Fredericksburg Restaurant Inspections: Violations At 15 Places

The Rappahannock Area Health District found more than a dozen restaurants around Fredericksburg with violations during recent inspections.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected several restaurants across the region from Sept. 15 to Sept. 21; they cited more than a dozen restaurants with violations.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Hard Times Café, 5099D Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Sept. 15
    • Total violations: 3
      • Observed wet stacking of stainless steel and plastic containers on the warewashing shelving unit. The person in charge removed the containers to be cleaned and sanitized.
      • Observed working containers of soy flakes, cornbread mix and flour which were not labeled to identify the contents.
      • Observed the following TCS foods cold holding at improper temperatures in prep cooler #2: Diced tomatoes (42 degrees F), Cut cabbage (49 degrees F), Sliced tomatoes (42, 44 degrees F), Cut lettuce (46 degrees F), salsa (45 degrees F) and hard boiled egg halves (46 degrees F). Observed multiple food containers on the top of the unit overfilled. The person in charge reported the foods were pulled from the walk in cooler at 11:30 a.m., temperature was taken at 2:15 p.m. The person in charge directed food staff to remove the majority of the food to place in the walk in cooler on ice and small portions of foods were left in the prep cooler, using a 4 hour time hold. A food employee placed a note on the cooler to discard foods by 3:30 p.m. The unit temperature was lowered during the inspection.
  • Wendy's #6559, 145 Garrisonville Road Stafford, VA 22554
    • Date inspected: Sept. 16
    • Total violations: 2
      • Observed 4 spray bottles stores next to single use gloves. Person in charge relocated the gloves.
      • Observed the walk-in-freezer door in disrepair. Observed ice built up around the door and to the sides. Observed the storage shelves in disrepair.
  • Cardinal Village, 4621 Spotsylvania Pkwy Fredericksburg, VA 22408
    • Date inspected: Sept. 16
    • Total violations: 3
      • Observed one food employee wash hands in less than 5 seconds and other food employee scrub hands under running water. EHS informed food employees of proper handwashing procedure. Food employees rewashed hands before returning to work.
      • The sanitizer at the 3 compartment sink is not being pulled through the tubing. During the inspection, the food employee set up the sanitizer manually.
      • Observed 1 box of thickened sweet tea nectar, 1 box of foam hinged lid containers, and 1 box of cranberry nectar on the floor in the dry food storage area.
  • Wendy’s (Caroline), 23890 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: Sept. 17
    • Total violations: 6
      • Observed carafe lids in the hand washing sink in the drive-thru area. Lids were relocated.
      • Observed a bottle of restroom cleaner stored next to single serve articles in the drive-thru area. The person in charge relocated the bottle.
      • Observed several flies throughout the facility.
      • Observed several pans on shelves over the 3 compartment sink that were stacked while wet after cleaning and sanitizing. A food employee dried them using paper towels.
      • The prep unit cooler is not operating properly, the following were out of temperature 1. A container of cheddar cheese 46F, 2. Two containers of cut tomatoes 44F- 57F, 3. cut lettuce 54F-60F, 4. cheddar spread 50F. The person in charge disregarded the foods. Facility is going to use time as a public health control unitl unit can be repaired.
      • Observed the following in need of repair: 1. The 3-compartment sink faucet is leaking, 2. 3 compartment sink needs re-caulking, 3. broken light shield cover in the dry storage area, 4. hand washing sink needs to be re-caulked in the drive-thru area.
  • Rocky Run Elementary, 95 Reservoir Road Fredericksburg, VA 22406
    • Date inspected: Sept. 17
    • Total violations: 3
      • Observed hot water in the employee toilet room at 74 degrees F.
      • Observed the Quaternary Ammonium test kit expired in June 2021. The person in charge contacted their supervisor to address the issue.
      • Observed long hose with a spray nozzle attached to the water supply under the dishwasher drainboard without backflow prevention.The water was not on during the inspection. The person in charge removed the hose during the inspection. Provided backflow prevention handout during the inspection.Corrective Actions: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at ea
  • Ni River Middle School, 11632 Catharpin Road Spotsylvania, VA 22553
    • Date inspected: Sept. 17
    • Total violations: 2
      • The interior surfaces of the ice machine were observed soiled. Surfaces were cleaned during inspection.
      • Observed 5 ceiling light fixtures in the kitchen that are not working.
  • Spotsylvania Regional Medical Center, 4600 Spotsylvania Pkwy Fredericksburg, VA 22408
    • Date inspected: Sept. 17
    • Total violations: 3
      • The following foods were observed out of temperature due to improper cooling, per discussion with the person in charge, foods were cooked the day prior 9/16: 1. Two pans of beef chili (45 degrees) 2. One pan of chicken chilli (43-44 degrees) 3. Three pans of white rice (45-45-47 degrees). During the inspection the person in charge discarded the out of temperature foods for safety.
      • The grout of the kitchen floor tiles is missing and in need of repair.
      • Observed the floors of the dry storage area and floors underneath equipment in need of cleaning.
