Fredericksburg Restaurant Inspections: 36 Violations At One Place

Inspectors said they found "living quarters," roaches, and 34 other violations during a recent inspection at a restaurant in Stafford.

FREDERICKSBURG, VA — During the most recent round of restaurant inspections in the Fredericksburg area, 22 different locations were cited with violations. The inspections took place between Nov. 10 and Nov. 16.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

      • Observed the following TCS foods cold holding at improper temperatures: Plat nacho cooler: Diced tomoatoes (47 degrees F), Cut lettuce (44 degrees F) and Shredded cheese (50 degrees F). The person in charge reported the TCS foods were pulled from the walk in cooler and placed in the unit at 11:00 a.m. The facility will use time as a public health control to use time, in lieu of temperature control, for TCS foods in the unit. The person in charge placed a sign at the unit to discard TCS foods at 3:00 p.m. The person in charge reported a technician has serviced the machine. Also observed sliced cheese (43, 45 degrees F) cold holding at improper temperatures in the fryer drawer cooler. The person in charge reported the cheese has been in the unit overnight and voluntarily discarded for safety. Discussed lowering the temperature of the unit to 38 degrees F or less to ensure adequate cold holding
      • Observed stainless steel containers and trays stacked while wet after sanitizing on shelving at the warewashing area.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the high temperature dishwasher. The person in charge reported the equipment was on order.

    • Date inspected: Nov. 12
    • Total violations: 20
      • Improper handwashing procedures observed. A food employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.

      • Observed several food employees change tasks and proceeded to engage in food preparation without first washing theirs hands and donning gloves. Proper hand washing procedures were discussed with the person in charge.
      • Observed uncovered containers of food stored in the walk-in cooler. The items were covered and re-located during the inspection.
      • Observed raw meat and shelled eggs stored on top of ready to eat food
      • Observed the interior of the microwave and toaster oven in need of cleaning.
      • Observed 1 lid and 2 knives that were soiled to the sight and touch: several lids, knives. Items were removed to be washed, rinsed and sanitized.
      • Observed that the can opener is soiled. The person in charge moved the item to be cleaned.
      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods.
      • Observed boxes and containers of food stored directly on the floor in the walk-in cooler.
      • Observed several wet wiping cloths on the counter. The person in charge placed the wet cloths in the sanitizer bucket.
      • Single service containers were saved and re-used to store other food items.
      • Observed unwrapped single service knives stored with the lip-contact surface in different directions, in a drawer in the dining room area. The person in charge discarded the knives.
      • Observed the three compartment sink needs to be re-caulked.
      • Observed gaps in the filters in the hood system.
      • Observed the exterior of the rice cooker in need of cleaning.
      • The outdoor waste container is left uncovered.
      • Observed the air vent cover in the walk-in cooler in need of cleaning.
      • Observed several employees' personal belongings stored next to food and equipment throughout the facility.
      • Observed the hood system and filters soiled with grime and debris.
  • Pork Junkies (VA JUNKIES), 10501 Big Horn Drive Fredericksburg, VA 22407
    • Date inspected: Nov. 13
    • Total violations: 4
      • Observed accumulation of debris on hood vents above the fryers.
      • Accumulation of debris was noted on the magnetic holders for utensils above the prep table and gaskets of the small reach in cooler. Observed black mold-like growth on the gaskets.
      • Observed food employees without proper hair and beard restraints.
      • The person in charge could not provide verification of employee health training but was knowledgeable of health reporting requirements. The person in charge reported the signed documentation was at home. Instructed the person in charge to email signed employee health policies and ensure the documentation is available for inspections.
  • Arby’s #7615, 2315 Salem Church Road Fredericksburg, VA 22407
    • Date inspected: Nov. 15
    • Total violations: 9
      • Observed the in use Quaternary Ammonium Sanitizing solution at 150 ppm in the sanitizer basin of the 3 compartment sink. The manufacturer specifications state at least 200 ppm is required for sanitizing. The person in charge emptied the sanitizer basin and retitrated to 200 ppm and 75 degrees F. The trays being sanitized were removed to be cleaned and sanitized.
      • Observed interior surfaces of the ice machine in the back of the house soiled.
      • Observed container of cooked breaded onions, which are held for time in lieu of temperature control, were not marked to indicate when the 4 hour time period expires. The person in charge reported the onions were cooked at 10:00 a.m. and placed a sticker on the container to indicate the discard time.
      • Observed the handsink by the ice machine and 3 compartment sink are not sealed to the adjacent walls.
      • Observed the cutting boards at the prep coolers, which are used to cut food, are scratched. Also observed two green cutting boards stored at the prep sink which are scratched and have large black marks on them.
      • Observed accumulation of debris on the following nonfood contact surfaces: Interior surfaces of the prep coolers and beverage cooler, shelving throughout, inside fryers, wall behind fryers, and exterior surfaces of the knife holder box.
      • Observed long hose attached to a spray nozzle attached to the mop sink, which was turned on during the inspection, without correct backflow prevention. The mop sink had a vacuum breaker, however, a dual check valve is required for continuous pressure. The water was turned off during the inspection and the nozzle was removed. The person in charge reported employees will turn off the water and disconnect the spray nozzle in the future.
      • Observed the following in poor repair: Grout is in need of replacement on the floor tiles in various areas of the kitchen, damaged or missing coving tiles, cracked or ill fitting ceiling light covers in the kitchen.
      • Observed the following in need of cleaning: Floors and walls throughout the kitchen, ceiling vents in the toilet rooms, caulk at the 3 compartment sink.
  • Winding Creek Elementary School, 475 Winding Creek Road Stafford, VA 22554
    • Date inspected: Nov. 15
    • Total violations: 3
      • No supervisors or managers are certified food protection managers. The person in charge reported during routine inspection conducted on 11/1/2021 a class is scheduled for March 2022.
      • Observed the dishwasher drainboard and temperature gauge unit on top of the dishwasher are no longer sealed to the adjacent wall or equipment.
      • Observed the wash solution temperature gauge on the high temperature dishwasher is malfunctioning. The temperature gauge does not deviate from 120 degrees F during wash operations.

