Fredericksburg Restaurant Inspections: Captain D's, Sam's Pizza
Several restaurants around the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District.
FREDERICKSBURG, VA — Inspectors recorded several violations at restaurants around the Fredericksburg area during recent visits. The most recently published inspections occurred between March 16 and March 22.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Bowlero Fredericksburg, 12100 Kilarney Drive Fredericksburg, VA 22407
- Date inspected: March 16
- Total violations: 13
- Facility does not have a person in charge that oversees the dining facility, and the lack of active managerial control is evident. Person in charge could not provide an employee health policy, several risk factors were observed during inspection, could not correctly answer cooking temperature of raw TCS food, could not correclty answer cold holding temperatures and equipment and food temperatures are not monitored.
- Person in charge could not provide a written employee health policy. EHS provided a copy to the person in charge.
- Observed the interior surfaces of the cookie oven and microwave oven are soiled.
- The following TCS foods were found cold holding at improper temperatures: 22 packs of steak strips (48-50 degrees F), 20 packs of chicken strips (47-50 degrees F), 6 raw burger patties (45-47 degrees F), 1 large pack of hot dogs (45 degrees F), sliced american cheese (45 degrees F), sour cream (48-49-50 degrees F), 50 packs of shreeded cheese (47-49-50 degrees F). During the inspection the person in charge dicarded the food for safety.
- Observed 3 spray bottles with toxic chemicals unlabeled. During the inspection the person in discarded the chemicals.
- Observed 3 spray bottles of toxic chemicals stored near open containers of single service use wrappings. During the inspection the person in charge relocated the spray bottles.
- Observed the following: 1.Ice build up at the back of the walk in freezer condenser. 2. The prep unit (44-46 degrees) by the cooks line is not holding the required temperature of 41 degrees or less.
- Observed cutting board of the pizza prep station is heavily stained and no longer smooth and easily cleanable.
- Observed the following: 1. The interior of the fryer cabinets, and interior surfaces of the under counter cooling and frezer units are in need of cleaning. 2. Inside the (unplugged) warming cabinet there were two pizzas that had not been discarded. Person in charge could not say how long pizzas had been left in the unit for.
- Observed water leaking from the kitchen 3 compartment sink faucet.
- Observed debris and cardboard around the dumpster and grease barrel.
- Observed the wall plastic panels of the kitchen hallway are loose.
- Walls and floors of the entire kitchen prep, storage areas, walk in cooler and freezer are in need of deep cleaning.
- Pizza Hut, 4701 Plank Road #103 Fredericksburg, VA 22407
- Date inspected: March 16
- Total violations: 5
- Person in charge was unable to provide a varifiable employee health policy signed by employees. During the inspection EHS provided a copy of a health policy and person in charge went over the policy with food employees.
- Per discussion with the person in charge the food contact surface equipment (pizza slicer and utensils) get washed every 5 hours instead of the required every 4 hours for food contact surfaces used with TCS foods. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods.
- Observed food employees working without a hair and beard restraints. During the inspection the food employees put on hair and beard restraints.
- Observed wire shelving holding clean equipment and storing equipment was soiled to the sight and touch.
- Observed walls and floors and floor drain of the dish washing area in need of cleaning.

- Observed that the facility does not have a certified food protection manager. EHS provided information on local classes.
- Observed raw shell eggs stored over cream cheese and juice in the reach in refrigeration units. The eggs were relocated.
- Observed that the orange juice machine requires cleaning.
- Observed yogurt at 45'F in the display case and 1% milk at 47'F on the breakfast bar. Discussed with the person in charge using time as a public health control.
- The kitchen employee is not wearing a proper hair restraint. EHS provided one.
- Observed that the handles are missing on the reach in refrigeration units.
- Domino’s Pizza (CH), 10740 Courthouse Road Fredericksburg, VA 22408
- Date inspected: March 17
- Total violations: 7
- The PIC could not provide an employee health policy for the food employees working. The food employees did not know any of the foodborne illnesses. EHS provided the PIC a sample copy of the employee health policy agreement.
- The sanitizer at the 3 compartment sink is not being pulled throught the tubing of the dispensing system. During the inspection, the PIC set up the sanitizer (manually) at the 3 compartment sink. (Corrected)
- The following were not properly labeled: 1. Three spray bottles containing Inside Out kitchen equipment cleaner hanging from a rack. 2. Two spray bottles containing glass cleaner hanging from a hook. During the inspection, EHS assisted PIC with labeling the spray bottles. (Corrected)
- Observed food employee preparing food without a beard restraint.
- Observed the floor tile coving chipped or missing in the following locations: 1. To the right of the walk-in cooler door. 2. Between the hand sink and pizza prep. 3. The corners in the hallway between the Coca Cola beverage cooler and dry food storage rack.
- The following are in need of cleaning: 1. The ceiling tile surrounding the vent over the dishwashing area are covered in debris. 2. The floor under the 3 compartment sink is soiled. 3. The ceiling vent in the employee restroom is covered in debris.
- The light intensity in the walk-in cooler is 3.4 foot candles.

