Fredericksburg Restaurant Inspections: Violations At 16 Locations
The Rappahannock Area Health District found violations at several restaurants across Stafford County and the City of Fredericksburg.
FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected multiple restaurants across the region from July 8 to July 13; they found more than a dozen restaurants with violations.
As coronavirus restrictions continue to loosen around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department also performs a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

- Date inspected: July 7
- Total violations: 4
- Observed the following TCS foods hot holding at improper temperatures in hot holding station: Breaded cooked chicken (120, 110, 117, 120 degrees F). The person in charge reported the TCS foods have been in the unit approximately 20 minutes.
- The back door does not have a tight-fitting seal to prevent the intrusion of pests. Observed gaps along the base and top of the door.
- Observed harboraging conditions in the food establishment. The following conditions were observed: the back door is not tight fitting to prevent entry of pests, significant accumulation of debris under the shelving of dry storage area including food debris, food debris noted under and behind cooking equipment.
- The walls and floors throughout the facility are in need of cleaning. Observed soiled ceiling tiles in the dining room and areas of the kitchen.

- Total violations: 4
- Observed 1 box of clamshell paper containers on the floor next to the broken fryer and in grease. During the inspection, the person in charge removed the box and discarded the bottom layer of containers.
- The following are in need of repair: the fryers, the 4 door refrigerator, and the 1 door prep unit. Person in charge stated maintenance company had been called for repair.
- The floor under all equipment is in need of cleaning.
- Observed a personal backpack, 1 purse, and beverage container stored stored on a shelf with single-service/single-use items.
- Applebee’s Neighborhood Grill & Bar, 1000 Stafford Market Place Stafford, VA 22556

- Total violations: 3
- Observed ceramic plates, the interior of stainless steel containers that require cleaning.
- Observed Pico de Gallo at 54'F, mixed cheese at 53'F, chicken pieces at 59'F, spinach dip at 54'F, sour cream at 51''F in the sandwich prep units on the line cold holing at improper temperatures. All foods were discarded.
- Observe pico de gallo in the waitstaff area that expired on 7/7/21 and cut cabbage that expired on 7/6/21. The foods were discarded.

- Observed the dishwasher leaking when checking temperature. Person in charge stated dishwasher was scheduled for repair. The facility will discontinue use and use 3 compartment sink.
- The floors and floor drains are in need of cleaning.

- Observed dead pests under the 3-compartment sink, in the drain near the mop sink, and near the walk in cooler.
- Observed live pest crawling on the wall near the kitchen entrance door. Observed small flies in the storage room.
- McDonald’s of Stafford #1630, 1212 Jefferson Davis Highway Fredericksburg, VA 22401
- Date inspected: July 9
- Total violations: 6
- Observed employee apply soap and scrub under running water. Corrected during inspection employee re-washed hands using the proper hand washing procedure.
- Observed employee using handwashing sink to fill metal container. EHS discussed use of hand sink with employee.
- Observed food metal containers fall to the floor and employee placed on clean drying rack. Observed a container with soiled equipment on clean rack shelf. Items were placed at the three compartment sink.
- Observed an employee wearing a watch and bracelets while washing dishes and another employee wearing a bracelet while handwashing. Employees removed bracelets and watch.
- Observed the following in need of repair: Pastry oven door gasket, ice build up in fry cabinet, walk in cooler leaking.
- Observed two employees personal cell phones on top shelf over food prep line. Observed employees personal keys on packaging counter. Employees relocated.

- Total violations: 4
- Absence of a consumer advisory noted on the menu posted on the exterior of the mobile unit for eggs which may be prepared undercooked upon a customer's request.
- Observed accumulation of debris on the following nonfood contact equipment in the mobile unit: Racks inside the oven, containers used to store food contact equipment, and wells of hot holding equipment.
- Observed duct tape around the plumbing connection under the middle basin of the three compartment sink and the fixture was dripping when the water is running.
- The floors, walls, and sinks of the mobile unit are in need of cleaning.

