Fredericksburg Restaurant Inspections: 31 Restaurants Cited
More than 30 restaurants in the Fredericksburg area had violations during recent inspections, according to the Rappahannock Health District.
FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected several restaurants across the region from Aug. 18 to Aug. 25; they found more than two dozen restaurants with violations.
As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Most restaurants receive at least one violation during their routine inspections, and this list does not include facilities that were not cited for violations.

- Observed accumulation of food debris on the rack inside the salamander broiler and interior surfaces of the left side microwave located above the ice cream deep freezer. The person in charge instructed employees to clean the microwave oven and remove the salamander rack for cleaning.

- Observed stainless steel bowls and rectangular containers used to store food stacked while wet after cleaning and sanitizing.
- Observed the following in poor repair: Missing vent filters on the right side of the hood system, ceiling tiles above the dry storage area is not securely attached to the frame, grout in the floor tiles has become eroded in sections of the kitchen.

- Observed food employee lather their hands for less than 10 seconds prior to rinsing under water. Discussed correct handwashing procedures and instructed food employee to wash their hands.
- Observed hot water in the female toilet room at 84 degrees F.
- Observed green peppers stored inside a plastic shopping bag in the walk in cooler. The person in charge removed peppers from the shopping bag and placed then in a plastic storage container.
- Observed 2.8 foot candles of light inside the men's toilet room. Observed ceiling lights blown out or dimmed.

- Person in charge was unable to provide an employee health policy.
- Observed a food employee handling raw chicken with gloved hand, switch tasks without washing their hands.
- Observed no hand soap available to wash hands at the handsink. Person in charge provided soap.
- Observed no paper towels available at the kitchen handsink (corrected) and in the bathrooms.
- Observed raw foods (chicken and lamb) over RTE foods. Person in charge rearranged foods during the inspection.
- Observed food containers of cilantro, lemons, and salad uncovered in the reach in cooler. During the inspection the person in charge covered the items.
- Observed the following soiled food contact surfaces stored with clean food contact surfaces: plastic bowls, knives on the metal knife rack and tongs hanging above 3 compartment sink and food dicer. Soiled equipment was removed for washing during the inspection.
- Observed the interior surfaces of the microwave heavily soiled.
- Observed a gallon of milk with expiration date of 8/23. During the inspection the person in charge discarded the milk.
- Observed small flying insects in the food storage area.
- Observed wet wiping cloths stored under the food prep table, by the grll and stove top.
- Observed reusing of manufactured jars for spices.
- Observed the gaskets and interior of the coolers soiled.
- Observed the trash and debris in the dumpster area.
- Observed the ceiling tiles of the bathroom stained.
- Observed the 30 foot candles of light in the food prep area.

- Observed two heavily stained cutting boards on the cooks line and a soiled potato cutter. Equipment was taken to the dish machine to be cleaned.
- Observed many of what appeared to be fruit flies throughout the kitchen.
- Observed in use serving utensils store in water measuring 115 degrees. Temperature of water was corrected to 145 degrees during inspection
- Several insert pans were observed stacked while wet after chemical sanitization.

- The walk in freezer door heat strip is not working, resulting in a build up of ice that is preventing the freezer door from closing tightly.
- Observed a non functioning ceiling light fixture in the dishroom.
- Less than 10 foot candles of light was noted in the walk in cooler. Observed 0.6 foot candles in this area.

- Observed absence of hand drying provision at the front and warewashing handsinks. The person in charge placed disposable paper towels at the handsinks.
- Observed the absence of hand soap at the warewashing hand sink. The person in charge reported hand soap was not available in the facility and placed dish washing detergent at the sink until hand soap is obtained. Discussed instructing an employee to purchase a hand soap dispenser by close of business.
- Observed a food employee place raw bacon on the flat top wearing single use disposable gloves and then proceed to handle cooking utensils at the cook line without removing gloves and washing their hands. Instructed the food employee to remove gloves and wash their hands. Discussed correct glove use with the person in charge during the inspection.
- Observed the can opener soiled and the person in charge reported soda and ice tea dispenser nozzles are removed for cleaning and sanitizing every other day. The can opener was removed to the three compartment sink during the inspection.
- Observed food contact surfaces of the only meat slicer in the facility and the interior of the blender container soiled. The person in charge reported the meat slicer was not used during the morning hours. Instructed the person in charge to clean and sanitize the meat slicer prior to next use. The blender was removed to the three compartment sink for cleaning and sanitizing.
- Observed in use chlorine sanitizing solution at the three compartment sink at 25 ppm and 95 degrees F, instructed the person in charge to discard sanitizing solution and remove the food contact equipment. The person in charge instructed a food employee to drain the sink and retitrate the sanitizing solution.
- Observed the enchilada sauce in the hot holding steam well at 158 degrees F. The person in charge reported the enchilada sauce was pulled from the walk in cooler at 9:30 am and temperature was observed at 11:35 am. The person in charge voluntarily discarded the enchilada sauce for safety.
- Observed cut tomatoes (47 degrees F), cold holding at improper temperatures in prep cooler 1. The person in charge reported the tomatoes had been in the unit overnight. The person in charge voluntarily discarded the tomatoes for safety.
- The food establishment does not have a date marking system to identify the date of preparation or disposal for ready to eat foods in the food establishment. During the inspection the person in charge placed date labels on ready to eat foods prepared in the food establishment .Provided date marking handout to the operator.
- The food establishment does not have a date marking and dispostion system to identify date by which ready to eat TCS foods shall be discarded. Worked with the person in charge to date mark TCS foods during the inspection.
- Absence of ambient air thermometers observed in prep cooler 1, prep cooler 2 and pepsi cooler.
- Observed food employees working in the kitchen without hair restraints.
- Observed the three compartment sink and one compartment sink are not sealed to the adjacent wall.
- Observed plastic shopping bag being used in direct contact with tortillas. The plastic bag was discarded during the inspection.
- Absence of alarm or line of sight on the dishwasher to inform of low sanitizer or detergent levels.
- Observed accumulation of debris on the following non food contact surfaces: Interior and gaskets of refrigeration equipment and spray nozzle next to the three compartment sink.
- Observed ceiling tiles in the dry storage area are absorbent and not easily cleanable. Observed ceiling tiles soiled.
- Observed the following in poor repair: Damaged ceiling tiles in the kitchen, observed ceiling tiles starting to peel or scratched. Ceiling vent in the mens toilet room is not secured to the ceiling.