- In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. Observed two uncovered drinks on the prep table next to food preparation. Drinks were discarded during inspection.
- Observed food employees using improper handwashing procedures. Observed two employees both washing their hands for only 5 seconds. Employees were instructed to wash their hands fir at least 20 seconds.
- The handwashing facility located at the bar is blocked, preventing access by employees for easy handwashing. Observed a bucket of water and wiping cloths stored in the sink. Bucket was removed during inspection.
- The interior surfaces of the ice machine were observed soiled.
- There were no temperature measuring devices located inseveral cold holding units.
- Wiping cloths improperly stored between use. Observed a wet wiping cloth stored on the prep table at the back prep area. Cloth was placed in sanitizer during inspection.
- The hot water at the handsink near the cook's line was turned off due to a leak from the hot water faucet.
- Observed two light fixtures in the prep area, two on the cook's line and two in the server's area that are not working.
- Observed 2 cell phones store in food preparation areas while food preparation was taking place. Phones were re-located during inspection.

- Total violations: 11
- The food-contact surface of the coffee maker dispersion nozzle is not cleaned at an appropriate frequency. Nozzle was cleaned during inspection.
- The (cavity, door seal) of the microwave oven is observed soiled. Oven was cleaned during inspection.
- The cutting board along the saute prep unit was observed heavily stained .
- Observed a spray bottle of de-greaser stored on the prep table with the dough press. Bottle was removed during inspection.
- Harborage conditions exist. Observed 5 gallon bucket half full of grease/oil near the pizza oven.
- Observed two employees with head coverings in the kitchen upon arrival and one person without a beard guard.
- The following non-food contact surfaces were observed soiled: 1-outer surface of the walk in cooler, 2-the walk in cooler door, 3-the scales above the pizza prep unit, 4-the upsplash of the coffee and tea maker, 5-the checklist binder and recipe cards above the prep unit on the cook's line, 6-the top of the soup hot holding tureen, 7-the pot under the pizza oven conveyor belt, 8-inside of ALL reach in coolers, freezers and prep units, 9-the gaskets on all of the coolers, 10-the outer surfaces of ALL of the equipment in the kitchen, 11-the outer surfaces of the trash cans in the kitchen, 12-the black plastic cart, 13-the outer surfaces of the dough press, 14-the step stool, 15-the outer surfaces of the pizza pans. IN GENERAL, ALL INNER AND OUTER SURFACES OF THE EGUIPMENT IN THE KITCHEN IS IN NEED OF THOROUGH CLEANINGS.
- Observed two mops being stored in dirty mop water. Mops were rinsed and hung up to dry during inspection.
- The following physical facilities are in need of cleaning: 1-the walls throughout the kitchen, 2-the floors throughout the kitchen, especially under the equipment, 3-the floors in the walk in cooler and freezer. IN GENERAL, THE WALLS AND FLOORS THROUGHOUT THE KITCHEN AND WALK IN COOLER AND FREEZER ARE IN NEED OF A DEEP CLEANING.
- The filters in the ventilation system and the ceiling vents in the kitchen were observed heavily soiled.
- Observed employee clothing and other possessions stored in areas that may contaminate food, food-contact surfaces of equipment, single-service items, or single-use items. Observed two jackets and dirty aprons on the dry storage rack on or next food. Also observed a cell phone on a prep table.
- Potbelly Sandwich Shop, 1240 Carl D. Silver Pkwy Fredericksburg, VA 22401
- Date inspected: March 17
- Total violations: 5
- Person in charge could not provide a verifiable way to demonstrate employees have been trained in foodborne illnesses and symptoms through an employee health policy. During the inspection EHS provided the person in charge with an employee health policy, person in charge had employees read and sign the copy.
- Observed a large knife inside of the kitchen hand washing sink basin. During the inspection the knife was moved to the 3 compartment sink and EHS explained to the person in charge handwashing sinks are to be used for handwashing only.
- Per discussion with the person in charge the in use utensils at the prep stations and hot holding stations only get switched out for washing and sanitizing during shift changes instead of the required every 4 hours when used with TCS foods.
- Observed the following: 1.The cutting board of the salad prep station and the cutting board of the load station need replacing, they are heavily stained with black markings throughout the cutting surface. 2. The meat and veggie slicer had food residue left from early in the day usage.
- Observed the following: floor drains under the prep sink and floor drain under the soda machine need cleaning, floors underneath equipment in the kitchen and walls around the dish washing area are in need of cleaning.

