Fredericksburg Restaurant Inspections: Truck Tires In A Cafe

The Rappahannock Area Health District cited another 25 restaurants with violations during recent inspections in Fredericksburg and Stafford.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected multiple restaurants across the region from Aug. 4 to Aug. 1o; they found 25 restaurants with violations. One location even had truck tires (with rims) in their restaurant.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department also performs a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Fork N Biscuit, 715 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Aug. 4
    • Total violations: 13
      • A food employee touched their face mask with their bare hands and then proceeded to don single use disposable gloves to engage in food preparation tasks and 2- Observed a food employee batter raw chicken wearing single use disposable gloves, remove gloves and then proceed to open the prep cooler without washing their hands. Instructed the food employee to wash their hands and discussed hand washing requirements with the person in charge.
      • Observed soiled tongs and spatula on shelving unit above the hot holding station and 2 knives stored in a container at the prep area. The soiled equipment was relocated to the three compartment sink to be cleaned and sanitized.
      • Observed raw shell eggs on a shelf near the cook line at 69 degrees F. A food employee reported the eggs were taken out of refrigeration at 10:15 a.m, temperature was taken at 11:11 a.m. The eggs were relocated to the 2 door upright cooler to cool to 41 degrees F or less by 2:15 p.m.
      • Observed the following TCS foods cold holding at improper temperatures inside the prep cooler: Pimento cheese spread (48 degrees F), Sliced tomatoes (48 degrees F), Corned beef (43 degrees F), Sliced ham (48 degrees F), Smoked salmon (46 degrees F), Cole slaw (46 degrees F), Sliced cheese (53 degrees F), and Raw steak (50 degrees F). The person in charge reported the TCS foods have been in the unit overnight and voluntarily discarded TCS foods for safety. Instructed the person in charge to discontinue of the refrigeration unit to cold hold TCS foods until evaluated by a technician.
      • Observed four gallons of milk in the walk in cooler with an expiration date of 8/1/2021, one gallon of milk in the walk in cooler with an expiration date of 8/3/2021 and one gallon of milk in the bar reach in cooler with an expiration date of 7/14/2021. The person in charge voluntarily discarded the milk for safety.
      • Observed food employees working in the kitchen without proper hair and beard restraints.
      • Observed one wet wiping cloth stored on top of the prep cooler and one wet and soiled wiping cloth stored on the prep table in the bakery section of the kitchen. The soiled wiping cloth was removed to be cleaned and the wiping cloth stored on top of the prep cooler was placed in a wiping cloth bucket with Quaternary Ammonium sanitizer.
      • The person in charge could not provide a testing kit for the Lactic Acid food grade sanitizer (Monogram Sink and Surface Sanitizer).
      • Accumulations of debris were noted on the following nonfood contact surfaces: 1- Interior and gaskets of prep cooler, 2- inside fryers and 3- Base of convection oven.
      • Observed the following in poor repair: 1- Missing coving tile in the bakery prep area, 2 - Cracked floor tiles near the prep cooler, 3- Missing ceiling tiles in the basement over covered single use article storage and above the chemical storage.
      • Ceiling tiles in the female toilet room are not smooth, easily cleanable or nonabsorbent.
      • The floors throughout the facility, especially under equipment, soiled ceiling tiles above the bar and ceiling vents in the toilet rooms are in need of cleaning.
      • Observed less than 10 foot candles of light in the dry storage area of the basement , which is used to store dried food goods, beverages and single use articles. Observed blown out light bulbs in the vicinity of the shelving units.
  • Dunkin Donuts/Baskin Robbins, 5101 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Aug. 5
    • Total violations: 2
      • The surfaces of the cutting boards along t he prep units were observed stained and soiled.
      • Both hand sinks in the kitchen need to be re-sealed to the walls.
  • Comfort Inn & Suites, 24058 Welcome Way Dr Ruther Glen, VA 22546
    • Date inspected: Aug. 5
    • Total violations: 12
      • Absence of active managerial control observed. The person in charge is not monitoring temperatures of TCS foods during cold holding, preventing bare hand contact with ready to eat foods, absence of food grade sanitizer in the facility and communication with food staff demonstrates an absence of sanitizing taking place for food contact equipment and utensils.
