Fredericksburg Restaurant Inspections: Taste Of China, Worst Deli, Subway
Several restaurants around Fredericksburg and Stafford recorded violations during recent inspections from the local health department.
FREDERICKSBURG, VA — Several restaurants in the Fredericksburg area recorded violations during recent inspections from the local health department. In total, 19 restaurants had violations.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Chick Fil-A, 1698 Carl D. Silver Parkway Fredericksburg, VA 22401
- Date inspected: Feb. 23
- Total violations: 4
- The PIC could not provide a signed employee health policy agreement for the foode employees working in the kitchen. Food employee was unable to provide the foodborne illnesses and symptoms.
- Observed food employee drinking from a cup at the veggie wash prep sink. EHS informed PIC of observation. (Corrected)
- Observed 2 food employees wash their hands under running water and in less than 15 seconds. EHS demonstrated the proper handwashing procedure for food employees. Food employees re-washed hands before returning to work. (Corrected)
- The following are in need of cleaning: 1. The mop sink wall. 2. The wall next to the breading station. 3. The floor throughout the kitchen.
- Fortune House, 282 Deacon Road Unit 108 Fredericksburg, VA 22405
- Date inspected: Feb. 23
- Total violations: 2
- Observed several damaged cans stored in the dry storage area. The person in charge relocated the cans.
- Observed shelled eggs stored over ready to eat food.
- Los Dos Amigos, 117 N Main St Bowling Green, VA 22427
- Date inspected: Feb. 23
- Total violations: 3
- Employees observed working in the food service area without proper hair restraints.
- The clothespins, and L brackets used to hold tickets hanging over the flattop grill are not easily cleanable.
- The dish machine staging tables and the hand sink need to be sealed to the wall.
- Taste of China, 5030 Plank Road Fredericksburg, VA 22407
- Date inspected: Feb. 23
- Total violations: 13
- Observed the following: 5 bins of uncovered chicken stored on the floor of the walk in cooler and 6 trays of uncovered egg rolls stored on a rolling rack inside the walk in cooler. During the inspection the person in charge relocated the bins of chicken to a shelf and covered the trays of egg rolls
- Observed the following: the interior surfaces of the two kitchen microwaves were soiled, and the interior of the upright grill was left soiled from the usage of the previous day.
- Observed the back door of the facility is not self closing and the screen door of the kitchen back door is damaged and off set from the frame.
- Observed numerous boxes of cooking oil, numerous packages of canned sodas and bottled water, sacks of potato flour and white flour being stored on the floor of the dining room. In discussion with the person in charge the facilities dining room has been turned into a storage room.
- Observed numerous boxes of food stored on the floor of the walk in freezer and walk in cooler.
- Observed numerous boxes of single service used items(cups, go to boxes, toilet paper, napkins, plastic food containers, plastic food containers, etc.) stored on floor of the dining room. In discussion with the person in charge the facilities dining room has been turned into a storage room.
- Observed single service use containers being reused to hold foods and spices.
- Observed the kitchen handsinks, 3 compartment sink and dish machine area need caulking.
- Observed the following: the bottom of a prep table holding equipment is heavily rusted, shelves inside the walk in cooler are rusted, the door of the walk in freezer is in poor repair and no longer seals shut (ice biuld up was observed on the exterior of the door), floors of the walk in cooler and freezer were noted in poor repair.
- Observed the dumpster was uncovered and one of the doors of the dumpster is missing.
- Observed the following in poor repair: Floors of the walk in cooler and freezer are severly damaged and chipped, paint is chipping away of the employee bathroom door and wall behind one of the kichen handsinks.
- The following were observed in need of cleaning: Walls througout the kitchen are heavily soiled and stained and food debris was observed, several ceiling tiles are stained, floors of the walk in cooler, freezer and floors underneath equipment throughout the kitchen, exterior vents of the hood vent area, AC and ceiling vents of the kitchen and bathrooms were dusty. Overall recommend a deep and thorough cleaning of the kitchen.
- Observed employees personal belongings masks, water bottles, coffee mugs, keys, and cell phones, stored above a food prep table. During the inspection the person in charge relocated employees belongings stored away from all food storage and food prep areas.
- Bo’s Cafe, 10392 James Madison Parkway King George, VA 22485
- Date inspected: Feb. 23
- Total violations: 5
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The food-contact surface of the can opener blade is not cleaned at an appropriate frequency.
- Observed a large gap under the screen door at the rear of the kitchen.