      • The dishwasher was not sanitizing (0 ppm) equipment and utensils. During the inspection, the PIC replaced the empty bottle of sanitizer. (Corrected)
      • The ranch dressing in the cold holding unit at the server's station was at 48 F. The ranch dressing had exceeded the 4 hours. During the inspection, the PIC discarded the ranch dressing. (Corrected)
      • Observed 1 spray bottle of water over the grill prep unit and 1 spray bottle of DC33 on top of the dishwasher that were not labeled. During the inspection, the food employees labeled the spray bottles. (Corrected)
      • Observed numerous fruit flies in the bar area.
      • Observed a food employee preparing food without a beard restraint.
      • The following are in need of repair: 1. The hand sink faucet next to the soda boxes. 2. The grill prep unit is not holding temperature. 3. The dishwasher draining pipe. 4. The reach in cooler between the fryers and flat top grill. 5. Light out over the dishwasher area.
      • The following are in need of repair: 1. The wall paint is peeling between the walk in cooler and walk in freezer. 2. The ceiling paint is peeling from the cook/prep line to the dry storage area.
      • Observed wet mops sitting in the mop sink basin.
      • The following are in need of cleaning: 1. The ceiling from the cook/prep line to the dry storage area. 2. The mop sink. 3. The floor drains. 4. The wall between the 3 compartment sink and drying racks.

      • The caulking at the 3 compartment sink is heavily soiled.

      • Observed a heavy accumulation of ice build up inside of the deep freezer.
      • Person in charge could not provide a test kit for the sanitizing solution used in the facility.