      • The food establishment does not have a certified food manager.
      • Observed a food employee wash their hands in the three compartment sink. Instructed the food employee hands are required to be washed in the hand sink. The food employee washed their hands in the hand washing sink.
      • Observed a food employee cut a lemon to be placed in a container of water with their bare hands. Informed the food employees bare hand contact with lemons to be placed in water for customers is prohibited. The food employee voluntarily discarded the lemons for safety.
      • Through discussion with food employees and absence of food grade sanitizer in the food establishment, the food establishment is not sanitizing food contact equipment and utensils after cleaning. A food employee reported food contact equipment are only being washed. Instructed the person in charge food contact equipment and utensils are required to be sanitized after cleaning. Instructed the person in charge to clean and sanitize all food contact equipment and utensils once sanitizer is purchased and to discontinue use of any equipment which has not been sanitized.
      • The person in charge reported the juice and coffee machine dispenser nozzles are removed for cleaning every two days.
      • Observed the following TCS foods cold holding at improper temperatures: 1- Whirlpool residential refrigerator: Milk (47, 49 degrees F) and single use container of cream cheese (48 degrees F) and 2- Small reach in cooler in the dining room - Single use container of yogurt (51 degrees F) and single use container of cream cheese (47 degrees F). The person in charge reported the TCS foods have been in the refrigeration units overnight and voluntarily discarded the foods for safety. Contact technician to evaluate the cold holding capacity of the refrigeration units. Discussed trying to lowering the temperature in the units with the person in charge.
      • Absence of ambient air thermometer observed inside the Whirlpool and Haico homestyle refrigerators.
      • Absence of food labels observed on cereal dispenser and cabinet used to store muffins/danish/bagels which are available for customer self-service in the dining room.
      • Observed the following in poor repair: Broken handles of the Whirlpool homestyle refrigerator and Frigidare 1 door upright cooler and right side cabinet under the juice machine in the dining room does not close completely.
      • The person in charge provided a Quaternary Ammonium testing kit, however, the facility uses Chlorine bleach. Instructed the person in charge to purchase a Chlorine testing kit to monitor the concentration of Chlorine sanitizer.
      • Absence of a food grade sanitizer in the food establishment. Instructed the person in charge to purchase plain unscented Chlorine bleach.
  • Merge Cafe, 44 Mine Rd 7 Stafford, VA 22554
    • Date inspected: Aug. 5
    • Total violations: 9
      • Observed the following hand washing violations: -Observed a food employee handling food equipment, waited on a customer, then she proceeded to make a customers drink without washing thier hands. Observed a food employee touch his hair, then don gloves without washing his hands.
      • Observed no paper towels available at the hand washing sink.
      • Per conversation with the person in charge, the facility is not sanitizing the food contact surfaces and not wash, rinse and sanitizing the continuous use equipment every 4 hours. During the inspection the EHS discussed the correct cleaning procedures with the person in charge.
      • Observed the interior of the ice machine and the the lids of the two blenders and 2 shaker lids soiled.
      • Observed the following items cold holding at improper temperatures: -whole milk in the reach in cooler 42 degrees -mango mix in the reach in cooler 52 degrees -whole milk in the rear reach in cooler 43 degrees
      • Observed cooked pork and beef on the front holding line not date marked.
      • The person in charge was unable to provide a bleach sanitizer test kit. EHS provided the person in charge with a test kit
      • The person in charge was unable to provide a sanitizer for the three compartment sink. During the inspection the several bottles of bleach were found the chemical storage area.
      • Observed the following non food contact items require cleaning: -storage shelves on the front line -top of the oven -inside of refrigeration units
  • Poke Mian, 325 Garrisonville Road #105 Stafford, VA 22554

