Fredericksburg Restaurant Inspections: Majestic Restaurant Bar & Grill, Stir Fry 88
A handful of restaurants around the Fredericksburg area were cited with violations during the health department's final inspections of 2021.
FREDERICKSBURG, VA — Several restaurants around the Fredericksburg and Stafford region were cited with violations during the Rappahannock Area Health District's final inspections of 2021. The health department has not posted any inspections in 2022 yet, due to the New Year's holiday and last week's snowstorm, which knocked out power to thousands of homes and restaurants around the area for several days.
As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Merit School of Stafford Lakes #25, 251 University Blvd Fredericksburg, VA 22406
- Date inspected: Dec. 29
- Total violations: 3
- Observed the handsink and drainboard of the three compartment sink are not sealed to the adjacent walls or equipment.
- Accumulation of debris on the following nonfood contact surfaces: Base of the convection oven, exterior surfaces of the cabinets under the 3 compartment sink, countertop around the prep sink and shelving above the 3 compartment sink.
- Observed the following in need of cleaning: Rubber mats in the kitchen, ceiling tiles in the kitchen and interior of the mop sink.
- Majestic Restaurant Bar & Grill, 3987 Jefferson Davis Highway Stafford, VA 22554
- Date inspected: Dec. 29
- Total violations: 13
- Observed mold like growth on shredded mozzarella cheese inside the walk in cooler. The person in charge voluntarily discarded the cheese for safety.
- The person in charge could not provide parasite documentation for salmon and tuna which may be served raw and/or undercooked upon a customer's request. Instructed the person in charge to obtain parasite destruction documentation from the fish supplier.
- Observed absence of date marking to identify the date of preparation/opening or discard date for the following TCS foods: Container of mashed potatoes (prepared 12/24/2021) and opened container of whole milk (opened on 12/23/2021) in the walk in cooler. The person in charge placed a date label with the date of preparation on the container of mashed potatoes. Instructed the person in charge to discard the gallon of whole milk by the end of the day (day 7 from date of opening). Discussed date marking requirements for commercially processed TCS foods.
- The menus (both everyday and brunch) does not include a disclosure to indicate which foods on the menu may be cooked to order upon a customer's request (steak, salmon, burgers and eggs), through identification such as an asterisk, to the reminder statement on the menu.
- Observed three bulk containers which were not labeled to identify the contents.
- Observed boxes of wine and two pickle buckets stored on the floor in the walk in cooler.
- The person in charge could not provide a Quaternary Ammonium (QT-10) test kit to monitor concentration of Quaternary Ammonium sanitizing solutions.
- Observed accumulation of debris on the following nonfood contact equipment: Cutting board on prep cooler (not used in contact with food), shelf under char broiler, shelf next to range, interior doors of the convection oven, equipment used to store cleaned utensils at the cook line. Discussed removing utensils for cleaning.
- Observed dumpster stored on grass/gravel.
- Observed ceiling vents in need of cleaning in the facility and black build up near the faucets at the 3 compartment sink.
- Observed two mops with the mop head sitting inside the bottom of a bucket.
- Observed the following in poor repair: Missing ceiling tile above the ice machine, damaged wall by the meat slicer, and the floor at the bar area is lifting up, observed accumulation of water on the floor adjacent to the bar reach in cooler.
- Observed the ceiling tiles in the male and female toilet rooms are absorbent and not easily cleanable.

- Date inspected: Dec. 29
- Total violations: 6
- Observed in use Quaternary Ammonium sanitizing solution in the sanitizing basin of the 3 compartment sink at 68 degrees F. The sanitizing solution was discarded and worked with food staff to retitrate to 102 degrees F (tested at 75 degrees F) and 200 ppm. Informed the person in charge Quaternary Ammonium sanitizing solutions are required to be at least 75 degrees F for use. Food contact equipment recently sanitized and drying on the drainboard were removed for cleaning and sanitizing.
- Accumulation of grime and debris was noted on nonfood contact surfaces of all equipment including inside refrigeration equipment, inside fryers, and shelving units. Discussed thorough cleaning of nonfood contact surfaces.
- Observed what appears to be spilled and dried shortening (white build-up on the ground inside and exterior of the enclosure area) . Discussed cleaning the area to prevent attraction of pests.
- The floors, walls, ceiling tiles (in both the back of the house and dining room) and air vents in dry storage area are in need of cleaning. Recommended a deep cleaning of the food establishment.
- Observed the following in poor repair: Burned out ceiling lights, ceiling light does not work in the walk in freezer #2 (outside), floor tiles are in need of grouting, and rusted air vent in the female toilet room.
- Observed accumulation of grime and debris on vent filters above the fryers.
- Stir Fry 88, 137 Spotsylvania Mall Drive #8006 Fredericksburg, VA 22407
- Date inspected: Dec. 29
- Total violations: 2
- The 2 faucets at the 3 compartment sink are leaking.
- The pre-rinse hose is hanging below flood level rim at the 3 compartment sink. During inspection, PIC removed pre-rinse hose.
- Popeye's, 735 Warrenton Road Fredericksburg, VA 22406
- Date inspected: Dec. 30
- Total violations: 7
- Observed a food employee handle raw chicken wearing a single use disposable glove and then proceed to enter the walk in cooler using the same glove which had observable debris on the gloves. Discussed the issue with the person in charge who instructed a food employee to sanitize the handle of the walk in cooler to prevent contamination of other staff. Discussed removing gloves when handling raw animal foods prior to entering the walk in cooler.
- Observed covered container of raw chicken stored over covered pickle buckets on shelving inside the walk in cooler. The person in charge instructed a food employee to relocate the pickle buckets to prevent contamination.
- Observed interior surfaces of the ice machine soiled.
- Observed improper cooling of prepared rice (89, 85 degrees F) Observed the rice cooling in two large and covered plastic containers on a cart by the walk in freezer. The person in charge reported the rice was prepared and placed in the containers at 10:12 a.m. and temperatures were taken at 2:01 p.m. The food establishment had a temperature log but did not document the cooling temperatures. The person in charge voluntarily discard the rice for safety. Discussed cooling time and temperature requirements and discussed methods to achieve rapid cooling.
- Observed improper methods to cool cooked riced in the food establishment. Observed rice, which was prepared with hot water, cooling in two large and covered plastic container on a cart by the walk in freezer. The rice was prepared at 10:12 a.m. and temperatures were taken at 2:01 p.m., internal food temperatures were observed at 89 and 85 degrees F. Discussed methods to achieve rapid cooling and time and temperature cooling requirements with the person in charge.
- Observed the following in poor repair: Ceiling light cover near the chicken breading station is not secured to the frame, ceiling tile overlay is separating from the tile over the shelving area by the fryers, floor tiles in the kitchen are in need of grouting.
- The following were observed in need of cleaning: Interior of ceiling light covers, black mat inside the walk in freezer, floor inside the walk in cooler, floor drain under the prep sink, interior of mop sink and air vent in the vicinity of the warewashing area.
However, not all the restaurants that the health department visited had violations. Here is a list of locations that were visited, but had no violations. Some were visited as part of pre-opening inspections, while others received routine or follow-up visits.
