Fredericksburg Restaurant Inspections: Sam's Pizza, China King
The Rappahannock Area Health District recorded violations at multiple restaurants around Fredericksburg, Stafford, and Spotsylvania.
FREDERICKSBURG, VA —
- Touch Hearts Center For Development, 2094 Jefferson Davis Highway Stafford, VA 22554
- Date inspected: March 30
- Total violations: 3
- Observed two chlorine spray bottles which exceeded 200 ppm when tested with a Chlorine test kit. Worked with the person in charge to dilute the Chlorine to 50 and 100 ppm, respectively.
- The person in charge could not provide a thin-probed food thermometer to monitor internal temperatures of thin-massed foods. The facility had a bimetallic thermometer which the sensor is not observable. Instructed the person in charge to purchase a thin-probed food thermometer.
- Observed two boxes of canned goods stored on the floor by the refrigerator. The person in charge reported they were purchased the week prior.

- Observed what appeared to be fruit flies throughout the facility, the premises is not being routinely inspected for the presence of insects, rodents, and other pests.
- Observed the following in need of repair: 1) The garbage disposal unit under the dishwasher sink; 2) a crack on the wall by the dishwasher machine, 3) re-caulk the dishwasher drain boards.

- Observed the employee finish eating, handle a push cart, go to the walk in cooler, and then handle tomatoes without wash her hands. The person in charge instructed the employee to wash her hands.
- Sam’s Pizza & Subs, 2142 Jefferson Davis Highway #Suite 101-102 Stafford, VA 22554

- Observed light emitting from the bottom and right side of the back door of the food establishment which leads to the dumpster area. A storage area exists between the back door leading to the exterior of the building and the back door of the food establishment, which is used to store packaged dry goods
- Observed 3 bags of onions stored on the ground in the storage area between the exterior door of the food establishment and the door leading into the dumpster area.
- Observed the following equipment in poor repair: Rusted shelving inside the walk in cooler, walk in cooler flooring is lifting up towards the back of the unit, crack inside the far left door of the pizza prep cooler and missing vent cover for the pizza prep cooler.
- Observed the Quaternary Ammonium test kit expired in 2021 and the person in charge could not provide a Chlorine test kit to monitor sanitization of the low temperature dishwasher.
- Observed accumulation of debris on the following nonfood contact surfaces: Base of pizza oven, shielded lights above the pizza oven and hood vent at the cookline, shelving and back of the flattop grill, inside fryer doors, base of 2 door upright freezer and shelving in the walk in cooler.
- Observed ceilings, walls and air vents in need of cleaning.
- Observed hood vent filters on the left side are soiled and also observed oily residue emitting from the vents on the front of the hood system.
- Observed 2.7 foot candles of light inside the back of the walk in cooler.
- Vinny’s Italian Grill, 20 Plantation Drive Suite 135 Fredericksburg, VA 22406

- Total violations: 14
- Facility does not have a certified food protection manager.
- Observed foods in the sandwich prep unit, reach in freezer, and walk in cooler that were not protected from possible contamination. The person in charge covered the items.
- Observed that the floor mixer, ice chute, can opener, and ice machine require cleaning, The person in charge instructed an employee to clean the items.
- Observed an oversize stock pot of pizza sauce that was made last night that measures at 77'F. The person in charge discarded the product.
- Observed a large stock pot of pizza sauce that was not adequately cooled. The product was discarded.
- Observed chicken sitting in water in the walk in cooler, ROP fish thawed inside of it's original packet in the walk in cooler, and calamari sitting in a pan on the prep sink thawing improperly. The fish was discarded.
- Observed several dead pests behind the reach in cooler.
- Observed one live roach inside of the can opener holder. The person in charge killed the roach.
- Observed the employees working in the food prep area without beard restraints. The line cook put on a restraint.

- Observed that the doors to the dish machine require cleaning.
- Observed that the exterior of several stainless steel pans and the can opener holder require cleaning.
- Observed that the floors behind and under the equipment is in need of detailed cleaing to include the rear storage room.
- Observed employee's phone and keys stored on the prep table. The person in charge removed the items.
- China King, 29 Banks Ford Parkway Suite 113 Fredericksburg, VA 22406
- Date inspected: March 31
- Total violations: 15
- The person in charge could not provide verification of employee health training but was knowledgeable of health reporting requirements. Emailed employee health documents in Spanish and Chinese to the operator.
- Observed an uncovered container of tea on the hot holding steam table which may contamination food if spills occurred. The food employee relocated the beverage container to the register desk.
- Observed a food employee handle a bleach container with their bare hands and then proceed to relocate a food container into the walk in cooler without washing their hands. Instructed the food employee to wash their hands. Discussed washing hands when there is a change in task.
- Observed uncovered containers of string beans, cut cabbage and tops of onions cut in the walk in cooler. Also observed various sauces and spices uncovered at the cook line which were not actively being used. The person in charge covered the food in the walk in cooler. Discussed covering the items at the cook line during the inspection but the person in charge did not place a protective cover over the containers.
- Observed raw shell eggs stored in a bowl with a head of cut cabbage. The person in charge discarded the cut cabbage during the inspection.
- Observed numerous stacked plastic and stainless steel containers on shelf above the 3 compartment sink which were soiled to the touch, interior surfaces of the raw chicken mixing and slicing equipment were also soiled. The person in charge reported the chicken equipment was not used today. The soiled containers over the 3 compartment sink were removed for cleaning and sanitizing. Instructed the person in charge to clean the chicken mixer and slicing machines prior to next use. Both pieces of equipment are cleaned in place.
- The person in charge reported the tongs are removed for cleaning daily. Instructed the person in charge to remove the tong, which are used in contact with TCS and Non-TCS foods at least once every four hours, he agreed to comply.
- Observed the following TCS foods were not dated to identify the date of preparation or discard. The person in charge reported the foods are held in reach in refrigeration units for a period up to 3 days: 1- Walk in cooler: Five containers of cooked pork dumplings (prepared morning of 3/30/22) and cooked chicken( pulled from freezer 3/30/22) and 1 door upright cooler #2- Two containers of cooked pork which were pulled from the walk in freezer on 3/30/22. Instructed the person in charge to date mark containers of TCS foods held longer than 24 hours. Discussed changing the date when foods are pulled from the walk in freezer.
- Observed numerous spices stored in take out containers which were not labeled to identify the contents.
- Observed soy sauce buckets stored on the floor in the front dry storage area and inside the walk in freezer. Also observed an opened box of broccoli stored less than one foot from the warewashing handsink.
- Accumulation of debris observed on the back of the fryers and shelving throughout.
- Observed accumulation of empty boxes/containers in the back of the facility near the back storage area. Remove refuse from the facility frequently to prevent attraction of pests.
- Observed ceiling tiles in the toilet room are absorbent and not easily cleanable.
- Observed employee personal articles such as cell phones, food and personal cooking equipment are scattered throughout the food establishment. Discussed segregating and labeling employee's food in one container in the walk in cooler. Discussed storage of employee personal articles in locations which will not contaminate food or equipment.
- Observed 13 foot candles of light under the hood vent at the cooking line. Discussed replacing the shielded lightbulbs.