- Total violations: 10
- PIC does not demonstrate active managerial control based on the violations and the following: 1. At the beginning of the inspection, no sanitizer was available and when asked about the 3 compartment sink set up the PIC forgot the sanitizing step. 2. The PIC could not provide the proper cooling procedure for the rotisserie chicken on the prep table.
- No certified food protection manager.
- No paper towels at handwashing sink next to prep unit. During inspection, the PIC provided paper towels. (Corrected)
- The ceviche de pescado (raw tilapia) offered on the take out menu is not linked to the disclosure statement. During the inspection, the PIC had the employees place an asterisk on the ceviche and the disclosure statement. (Corrected)
- No working thin probe thermometer available to verify cooking temperatures.
- Observed several bags of frozen cut peppers and California medley on the floor in the walk-in freezer. During the inspection, the food employee placed the food on the racks. (Corrected)
- Observed jalapenos being stored directly in 16 carry out plastic bags in the walk-in freezer. During the inspection, the PIC discarded the jalapenos. (Corrected)
- Observed 2 cracked light shields; one in the men's restroom and one between the 3 compartment sink and prep table.
- The light intensity in the walk-in cooler ranges between 1.1 - 4.3 foot candles.
- Observed personal items (1 bottle of magnesium, ball, bottled water, car keys, mirror, pink glitter bag, 1 bottle of hydrogen peroxide) throughout the kitchen.
- Hardee’s of Spotsylvania Courthouse #2996, 9811 Courthouse Road Spotsylvania, VA 22553

- The following are in need of repair: 1. The front cover to biscuit reach in cooler is missing. 2. The mop sink coating is heavily chipped. 3. The area around doors into the walk-in cooler and walk-in freezer is peeling.
- The non food contact surfaces of all equipment along the prep and cook line were observed soiled.
- The following are in need of repair: 1. The floor tile need regrouting throughout the kitchen. 2. The floor threshold strip at the walk-in freezer is loose.
- The following are in need of cleaning: 1. The floor under and around all prep tables and cooking equipment. 2. The ceiling tile beams throughout the kitchen are heavily soiled.
- The light intensity in the walk-in freezer is 2.1 foot candles.

- Total violations: 8
- Observed the interior surfaces of the 3 microwave ovens and bread toaster are in need of cleaning.
- Per discussion with the person in charge the shrimp devainer machine is not currently undergoing the wash step of the wash rinse and sanitize cleaning method. Food employees only spray sanitizer in between each use of the machine.
- Observed several stainless steel containers that were stored stacked while still wet.
- Observed the front handsink, kitchen handsink, food prep sink and 3 compartment sink are no longer adjoined to the wall and need caulking.
- Observed the top covering of the three microwave ovens is loose and the microwave of the prep area had black tape sealing a crack near the opening.
- Observed the following non food contact surfaces that are in need of cleaning: 1. The shelf holding the microwave above the food prep sink. 2. The rolling carts in the walk in cooler had a lot of encrusted and visible food debris. 3. The shelf holding clean equipment in the prep room area. 4. The shelf above the hot table is very dusty. 5. Cobwebs were observed above the shelf of the 3 compartment sink area. 6. Inside and outside surfaces of ALL reach in cooling and freezer units. 7. Top surfaces and inside and outside surfaces (cabinets) of the front service areas including the self service soda machine area.
- Observed walls, floors under equipment and ceiling and ac vents and floor drains are in need of a deep and thorough cleaning.
- Observed the following: floors throughout the kitchen area are cracked and severely damaged, the door frame of the stock room is severely damaged, the inside of the walk in cooler door is rusted.

