Fredericksburg Restaurant Inspections: Violations At 23 Locations
23 restaurants and other locations in the Fredericksburg area accumulated violations during recent health department inspections.
FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected several restaurants across the region from Oct. 20 to Oct. 25. More than 20 restaurants recorded violations in Stafford, Fredericksburg, and the surrounding area.
As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

- Arby’s, 5426 James Madison Parkway King George, VA 22485
- Date inspected: Oct. 25
- Total violations: 4
- 2 Cutting boards were observed heavily stained on the clean equipment rack.
- The large scale on the rear prep table was observed in a state of repair and condition preventing effective maintenance and easy cleaning. The scale is pitted and covered with rust.
- The surface of the cutting board along the make line was observed heavily scored, preventing it from being thoroughly cleaned and sanitized.
- The following non-food contact surfaces were observed soiled: 1-the interior of the deep fryer cabinets. 2-the top of the fry hot holding unit, 3-the outer surfaces of the convection oven, 4-inside of the utensil and hot holding drawers, 5-The door of the ice machine.

- Observed the food and equipment shelving of the kitchen and walk in cooler in need of cleaning.
- Observed in the men's bathroom, the ac vent cover is missing.
- Curitiba Art Cafe, 919 Caroline Street Fredericksburg, VA 22401

- Observed the interior surfaces of the ice machine are soiled.
- Per discussion with the person in charge the prep table and blender only get washed rinse and sanitized at the end of the day. EHS discussed proper cleaning procedures for food contact surfaces of TCS foods.
- Observed the following foods cold holding at improper temperatures due to faulty cooling units: In the upright cooler (52 degrees F) one container of pico de gallo 43 degrees F, one container of in house made ranch 44 degrees F, one container of in house made caesar 46 degrees F, one container of chicken salad 47 degrees F, one container of cut lettuce 46 degrees F. In the display cooler (46 degrees F) 6 empanadas/empadao 46 degrees F. During the inspection foods cold holding at improper temperatures were discarded for safety.
- Observed the facility does not have any thermometers in their cold storage units.
- Observed the interior of the oven with an accumulation of debris.
- The lightbulbs in the basement/dry storage area are in need of replacement.
- The following were observed in need of cleaning: 1.Floors underneath equipment. 2.Fans of the men and women's bathroom.
- Hyatt Place Fredericksburg, 1241 Jefferson Davis Hwy Fredericksburg, VA 22401
- Date inspected: Oct. 26
- Total violations: 9
- Facility does not a have person who is a Certified Food Protection Manager.
- Observed a food employee handle dirty dishes then proceed to don a new pair of gloves before handwashing first. EHS directed employee to hand sink for hand washing and discussed with the food employee when handwashing is to be done.
- Per discussion with the person in charge the steam wand of the espresso machine does not currently under go the wash rinse sanitize step every 4 hours. EHS discussed proper cleaning and sanitizing of food contact surfaces with the person in charge.
- Observed boxes of of food stored on the floor of the dry storage area.
- Observed numerous stacks of to go containers and lids stored upwards on a shelf. During the inspection the person in charge inverted the stacks of containers and lids.
- Observed the cutting board of the prep table heavily scored and stained with what appears to be mold-like growth throughout the cutting board.
- The following non food contact surfaces were observed in need of cleaning: the pizza/toaster oven, the top of the prep table unit.
- Observed debris (wood, chairs) around the dumpster area.
- Observed the following in need of cleaning: Floors underneath equipment, floors of the dry storage room, vent openings under the hood vent area.

- The raw animal food on display is stored in a manner that may cause cross contamination of ready-to-eat food as follows: Observed a bowl of raw chicken stored sitting on a box of carrots. Chicken was removed during inspection.