- Observed containers of food sitting inside of each other in the sandwich prep unit. The person in charge separated the foods and covered each one. Observed pooled egg and baked potatoes uncovered. The person in charge covered the items.

- Observed pupusia mix, sliced ham, pico de gallo cooked beans, and cooked rice that were not dated.
- Observed chemical spray bottles stored with the single service straws and on the hand sink. The person in charge removed the spray bottles.

- The following were observed in need of cleaning: Walls and floor in the warewashing area and ceiling vents in the toilet rooms.
- Accumulations of debris were noted on the following nonfood contact surfaces: Interior cabinet under the front handsink. container used to store various utensils on the warewashing shelving, backsplash of the juice machine, spill under the soda machine cabinet.
- Observed equipment used to fill juice machines with water stored inside the cabinet under the soda machine which may be exposed to contamination. Instructed the person in charge to clean and sanitize the equipment prior to next use and store in a way which will prevent cross contamination. Discussed wrapping or storing the connected in a covered container to prevent contamination. Also observed single use packets of sugar and disposable cups and carry out sauce containers stored under the plumbing connections of the front handsink. The person in charge voluntarily discarded the articles for safety.
- Observed the back door to the food establishment does not adequately seal at the bottom, observed light coming through the bottom right side of the door.

- A food employee was observed handling phone while decorating a cake. EHS discussed observation with the person in charge and advised the PIC to speak with the food employee.
- The following were observed cold holding at improper temperatures: tomatoes 52°F, American cheese 45°F, Swiss cheese 50°F, chili 44°F, sliced tomatoes 46°F. EHS discussed observation with the PIC and advised not to overfill containers and adjust the unit internal temperature to 37°F degrees. Food outside of temperature control held longer than 4 hours and were subsequently voluntarily discarded.
- The walk-in freezer door was noted with accumulation of froze surrounding the door.
- Caulking was noted damaged at the 3-compartment sink.
- Trash bags were noted around the dumpster area.

- Observed a food employee lather their hands under running water at the kitchen handsink. Employee hands were not effectively cleaned due to handsoap being flushed down the drain. Instructed the food employee to wash their hands correctly, explaining the correct way to lather their hands.
- Observed hot water at the handsinks in the left and right toilet rooms in the dining rooms at 88 and 86 degrees, respectively.
- Observed the low temperature dishwasher sanitizing food contact equipment exceeding 200 parts per million at 110 degrees F when measured on irreversible registering temperature indicator plate. Instructed the person in charge to discontinue use, set up the three compartment sink to wash, rinse and sanitize food contact equipment and utensils and contact a technician for repair.
- The wash and final rinse temperature gauges on the low temperature dishwasher are broken, temperature gauges do not move during operation of the dishwasher.
- Observed the ceiling in the toilet rooms and dry storage of food are absorbent and not easily cleanable.

- Observed that the ice machine and panini press requires cleaning.

- Observed all shelving rusted in the dish washing room. The caulking at the pre-rinse sink and the mop sink are soiled.
- Observed numerous pizza pans heavily soiled. During the inspection the person in charge removed the pizza pans.

- Observed the interior surfaces of the microwave oven soiled. A food employee cleaned the interior of the microwave during the inspection.
- Observed the following in poor repair: Numerous blown out ceiling lights in the kitchen, soiled ceiling tiles in the hallway by the toilet rooms and dining rooms and damaged ceiling above the soda machine which appear to have mold-like growth. Observed mold-like growth on the ceiling in the front dining room which significant damage to the ceiling, Two large areas of the dining room, where significant damaged ceiling tiles are located, are blocked for guest use.

- The following are in need of repair: hole in the wall in the dry storage room and paint is peeling above the mop sink basin.

- The quaternary ammonium was measured above 400ppm. During inspection, the PIC diluted the solution to 200ppm.
- The cheese dispenser bag was not cut at a 45 degree angle. EHS advised the PIC to cut the bag for the residual cheese to drain.
- The base at the front counters were noted rotting due to the drain immediately above with an open drain system. EHS advised to repair or replace the area to prevent further growth.
- 5 lumens were measured at the dry storage area.

- Observed a food employee use a Sharpie marker to write on a board and then proceed to don single use disposable gloves to engage in a food preparation task. Instructed the food employee to remove the gloves and wash their hands and discussed handwashing requirements when there is a change in task.

- ROP salmon not thawed properly. Observed several thawed packages of salmon completely thawed in the reach in cooler. Salmon was voluntarily discarded during inspection.
