Fredericksburg Health Inspections: Water's End Brewery, Hissho Sushi

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Oct. 18 and Oct. 24.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Taste of Spice, 9819 Patriot Highway Fredericksburg, VA 22407
    • Date inspected: Oct. 18
    • Total violations: 1
      • Upon arrival to conduct a follow-up inspection, observed absence of adequate hot water in the food establishment to meet peak demand needs. Observed the following hot water temperatures in the kitchen: Cookline handsink (70 degrees F), warewashing handsink (78 degrees F) and 3 vat sink (79 degrees F). Instructed the operator water will need to be heated to 110 degrees on the range for wash and rinse basins and sanitizer solution shall be at least 75 degrees F. Advised the person in charge to fill the wash and rinse basins approximately halfway with water from the 3 vat sink and heat water on the range to achieve at least 110 degrees F, monitoring the water temperature with a calibrated food thermometer. During the inspection the operator contacted a plumber to evaluate the hot water supply. Hot water heater is set to 150 degrees F.
  • Lin's Gourmet (Ladysmith), 18045 US-1 Ruther Glen, VA 22546
    • Date inspected: Oct. 18
    • Total violations: 5
      • The interior of the microwave oven was o served soiled. Oven was cleaned during inspection.
      • Observed bulk food storage bins at the rear of the kitchen that were not labeled. Bins were labeled during inspection.
      • Observed several employees in the kitchen preparing food without hair restraints.
      • The top shelf of the expo station and the top of the smoker was observed heavily soiled.
      • The ceiling vents in the kitchen as well as the ceiling tiles and the light fixture covers around the are in need of cleaning.
  • Subway, 5769 Plank Rd Fredericksburg, VA 22407
    • Date inspected: Oct. 18
    • Total violations: 1
      • Facility could not provide clean-up procedures for responding to vomiting and diarrheal events. EHS provide a sample to the person in charge.
  • Papa John's Pizza, 570 Celebrate Virginia Parkway Suite #105 Fredericksburg, VA 22406
    • Date inspected: Oct. 18
    • Total violations: 2
      • Observed two sanitizing spray bottles next to single use articles on a prep table. Spray bottles were relocated.
      • Observed shelving throughout the kitchen in need of cleaning.
  • Jeff Rouse Swim and Sport Center, 1600 Mine Rd Stafford, VA 22554
    • Date inspected: Oct. 18
    • Total violations: 9
      • Observed the microwave on the cook line soiled.
      • Low temperature dishmachine reading 0ppm after running several cycles.
      • Observed the can opener blade soiled. Item was removed to the three compartment sink for cleaning.
      • Observed several stainless steel containers with debris on them. Items were removed to the three compartment sink for cleaning.
      • Observed a spray bottle containing bleach stored next to plastic containers and a disinfectant spray bottle hanging from a shelve where food prep is conducted underneath. Both spray bottles were relocated.
      • The temperature gauge of the dish machine is not working after several cycles.
      • Test strips for sanitizer expired in the year of 2020.
      • Observed the following in need of cleaning: 1) shelving throughout the kitchen. 2) up splash and backslash of the soda machine. 3) fryers. 4) prep cooler unit.
      • Observed the following in need of cleaning: 1) flooring underneath equipment. 2) walls. 3) mop sink basin. 4) floor drains.
  • Anderson Center (Concession Stand), 1301 College Ave Fredericksburg, VA 22401
    • Date inspected: Oct. 19
    • Total violations: 1
      • Observed the non food contact surfaces for the reach in freezer, refrigerator, and the hot holding unit in need of cleaning.
  • Vocelli Pizza, 1301 College Ave. Eagles Nest Fredericksburg, VA 22401
    • Date inspected: Oct. 19
    • Total violations: 6
      • Person in charge was unable to provide a verifiable way that shows employees have been trained on foodborne illnesses and symptoms related. EHS provided a written copy.
      • Person in charge could not provide written procedures for how the facility tends to bodily fluid clean up procedures. EHS provided a copy.
      • Per discussion with the person in charge the pizza pans do not get cleaned at least once every 24 hours.
      • Observed a food employee working without a beard restraint.
      • Observed cutting boards that are heavily scratched and scored.
      • Person in charge could not provide a test kit for the sink and surface sanitizer used at the 3 compartment sink.