- Observed the upsplash area of both the soda machine at the drive through and dining area in need of cleaning.
- The following were observed in need of cleaning: floors of the walk in cooler and walk in freezer, floors underneath and behind equipment throughout the kitchen.
- Tito’s Diner, 1695 Carl D. Silver Parkway Fredericksburg, VA 22401
- Date inspected: Oct. 22
- Total violations: 8
- Observed a food employee scrub hands under running water. EHS informed employee of the proper procedure of handwashing, food employee re-washed their hands.
- Observed the blade of the can opener soiled. Can opener was taken to the dish washing area for rewashing and sanitizing.
- Observed the following holding at improper cold holding temperatures: 1. One container of gluten free batter (50 degrees F). Batter had been left in the unit overnight, VDH thermometer read unit temperature at 50 degrees F. Person in charge discarded the batter for safety. 2. Portioned out rice pilaf (50 degrees F), one container of raw chicken (50 degrees F), one pack of opened up raw hamburgers (50 degrees F). Foods had been put in there 2 hours prior to the inspection. Person in charge will use time as a public health control until unit gets fixed.
- Observed one container of chicken salad with a date made on label of 10/8, and one container of tuna salad with a date made on label of 10/15. Both containers of food had passed the 7 day maximum cold holding days. Person in charge discarded the foods.
- Observed a bottle of an employees medication and an unused syringe on the shelf of the dessert cooler. Person in charge stored the employees medication in a container, labeled it and repositioned it to the bottom of the cooler.
- Facility does not have a small probe thermometer.
- Observed the following: 1. A prep cooler refrigerator and hot line cooling drawers at 50 degrees F. 2. A heavy accumulation of ice at the back of the walk in freezer, an under counter freezer with two loose doors.
- Observed the following in need of cleaning: Floors underneath equipment and dry food storage area, AC vent above clean equipment and food prep area.
- Lee’s Hill Golf Club, 10200 Old Dominion Parkway Fredericksburg, VA 22408
- Date inspected: Oct. 22
- Total violations: 14
- The following manager responsibilities were not being met: 1-Employees were not informed about the need to sanitize equipment and utensils before they are reused, 2-There was no Employee Health Policy in place.
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The person in charge is not able to provide a verbal or written policy for preventing the transmission of foodborne disease by an employee who has a disease or a medical condition. A sample Employee Health Policy was left with the person in charge.
- The interior of the microwave, the deep fryer baskets and the flattop grill were observed soiled.
- During discussions with an employee and the person in charge, it was discovered that equipment and utensils were not being sanitized after cleaning. They had no knowledge of the sanitizing process. EH demonstrated how to sanitize equipment.
- Observed bottles of Liquid Plumber stored above the 3 compartment sink. Bottles were relocated during inspection.
- Observed a heavy layer of frost and ice inside of the small chest freezer.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
- The following non-food contact surfaces were observed in need of cleaning: 1-outer surfaces of the microwave, 2-theh toaster, 3-the upspash of the soda dispenser, 4-the interior and exterior of the deep fryer cabinets, 5-outer surfaces of the cooktop/oven, 6-the interior and exterior of the prep unit, 7-the interior and exterior of the reach in cooler, 8-the cook top. IN GENERAL ALL EQUIPMENT SURFACES ARE IN NEED OF A DEEP CLEANING.
- The connections of the faucet at the 3 compartment sink and the drain lines under the hand sink are leaking.
- Observed that mops are improperly stored between use. Mops were hung up during inspection.
- A ceiling vent cover is missing in the kitchen.
- The following physical features were observed in need of cleaning: 1-The floors under all equipment in the kitchen and storage areas, 2-The stainless steel wall behind the equipment on the cook's line. IN GENERAL, THE FLOORS AND WALLS ARE IN NEED OF A DEEP CLEANING.
- Observed the ventilation hoods and the hood filters heavily soiled with grime and debris.
- Garnjana Thai Restaurant, 4416 Lafayette Boulevard Fredericksburg, VA 22408
- Date inspected: Oct. 22
- Total violations: 13
- Paper towels were not available at the handwashing sink in the back of the house.
- Observed that following items cold holding at improper temperatures: 2 containers of egg rolls (43-47'F) in the home-style refrigeration unit in the back of the house, 2 containers of individually wrapped sticky rice (47-50'F) in the home-style refrigeration unit in the front of the house. The items were discarded.
- Observed the following foods not properly dated for disposition: 2 containers of egg rolls, tray of cooked chicken, several portions of rice.
- Observed 3 bottles of cleaning chemicals stored next to single serve articles and equipment. The person in charge relocated the bottles.
- Observed an electric flying swatter stored on the prep table near food or equipment.
- Observed several stained wiping cloths hanging near clean equipment near 3 compartment sink.
- Observed several bags of rice stored on the floor.
- Observed a wet wiping cloth stored on the counter. The person in charge placed the wiping cloth in a chemical sanitizer solution.
- Observed lettuce and lemon grass stored directly in a shopping bag that is not food grade.
- Observed several boxes of single service articles stored directly on the floor.
- The ceiling tiles throughout and wall behind the cook line were noted in need of cleaning
- Mops not hung up to air dry.
- Observed the hood system and filters soiled with grime and debris
- Otani Japanese Steakhouse, 12131 Amos Lane Fredericksburg, VA 22407