- Date inspected: July 12
- Total violations: 15
- Observed the floors, walls and ceiling vents throughout the facility in need of cleaning.
- Observed accumulation of debris around the dumpster area.
- Observed spray nozzle attached to a long hose, which was located on the floor of the mop sink, without proper backflow prevention device. The water was turned on during the inspection.
- Observed accumulation of debris on the following nonfood contact surface: Gaskets and exterior doors of reach in refrigeration units, up splash of the coffee brewer, wheels of mobile carts, dry storage shelving, base of 2 door upright freezer and inside fryers.
- The facility has Quaternary Ammonium test kit with an expiration date of February 2021.
- Observed multiple stacks of cleaned and sanitized food storage containers staked while wet.
- Observed multiple dispensing utensils held in a container of water observed at 79 degrees F. at a prep table near the cooking line.
- Observed multiple employees wearing wrist jewelry.
- Observed 3 sauces used to brush on top of the biscuits which were not labeled to identify the product. The person in charge reported the sauces included a margarine blend, cinnamon topping and oil.
- Observed absence of a working thermometer in the following cold hold equipment: Walk in freezer, drawer cooler, 2 door upright freezer.
- Observed the food establishment using time as a public health control without prior approval from the Health Department. According to the person in charge, the facility reheats french fries for 3 minutes and holds them in a stainless steel pan for a period of 1 hour. The fries are placed into the fryer for 1 minute to be reheated for immediate service. The person in charge reported a timer is used to monitor the holding time.
- Observed a pan of commercially processed macaroni and cheese reheated in a pan of hot water and dispensed into a stainless steel container to be placed into hot holding, Observed internal temperatures of 130, 122, 117 degrees F. The employee stirred the product prior to taking temperatures. Improper reheating for hot holding was observed during the inspection.
- Observed the following food contact equipment soiled:2 stainless steel containers on open shelving next to the 3 compartment sink, flour sifter stored on shelving above the 3 compartment sink, stainless steel containers inside a new 1 door reach in cooler not being used.
- Observed employee submerge a pot in the Quaternary Ammonium sanitizer at the 3 compartment sink for 10 seconds and then rinsed the sanitizer off under running water. The food contact equipment was not adequately sanitized after cleaning.
- Observed a food employee, wearing a single use disposable glove, touch a soiled surface at the hot holding station and use the same glove to handle a biscuit to prepare a customer's order.

- Total violations: 9
- Observed that the inside of the reach in refrigerators require cleaning.
- Facility does not have a temperature measuring device for the high temperature dish machine.
- Observed that the paper towel dispenser in the kitchen is in disrepair. Observed that the leg on the prep table is in disrepair.
- Observed refrigerator shelf and plastic container lids that were stored on the floor.
- Observed boxes of cereals stored on the floor.
- Observed cream cheese on the bar at 43'F cold holidng at improper temperatures.
- Observed the top of the coffee machine and juice machine are in need of detailed cleaning.
- Observed that both microwaves require cleaning.
- Observed dirty tongs and knives in the clean dish rack. Items were placed on the wash side of the 3-compartment sink. Observed dirty plates under the bar.

- Observed that the large can opener is in need of detailed cleaning. The person in charge placed the can opener in the 3-compartment sink for cleaning.
- Observed that the interior of the ovens are in need of detailed cleaning.

- Total violations: 8
- Observed 3 microwaves heavily soiled on the interior. Microwaves were cleaned during inspection.
- The equipment being cleaned and sanitized in the dishmachine were not observed sanitized. The rinse water was not reaching sufficent temperature (160 degrees F) to achieve sanitization. The temperature of the rinse cycle measured only 148 degrees F. Person in charge called the repair company during inspection. The 3 compartment sink was set up to sanitize the equipment.
- Clean equipment/utensils were found stored in between the prep table and the wall. Equipment was removed during inspection.
- The pressure gauge for the high temperature final rinse warewash machine is not working.
- The non food contact surfaces of the cooking equipment in the kitchen were observed heavily soiled.
- The floor underneath the equipment on the cooks line and the caulking throughout the kitchen is in need of a thorough cleaning.
- The floor grout throughout the kitchen was observed heavily worn and in need of replacement.
- Observed the intake vents in the kitchen were heavily soiled.