- Observed a heavy buildup of ice in the 2 chest freezers at the rear of the kitchen.
- The dry storage shelving was observed soiled with grime and debris.
- Soulific Seafood (286-962), 6 Vaughn Ct Fredericksburg, VA 22405
- Date inspected: Feb. 23
- Total violations: 9
- The person in charge could not provide documentation of employee health training, he reported signed documents were held offsite. Instructed the person in charge to email documentation of employee health training. Ensure signed documents are available during inspections.
- Observed interior of the microwave oven soiled.
- Observed the handsink and 3 compartment sink basins are not sealed to the countertop.
- The person in charge could not provide Chlorine test kit. A pH test kit was available during the inspection.
- Observed accumulation of debris on the following nonfood contact surfaces: Interior of the prep cooler including the gaskets, exterior of the thin probed food thermometer and grate inside the fryers.
- Observed absence of adequate water pressure at the only handwashing sink on the mobile unit. The water has a slow and thin stream of water when turned on which is not adequate for effective handwashing. Instructed the owner to discontinue operations until the water pressure is restored.
- Observed absence of adequate hot water at the handsink (80 degrees F) and 3 compartment sink (85 degrees F). Contact plumber to evaluate the hot water capacity. Instructed the owner to discontinue operations until hot water is restored due to inability to heat water on the mobile unit.
- Observed leak under the 3 compartment/handsink and water leaking from the faucet at the 3 compartment sink.
- Observed the floors, walls and ceilings in the mobile unit in need of cleaning.
- Mason-Dixon Cafe, 11 Hope Rd Suite #115 Stafford, VA 22554
- Date inspected: Feb. 24
- Total violations: 2
- Observed the sealant on the concrete floor in the back of the house has eroded. The floor is no longer easily cleanable.
- The walls, ceiling tiles and caulking along the sinks were observed in need of cleaning.

- Total violations: 1
- Observed 4 containers of whipped cream in the walk in cooler that expired on 2/15/22. The PIC discarded the products.
- Pancho Villa Mexican Restaurant, 752 Warrenton Road Fredericksburg, VA 22406
- Date inspected: Feb. 25
- Total violations: 4
- Observed hand sanitizer bottle with a pump dispenser stored on a shelf above packaged spices. The hand sanitizer was relocated during the inspection.
- Observed shelving inside the walk in cooler and prep cooler has become rusted and is not longer smooth and easily cleanable. The person in charge reported new shelving has been ordered.
- Absence of alarm or other visual or audio means to alert staff if detergent or sanitizers are not delivered to complete the washing and sanitization cycle requirements.
- Observed numerous blown out ceiling lights throughout the facility and and ceiling light cover by the dry storage room is not secured to the frame and is hanging which presents a safety risk.

- Total violations: 4
- Observed the interior surfaces of the two microwaves and two toaster ovens were soiled with food debris and residue.
- Person in charge could not provide a test kit for the chlorine sanitizing solution used in the facility.
- Observed the interior surfaces and gaskets of the reach in cooler and freezer are in need of cleaning.
- Observed several ceiling tiles missing and some ceiling times damaged in the womens bathroom.
- Worst Deli, 1120 International Parkway #Suite 123 Fredericksburg, VA 22406
- Date inspected: Feb. 25
- Total violations: 11
- Observed food employee use single use disposable gloves for multiple tasks. A food employee touched raw beef wearing single use disposable gloves and then proceeded to handle cleaned equipment wearing the same disposable gloves. Instructed the food employee to remove the gloves and wash their hands to prevent contamination. Discussed proper glove use with the person in charge.
- The menu does not include a consumer advisory for eggs which may be cooked raw and/or undercooked per a customer's request.
- Observed improper thawing of raw animal foods during the inspection. Observed raw beef in a plastic storage container (24 degrees F) thawing on the prep sink drainboard during the inspection. The raw beef was relocated to the walk in cooler during the inspection.
- The person in charge could not provide a thin probed thermometer to monitor internal temperatures of thin-massed TCS foods.
- Observed soy sauce bucket and chili paste container stored on the floor by the prep table in the back of the house and a food container stored on the floor inside the walk in cooler.
- Observed 4 packages of raw beef being stored inside Carter's bread bags inside deep freezer #2. Informed the person in charge the bread bags are cleanable and not permitted for food storage. During the inspection the person in charge discarded the raw beef.
- Observed accumulation of debris inside reach in refrigeration units.