- Date inspected: Oct. 22
- Total violations: 4
- Observed stainless steel containers used to store food and a large wooden bowl stored on shelving units in the back of the house soiled. The person in charge removed the equipment to be cleaned.
- Observed accumulation of debris on the non food contact surfaces of the fryers, range and stand alone burner used to cook soup. Discussed thorough cleaning of the equipment during the inspection.
- Observed the following in poor repair: Coving tiles under the dishwasher requires resealing and the ceiling in the women's toilet room is peeling/separating. The person in charge reported a carpenter has been contacted to address the ceiling in the women's toilet room.
- The floors and walls in the vicinity of the range, fryers and stand along range used to cook soup are in need of cleaning.
- Hartwood Elementary School, 14 Shackleford’s Well Road Fredericksburg, VA 22406
- Date inspected: Oct. 22
- Total violations: 5
- The person in charge could not provide documentation of employee health training. The person in charge reported employees were trained and signed policies were sent to the school district. Discussed maintaining signed policies on site for review during inspections. Instructed the person in charge to emailed signed employee health policies once they are received at the district office.
- Observed hand lotions were storage in a basket on cinderblock shelf between a handwashing and prep sink. The person in charge relocated the lotion to the toilet room.
- Observed boxes of food stored on the top shelf of the walk in freezer which appears to have a leak under the condenser unit, observed significant accumulation of ice extending from the equipment and on the top shelf. The person in charge relocated the food to prevent contamination. Instructed the person in charge to contact maintenance for repair.
- Observed the pressure gauge in poor repair. At the beginning of the wash cycle the pressure gouge rests at 20 psi and during the wash and rinse cycle vibrates. Discussed contacting maintenance to repair/replace the pressure gauge.
- Observed the vents above the dishwasher, inside the dry storage room and inside the mop closet are in need of cleaning.
- Vocelli Pizza, 44 Mine Road #1A Stafford, VA 22554
- Date inspected: Oct. 21
- Total violations: 11
- The person in charge could not provide a written employee health policy. Environmental Health Specialist provided a sample of employee health policy.
- Observed Improper handwashing procedures. Employee washed hands for less than 10 seconds.
- Observed the following: 1. dirty aprons stored near food and equipment. 2. an uncovered container of Spanish in the refrigeration unit.
- Observed the following TCS foods cold holding at improper temperatures: 1) Small containers of ranch dressing in the sub prep area that were held at room temperature (had to be refrigerated as per the original container. 2) Mozzarella cheese in the pizza prep area (43'F).
- Observed the following foods not properly dated for disposition: several large plastic bags of cooked pasta, Pizza sauce, several portions of cooked chicken in the walk-in cooler. All items were properly dated during the inspection.
- Observed bottle of chemicals stored next to single serve articles. The person in charge relocated the bottle.
- The exterior door which is located in the kitchen is not tight-fitting. This may allow the entry of insects or rodents.
- Observed containers of food stored on the floor in the walk-in cooler.
- Observed a food employee prep food without proper hair restraints.
- No chemical test kit available.
- Observed two mops not hung up to air dry.
- Salsarita’s Fresh Cantina, 10117 Southpoint Parkway Fredericksburg, VA 22407
- Date inspected: Oct. 21
- Total violations: 10
- Through discussion with the person in charge the tortilla chips stainless steel holding container is washed and sanitized twice a week. Instructed the person in charge to wash and sanitize equipment at least once every 24 hours.
- Observed cooked steak (113°F, 116°F) hot holding at improper temperatures in the hot holding cabinet. The person in charge the steak was cooked at 9:45 a.m. and 10:45 a.m. and placed in the unit, temperatures were observed at 11:35 a.m. The TCS foods were removed to be reheated in the steamer to 165 degrees F by 11:45 a.m. and 12:45 p.m., respectively. Observed the steak cooked at 9:45 a.m. reheated to 175 at 11:45 a.m. and the second pan of steak was placed in the steamer at 12:00 p.m.
- Observed the following TCS foods holding at improper temperatures in the cold holding well unit: Cut lettuce (47°F), Pico de gallo salsa (46°F), shredded cheese (48°F) and salsa (45°F). The person in charge could not communicate how long the food has been in the unit and voluntarily discarded The person in charge also communicated the unit is under repair and is using ice to cold hold TCS foods. Instructed the person in charge to use time as a public health control until repairs are completed. Also discussed holding less product to reduce waste.
- Observed multiple sterno containers stored on open shelving above bottled beverages. The person in charge relocated the sternos to prevent contamination.
- Observed three wiping cloth buckets with no measurable concentration of Quaternary Ammonium sanitizing solution.
- Observed accumulation of debris on all non food contact surfaces of equipment. Discussed deep cleaning with the person in charge.
- Observed accumulation of litter around the dumpster.
- Observed oil storage unit in the waste enclosure in poor repair, the back right side of the lid is bent and not tight fitting which could attract the presence of pests.
- The following were observed in need of cleaning: pipes and floors under the 3 vat and prep sink, ceiling tiles throughout the facility, caulk along the 3 vat sink and ceiling vents in the female toilet room.
- Observed 5.7 foot candles of light inside the walk in cooler.