    • Total violations: 5
      • Observed a food employee wash hands for 15 seconds.
      • Observed raw eggs stored above vegetables in the walk in cooler.
      • Observed two knives hanging on the metal knife magnetic strip soiled with food debris.
      • Observed the toaster oven in the front heavily soiled.
      • Observed wet stacking of metal containers and wooden bowls. Items were taken back to the three compartment sink.
  • Fairfield Inn & Suites, 2784 Jefferson Davis Highway Stafford, VA 22556
    • Date inspected: Aug. 5
    • Total violations: 1
      • The food establishment does not have an employee who is a certified food manager. The person in charge reported she has registered for a certified food manager course.
  • Kobe Japanese Steakhouse and Cafe, 10119 Southpoint Parkway Fredericksburg, VA 22407

    • Total violations: 14
      • Observed cigarettes, a lighter and nail clippers stored on a shelf above the prep table.
      • Observed 4 large truck tires (with rims) behind the counter at the sushi station.
      • The following physical structures in the kitchen are not cleaned as often as necessary to keep them clean: 1-The walls throughout the kitchen, 2-the floors throughout the kitchen, 3-the mop sink, 4-the water heater.
      • Observed a knife and a bench scraper stored between the flattop grill and a prep table.
      • Food stored in a location where it is subject to splash, dust or other contamination. Observed frozen packages of raw beef stored on the floor of the walk in freezer. Two packages were being used a a support for the lower shelf on one of the racks.
      • Observed that the back door in the kitchen was propped open, allowing many flying insects into the kitchen.
      • The person in charge could not provide a temperature measuring device.
      • Observed bottles of aspirin, vitamins, energy drink mix and fish oil pills stored on a shelf above food preparation area. Items were removed during inspection.
      • Sushi rice for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time. Rice was labeled during inspection.
      • Rice in the walk in cooler, which was prepared the previous day measured an internal temperature of 47'F. Rice was voluntarily discarded during inspection.
      • The food contact surfaces of the deep fryer and the toaster oven at the sushi station was observed soiled.
      • Observed that the surfaces of all of the cutting boards in the kitchen were observed heavily stained. Boards were taken to the 3 compartment sink to be cleaned.
      • The interior surfaces of the ice machine was observed soiled.
      • The person in charge is not able to provide a verbal or written policy for preventing the transmission of foodborne disease by an employee who has a disease or a medical condition.
  • Dunkin Donuts & Baskin Robins, 305 Garrisonville Road Suite #102 Stafford, VA 22554
    • Date inspected: Aug. 5
    • Total violations: 2
      • Observed wiping cloths held in bucket with Quaternary Ammonium sanitizer by the hot chocolate machine and near the coffee brewers with no measurable sanitizer in the container. The person in charge discarded the sanitizer solution and retitrated to 200 ppm.
      • Accumulation of debris was noted on the following non food contact surfaces: Base/shelving of the self service beverage cooler, up splash of the hot chocolate machine, exterior surfaces of the ice bin at the ice cream station.