- Total violations: 4
- No certified food protection manager.
- Observed a spray bottle of all purpose cleaner on the prep table that was not labeled. During the inspection, the PIC labeled the spray bottle and relocated. (Corrected)
- PIC could not produce an appropriate food temperature measuring device to verify cooking temperatures. During the inspection, the PIC purchased a thin probe thermometer for the establishment. (Corrected)
- The following are in need of repair: 1. Light out in dry storage room. 2. Missing ceiling tile in the mop sink room.
- Domino’s Pizza, 511 Jefferson Davis Hwy Fredericksburg, VA 22401
- Date inspected: March 18
- Total violations: 6
- Person in charge could not provide EHS with an employee health policy. EHS provided a copy to the person in charge. During the inspection the employee policy was discussed, acknowledged and signed by employees.
- Observed a food employee scrub soapy hands under running water for less than 5 seconds. EHS demonstrated proper handwashing and employee rewashed thier hands. Person in charge will work in retraining employees with proper handwashing.
- Observed the sanitizer dispenser at the 3 compartment sink is not working properly. Person in charge placed a call to a technician and facility will manually make a sanitizing solution in the mean time.
- Observed a food employee wearing wrist jewelry.
- Observed all the wire racks holding dirty dishes, clean equipment, dry goods, above the 3 compartment sink and racks of the walk in cooler are soiled to the sight and touch.
- Observed the floors of the facility, walls behind the 3 compartment sink, ceiling fan vents and AC vents of the kitchen and bathrooms all require cleaning.
- Wendy's #6582, 588 Warrenton Road Fredericksburg, VA 22406
- Date inspected: March 21
- Total violations: 1
- Observed a container of cut tomatoes (43-46'F) a container of cut lettuce (48-50'F), and a container of shredded cheese (49-50'F) cold holding in the perp unit by the hot holding station at improper temperatures. The foods still had two hours for the temperatures to reach 41'F or below. The person in charge placed the items in the walk-in-cooler. EHS checked temperatures after one hour and all foods were at the proper temperatures.

- There is no handwashing sign or poster that notifies food employees to wash their hands provided at the only handwashing sink in the kitchen. Person in charge (PIC) provided a sign during the inspection.
- Observed a can of mandarin orange and a can of tomato sauce that were dented but not separated in the dry storage room. PIC relocated the items to a do not use area.
- El Rincon Del Sabor Bar & Grill, 6322 5 Mile Centre Park 307 Fredericksburg, VA 22407
- Date inspected: March 21
- Total violations: 5
- Person in charge provided employee health policy but had not informed employees of the foodborne illnesses and symptoms and the policy was not signed. During the inspection the person in charge went over the policy with employees.
- Observed the interior surface of the ice machine soiled.
- Observed a container of corn masa cold holding at 46 degrees F, and a container of chicken and cheese mix cold holding at 46 degrees F. In discussion with the person in charge items were made the day prior and were not cooled properly before storing in the upright reach in unit. During the inspection the person in charge discarded the items for safety.
- Observed the door of the food pep unit came off the hinge during the inspection. Person in charge will place a call to have it fixed.
- Observed a leaking pipe under the 3 compartment sink and the faucet of the prep sink also has a water leak.

- Date inspected: March 21
- Total violations: 5
- Observed that the front hand sink is being used as a dump sink. Observed rice and other foods in the hand sink. EHS discussed use of this sink with the person in charge.
- Observed vegetables in the sandwich prep unit and walk in cooler that were uncovered. The person in charge placed plastic over the items.
- Observed raw shell eggs stored over the sauces in the sandwich prep unit and observed raw shrimp stored over the noodles in the walk in cooler. Both items were rearranged.
- Observed that the interior of the breading machine, inside of the plastic containers used for seasoning, colander, and stainless steel containers stored over the 3-compartment sink are in need of detailed cleaning. The person in charge placed the items in the 3-compartment sink for cleaning.
- Observed ribs and sweet/sour chicken that were not dated for disposition. The person in charge dated the items.
- Kusina Filipino Restaurant and Gourmet, 10151 Patriot Hwy Fredericksburg, VA 22407

