Fredericksburg Restaurant Inspections: Violations At 18 Locations
Around the Fredericksburg area, 18 different locations had violations during recent health department inspections.
FREDERICKSBURG, VA — During the most recent round of restaurant inspections in the Fredericksburg area, 18 different locations were cited with violations. The inspections took place between Nov. 17 and Nov. 23.
As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Mountain View High School, 2135 Mountain View Dr. Stafford, VA 22556
- Date inspected: Nov. 17
- Total violations: 4
- Observed the following TCS foods cold holding at improper temperatures: String cheese (53 degrees F) and single use containers of yogurt (58 degrees F) in line 3 cold well and string cheese (44 degrees F) and single use containers of yogurt (48degrees F) in the line 4 cold well. Observed the TCS foods not being held inside the line 3 cold well, containers were stacked on top of other containers in the unit. The person in charge reported the TCS were placed in the unit at 8:00 a.m., temperatures were taken at 10:15 a.m. The foods were relocated to the walk in cooler to cool to 41 degrees F or less by 12:00 p.m.
- Accumulation of debris was noted on the following non food contact surfaces: Inside fryers and fans in the warewashing and main kitchen.
- Observed the ceiling tiles throughout the facility are not securely attached to the frame. The condition of the ceiling tiles may permit dust and debris in the food storage and preparation areas.
- Observed cleaning is needed in the following areas: Ceiling tiles, especially in the vicinity of the ceiling vents, ceiling vents in the kitchen and employee toilet room.
- Olde Town Steak & Seafood, 1612 Princess Anne Street Fredericksburg, VA 22401
- Date inspected: Nov. 17
- Total violations: 8
- The facility does not have a person who is a Certified Food Protection Manager.
- Observed a gap at the back kitchen door.
- Observed the following: serving plates at the cooks line and salad bar, water pitchers and several pots by the dishwashing area stored facing upwards and not inverted.
- The person in charge could not provide a test kit for the chemical being used in the facility. EHS provided a small sample of test kits until the person in charge places an order.
- The facility does not have a thermometer that registers the temperature of the high temperature dish machine of the facility.
- Observed the following non-food contact surfaces are in need of cleaning: 1. A heavy accumulation of grime and grease on the wheels of the cooks line equipment, sides of the broiler and inside cabinets of the fryers. 2. The metal shelves holding clean equipment by the dishwashing area and the metal shelves holding foods in the walk in cooler.
- Observed the following: Fan vents in the women's bathrooms and floors underneath cooks line equipment are in need of cleaning.
- Observed ceiling panels above the prep/cooks line are warped and loose.
- Courtyard by Marriott, 620 Caroline Street Fredericksburg, VA 22401
- Date inspected: Nov. 17
- Total violations: 3
- Observed 3 food employees preparing foods without a hair restraint.
- The facility does not have a high temperature registering thermometer for the ware washing machine in the kitchen.
- Observed the floors of the walk in cooler, walk in freezer and floors underneath equipment are in need of cleaning.
- Buffalo Philly’s, 10209 Southpoint Parkway Fredericksburg, VA 22407
- Date inspected: Nov. 17
- Total violations: 8
- The food establishment does not have an employee in a management or supervisory role who is a certified food protection manager. The person in charge reported two employees are scheduled for class in December 2021.
- Observed a food employee set up Chlorine sanitizing bucket and then proceed to handle aluminum foil to cover a container of food in the 1 door upright cooler without washing their hands. Instructed the food employee to wash their hands.
- Observed the can opener soiled. The person in charge reported the can opener was used the evening prior. The can opener was removed to the three compartment sink for cleaning and sanitizing.
- Observed gap at the bottom of the back door of the food establishment, light is visible emitted from the base of the door. The person in charge reported he has contacted the landlord for repairs.
- Observed presence of pests (flying insects) in the food establishment. The person in charge reported pest control is schedule to come to the facility in December. Instructed the person in charge to email pest control invoice.
- Accumulations of debris observed on the non food contact surfaces of the pizza oven and shielded light covers under the hood vents.
- Observed accumulation of brush and debris around the dumpster.
- Observed the walls, ceilings and ceiling vents and ceiling light covers throughout the facility are in need of cleaning.
- Shawn’s Smokehouse & BBQ, 7019 Harrison Rd 107 & 109 Fredericksburg, VA 22407
- Date inspected: Nov. 18
- Total violations: 3
- Observed interior surfaces (top of the microwave) soiled.
- Observed bottom of the back door of the food establishment is not sealed to the frame, observed light emitting from the lower left hand corner of the door.
- The 3 vat and prep sink are no longer sealed to the adjacent walls.
- Wing Stop #483, 5635 Plank Road Fredericksburg, VA 22407
- Date inspected: Nov. 18
- Total violations: 6
- Observed fries cooked at 10:15 am sitting on the speed rack at 116'F. EHS discussed this with the person in charge.
- Observed cooked fries sitting on a speed rack to cool.
- Observed the cook wearing a watch during food preparation.
- Observed that several stainless steel pans were stacked while wet.
- Observed that the following non-food contact surfaces require cleaning: outside of several stainless steel pans, tops of the floor-mounted equipment, and the top of the nacho cheese machine.
- Observed that the back flow prevention device is missing on the sprayer hose.