    • Date inspected: Aug. 6
    • Total violations: 10
      • The employee health policy does not include symptoms and illnesses requiring reporting. The current policy states employees are prohibited from working when they are sick. Provided sample employee health policy to train employees.
      • Observed a food employee touch their apron with their bare hands and then proceed to handle a ladle in the meatballs being reheated without washing their hands. Instructed the food employee to wash their hands and the ladle was removed to the 3 compartment sink for cleaning.
      • Absence of handwashing signage posted at the front handsink. Provided sample handwashing signage during the inspection.
      • The person in charge could not provide a thin probed food thermometer to monitor internal temperatures of food. The food thermometer in facility was not working.
      • Observed the back door to the food establishment does not adequately seal at the bottom, observed light coming through the bottom right side of the door.
      • Observed presence of flying insects in the kitchen and dining room. Contact pest contest control to conduct an inspection and treatment as needed. Observed presence of multiple trapping devices in the food establishment.
      • Observed equipment (attachment) used to fill juice machines with water stored inside the cabinet under the soda machine which may be exposed to contamination. Instructed the person in charge to clean and sanitize the equipment prior to next use and store in a way which will prevent cross contamination. Discussed wrapping or storing the connected in a covered container to prevent contamination. Also observed single use packets of sugar and disposable cups and carry out sauce containers stored under the plumbing connections of the front handsink
      • Accumulations of debris were noted on the following nonfood contact surfaces: Interior cabinet under the front handsink. container used to store various utensils on the warewashing shelving, backsplash of the juice machine, spill under the soda machine cabinet.
      • The following were observed in need of cleaning: Walls and floor in the warewashing area and ceiling vents in the toilet rooms.
      • Observed lighting inside the walk in cooler at 1 foot candle of light.
  • Benny’s Sub Shop, 16430 Consumer Row King George, VA 22485
    • Date inspected: Aug. 6
    • Total violations: 4
      • Observed two meat cleavers stored in between the prep unit and the prep table. Cleavers were removed during inspection.
      • The prepared ready-to-eat (RTE) egg rolls (3 tubs) in the walk in cooler were not labeled with a ""consume by"" date. Tubs were date marked during inspection.
      • The sides of the deep fryers and the flattop grill was observed covered with grease and debris.
      • Observed employee's cell phones, wallets and keys on the rear prep table and the food slicer. Items were moved during inspection.
  • Taco Bell, 10491 Hilltop Plaza Way Spotsylvania, VA 22553
    • Date inspected: Aug. 9
    • Total violations: 3
      • Observed food employee wearing gloves wipe gloved hands on apron and then wiped food contact surface of the taco rail. During inspection food employee discarded gloves and washed hands before returning to work
      • The non-food contact surfaces of the following were observed soiled: 1. The ice shoot on ice machine in the dining area. 2. The ice shoot on the ice machine at the drive thru window. 3. The clam shell grill.
      • The following are in need of cleaning: 1. The wall behind and ceiling vents over the chemical storage area. 2. The caulking at the 3 compartment sink. 3. The caulking at the mop sink.
  • Koi Tea Corporation, 418 Garrisonville Rd #110 Stafford, VA 22554
    • Date inspected: Aug. 9
    • Total violations: 7
      • Observed the employee wash his hands then turn the water off without using the paper towels.
      • The interior of the ice machine requires cleaning.
      • Observed cream of coconut cold holding at 48'F. The product was discarded.
      • Observed that the quaternary solution measured 400 ppm in the 3-compartment sink. The sanitizer solution was remade.
      • Observed several chemical spray bottles that were not labeled. The person in charge labeled the bottles.
      • The cutting board is scratched and scored.
      • Observed a hose with a sprayer nozzle attached at the 3-compartment sink that is missing the backflow prevention device.
  • Little Caesars, 1465 Stafford Market Place Suite 109 Stafford, VA 22554
    • Date inspected: Aug. 9
    • Total violations: 14
      • Observed several broken floor tiles in front of the 3-compartment sink and the wall tiles are coming off across from the 3-compartment sink.
      • The floors, walls, and ceiling tiles require cleaning along with the wall behind the 3-compartment sink, the floor drains, and the air vent covers.
      • Observed that the toilet room door is not self-closing.
      • Observed that the oven mitts, tops of the tongs, gaskets on all refrigeration units, and all the storage shelves require cleaning.
      • Observed that the blades on the pizza cutters are chipped and the pizza prep unit is leaking.
      • Observed plastic containers that were stacked wet.
      • Observed single service used wiping cloths stored on top of the oil machine.
      • Observed an employee wearing wrist braclets and a watch while preparing foods.
      • Observed a fly strip installed over the 3-compartment sink.
      • Observed chemical bottle hanging on the hand sink, stored on the shelf with the sauces, and stored on the front counter with the equipment. The person in charge moved the bottles.
      • Observed that the sausage, cheese, pepperoni, cut vegetables are not date marked.
      • Observed that the pizza rings are soiled. The person in charge moved the items to the 3-compartment sink for cleaning.
      • Observed that the water at the hand sink measured 75'F. EHS discussed this with the person in charge
      • Observed the employee wash their hands in cold water for less than 15 seconds. EHS discussed this with the employee

      • Observed absence of hand drying provision at the warewashing handsink. The person in charge directed an employee to restock the paper towels.
      • Observed interior surfaces of the ice machine soiled.
      • Observed single use container of chocolate milk (44 degrees F) cold holding at improper temperatures. The milk was stored near the door. The person in charge voluntarily removed the crate of chocolate milk to be discarded and turned down the temperature of the unit. The milk observed at 42 degrees F were placed on ice and the temperature was lowered during the inspection. At the end of the inspection the temperature was observed at 38 degrees F.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor the hot water sanitization of food contact equipment in the high temperature dishwasher.

      • Observed shell eggs stored above vegetables in the walk in cooler. The person in charge relocated the eggs.
      • Observed sliced meats, shredded cheeses, meatballs, and sauces in the refrigeration units that were not date marked.
      • Observed raw chicken stored in to-go plastic bags in the reach in freezer.
      • Observed single service items that were stored with the food contact surfacing upwards. The person in charge provided protection for the items.
      • Observed that the doors and sanitizer solution cup of the dish machine require cleaning.

