Fredericksburg Restaurant Inspections: 15 Violations At 1 Place

15 restaurants in the Fredericksburg area had violations during recent inspections, according to the Rappahannock Health District.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected several restaurants across the region from Aug. 26 to Aug. 31; they found more than a dozen restaurants with violations.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Most restaurants receive at least one violation during their routine inspections, and this list does not include facilities that were not cited for violations.

  • Panera Bread University of Mary Washington / Sodexo, 1301 College Ave. Fredericksburg, VA 22401
    • Date inspected: Aug. 26
    • Total violations: 6
      • The light intensity in the walk in cooler is between 0.7-7 foot candles.
      • The dishwasher does not have a visual or audible alarm to signal when sanitizer is being delivered.
      • Observed the condenser dripping and ice build up in the walk freezer.
      • Observed a food employee using a wiping cloth to wash dishes at the 3 compartment sink.
      • Observed a food employee wearing a bracelet.
      • The following were observed: 1.The sanitizer at the 3 compartment sink is not being pulled through the tubing. During the inspection the person in charge manually prepared the sanitizer to the proper concentration. 2. The sanitizer in the dishwasher is not being pulled into the dishwashing, therefore equipment and utensils are not being sanitized. During the inspection the person in charge replaced sanitizer bucket.
  • James Monroe High School, 2300 Washington Avenue Fredericksburg, VA 22401
    • Date inspected: Aug. 26
    • Total violations: 5
      • Observed 7 foot candles of lighting in the employee bathroom.
      • Observed the following soiled and in need of cleaning: ceiling and walls of the dish room area, ceiling vents in the employee bathroom, and the hallway area over the single service articles.
      • Observed the following in disrepair: missing two ceiling tiles in the dry storage room, and two lights out under the hood vents.
      • Observed soiled caulking at the pre-rinse sink and a rusted and chipped hook above the drain boards.
      • Observed the interior surface of the ice machine soiled.
  • Chick-Fil-A at Southpoint, 9900 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Aug. 26
    • Total violations: 5
      • Observed long hose with a spray nozzle attached and water dispensing when engaged, at the mop sink without any backflow prevention device installed. The water was turned off and spray nozzle removed during the inspection. Provided backflow prevention handout to install dual check valve with atmospheric vent to prevent contamination. Instructed the person in charge to remove the spray nozzle and turn off the water when not in use.
      • Observed plastic containers on the warewashing shelving area which were stacked while wet after sanitizing.
      • Observed improper cooling methods to rapidly cool cut lettuce. The food establishment is cooling cut lettuce, which is washed and cut in house, in a deep and covered plastic container in reach in and walk in refrigeration units. Discussed methods to rapidly cool cut lettuce including placing lettuce in smaller uncovered or vented containers and using ice baths. During the inspection the person in charge directed a food employee to place lettuce in smaller containers on ice and relocate to the walk in cooler.
      • Observed the following TCS foods cold holding at improper temperatures: 1- 2 door reach in cooler- Cut lettuce and egg for salads (43, 44 degrees F ), 2- 2 door reach in cooler #2- Cartons of pasteurized liquid eggs (46 degrees F), cartons of pasteurized scrambled eggs (44 degrees F) and pre-cooked chicken (44 degrees F) and 3- 2 door reach in cooler #2- Front - yogurt parfait ( 44 degrees F) and salads (45, 50 degrees F). The person in charge reported the pasteurized liquid eggs were placed in the units at 8:00 a.m. and were relocated to the walk in cooler to cool by 12:00 p.m. The person in charge reported the parfaits were placed the refrigeration unit at 5:30 a.m. and temperature was taken at 11:15 a.m. and the salads and cooked chicken have been in the refrigeration units overnight. The person in charge voluntarily discarded the foods for safety.
      • Observed approximately 3-4 large covered plastic containers of cut lettuce in the walk in cooler and 2 door reach in cooler #2 (48, 45 degrees F). The person in charge reported the lettuce was cut at 7:30 - 8:00 a.m. The person in charge directed a food employee to separate lettuce into smaller portions and place on ice in the walk in cooler. Temperature was observed at 44 and 45 degrees F at 12:20 p.m. TCS foods were not cooled appropriately within four hours. The person in charge voluntarily discarded cut lettuce for safety.
  • Sam’s Pizza, 3996 Lafayette Blvd Fredericksburg, VA 22408
    • Date inspected: Aug. 26
    • Total violations: 4
      • Observed damaged cans of tomato sauce and olives stored with the foods that will be sold and/or prepared. The person in charge relocated the cans.
      • Observed that the interior of the ice machine requires cleaning.
