Fredericksburg Restaurant Inspections: Mouse Droppings, Flies
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between March 8 and March 14.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Stafford County Head Start, 610 Gayle Street Fredericksburg, VA 22405
- Date inspected: March 8
- Total violations: 1
- The facility does not have a certified food protection manager with a current certificate. The person in charge's certificate expired 8/28/2022. Raw hamburger is offered for lunch- hamburger is cooked to above 165'F in the oven.
- King George High School, 10100 Foxes Way King George, VA 22485
- Date inspected: March 8
- Total violations: 1
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Taste of Jamaica, 263 Garrisonville Road Stafford, VA 22554
- Date inspected: March 8
- Total violations: 8
- Observed that the employee washed, rinsed, and sanitized the equipment but did not keep it in the sanitizer for 10 seconds. The person in charge had the employee rewash, rinse and sanitize the equipment correctly.
- Observed the lid to a stainless steel pan, a knife, the bulk flour scoop, a ladle, and a large dispensing spoon that require cleaning. The person in charge placed the items in the 3-compartment sink for cleaning.
- Observed curry chicken, curry goat, cooked kidney beans, and cut cabbage in the walk in cooler that were not dated for disposition. The person in charge dated the items.
- Observed small flies on the wall on the prep line and near the 3-compartment sink.
- The cutting surfaces of the wooden cutting board is heavily scratched and scored.
- Observed that the interior of the oven and the top of the stove require cleaning.
- Observed that the lighting measures 25 foot candles in the front and rear prep areas.
- Observed that the filters in the hood system require cleaning.
- Sonic Drive In, 240 McWhirt Loop Fredericksburg, VA 22406
- Date inspected: March 8
- Total violations: 13
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Observed the following: 1) employee handle the sanitizer bucket with single use disposable gloves, remove gloves and don new gloves to handle unwrapped single use article, 2) food employee touch a heavily soiled towel wearing single use disposable gloves, remove gloves and prepare to don gloves to engage in food preparation task without washing their hands prior to donning new gloves, 3) employee pick soiled cooking utensils off the floor with gloved hands then donned new gloves without washing their hands. 4) employee handle personal jacket and cellular phone with their gloved hands then handle single service in the food bagging area without washing hands and don new gloves. Instructed employees to remove gloves and wash their hands prior. Discussed handwashing requirements with the person in charge.
- Observed several stainless steel containers and cooking utensils stored on the shelving units to be soiled to the touch and sight. Items were moved to the 3 compartment sink to be washed, rinsed, sanitized.
- Through discussion with the person in charge the following were noted: 1) the milk shake machine wands are sanitized after every use but are not being properly washed, rinsed, and sanitized every four hours. Informed the person in charge equipment in contact with time/temperature control for safety foods are required to be cleaned and sanitized at least once every 4 hours. 2) the interior of the milk shake machines are cleaned and sanitized twice a week. Informed the person in charge the milk shake machines are required to be cleaned and sanitized once every 24 hours.
- Observed tater tots (109-126' F) hot holding at improper temperatures in the French fry hot holding equipment. The person in charge discarded the food for safety.
- Sanitizer dispensing unit in the front of the house by near the milk shake machine is not dispensing properly. The sanitizer solution is measuring 0 PPM. PIC posted a do not use sign- employees will use the other 3 compartment sink in the back of the house near the mop sink.
- Observed the following in need of repair: 1) Ice cream and milk shake unit collecting water on the bottom shelf where the products are stored (PIC covered the products with aluminum foil)2) sanitizer dispensing unit in the front of the house by near the milk shake machine is not dispensing properly. The sanitizer solution is measuring 0 PPM. PIC posted a do not use sign- employees will use the other 3 compartment sink in the back of the house near the mop sink. PIC will contact a technician to evaluate the units.
- Observed three wiping cloth buckets (that measured 0 PPM- requirement is 200-400 PPM) that had wiping cloths stored in them. Sanitizer solution was discarded and remade.
- Observed the floors, walls, and ceiling vents (to include walk in cooler) and light shields throughout the kitchen in need of cleaning.
- Observed accumulation of grime and debris on the following nonfood contact surfaces: 1) interior and exterior of reach in refrigeration equipment, 2) shelving throughout the facility, 3) heat lamp in hot holding unit.
- The floor throughout the kitchen is unsealed concrete.
- Observed 2.8 foot candles in the walk in cooler and 2.6 food candles in the walk in freezer.
- Observed several employees' personal belongings stored near food and equipment. Items were relocated.