  • Pizza Hut #1502, 35 Walpole Street #101 Stafford, VA 22554
    • Date inspected: Oct. 19
    • Total violations: 5
      • Person in charge could not provide documentation of training of the employee health policy. EHS provided documentation.
      • Observed many stainless steel containers soiled with debris. Items were relocated to the 3 compartment sink for cleaning.
      • Observed the following in need of cleaning: 1) interior and exterior of all equipment. 2) shelving throughout the kitchen. 3) encrusted sheet pans. 4) upslash of the soda machine.
      • Observed the following in need of cleaning: 1) floors underneath equipment. 2) ceiling tiles and vents. 3)walls. 4) floor drains. 5) mop sink basin.
      • Observed hood vents over the fryers in need of cleaning.
  • Hissho Sushi, 1301 College Ave Fredericksburg, VA 22401
    • Date inspected: Oct. 19
    • Total violations: 10
      • Person in charge was unable to provide a verifiable employee health policy. EHS provided a copy of an employee health policy.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • No parasite destruction records maintained for raw, raw marinated, partially cooked or marinated-partially cooked fish that are served or sold in ready-to-eat form.
      • Observed a container of raw shrimp stored above a container of raw chicken and stored near a container of ready to eat vegetables in the reach in cooler. Employee was instructed to re-locate the items.
      • Observed the following: 1) several stainless steel containers stored on the shelf by the cookline that were soiled to the touch and sight.
      • Observed the following hot holding at improper temperature for over 2 hours: 1) two large stainless steel containers of cooked rice (129-133 degrees F) in the hot holding box. 2) a container of cooked chicken on the display line (109 degrees F). Items were heated to 165 degrees F.
      • Observed the following items cold holding at improper temperatures: 1) a container of raw chicken (45-47 degrees F)- chicken was discarded due to being left at the unit overnight. 2) a container of cut cabbage (45 degrees F) that was stored in the cold holding display unit for about 2 hours- container was relocated to the RIC.
      • The person in charge was not able to provide records to indicate compliance is being met with regards to specialized food process.
      • There was no temperature measuring device located in the reach two cold holding units. PIC will purchase new thermometers.
      • No valid test kit available to test the lactic acid sanitizer solution.
  • Courtland Elementary School, 6601 Smith Station Road Spotsylvania, VA 22553
    • Date inspected: Oct. 19
    • Total violations: 3
      • The ceiling tiles near the rear door of the kitchen were observed with water damage.
      • Observed ceiling tiles above the back door of the kitchen that were not smooth and easily cleanable.
      • Observed that the ceiling air intake vents and the ceiling tiles around the HVAC vents were covered with dust.

      • Observed the non food contact surfaces for the reach in freezer, refrigerator, and the hot holding unit in need of cleaning.
  • Panera Bread at University of Mary Washington, 1301 College Ave. Fredericksburg, VA 22401
    • Date inspected: Oct. 19
    • Total violations: 3
      • Observed the inside surfaces of the microwaves in need of cleaning.
      • Observed food employees working while wearing wrist jewelry.
      • Observed several cutting boards that were stained and scored.
  • Himalayan Heritage VA, 10336 Spotsylvania Avenue Fredericksburg, VA 22408
    • Date inspected: Oct. 19
    • Total violations: 3
      • Facility could not provide documents on employee health policy. EHS provided documents to the person in charge.
      • Observed low temp dishwasher sanitizing at 10ppm after five cycles ran. Observed sanitizer tubing behind the dishwasher leaking which could be contributing to sanitizer issue.
      • Person in Charge was unable to provide test strips for chlorine sanitizer.
  • Chancellor High School, 6320 Harrison Road Fredericksburg, VA 22407
    • Date inspected: Oct. 20
    • Total violations: 2
      • Facility could not provide procedures for responding to vomiting and diarrheal events. EHS provided a sample to the person in charge.
      • Observed the following pieces of equipment that are non-functioning and unused. 1. Two large floor mixers 2. Two door oven 3. Tilt skillet Through discussion with the person in charge they have contacted management numerous times about removing the equipment from the facility.
  • Arby's, 23720 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: Oct. 24
    • Total violations: 6
      • Observed that the feeder hoses on the shake/ice cream machine were full of unknown particles. Machine should not be used until a service call is made and these tubes have been either cleaned or replaced.
      • Observed that the ice chutes on the soda dispenser in the dining room were soiled.
      • Observed many flying insects throughout the kitchen including flies and gnats.