- Observed the ramps in front of the reach-in-cooler between lines 1 and 2, and the ice cream box in disrepair.
- Observed broken wall tiles on line-1 and broken air curtains in the walk-in cooler.
- Observed the following in need of cleaning: 1. Light shield in dry storage area, 2. Wall over dishmachine, 3. Air vent cover in the walh-in cooler, 4. Air vents throughout the facility.

- Date inspected: Oct. 21
- Total violations: 6
- No parasite destruction letter from supplier for salmon.
- Observed the deli slicer soiled. During the inspection, the PIC cleaned and sanitized the deli slicer. (Corrected)
- Observed food employee working without hair restraints. During the inspection, the food employee applied a chef hat. (Corrected)
- The lid on the chest freezer in the basement is in need of repair.
- No temperature measuring device for high temp dishwashers.
- Observed a bottle of pain relieve medication on a shelf over the prep table and 1 personal water bottle on the prep table. During the inspection, the items were relocated. (Corrected)

- Date inspected: Oct. 21
- Total violations: 4
- The following equipment was observed in need of repair: 1-the torn gasket on the reach in cooler, 2-the ice build-up on the interior top and bottom of the reach in freezer.
- There is no pressure gauge installed at the hot water supply line immediately before entering the warewashing machine allowing pounds per square inch, PSI, to be monitored by a machine operator.
- The person in charge was unable to produce a test kit for the Dodecylbenzenesulfonic Acid sanitizer being used in the 3 compartment sink.
- Observed a non-functioning ceiling light fixture above the serving line.

- Date inspected: Oct. 21
- Total violations: 5
- The dishwasher was not sanitizing (0 ppm) equipment and utensils. The sanitizer container was low. During the inspection, the PIC replaced the sanitizer. (Corrected)
- The following were observed heavily soiled : 1. The wok station. 2. One fan on the prep table. 3. One fan on a shelf over a prep unit. During the inspection, the PIC removed the fans.
- The ceiling vents in the dry storage room and over the 3 compartment sink are in need of cleaning.
- The ceiling tile in the large storage room where the single service/use articles are be stored is not smooth and easy to clean.
- The light intensity in the large storage room where single service/use articles are being stored is 2.6 foot candles.