- Observed the toilet room door is not self-closing.
- The floors throughout the facility and the mop room are in need of cleaning.
- Observed numerous blown out ceiling lights in the food establishment and the air vent is not sealed to the ceiling in the toilet room. Discussed sealing the gap during the inspection.
- Observed 0.7 foot candles of light inside the walk in cooler and 6.5 foot candles of light inside the toilet room
- Subway (Leavells), 11105 Leavells Rd Suite 5 Fredericksburg, VA 22407
- Date inspected: Feb. 25
- Total violations: 10
- During the inspection observed the lack of active managerial control from the person in charge.
- Person in charge was unable to provide an employee health policy. EHS provided a copy an employee health policy.
- Observed a roll of paper towels and an empty stainless steel container inside the handwash sink. During the inspection items were removed from the handwash sink.
- In discussion with the person in charge, the in use utensils at the service line do not get washed every 4 hours. EHS discussed washing and sanitizing frequency of in use utensils with the person in charge.
- Observed the interior of the microwave was heavily soiled.
- Observed the only food employee not wearing a hair restraint.
- Observed water leaking from the fan of the walk in cooler.
- The shelving above the 3 compartment sink area and shelf above food prep table were soiled to sight and touch.
- Observed leaking water around the front hand sink. In discussion with the person in charge there is a potential water leak in the soda machine.
- Observed an employees personal belongings (cell phone and keys) on or above a food prep table.
- Target Store T-2175, 25 S. Gateway Drive Fredericksburg, VA 22406-1228
- Date inspected: Feb. 25
- Total violations: 6
- Observed the ceiling light covers and ceiling tiles throughout the back of the house and floor under the open shelving by the TurboChef in need of cleaning.
- Observed four ceiling tiles not secured to the frame and multiple burned out ceiling lights in the back of the house. During the inspection the person in charge instructed maintenance to address the ceiling tiles.
- Observed a Y connection at the mop sink with a hose sitting in the base of the mop sink connected to the faucet without adequate backflow prevention. Provided backflow prevention handout and during the inspection the hose was elevated off the base of the mop sink to create an adequate air gap.
- Observed spray bottle of TurboChef cleaner stored on a counter over packaged food. The person in charge removed the spray bottle to prevent contamination.
- Through discussion with the person in charge the steamer wands on the espresso machines are removed for cleaning once daily. Informed the person in charge the steamer wands are required to be removed once every 4 hours for cleaning and sanitizing due to contact with TCS beverages. The person in charge agreed to comply and reported the facility has other wands which can be used.
- Observed a food employee clean the counter under the espresso machine with Quaternary Ammonium sanitizer with their bare hands and then handle uncovered single use disposable beverage cup without washing their hands. Instructed the food employee to wash their hands and discussed handwashing requirements with the person in charge.
- Sonic Drive In, 10045 PATRIOT HWY Fredericksburg, VA 22407
- Date inspected: Feb. 28
- Total violations: 12
- Observed a food employee use the same disposable gloves to sanitize a nonfood contact surface with a Quaternary Ammonium sanitizing solution and then proceed to handle a disposable beverage lid without removing their gloves and washing their hands. Gloves were observed used for more than one task. Instructed the food employee to remove gloves and wash their hands and discussed correct glove use during the inspection. The disposable beverage lid was voluntarily discarded during the inspection.
- Observed interior surfaces of the ice machine soiled.
- Through discussion with the person in charge, utensils at the cook line, which are contact with TCS foods, are removed for cleaning and sanitizing every 6-8 hours. Informed the person in charge equipment and utensils in contact with TCS foods are required to be cleaned and sanitized at least once every 4 hours. The person in charge agreed to comply.
- Observed tater tots (120, 122 degrees F) hot holding at improper temperatures in the hot holding unit. The person in charge reported the tater tots were placed in the hot holding station at 10:00 a.m. and the temperature was taken at 10:15 a.m. The person in charge discarded the food during the inspection, discussed using time as a public health control.
- Observed packaged frozen chicken tenders stored under leaking lines inside the walk in freezer. The facility installed a shelf with thin trays stored under the leaking lines to catch dripping water. The person in charge instructed a food employee to relocate the packaged food to prevent contamination. Discussed contacting a technician to conduct repairs.
- Observed the following equipment in poor repair, equipment is no longer smooth and easily cleanable: Rusted shelving inside the 2 door upright cooler and shelving above the 3 compartment sink, numerous cracked and/or chipped plastic trays and food storage containers stored on shelving by the 3 compartment sink, 2 stainless steel containers with pitting which is creating a surfaces which is not easily cleanable. During the inspection the person in charge discarded the stainless steel containers.