- Observed that the baking equipment is in need of detailed cleaning. The person in charge moved the equipment to the dish machine area.
- Jersey Mikes Subs, 1607 Emancipation Highway Fredericksburg, VA 22401

- Observed brooms and dust pans blocking the handwashing sink. During the inspection, the brooms and dust pans were relocated. (Corrected)
- The maximum water temperature at the handwashing sink next to the prep sink is 73 F.
- Observed the following 4 metal containers of sliced tomatoes that were cut at 10:00 am (exceeded 4 hours during inspection) at temperatures ranging from 44F to 52F. During the inspection, the PIC discarded the tomatoes. (Corrected)
- Observed 2 metal containers of shredded lettuce (46F - 51F) on the prep line, at improper temperatures. During the inspection, the PIC discarded the lettuce. (Corrected)
- Observed several metal food container on a shelf over the 3 compartment sink that were stacked while wet after cleaning and sanitizing.
- The 3 compartment sink needs to be sealed to the adjoining wall.
- There is no test kit for monitoring the concentration of the chemical sanitizing solution at the 3 compartment sink.
- The sprayer at the 3 compartment sink is hanging below the flood level rim.
- No covered trash can in the women's restroom.
- The enclosed dumpster area is covered in leaves and debris.
- Observed wet mops sitting inside mop sink basin. During the inspection, the PIC hung mops. (Corrected)
- The following are in need cleaning: 1. All floors, ceilings, and walls. 2. The mop sink basin.
- Observed personal items (helmet, cell phone, personal mug, jackets) throughout the dry storage area.

- Observed containers of mac and cheese balls not date marked. Person in charge (PIC) stated the mac and cheese balls were prepared on 04/01/2022. During the inspection, the PIC date marked food. (Corrected)

- Employee was unable to provide documentation of training relating to the employee health policy for this facility. Ensure training records are kept in a location where they are easily accessible within the establishment. EHS provided an employee health policy sample- employees read and signed the form during the inspection.
- Observed a food employee cleaning the window with a chemical, wipe the counter with a towel then applied sanitizer on her hands and donned gloves; and was in the process of handling clean equipment for food preparation without first washing her hands. Employee was asked to wash her hands.
- Per discussion with the person in charge the food contact surfaces for equipment and utensils (utensils, bowls used for chicken, pizza slicer) used for time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the food regulations. The items sometimes get washed only once a day. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods.
- Observed the hood system and filters soiled with grime and debris.

- Observed a dented green beans can stored with five other undented cans on a cart in the kitchen. A food employee placed the cans on the cart to be opened, poured into a pan, heated, and served. The person in charge moved the dented can to a "Do Not Use" area.

- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Improper handwashing procedures observed. Observed sev3eral employees washing their hands for under 5 seconds. Employees were instructed in proper handwashing procedures.
- Observed the handwashing station at the bar being used to fill sanitizwer buckets. Person in charge was instructed to used hand washing sinks for handwashing only.
- Raw animal food stored over ready-to-eat food in the refrigeration unit. Observed raw chicken and beef stored over cooked chicken and cooked shrimp. Raw chicken and beef was re-located during inspection.
- The interior non-TCS food contact surfaces of the ice machine were observed soiled. Surfaces were cleaned during inspection.
- The prepared ready-to-eat (RTE) egg rolls and sweet and sour chicken in the refrigeration unit is not properly dated for disposition. Items were dated during inspection.
- Improper methods used to thaw pork ribs. Observed pork ribs being thawed in 105'F standing water. Water was drained and the internal temperature on the ribs measured 39'F. Ribs were placed in the walk in cooler.
- Observed cases and pans of chicken, beef and vegies stored on the floor in the walk in cooler.
- Wet wiping cloths were observed stored on the edge of the 3 compartment sink at the bar. Cloths were placed in sanitizer during inspection.
- Lockers or other suitable facilities are not provided for employees clothing and other possessions. Observed employee clothing stored on the dry storage shelving next to food items. Clothing was moved during inspection.
- The filters in the hood ventilation system and the hoods themselves were noted in need of cleaning.