      • Observed chemicals stored with the single service items and with the bread. The person in charge relocated the chemicals.
      • Observed that the air vent in the employee's restroom is coming off the ceiling.
  • A Taste of Elegance, LLC, 11513 Tidewater Trail Fredericksburg, VA 22408
    • Date inspected: Aug. 26
    • Total violations: 3
      • Front door is not self-closing.
      • There is not a covered garbage can in the employee restroom.
      • Several ceiling lights in the facility are not working.
  • Asian Diner Restaurant, 10048 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Aug. 28
    • Total violations: 15
      • The person in charge could not provide written documentation of employee health policy. EHS provided a copy of the employee health policy.
      • Observed a food employee washing their hands at the 3 compartment sink. EHS discussed proper handwashing with the person in charge.
      • Observed raw animal food and eggs stored over ready-to-eat food in the walk in cooler.
      • Observed a food employee wash their single service gloves.
      • Observed uncovered foods in the sandwich prep units, walk in cooler, and reach in refrigerator. The person in charge instructed the employee to cover the foods.
      • Observed a large container of chicken broth with chicken breasts stored on top of the stove to cool. The person in charge (PIC) stated that the container was left out overnight to cool. The PIC discarded the chicken and broth. EHS provided cooling log.
      • Observed bean sprouts at 48'F and shrimp at 57'F in the walk in cooler.
      • Observed spray bottles of cleaner stored with the single service equipment. The person in charge relocated the bottles.
      • The facility is using large pots to cool chicken in broth and leaving it out overnight. EHS discussed cooling methods with the PIC.
      • Observed a container of shrimp at 65'F sitting in standing water on the counter. The PIC placed the shrimp in a colander under running water.
      • Observed several containers stacked wet on top of each other. The PIC removed the containers to be rewashed, rinsed and sanitized then air dried.
      • Observed the door handle to the WIC is broken and the walk in cooler is not holding the cold foods at the required temperatures.
      • Observed dumplings stored in a cardboard box in the reach in cooler. The PIC discarded the items. Observed that rice bags are being used in the rice cooker.
      • The following physical facilities need cleaning: 1. hood filters. 2. the floors under the equipment. 3. the WIC floor. 4. the walls throughout the kitchen.
      • Observed employees personal belongings stored with food items. The PIC relocated the personal items
  • Walker Grant Center, 210 Ferdinand Street Fredericksburg, VA 22401
    • Date inspected: Aug. 30
    • Total violations: 3
      • No handwashing sign at hand sink in dishwashing room. During inspection, a sign was posted at the hand sink.
      • Observed 2 food employees working without hair restraints. During inspection, the food employees applied hairnets.
      • The following equipment are not holding proper temperature: 1. Observed water dripping onto the top shelf in the 3 door reach in cooler. 2. The milk box next to the hot holding table.
  • Foode, 900 Princess Anne Street Fredericksburg, VA 22401
    • Date inspected: Aug. 30
    • Total violations: 8
      • No handwashing sign at the server station hand sink. During inspection, a sign was posted at the hand sink.
      • Observed a plastic container with sliced beef brisket (45°F - 51°F) in the glass door reach in cooler that was prepared on 08/28/21. During the inspection, the person in charge discarded food.
      • Observed 2 food employees working without hair restraints. During the inspection, one food employee applied a beard guard.
      • The following equipment are in need of repair: 1. A/C unit in main kitchen. 2. The door on the salad prep unit.
      • No temperature measuring device for high temp dishwasher.
      • No covered receptacle in the employee restroom. During inspection, the person in charge replaced with a covered receptacle.
      • The following are in need of repair: 1. One missing ceiling tile in the main kitchen. 2. Two missing ceiling tile in the basement kitchen. 3. One light out in the kitchen basement.
      • The light intensity in the walk in cooler is between 2.2 - 6.3 foot candles.
  • Wildcat Cafe, 2135 Mountain View Dr. Stafford, VA 22556
    • Date inspected: Aug. 30
    • Total violations: 2
      • Observed accumulations of debris on the following nonfood contact surfaces: Base of the oven, shelf under the charbroiler, handle/lever of the smoker and pre-rinse spray nozzle.
      • The ceiling tiles in the kitchen do not seal completely to the frames, observed gaps at the corners and the ceiling tile next to the dishwasher is protruding from the ceiling frame.
  • Pancho Villa Mexican Restaurant, 155 Garrisonville Road Ste. 109 Stafford, VA 22554
    • Date inspected: Aug. 30
    • Total violations: 9
      • Observed the following food contact surfaces soiled: 1. The interior surfaces of the ice machine. 2. Knife stored between equipment (prep table and cooling unit). 3. Four knives stored on the magnetic strip. During the inspection the knives were removed to the 3 compartment sink area for rewashing.
