle's soiled.
      • Observed small flying pests in kitchen area.
      • Observed that food and non-food contact surfaces were not being allowed to air dry before being stacked and stored.
      • Observed racks in the two door reach-in cooler are in need of repair.
      • Observed facility without correct test strips for sanitizer.
      • Observed that non-food contact surfaces are soiled.
      • Observed the following: Leaking faucet on 3 compartment sink. Hand sinks are slow to drain.
      • Observed that the facility is soiled.