    • Total violations: 5
      • The following were observed: 1. One ceiling tile is missing and the wall is in need of repair in the mop sink room. 2. One ceiling tile is missing in the dishwashing area. 3. The ceiling in front of the ice machine is in need of repair.
      • The following are in need of cleaning: 1. The mop sink in the mop sink room. 2. The caulking along the drainboards at the high temp dishwasher. 3. The ceiling and walls next to the conveyor belt dishwasher.
      • Observed several flying insects in the following locations: 1. Next to a cart in the cold commissary area. 2. In the chemical room. 3. In the dry storage room next to the catering walk-on freezer.
      • Observed food employee rinse hands in the prep sink. EHS informed food employee to use handsink for handwashing. Food employee acknowledged and left kitchen.
      • No hand soap at the handwashing sink in the dishwashing area. During inspection, person in charge provided hand soap.
  • Taco Bell/KFC, 5400 James Madison Parkway Dahlgren, VA 22485
    • Date inspected: Aug. 10
    • Total violations: 3
      • The cutting board(s) along the make line are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
      • The nonfood contact surfaces of the following equipment have accumulations of grime and debris: 1-the hot holding drawer cabinet, 2-outer surfaces of the chicken hot holding cabinet, 3-outer surfaces of the taco shell hot holding cabinet, 4-the handles on all hot holding cabinets.
      • The floors underneath all of the equipment and shelving in the kitchen was noted in need of cleaning.

      • The non-food contact surfaces of the floor fan in the kitchen and the outer surfaces of the deep fryer have accumulations of grime and debris.

      • Observed 1 cracked and 1 warped light shield cover in the dishwashing area.

    • Date inspected: Aug. 10
    • Total violations: 6
      • Observed containers of uncovered tortilla chips and cut limes stored on top of a prep table which were not actively being used. The person in charge instructed a food employee to place protective wrapping on the containers to prevent contamination.
      • Observed a full container of brown rice (127, 131 degrees F) hot holding on the To-Go hot holding unit at improper temperatures. The person in charge reported the rice was pulled from the hot holding cabinet at 1:00 p.m. and voluntarily discarded the food for safety. Discussed holding less product on the hot holding table and stirring frequently.
      • Observed cut lettuce (45 degrees F) cold holding at improper temperatures at the To-Go cold holding unit. The person in charge reported the lettuce was pulled from the walk in cooler at 12:30 p.m. and placed in the unit. The person in charge relocated the lettuce to the walk in cooler to cool to 41 degrees F or less by 4:30 p.m. Discussed holding less product in the cold holding unit to ensure adequate cold holding temperatures are maintained.
      • Observed presence of small flying insects in the food establishment and observed the back door propped open during the inspection. The person in charge closed the back door during the inspection. Instructed the person in charge to contact pest control to conduct an inspection and treatment as needed.
      • Observed numerous stainless steel food contact equipment containers and bowls which were stacked while wet after chemical sanitizing.
      • Observed oil, to be discarded, is being maintained in the back of the food establishment for an unknown period of time. The person in charge could not communicate how often the oil is being picked up for disposal from the waste disposal company. Discussed purchase of a oil storage unit to be placed in the dumpster area to prevent storage in the food establishment.
  • Moe’s Southwest Grill, 1495 Suite #111 Stafford Market Place Suite# 111 Stafford, VA 22556
    • Date inspected: Aug. 10
    • Total violations: 12
      • Observed that the following physical facilities must be cleaned: hood equipment, ceiling tiles throughout the facility, and the air vent covers in the walk in cooler.
      • Observed that the front hand sink is coming off the wall, lights out throughout the kitchen, duct tape was used to repair the hole in the wall near the entrance way, leak in the cabinet in the lobby, and ceiling tiles in the restroom and over the ice machine are in disrepair.
      • Observed that the cabinet under the fryer and plastic holder in the lobby where the portioning cups are stored.
      • Observed broken knives on the metal knife rack and broken lids on the chip containers.
      • Observed wet containers on the front line and on the clean equipment rack.
      • Observed boxes of equipment stored on the floor. Observed that a splashguard is needed at the front hand sink.
      • Observed that the inside of the ice machine requires cleaning.
      • Observed that the employee washing the equipment did not leave them in the sanitizer for 30 seconds. The person in charge discussed this with the employee.
      • Observed that the hand washing sign was missing at the front hand sink and the in men's restroom.
      • Observed the employee wash his hands in the 4-compartment sink. The PIC discussed this with the employee.
      • Observed the employee touch the counter top, scoop chips and placed them in a bag, work on the cash register, then donned gloves. Observed that the employee is handling soiled equipment then clean equipment without washing his hands. The person in charge discussed this with the employees.
      • The employees are not aware of health procedures.