      • Observed a food employee preparing ready to eat food with their bare hands. The food was discarded.
      • Observed a bucket of water and a sponge in the hand washing sink in the bar area. Items were relocated.
      • Observed spoiled vegetables in the walk-in-cooler. The person in charge stated that the items are for personal use and discarded the items.
      • Observed different types of raw animal foods stored in such a manner that may cause cross contamination as follows: 1) shelled eggs over ready to eat food. 2) raw chicken over ready to eat food, 3) raw chicken over raw beef. *Observed several uncovered containers of food in refrigeration units that were not protected from cross contamination.
      • Observed accumulation of debris on several equipment in the kitchen.
      • The following food-contact surfaces were observed soiled to sight and touch: 1) three containers of utensils on the prep table stored in soiled containers. 2) containers of clean silverware that was stored near soled equipment, 3) dough mixers in back storage area.
      • The following time/temperature control for safety (TCS) foods were observed cold holding at improper temperatures: 1) a mixture of oil and garlic (75'F) by the prep line, 2) cut tomatoes (52'F) stored above the fill line in the prep unit by the cook line, 3) cut cabbage (59'F) on the prep table, 4) a container of cooked beef (66'F) stored above the fill line in the prep unit, 5) rice (126'F). The person in charge discarded the items that were held at improper temperature for over four hours. Environmental Health Specialist discussed proper handling of TCS food during rush hours.
      • Observed the prepared ready-to-eat (RTE) food in the refrigeration units throughout facility were not properly dated for disposition.
      • *Observed two bug spray bottles of degreaser stored in the shelving unit near food and equipment. The bottles were re-located. *Observed two spray bottles of unlabeled liquid, one had water and the other had chemicals. the person in charge labeled the bottles.
      • Observed improper thawing methods used to thaw raw Pork Lion. Beef was observed sitting out at room temperature to thaw. The item was placed in the Walk-in-cooler. Observed thawed out salmon in the walk-in-cooler that has to be kept frozen, per the instructions listed on the package. The person in charge seperated the items to be used for personal use.
      • Observed several roaches under the three compartment sink
      • The exterior door which is located in the kitchen is not self-closing. This may allow the entry of insects or rodents.
      • Observed food stored on the floor.
      • Observed employees in the back of the house without proper hair restraints.
      • Observed several wet wiping cloths on the counter. The person in charge placed the wet cloths in the sanitizer bucket.
      • Observed two rice scoops stored in standing water at a temperature of 72'F.
      • Observed boxes of single service articles stored directly on the floor.
      • Single use containers were saved and re-used to store other food items.
      • The following were observed in need of repair. 1) a leak in refrigeration unit in the bar area, 2) the grill was being used while missing some of the hood filters.
      • Observed the hand washing sink by the prep area and in the ladies restroom are not sealed to the adjacent wall.
      • Observed duct tape on the three compartment sink on the drain board.
      • Observed food being stored directly in a non-food grade bags.
      • Observed not easily cleanable bag, filter, and steamer stored on the shelving unit.
      • The pressure gauge in the hot water sanitizing rinse is not in a working state.
      • No chlorine test kit available.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher.
      • Observed Accumulations of debris were noted on the following nonfood contact surfaces: Interior, including gaskets, of reach in refrigeration units. sides of flattop grill, fryer cabinets.
      • Observed the hand washing sinks in the back of the house and the three compartment sink caulk to be soiled.
      • Observed the walk-in-cooler, walk-in-freezer, floors, ceiling, and walls in need of cleaning throughout the facility.
      • Observed the following in need of repair: 1) Three compartment sink needs re-caulking, 2) hand washing sink by the cook line need to be re-caulked 3) several ceiling tiles throughout the facility need to be re-adjusted. 4) a hole in the wall near the exterior door. 5) masking tape on the light shield in the walk-in-cooler.

      • Observed several employees' personal belongings stored next to food and equipment.
  • Umi Japanese Fine Dining, 1500 Jefferson Davis Highway Fredericksburg, VA 22401
    • Date inspected: Nov. 10
    • Total violations: 9
      • Observed a container of cherry tomatoes and a bowl inside the handwashing sink of the kitchen. During the inspection the person in charge removed the tomatoes and bowl.
      • Observed the following: raw scallops, raw salmon, raw trout fillet, raw mackerel fish being stored above ready to eat foods in the walk in cooler. During the inspection the person in charge re-arranged the items mentioned to the bottom shelf.
      • Observed the under counter reach in cooler cooling at a temperature of 50 degrees. Foods in the unit had been less than 1 hour and were moved to a working cooler nearby. Person in charge will put in a service call.
      • Observed large tears/holes on the mesh screen of the back door.
      • Observed two knives being stored in between a prep table and equipment storage cabinet. During the inspection the person in charge removed the knives.
      • Observed the caulking of the dish wash area needs replacing.
      • Observed the interior surfaces of the all the freezer units are in need of cleaning.
      • Observed the coving tiles near the kitchen door are missing, and paint above the coving tiles is chipping.
      • Observed the following in need of cleaning: Floors underneath equipment, walls throughout the kitchen.