      • Observed the interior surfaces of the microwave soiled.
      • Observed plastic bags of yellow rice in the walk in cooler, cold holding at an improper temperature of 51 degrees F. EHS advised the person in charge to discard for safety.
      • Observed wetstacking of stainless steel containers stored on a shelf.
      • Observed manufactured tape not removed from a food prep unit.
      • Observed the upsplash of the coffee maker soiled.
      • Observed plastic crates around the dumpster area.
      • The following are in need of cleaning: 1.Floors underneath equipment of the cooks line. 2. Ceiling tiles and vents of the kitchen.
      • Observed the hood vent filters in disrepair.
  • Stafford Kabob Room, 2777 Jefferson Davis Highway #113 Stafford, VA 22554
    • Date inspected: Aug. 30
    • Total violations: 3
      • Observed no hand soap at the hand sink. The PIC provided hand soap.
      • Observed no paper towels at the hand sink. The person in charge provided paper towels.
      • Observed that the microwave requires cleaning.
  • Essential Christian Academy, LLC, 628 Cambridge Street Suite # 101 Fredericksburg, VA 22405
    • Date inspected: Aug. 30
    • Total violations: 1
      • The person in charge could not provide the employee health policy during the inspection. Food employees are knowledgeable of health reporting requirements. Provided sample employee health policy during the inspection.

      • Observed the ceiling in the toilet rooms and dry storage of food are absorbent and not easily cleanable.
  • The Timbers Restaurant, 8026 Prosperity Way Ruther Glen, VA 22546

    • Total violations: 8
      • The interior non-TCS food contact surface of the ice machine were observed soiled.
      • Observed the following food contact surfaces soiled: 1-all of the cutting boards throughout the kitchen, 2-all of the equipment on the narrow wire shelf near the dish machine
      • ROP packed not thawed properly. Observed ROP fish thawed in their package without cutting open the bag or removing the fish from the package.
      • Observed many live flying insects throughout the kitchen.
      • Observed a soiled extension draped around a shelving unit holding clean equipment in the dishroom.
      • The following nonfood contact surfaces have accumulations of grime and debris: 1-all of the plastic storage drawers in the kitchen, 2-the black metal shelf in the kitchen, 3-the outer surfaces of the ice machine, 4-the inner surfaces of the homestyle freezer and reach in cooler, 5-the top of the same units, 6-the trash can and lid in the dishroom, 7-the covers of the condenser fans in the walk in cooler, 8-the plastic cart in the dishroom, 9-the narrow wire shelf near the dish machine, 10-the inner and outer surfaces of the dish machine, 11-the back door, 12-the plastic squeeze bottles on the cook's line, 13-the sides of the cooking equipment on the cook's line.
      • The following physical facilities are not cleaned as often as necessary to keep them clean: 1-the walls throughout the kitchen, 2-the floors in the dishroom and under the equipment and shelving throughout the kitchen, 3-the light covers throughout the kitchen, 4-the walls near the mop sink, 5-all of the walls throughout the kitchen, 6-the ceiling tiles near the air ducts in the kitchen, 7-the stainless steel walls on the cook's line..
      • Intake and exhaust air ducts are not being cleaned. the filters in the hood ventilation vents on the cook's line and over the pizza oven are soiled.
  • Tarntip Thai Cuisine, 507 Jefferson Davis Highway Fredericksburg, VA 22401
    • Date inspected: Aug. 31
    • Total violations: 12
      • Observed 3 soiled knives hanging on the metal wall strip. Knives were removed to the 3 compartment sink for handwashing
      • Observed a carton of half and half cold holding at 47 degrees. During the inspection the person in charge discarded for safety.
      • Observed no date marking being used in the facility.
      • Observed a bottle of clorox wipes and a bottle of hand sanitizer in the food prep area. During the inspection the person in charge removed these items.
      • Observed a gap at the back door.
      • Observed boxes of food being stored on the floor of the walk in freezer.
      • Observed peanuts and a prepared batter stored in the container not original to the food item.
      • Observed shelving used throughout the facility to store equipment and food in the walk in cooler, rusted heavily.
      • Observed a cut eggplant stored in a grocery bag.
      • Observed non food contact surfaces of equipment sticky to the touch.
      • Observed the following in disrepair: 1.Loose coving tiles by fryer 2.Ceiling tiles in a corner of the kitchen 3. A row of ceiling tiles with apparent water staining 4. Chipping of paint on both sided of the urinal stall.
      • Observed employees personal belongings (coffee mug, water bottles, a bag with belongings) stored near food prep areas.