- Observed a lot of cobwebs on the walls over the reach in freezer.
- Domino's Pizza #4274, 556 Garrisonville Road Suite #100 Stafford, VA 22554

- Total violations: 4
- Observed that the pizza and marinara sauces does not have the time that it was removed from temperature control. The person in charge placed a time of the products.
- Observed an open mouse trap under the coke/sprite storage racks.
- Observed mice droppings under the soda racks in the rear prep area.
- Observed that the floors under the storage shelves, floor/wall junctures throughout the facility, ceiling tiles near the pizza oven, and drain pipes under the 3-compartment sink require cleaning.
- Casey's, 622 Kenmore Avenue Fredericksburg, VA 22401
- Date inspected: March 9
- Total violations: 5
- Person in charge was unable to provide the facilities written plan for how to clean up bodily fluids. EHS provided a copy.
- Observed the following non food contact surfaces that require cleaning: 1. sides of fryer 2. top surfaces of cook range 3. wire racks in the walk in cooler 4. inside surfaces of the upright coolers/freezers.
- Observed the following facilities in need of cleaning: 1. floors under cook line equipment 2. ceiling of walk in cooler 3. floors of walk in cooler 4. walls leading to the walk in cooler.
- Observed porous ceiling tiles in the men's bathroom.
- Observed the the following facilities in poor repair: 1. cracked floor tiles at the bar area 2. cracked wall tiles near the window in the women's bathroom 3. damaged and peeling ceiling tiles throughout the kitchen area and dish room 4. damaged and peeling wall panel under the window in the kitchen.
- Tacos Y Pupuseria Diner, 3650 Richmond Highway Stafford, VA 22554
- Date inspected: March 9
- Total violations: 5
- Observed that the consumer advisory is missing on both menus along with the mark (*) that goes with it to advise the customers about consuming raw or undercooked foods. The person in charge posted a sign to inform the customers about consuming raw or undercooked foods.
- Observed that the large 3-door reach in cooler and the upright reach in cooler are broken.
- Observed that the interior of the dish machine require cleaning.
- The Goddard School, 301 Highpointe Blvd Stafford, VA 22554
- Date inspected: March 10
- Total violations: 7
- Per conversation with the person in charge, the parents are providing the foods and the teachers are reheating them.
- Observed that the fruit slicer, dispensing spoons, and knives require cleaning. The person in charge placed the items in the 3-compartment sink for cleaning.
- The person in charge could not provide a small probe thermometer.
- Observed that the kitchen staff is not wearing a hair restraint.
- Observed that the 3-compartment sink is not sealed to the adjourning wall.
- Observed that the facility does not have chemical test strips.
- Observed that the facility is using splashless bleach to sanitize the equipment.

- Date inspected: March 13
- Total violations: 7
- The person in charge could not provide documentation the immunity and energy boosters, used in smoothies, come from approved food sources. The person in charge could not provide documentation the manufacturer is registered with the FDA. Instructed the person in charge to discontinue use until documentation is received. Provided VDH Dietary Supplements handout during routine inspection on 4/5/2022 and during today's inspection.
- Through discussion with food employees, food thermometers are cleaned and sanitized once daily. Informed the person in charge food thermometers are required to be cleaned and sanitized at least once every 4 hours when contact with TCS foods occur. Discussed methods to clean and sanitize food thermometers.
- Beverage menu does not include a disclosure for immunity and energy boosters to inform customers. Menu requirements were discussed during the routine inspection conducted on 4/5/2022 and during today's inspection.
- Observed food employees preparing food and beverages without proper hair and beard restraints.
- Observed accumulation of debris on the base and shelf of both Merrychef ovens and shelving in the drive through area.
- Observed a hose connected to a Y-connector at the mop sink sitting in the base of the mop sink, absence of an adequate air gap. An Atmospheric vacuum breaker is installed behind the faucet of the mop sink. During the inspection the hose which looped around the faucet to create an adequate gap. Discussed cutting the hose which is used to fill mop buckets.
- Observed the following areas in need of cleaning: Caulk at the 3 vat sink, floors and side panel in the vicinity of the 2 door reach in cooler by the drive thru and ice machine floor trough.