      • Observed raw chicken stored above raw meat in the walk-in cooler. The person in charge relocated the items.

      • Observed no date markings on the cooked and ready to eat time/temperature control for safety (TCS) containers. The items were labeled during the inspection.
      • Observed the following near food and equipment: 1) bottle of bug spray on the shelving unit by the ice machine 2) unlabeled bottle of chemical in the bar area. The person in charge labeled the unlabeled bottle and re-located both bottles.
      • Observed a stone bowl being used directly to serve food.
      • Observed the hand washing sink in the employees' restroom and the 3 compartment sink are not sealed to the adjacent wall.
      • Observed visible debris, grime and soil in the interior of the dishmachine and all shelving unit throughout the kitchen.
      • Observed all the cooling units including the gaskets in need of cleaning.
      • Observed the hose in the basin of the mop sink without a air gap or air a backflow prevention device. The hose was removed from the basin during the inspection.
      • A covered receptacle was not provided at the employees' toilet room.
      • Observed the following in poor repair: 1) sealant between the coving and the wall at three compartment sink has become eroded, 2) several broken tiles throughout the facility, 3) tiles have to be re-grouted 4) numerous blown out lights in the kitchen, 5) ceiling tiles is not sealed to the frame throughout the facility. 6) three compartment sink and hand washing sink need to be re-caulked, 7) broken light shield in the employees' restroom. 8) chipped paint in the service area, 9) several blown out lights in the kitchen.
      • Observed the walls, floors, ceiling, ceiling vents, ceiling tiles in need of cleaning throughout the facility.
      • Observed the following employees' personal belongings stored next to food and equipment. 1) a shirt stored over a box of single service article in the dry storage area, 2) medicine stored in a container with equipment on the shelving unit near the ice machine. n the prep table near food, 3) two cell phones stored by equipment. The person in charge re-located the items.
  • El Asador Restaurant, 100 Olde Greenwich Drive Fredericksburg, VA 22408
    • Date inspected: Nov. 10
    • Total violations: 9
      • Observed a large container of chicken that was cooked at 9:30 inside of the reach in cooler. The chicken measured 72'F. The person in charge reheated the chicken for proper cooling.
      • Observed tamales, salsa, soups, and hochata that were not date marked.
      • Observed that the items used for the pupusias are not annotated with a time.
      • Observed a wiping cloth bucket stored on top of the counter near the single service items.
      • Observed chemical spray bottle that was not labeled. The person in charge disposed of the bottle.
      • Observed that large plastic containers are used for cooling. EHS discussed this with the person in charge.
      • Observed wet wiping cloths stored in plain water. The person in charge added bleach to the wiping cloth bucket.
      • Observed that the facility does not have a mop sink.
      • Observed that the air vent covers and the ceiling tiles are in need of cleaning.
  • Sky Zone Fredericksburg, 1410 Central Park Blvd. Fredericksburg, VA 22401
    • Date inspected: Nov. 10
    • Total violations: 4
      • The hot dog warmer and pizza warmer were observed soiled.
      • The quaternary ammonium sanitizer at the 3 compartment sink is 0 ppm. During the inspection, the PIC made chlorine sanitizer. (Corrected)
      • Observed no thin probe thermometer to measure food temperatures.
      • Observed no test strips for chlorine sanitizer.
  • Courtland High School, 6701 Smith Station Road Spotsylvania, VA 22553
    • Date inspected: Nov. 10
    • Total violations: 3
      • After being cleaned, equipment food-contact surfaces of equipment being cleaned in the high temperature sanitizing dish machine was not observed being sanitized. The temperature of the maximum temperature reading thermometer only read 139'F during the rinse cycle. Heat temperature strips also indicated that the correct temperature (160'F) was not being achieved in the machine. The 3 compartment sink was set uo to sanitize the equipment.
      • The person in charge was unable to produce a test kit for the Dodecylbenzenesulfonic Acid sanitizer being used in the 3 compartment sink
      • The ceiling air intake vents in the kitchen were observed soiled.