- The light intensity over the handsink at the service area next to the kitchen is 1.2 foot candles and the 0.7 foot candles at the 3 compartment sink in the upstairs bar.
- Paint peeling over the mop sink area.
- All floors and floor drains the kitchen are in need of cleaning.
- Observed the dumpster/trash compactor area heavily soiled and an excess of unnecessary items.
- Observed the following handsinks leaking: 1. Handsink next to dishwashing area. 2. Handsink next to the dishwasher in the main bar.
- The following items are in need of repair: 1. The reach in freezer next to the fryer has ice buildup and is not maintaining temperature. During inspection, 1 container of chicken potstickers (44 - 46 degrees F and exceeded 4 hours) was discarded. 2. The door gasket and ice build-up in the ice cream reach in freezer.
- Observed flies at floor drain next to dishwasher and at the soda gun in the upstairs bar.
- The menu has the footnote, but is missing the asterick to link to the footnote.
- Observed the soda gun holder heavily soiled. During the inspection, the employee cleaned and sanitized the holder.
- No hand soap at handsink in service area next to restrooms. During inspection, person in charge provided hand soap.
- The following were observed in the upstairs bar: 1. Drinking staws and lime wedges in the handsink. 2. Observed employee rinse the 3 compartment sink wash brushes at the handsink. EHS informed person in charge and employee the only use of the handsink
- No papertowels at handsink next to dry food storage area and the handsink in the service area next to restrooms. During inspection, paper towels were provided.

- The following were observed in poor repair: Damaged flooring under the coffee brewers in the beverage area and rusted ceiling frames throughout the facility.

- Observed the following in poor repair: 1- low flow at the hand sink by the prep sink when the water is turned on and 2- McCafe machine leaks water when the machine is turned on.
- Accumulations of debris were noted on the following nonfood contact surfaces: Interior, including gaskets, of reach in refrigeration units. sides of flattop grills, up splash of the drive-thru soda machine, cabinets under the soda machine in the lobby.
- Observed stack of plastic containers used to store food stacked while wet on shelf next to the flattop grill.
- Observed employees in the back of the house without proper hair restraints.
- Observed a spray bottle of degreaser stored hanging on the shelving unit above the three compartment sink used to store cleaned and sanitized food contact equipment.
- Observed the following TCS foods cold holding at improper temperatures: 2 door reach in cooler: Containers of shredded lettuce (44, 46, 49 degrees F) and scrambled eggs/sausage mix for breakfast burritos (48 degrees F) and reach in cooler by prep line: Containers of sliced cheese (49, 52 degrees F). The person in charge reported the TCS foods in the 2 door reach in cooler have been in the unit overnight and the sliced cheese has been in the reach in cooler by the prep line for an unknown period of time.
- Observed egg patties (95 degrees F) hot holding in the hot holding station at improper temperatures. The person in charge reported the food was placed in the unit around 9:00 a.m. and temperatures were taken at 9:30 a.m.
- Observed the following food contact equipment soiled: Three food trays stored on shelf above the microwave ovens and 2 fry baskets stored under the fry freezer, which the person in charge reported were not used during the morning.
- Observed a food employee place hand soap on their hands and lather their hand under running water at the hand sink located near the convection ovens. Hands were not effectively cleaned due to hand soap being flushed down the drain during hand washing.
- Kenmore Club, 632 Kenmore Avenue Fredericksburg, VA 22401
- Date inspected: July 13
- Total violations: 5
- Observed no testing strips for the quaternary ammonium sanitizer at the 3 compartment sink.
- The following are in need of repair: 1. The faucet at the 3 compartment sink is loose. 2. The pre-rinse sprayer is leaking.
- Observed employee wearing a watch and bracelet while preparing meal. During inspection, food employee removed watch, but was not able to remove bracelet.
- The sanitizer at the 3 compartment sink is not being pulled through the tubing. The facility ran out of quaternary ammonium. EHS assisted with setting up chlorine sanitizer at the 3 compartment sink.
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