- Total violations: 7
- Observed the interior surfaces of the microwave were soiled.
- Observed food employees working without hair restraints.
- Observed the following: gaps between hood filters over the fryer, ice build up in the walk in freezer and the walk in cooler is missing the door threshhold.
- Seal 3 compartment sink to adjoining wall.
- Observed several cracked or missing floor tiles throughout the kitchen and the floor coving near the mop sink and ice machine are in need of repair.
- Observed the mop sink was soiled and the floors of the dry storage and underneath equipment are in need of cleaning.
- Observed 8 ceiling tiles that are not smooth and easy to clean throughout the kitchen.
- The Alpine Chef, 200 Lafayette Blvd. Fredericksburg, VA 22401
- Date inspected: Nov. 18
- Total violations: 6
- Observed the blade of the can opener was soiled. The can opener was moved to the 3 compartment sink for rewashing and sanitizing during the inspection.
- Observed the 3 compartment sink and the 2 hand wash sinks in the kitchen need to be sealed to the wall.
- Observed ice build up at the back of the walk in freezer and on the air curtains.
- Observed a heavy build up of grease on the sides of the deep fryers and sides of the broiler/cooks line.
- Observed the walls behind equipment and floors underneath equipment are in need of cleaning.
- Observed several ceiling tiles that are not smooth and easy to clean.
- Gayle Middle School (NEW), 100 Panther Drive Fredericksburg, VA 22406
- Date inspected: Nov. 18
- Total violations: 6
- Observed container of tomato paste with mold-like growth. The person in charge voluntarily discarded for safety.
- Observed string cheese (44 degrees F) cold holding at improper temperatures in the 3 door upright cooler. the person in charge reported the TCS foods have been in the unit overnight and voluntarily discarded for safety.
- Observed the employee room faucet is loose and required resealing to the handsink basin and the temperature gauge on top of the dishwasher is not long sealed.
- Observed the pressure gauge on the high temperature gauge is malfunctioning. The pressure gauge rests at 20 psi when the dishwasher commences the wash cycle and drops to 0 psi during the hot water sanitization rinse, which is not meeting the equipment specifications. Discussed contacting maintenance for repair or replacement.
- Observed the following areas in need of cleaning: Ceiling vent in the employee toilet room, interior of ceiling light covers, ceiling tiles in the vicinity of the air vents, caulk along drainboard and closed window grate at the dishwasher area
- Observed 15 foot candles of light at the dishwasher area and observed burned out ceiling lights.
- Wendy's Store #10802, 9910 Southpoint Parkway Fredericksburg, VA 22407
- Date inspected: Nov. 18
- Total violations: 4
- Observed several food employees preparing food without hair restraints.
- The 3 compartment sink and prep sink need to be sealed to the adjoining wall.
- The following are in need of repair: 1. Missing floor tile under the pressure fryer. 2. Several chipped or missing floor tile throughout the kitchen. 3. The floor tile coving next to the water heater and below the 3 compartment sink drainboard. 4. Five cracked light shields throughout the kitchen.
- Observed 1 ceiling tile over the Coca Cola machine and 5 ceiling tile throughout the kitchen that are not smooth and easy to clean.
- Holy Cross Academy, 250 Stafford Lakes Parkway Fredericksburg, VA 22406
- Date inspected: Nov. 19
- Total violations: 8
- The person in charge could not provide documentation employees have been trained on the employee health policy. The person in charge provided a training roster of various food safety topics but employee health was not included. Sample employee health policy was provided during the inspection.
- Observed a food employee remove a wiping cloth bucket to dispose in the pre-wash sink and then proceed to open the hot holding unit without washing their hands. Instructed the food employee to wash their hands.
- The Quaternary Ammonium testing kit expired September 2021. instructed the person in charge to purchase a new test kit.
- Observed hose with a spray nozzle attached to the water supply under the right side dishwasher drainboard without backflow prevention. The hose is directly connected to the water source. The water was not turned on during the inspection. Instructed the person in charge to contact maintenance and install an approved backflow preventer. Provided handout during the inspection.
- Observed soiled ceiling tiles by the convection oven and inside the dry storage room and the floor under the convection oven is in need of cleaning.
- Observed ceiling tiles over the 3 compartment sink, vent hoods and the dry storage room are not sealed to the frame.
- Observed the ceiling tiles in the dry storage room are absorbent and not smooth or easily cleanable. Also observed ceiling tiles are soiled.
- Observed 3.5 foot candle of light inside the walk in cooler.