- There is no valid quaternary ammonium test kit in the facility for monitoring the concentration of the chemical sanitizing solutions.
- Charlie G's, 314 Emancipation Highway Fredericksburg, VA 22401

- The cutting board along the cook's prep unit was observed stained. Board was taken to the 3 compartment sink to be cleaned.
- The interior of both microwaves at the end of the cook's line and the homestyle flattop grill being used to toast buns were observed soiled. Ovens and grill were cleaned during inspection.
- The wire shelving in the walk in cooler is rusty and is no longer cleanable.
- The following non-food contact surfaces were observed soiled: 1-the outside of the ventilation hood over the cook's line, 2-outer surfaces of the deep fryers and the oil wand next to them, 3-outer surfaces of all food storage containers, 4-the wire cart near the walk in cooler, 5-the black plastic cart, 6-the front filters of the ice machine, 7-outer surfaces of the bulk storage bins, 8-the oil filter pump, 9-the shelving behind the bar.
- The walls throughout the kitchen, especially near the 3 compartment sink, and the floors under the equipment at the bar were observed heavily soiled.
- The filters in the ventilation hood over the cook's line were observed soiled.
- Observed less than 50 foot candles of light in the following food preparation areas: the cook's line (13 foot candles) and the rear prep area (8 foot candles).

- Vent filters in the hood system are not being maintained in a clean condition.
- Outback Steakhouse #4718, 2941 Plank Road Fredericksburg, VA 22401
- Date inspected: March 14
- Total violations: 11
- Observed the following: 1. One package of shredded lettuce stored below trays of raw meat 2. One container of ice cream uncovered. During the inspection, the package of lettuce was relocated and the ice cream was covered by the person in charge.
- Observed the hot water sanitizing dish machine was not working properly and sanitizing dishes. After several attempts the highest recorded temperature recorded was 157 degrees F. EHS advised employees and person in charge to ensure dishes are manually sanitized until a technician comes to service the machine.
- Observed numerous sheet tray pans extremely soiled with encrusted grease deposits and build up on the food contact and non food contact surface.
- Observed a food employee working while wearing wrist jewelry.
- Observed a food employee working without a beard restraint.
- Observed numerous plastic and stainless steel containers that were stored stacked while still wet.
- Observed caulking of two kitchen hand sinks is missing in places, and the caulking at the 3 compartment sink is heavily soiled with mold like growth.
- Observed the following non food contact surfaces that require cleaning: 1. speed rack carts 2. shelving throughout.
- Observed at the mop sink an air gap was missing between the water faucet connection and hose connected directly connected to it. During the inspection the water hose was wrapped around basin of faucet.
- Observed the following in need of cleaning: 1. walls of the prep room 2. floors of walk in cooler and walk in freezer 3. floors and walls in the ware washing area 4. dusty ac vents in the bathrooms.
- Observed the following: 1. cracked floor tiles throughout kitchen and bar area 2. damaged coving tiles in the dry storage room 3. loose/missing ceiling tiles in the dry storage room 4. damaged/loose upper lining coving tiles in the women's bathroom 5.floor grout missing in womens bathroom 6. rusted ceiling time framing in the kitchen 7. peeling wall paint behind the steamer.

- The person in charge could not provide documentation concerning employee health training.
- The person in charge could not provide written documentation concerning the cleaning of bodily fluid accidents.
- Observed that the hand sink was blocked with two large pans. The person in charge removed the items.
- Observed several knives on the metal knife rack that were soiled to sight. The person in charge placed the knives in the 3-compartment sink for cleaning.
- Observed pre cooked bacon at 54'F, shell eggs at 46'F, and scrambled eggs packages at 52'F in the reach in refrigerator and waffle batter at 69'F on the breakfast bar - all cold holding at improper temperatures.
- Observed that the coving is off the wall near the entrance door.
- Observed that the ceiling tiles are not smooth and easily cleanable.
- Soulific Seafood (286-962), 6 Vaughn Ct Fredericksburg, VA 22405
- Date inspected: March 14
- Total violations: 5
- The person in charge (PIC) could not provide documentation of employee health training, he reported signed copies are held offsite. EHS provided an employee health policy sample to be read and signed during the inspection- the form will be kept on the unit.
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. PIC was informed of the Virginia Food Regulations update on May 14, 2022. EHS provided a bodily fluids clean up sample.
- No chlorine test kit available on site.
- The nonfood contact surface of the interior of the prep unit and fryer station are noted with an accumulation of grime and debris.
- Observed the floors, walls and ceiling (to include ceiling air conditioning vent and light shield) in the mobile unit in need of cleaning.