      • The following non-food contact surfaces were noted in need of cleaning: 1- the top of the convection oven, 2- the fan covers in the walk in cooler, 3- the interior of the liquid cheese dispenser. IN THE GENERAL, THE OUTER SURFACES OF ALL EQUIPMENT IN THE KITCHEN IS IN NEED OF A THOROUGH CLEANING.
      • Observed several light covers hanging down from the light fixtures.
      • The following physical facility items were observed soiled and in need of cleaning: 1- The floors in the walk in cooler and freezer, 2- the floors throughout the kitchen especially under the equipment and shelving.
  • Winding Creek Elementary School, 475 Winding Creek Road Stafford, VA 22554
    • Date inspected: Oct. 24
    • Total violations: 2
      • The high temperature dish machine is not reaching its minimum requirement of 160 degrees F of the wash cycle. After several cycles, VDH thermometer read 152 degrees F and temperature gauge of the wash cycle read 158 degrees F.
      • The temperature gauge for the rinse cycle of the high temperature dish machine is 0 degrees F after running several cycles.
  • McDonalds, 23781 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: Oct. 24
    • Total violations: 2
      • Observed the following non-food non-contact surfaces in need of cleaning: 1- inside of the burger freezer, 2- the floor fan, 3- the drink dispenser backsplash and counter in the dining room, 4- the outside of the hot holding drawers.
      • The floors in the walk in freezer and walk in cooler are in need of cleaning.
  • Dunkin Donuts/Baskin Robbins, 1800 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Oct. 24
    • Total violations: 5
      • Observed food service worker put sanitizer solution in the wiping cloth bucket, handle various other equipment, and then proceed to go back to decorating donuts. I spoke to the worker and they removed their gloves, washed their hands, and reapplied the gloves.
      • Observed the following: 1. ice bucket stored hanging on the chemical storage rack; 2. a spray bottle of bleach cleaner next to a thermometer and paper wrapped straws; 3. a spray bottle of bleach sitting next to coffee syrups. The ice bucket was moved to another location and the spray bottles were relocated to the chemical storage rack.
      • Observed the only thermometer available was not a thin probe thermometer.
      • Observed a large number of flying insects in the kitchen and in the dining area. The person in charge spoke to Walmart Lead about getting the air curtain fixed. The Lead put in a work order. The person in charge has added a plastic curtain on the donut case and is keeping the other foods covered to try to control the issue and EcoLab was out 10-14-23.
      • Observed the pipe attached to the back of the ice machine and connected to the ceiling with a large amount of white tape wrapped around the part connected to the ceiling and the curved part near the ice machine.
  • Water's End Brewery, 525 Caroline St Fredericksburg, VA 22401
    • Date inspected: Oct. 24
    • Total violations: 12
      • The facility does not have a person that is a Certified Food Protection Manager.
      • Observed three containers of sauces on a steam table uncovered. During the inspection person in charge provided lids for the sauces.
      • Observed the blade of the can opener soiled. During the inspection the can opener was relocated to the ware washing area.
      • Observed the griddle at the cook line heavy soiled with grease and grime build up.
      • Observed a bottle of roach and ant Raid on a storage shelf next to gloves and single service use articles. During the inspection the person in charge discarded the Raid. Also discussed with the person in charge to ensure and contact pest control if needed.
      • Observed the following non food contact surfaces in need of cleaning: 1.inside surfaces of all reach in coolers and freezers 2. shelving throughout the facility 3. sides of equipment at the cook line 4. inside the fryer cabinets.
      • Observed a direct connection in plumbing under the 3 compartment sink. No air gap was observed and is needed. Discussed with the person in charge.
      • Observed a leaky faucet at the 3 compartment sink.
      • Observed the following facilities in need of cleaning: 1. walls behind food prep areas 2. floors behind and around the cook line equipment 3. dusty ceiling tiles in the bathrooms 4. window sills
      • The following facilities are in need of repair or replacing: 1. coving next to fry area and in the dish room area 2. floor tiles peeling and soft to the step in front of the cook area 3. caulking at the 3 compartment sink, kitchen handsink and bathroom handsinks. 4. ceiling tiles missing or loose in the downstairs/basement area.
      • Observed a lot unnecessary and unused equipment in the facility. Discussed with the person in charge to remove all equipment ( and foods from prior permitted to the facility) that will not be used.
      • Observed employees personal belongings (coats, backpack, speaker, water bottle) near food prep and or above stored single service use articles.