- Total violations: 3
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Improper methods used to thaw fish. Observed fish thawing in standing water in the prep sink. Fish was placed in the walk in cooler to properly thaw.
- Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials. The filters in the ventilation system over the cook's line were also noted in need of cleaning.
- Sam’s Pizza & Subs, 2142 Jefferson Davis Highway #Suite 101-102 Stafford, VA 22554
- Date inspected: March 22
- Total violations: 15
- Observed a food employee, wearing single use disposable gloves, touch soiled ambient air thermometer inside the pizza prep cooler and then proceed to use the same glove to handle shredded mozzarella cheese. Instructed the food employee to discard the cheese and wash their hands. Discussed requirements with the person in charge.
- Observed the following TCS foods cold holding at improper temperatures: Sandwich prep cooler: Sliced American cheese (45 degrees F), pizza prep cooler: Pre-cooked sausage (48 degrees F) and crumbled feta cheese (46 degrees F) and Keg cooler: Single use containers of half and half dairy creamer (45 degrees F). Observed the American cheese was sitting on top of a food storage container and was not being held inside the refrigerator. The person in charge reported the cheese was pulled from the bottom of the unit at 11:00 a.m. Instructed the person in charge to sell or discard by 3:00 p.m. The person in charge reported the pre-cooked sausage and feta cheese were held in the pizza prep cooler overnight and voluntarily discarded for safety. Discussed keeping the pizza prep cooler lid closed when not in use. The person in charge reported the half and half creamers have been in the keg cooler overnight and voluntarily discarded for safety.
- Observed Quaternary Ammonium spray bottle at 500 ppm. Worked with food staff to dilute the spray bottle to 200 ppm. Observed the dispenser unit at the mop sink is dispensing the sanitizer at 500 ppm. Discussed monitoring the concentration of sanitizing solutions and diluting to achieve a concentration strength of 200-400 ppm until the dispenser is properly calibrated and serviced. The person in charge contacted the sanitizer company for service during the inspection.
- Observed chemical spray bottle of Quaternary Ammonium sanitizer which was not labeled to identify the contents. During the inspection a food employee labeled the spray bottle.
- Observed light emitting from the bottom and right side of the back door of the food establishment which leads to the dumpster area. A storage area exists between the back door leading to the exterior of the building and the back door of the food establishment, which is used to store packaged dry goods.
- Observed presence of crawling insects in the vicinity of the dishwasher. Instructed the person in charge to contact pest control for service and treatment. Also observed harboraging conditions including presence of water on the floors in the warewashing area and food and other debris sitting in the drain under the 3 compartment sink.
- Observed six pickle buckets stored on the floor inside the walk in cooler and one bag of flour stored on the floor by the flour storage bin. During the inspection the person in charge instructed a food employee to relocate the bag of flour.
- Observed cut onions stored in a black trash bag inside the walk in cooler. Informed the person in charge trash bags are not food safe or food grade. The person in charge voluntarily discarded the onions for safety.
- Observed the following equipment in poor repair: Rusted shelving inside the walk in cooler, walk in cooler flooring is lifting up towards the back of the unit, crack inside the far left door of the pizza prep cooler and missing vent cover for the pizza prep cooler.
- Observed the Chlorine and Quaternary Ammonium testing kits expired February 2021 and 2013, respectively. Instructed the person in charge to purchase new test kits.
- Observed accumulation of debris on the following nonfood contact surfaces: Base of pizza oven, shielded lights above the pizza oven and hood vent at the cookline, shelving and back of the flattop grill, inside fryer doors, base of 2 door upright freezer and shelving in the walk in cooler.
- Observed a long hose
- with a spray nozzle attached to a Y-connector at the mop sink faucet without adequate backflow protection. Water was flowing from the spray nozzle when turned on. Provided the person in charge backflow handout to install a dual check valve with atmospheric vent. During the inspection the water was turned off and the spray nozzle was disconnected.
- Observed ceilings, walls, air vents and drain under the 3 compartment sink are in need of cleaning.
- Observed 2.7 foot candles of light inside the back of the walk in cooler.
- Observed hood vent filters on the left side are soiled and also observed oily residue emitting from the vents on the front of the hood system.