- Observed fruit flies at the bar.
- Rowser Senior Citizen Center, 1739 Jefferson Davis Highway Stafford, VA 22554

- Observed several food contact surfaces soiled to the sight and touch. The person in charge removed the items to be washed, rinsed, and sanitized.
- The exterior door is not tight-fitting. This may allow the entry of insects or rodents.
- Observed mice droppings on the cabinet shelf.
- Observed several flies throughout the facility.
- Observed utensils stored in the drawer with the lip-contact surfaces facing different directions. The person in charge removed the items to be washed, rinsed, and sanitized.
- Observed the sinks throughout the facility need to sealed.
- Observed the following nonfood contact surfaces with an accumulation of grime and debris:1) the interior of the reach-in- cooler, 2) reach-in-freezer, 3) cabinets, 4) shelving units.
- Observed refuse on the ground around the dumpster.
- The outdoor waste container cover is broken.
- Observed the ceiling in the restrooms to not be smooth and easily cleanable.
- Observed the following: 1) one of the kitchen's doors to be broken, 2) several ceiling tiles need to be re-adjusted.

- Total violations: 5
- Observed 15 foot candles of light at the 3 compartment sink and 11.5 foot candles of light at the cook line. Observed numerous ceiling lights blown out.
- Observed cleaning is needed at the walls behind the 3 compartment sink and floors/piping under the cook line.
- Accumulation of debris was noted inside the fryers, track under the hood vents and interior of the prep cooler. Significant accumulation was noted on the prep cooler gasets.
- Observed the 1 compartment and 3 compartment sinks are not sealed to the adjacent walls.
- Observed containers of beef, wontons and egg rolls uncovered in containers inside the deep freezer. The person in charge placed plastic wrap over the containers to prevent contamination during the inspection.

- Date inspected: Nov. 22
- Total violations: 5
- Observed the blade of the can opener was soiled. During the inspection the can opener was moved to the 3 compartment sink for washing and sanitizing.
- Observed the caulking behind the 3 compartment sink needs replacing.
- Observed the upsplash of both soda machines (dining room and drive thru) were soiled.
- The following were observed in poor disrepair: 1. peeling paint on the wall behind the handwashing sink 2. numerous ceiling tiles throughout the kitchen and dry storage room have mold like growth, paint peeling and are loose fitting. 3. in the dry storage room the wall paneling behind a food shelf is coming undone 4. a coving tile is missing in the broom closet and a coving tile is cracked near the soda room.
- Observed the floors besided the hot cook line is in need of cleaning.

- Date inspected: Nov. 22
- Total violations: 4
- Observed three spray bottles without a proper identifying label. During the inspection the person in charge labeled the spray bottles.
- Observed shelves of the walk cooler and shelves of the prep unit coolers are rusted.
- The following were observed in need of repair: 1. One light bulb in the hood vent area 2. Floors of the food prep room, dish room, two hallways and walk in cooler are chipping away with paint.
- The following were observed in need of cleaning: floors underneath equipment, ac vents throughout the facility, vents on the outside of the hood vent, floors of the walk in cooler, floors of the dish room area.
- Chick-Fil-A (Lee's Hill), 5322 Jefferson Davis Hwy Fredericksburg, VA 22408
- Date inspected: Nov. 22
- Total violations: 6
- All equipment and utensils need to be cleaned and sanitized
- Final rinse temperature on the high temperature sanitizing dish machine need to be at least 160'F on the surface of the equipment being cleaned.
- Hand wash signs need to be hung up at all hand sinks, including restrooms.
- Need copy of Food Safety Manager Certification.
- Install covered trash cans for sanitary napkins in women's restroom stalls.
- Install hand wash signs at all hand wash sinks.
- Bowling Green Health & Rehabilitation Center, 120 Anderson Avenue Bowling Green, VA 22427
- Date inspected: Nov. 12
- Total violations: 23
- The wall covering in the mop sink closet is not smooth and easily cleanable. The walls are visibly soiled and appear to be finished with flat paint and cannot be easily cleaned.
- Observed the following in need of repair: 1) Hand washing sink needs to be re-caulked, 2) missing and broken floor tiles throughout the facility, 3) several blown out lights, 4) broken light shield by the three-compartment sink, 5) mop sink door, 6) re-adjust ceiling tiles throughout the facility.
- Observed the walls, floors, and ceiling (including vents) throughout the facility in need of cleaning.
- Hand washing sink in the female restroom is in need of cleaning.
- Direct connection exists at the 3 compartment sink sewer line
- Observed the following in need of repair: 1) The 3-compartment sink faucet is leaking, 2) hose is leaking in the mop sink
- Several shelving units were noted with an accumulation of grime and debris.
- The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher.
- Observed several flies throughout the facility.
- The exterior door which is located in the kitchen is not tight-fitting, this may allow the entry of insects or rodents.
- The following equipment food contact surfaces were observed soiled to sight and touch: 1) two knives stored against soiled wall, 2. one whisk, 3) one pot, 4) one pan, 5) two tongs. The person in charge placed the items in the handwashing sink to be washed, rinsed, and sanitized.
- The person in charge was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy.