  • Rosie's Pizza & Doener Kebab (VA92-589), 33 Perchwood Dr 101 Fredericksburg, VA 22405
    • Date inspected: Sept. 18
    • Total violations: 1
      • Observed food employee leave the cab of the mobile unit and don single use disposable gloves to engage in food preparation activities without washing their hands. Instructed the food employee to wash their hands.
  • Taco Bell #3921, 255 Garrisonville Road Stafford, VA 22554
    • Date inspected: Sept. 20
    • Total violations: 2
      • Observed stainless steel food containers stacked while wet after chemical sanitizing on the open shelving in the warewashing area.
      • Observed accumulation of debris on the following nonfood contact surfaces: Interior of Pitco fryer, backsplash and upsplash of the drive-thru soda machine and cabinets under the dining room soda machine.
  • Salem Elementary School, 4501 Jackson Road Fredericksburg, VA 22407
    • Date inspected: Sept. 20
    • Total violations: 4
      • The interior surfaces of the ice machine bin were observed soiled. The bin was cleaned during inspection.
      • The food contact surface of the garbage bags covering the floor mixer are not safe. Bags were removed during inspection.
      • The following physical facility items and in need of cleaning: 1-the floors in the dry storage room, 2-the floors under the equipment in the kitchen.
      • The ceiling tile located throughout the kitchen is absorbent and not easily cleaned. The ceiling tiles are porous acoustical tiles.
  • Feast Buffet, 2388 Plank Rd Fredericksburg, VA 22401
    • Date inspected: Sept. 20
    • Total violations: 12
      • The following foods were observed cold holding at the improper temperatures, (per discussion with the person in charge foods were held in the units overnight): -In the prep unit: (62 degrees, VDH thermometer) tofu (45 degrees), raw beef (64 degrees), raw shrimp (47 degrees), raw fish (50 degrees), chicken chunks (44 degrees), raw chicken (50 degrees), cubed ham (52 degrees), noodles (49 degrees) hard boiled egg (40 degrees) -In the walk in cooler: (46 degrees, VDH thermometer): cut watermelon (47 degrees), cut cantaloupe (46 degrees), cut tomatoes (47 degrees), crab salad (47 degrees). -In the reach in unit: (50 degrees, VDH thermometer) raw chicken (46 degrees), raw beef (46 degrees) During the inspection, person in charge discarded the foods for safety.
      • Observed numerous containers of food in the walk in cooler and reach in without date marking.
      • Observed several containers of breading ingredients and spices without labels.
      • Observed a gap underneath the back screen door.
      • Observed the presence of flying insects throughout the kitchen.
      • Observed fly strips above food prep and storage areas.
      • Observed several food employees without a hair restraint .
      • Observed a wiping cloth being used to hold a cutting board in place.
      • Observed food stored in the freezer in trash bags.
      • Observed the interior of the fryer cabinets with a heavy build up of grease.
      • Observed items (plastic containers, cart, shelves) surrounding the dumpster bin.
      • The following were observed in need of cleaning: 1. AC vents of the men and womens bathroom. 2. Floors underneath equipment, and in the walk in cooler. 3. Walls throughout the kitchen.
  • Childtime Learning Center, 10628 Courthouse Road Fredericksburg, VA 22407
    • Date inspected: Sept. 21
    • Total violations: 1
      • Observed utensils stored under the paper towel holder next to the hand sink.
  • Food For Functions by Michele, 22093 Landora Bridge Road Ruther Glen, VA 22546
    • Date inspected: Sept. 21
    • Total violations: 7
      • The person in charge could not provide an employee health policy but was aware of symptoms and illnesses requiring reporting. Sample employee health policy will be emailed to the owner.
      • Observed a food employee set up the sanitizer basin of the three compartment sink with Steramine Quaternary Ammonium tablet and then proceed to don single use disposable gloves to engage in food preparation tasks. Instructed the food employee to remove gloves and wash their hands.
      • Observed counter mounted can opener soiled. The person in charge removed the can opener to the three compartment sink for cleaning and sanitizing.
      • Observed three chemical spray bottles on the top shelf above the three compartment sink in contact with disposable paper towels which where removed from their protective wrapping. The chemical spray bottles were relocated to prevent contamination.
      • The person in charge reported the previous water sampling was conducted September 2020. Instructed the person in charge water sampling for total Coloform and Nitrate are required to be conducted at least annually.
      • Observed six rolls of disposable paper towels, which have been removed from their protective wrapping, stored on shelving unit above the three compartment sink which may be exposed to splash. Discussed relocating disposable paper towels during the inspection.
      • Observed the Quaternary Ammonium testing kit expired 9/1/2021. Discussed purchasing a new test kit with the person in charge.
  • Chen’s China King, 10211 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Sept. 21
    • Total violations: 4
      • Observed 2 dented cans of bamboo shoots on the dry storage shelf. Cans were re-located during inspection.
      • The wire shelving in the dry storage area and in the walk in cooler is covered with rust, making it impossible to clean.
      • The prep sink and 3 compartment sink are not sealed to the adjoining wall.
      • The top shelf over the prep unit was observed heavily soiled.