- Montego Bay, Grille & Sports Bar, 9915 Southpoint Pkwy Fredericksburg, VA 22407
- Date inspected: March 22
- Total violations: 21
- The person in charge has failed to ensure employees are complying with the requirements of the Regulations. Observed temperature violations of cold and hot holding, a lack of date marking, and a lack of hand washing at required points during food preparation.
- Person in charge was only able to provide a certificate of course completion, not a a certificate that states the person is a certified food protection manager.
- Person in charge was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy.
- Observed a food employee talking on their personal cellular phone, walked outside, then walked back in the kitchen, and handled clean bowls to prepare food without first washing his hands. Employee was asked to wash his hands- bowls were placed to be washed.
- Observed a food employee fill a food container with water from the handwashing sink. EHS informed the food employees of the proper use of the handwashing sinks.
- No hand drying methods provided at any of the handwashing sinks. The person in charge provided paper towels during the inspection.
- Observed a container of cooked green beans in the walk-in-cooler- the green beans were spoiled. The container was not date marked. The person in charge discarded the green beans.
- Observed several knives, cups, containers stored on the shelving unit by the 3 compartment sink that were soiled to the touch and sight. The person in charge removed the items to be washed, rinsed, and sanitized.
- Observed 4 beef patties hot holding at improper temperature (125-127'F), the person in charge (PIC) reported the patties were cooked in the oven and placed in the hot holding unit for less than two hours. EHS instructed PIC to reheat the patties to 165'F and adjust the temperature of the unit. Corrected during the inspection.
- Observed the following: 1) a container of flour that contained raw shrimp used for breading that was left overnight on the counter. The person in charge reported the breading container used for raw shrimp is being stored on the counter and changed every 24 hours without sifting. The person in charge discarded the item. 2) Observed the following time/temperature control for safety (TCS) foods cold holding at improper temperatures overnight in the walk-in-cooler: a) one pot of curry goat (44-46'F); b) a container of homestyle chicken (43-44'F); c) a container of Oxtail (44-46'F); a container of cooked green beans (43-45'F). The person in charge discarded the items Walk-in-cooler thermostat was set at 44'F, the PIC lowered it to 37'F.
- A date marking system is not in proper use for the time/temperature control for safety (TCS) foods.
- Observed an unlabeled clear bottle on the counter that contained chemical. The person in charge labeled the bottle during the inspection.
- Observed four packs of thawed out fish stored in the walk-in-cooler. The instructions on the box stated that the product has to be removed from the plastic bags prior to thawing. The person in charge discarded the items.
- No thin-probed food thermometer available.
- Observed several wet wiping cloths throughout the facility. The person in charge placed the wet cloths in the sanitizer bucket.
- Observed an employee wrapping the utensils without gloves, the utensils were stored with the lip-contact surface facing upward.
- No chlorine test kit available.
- The following were noted with an accumulation of grime and debris: 1) The nonfood contact surfaces of the interior and exterior of the reach in refrigerators; 2) rice cooker.
- Observed a leak in the prep sink by the dry storage area.
- Observed the walls throughout the facility in need of detailed cleaning.
- Observed the following employees' personal belongings stored next to food and equipment. 1) a cellular phone, 2) two tumblers, 3) personal keys.

- Observed no sanitizer was detected at the 3 compartment sink and two wiping cloth buckets. During the inspection the person in charge changed out the sanitizer and the solution tested at 200ppm.
- Observed the following foods cold holding at improper temperatures: 4 bags of mixed salad (44-52 degrees F), 1 bag of spinach (44 degrees F), 4 bags of mozzarella cheese (43-46 degrees F), 4 containers of vanilla yogurt (44-51 degrees F), 9 trays of drinkable flavored yogurt (46-44 degrees F)2 boxes of yogurt cups (43-44 degrees F). During the inspection the person in charge discarded the items for safety.

- The following are in need of repair: 1. Missing several floor tiles between the walk-in cooler and the 3 compartment sink. 2. Several water stained ceiling tile over the walk-in cooler and dishwashing area. 3. Two cracked light shield covers in the back prep area/storage room. 4. One peeling ceiling tile over the entrance into the walk-in cooler. 5. The ceiling is caving in between the walk-in cooler and exterior back door. 6. A hole in the wall of the employee restroom.