- Observed accumulation of debris on the following nonfood contact surfaces: Steamer basket, top of the toaster, shelving at the 3 compartment sink, black cart, debris at the back of the flattop grill.
- Observed the oil storage barrels do not have a lid and the side doors of the dumpsters were not closed to prevent attraction of pests.
- Observed accumulation of debris around the dumpsters and oil on the exterior of oil storage receptacles and ground near the oil storage area in the dumpster area.
- Observed the following in poor repair: Ceiling tiles in the hallway by the walk in refrigeration units are not sealed to the frame and the floor grouting in the vicinity of the cook line has eroded.
- Observed the floors, walls, air vent by the back door and drain under the 3 compartment sink in need of cleaning.
- Observed 7.2 foot candles of light inside the walk in cooler.
- Pupuseria Dona Azucena, 60 Susa Dr STE 127 Stafford, VA 22554

- Observed that the oven requires cleaning.
- Charlie G's, 314 Emancipation Highway Fredericksburg, VA 22401
- Date inspected: March 1
- Total violations: 5
- The following are in need of cleaning: 1. The floor and walls along the prep and cook line are heavily soiled with food debris. 2. The wall behind the dishwasher is soiled with black debris. 3. The mop sink basin is soiled.
- The following are in need of cleaning: 1. All equipment along the prep and cook line were observed soiled with food debris. 2. Standing water at the bottom of the keg coolers.
- The following are in need of repair: 1. One light out over grill. 2. Gaps between hood filters over cook line.
- Observed 2 thawed ROP salmon in a plastic food container in the walk-in cooler. The label on the ROP stated, "If thawing remove from vac pack thaw under refrigeration temperature." During the inspection, the PIC discarded the 2 ROP salmon. (Corrected)
- The following were not labeled: 1. A spray bottle containing bleach sanitizer sitting on a shelf next to a stack of clean plates. 2. A spray bottle containing Windex hanging on the counter in the bar area. During the inspection, the PIC labeled the spray bottles. (Corrected)

- The interior surfaces of the ice machine were observed soiled. The machine was cleaned during inspection.

- The food establishment does not have a certified food manager and uses time as a public health control for waffle batter.
- The person in charge could not communicate symptoms and illnesses requiring reporting, restriction or exclusion and could not provide documentation food employees have been trained on an employee health policy. Provided sample policy during the inspection.
- Observed interior of the microwave oven soiled.
- Observed significant presence of mouse droppings on the floor and under shelving units behind the door, inside cardboard box used to hold single use containers of cereal and towel used to store bananas in the kitchen. Through observation of the suspected rodent activity, it appears staff are not monitoring the facility for the presence of pests. The person in charge reported pest control has been contacted.
- Observed sealed single use containers of cereal stored in a cardboard box and bananas stored on a towel with what appears to be mouse droppings. During the inspection the person in charge voluntarily discarded the food for safety.
- Observed QT-10 and QT-40 test kits expired in 2017 (QT-10) and 2018 (QT-40). Instructed the person in charge to purchase new test kits.
- Observed accumulation of debris on the following nonfood contact surfaces: Shelving throughout the kitchen, base and interior door of the convection oven, upsplash of the coffee brewer and nonfood contact ceramic container near the microwave oven
- Observed numerous ceiling lights blown out and ceiling tile behind 2 door upright cooler #1 is not tightly secured to the ceiling frame.
- Observed ceiling light cover above the 2 door upright cooler #2 and floors throughout the facility in need of cleaning. Observed significant presence of mouse droppings on the floor behind the door and shelving unit near the kitchen entrance.
- Observed 14 foot candles of light in the vicinity of the warewashing area and observed numerous ceiling lights blown out.
- Merit School of Leeland #31, 115 Primmer House Road Stafford, VA 22405
- Date inspected: March 1
- Total violations: 3
- Observed the filters in the hood system to have a build up of what appeared to be dust.
- No quaternary ammonium testing kit available. Person in charge will purchase a test kit.
- The concentration of the quaternary ammonium sanitizer solution at the 3 compartment sink and the sanitizer bucket measured 100 PPM, according to the manufacture instructions the concentration should be between 150-400 PPM. The person in charge contacted a technician to come service the dispensing device. The technician will be at the facility within a couple of hours.