- Observed the fan vents of the men and women's bathroom need cleaning.
- Observed a shelf used to store clean and sanitized equipment stored directly next to a hand washing sink.
- Observed several metal sheet pans stored and stacked wet.
- Observed the interior surface of the ice machine soiled. During the inspection the person in charge cleaned the ice machine
- Walker Grant Middle School, 1 Learning Ln. Fredericksburg, VA 22401
- Date inspected: Oct. 21
- Total violations: 3
- Observed food employee adjusting face mask while attempting to prep food. EHS informed the food employee of when to wash. The food employee washed their hands before returning to prep food. (Corrected)
- Observed several ceiling tile in the dry food storage room that are not smooth and easy to clean.
- Observed several ceiling vents over the dishwashing area heavily soiled.
- H.H. Poole Middle School, 800 Eustace Road Stafford, VA 22554
- Date inspected: Oct. 20
- Total violations: 4
- Observed hot dogs (55 degrees F) stored in stainless steel pans on a cart by the steamer holding at improper temperatures. A food employee reported she pulled the hot dogs from the walk in cooler to place some pans in the steamer to be reheated at approximately 9:45 a.m. The cart of hot dogs were relocated to the walk in cooler until they are ready to be cooked. Discussed pulling the quantity of pans needed to be reheated at a time.
- Observed boxes of food and a waffles stored in a stainless steel pan covered with plastic wrap in the walk in freezer under leaking and frozen water lines. Observed frozen ice suspended from the unit and ice build-up on top of the container of waffles. The person in charge voluntarily discarded the pan of waffles and relocated the boxes of food to prevent contamination. Discussed contacting maintenance to service and discontinue storing food in this area.
- Observed five rolls of paper towels, which have been removed from their protective wrapping, stored on a shelf in the employee toilet room which may be exposed to splash and contamination. During the inspection the person in charge placed the paper towels in an cabinet to protect from contamination.
- Observed the final rinse temperature and pressure gauges are not functioning appropriately. The final rinse temperature gauge registers 110 degrees F and the final rinse temperature was observed at 158.5 on the irreversible registering temperature indicator (plate). Additionally, the pressure gauge registers 60 PSI when then dishwasher is turned on and drops to 50 PSI during the final rinse cycle. Discussed contacting a technician to replace/repair the gauges.

- Total violations: 16
- Observed the following foods uncovered in the walk in cooler: one pan of chicken, one pan of beef, and 3 trays of spring rolls. During the inspection the person in charge protected the foods by covering them with a tray and plastic.
- Observed the interior surface of the ice machine was soiled. During the inspection the person in charge cleaned the ice machine.
- Observed the following: 1. The fryer baskets with a heavy accumulation of oil and food debris. 2. The interior surfaces of the microwave were soiled. During the inspection the person in charge cleaned the fryer baskets and the microwave.
- Observed a pot of broth with temperatures ranging 43-44 degrees F. Per discussion with the person in charge the broth was cooked the day prior and temperatures were not taken during the cooling process. EHS discussed proper cooling methods with the person in charge. The pot of broth was discarded for safety.
- Observed no date marking of foods throughout the facility. EHS discussed a system of date marking with the person in charge.
- Observed the facility using time as a public health control for the sprouts without written procedures in place. EHS discussed with the person in charge and will provide the documentation needed.
- Observed a thawing bowl of tripe at 100 degrees F. Per discussion with the person in charge it was discovered improper thawing methods were carried out to thaw out a bowl of tripe. Tripe was thawed under hot water. EHS discussed the correct thawing methods. Person in charge relocated the tripe into the walk in freezer. EHS re-checked the temperature of the tripe before the inspection and it was 34 degrees.
- Observed several containers of sauces and spices without a label.
- Observed boxes of food being stored on the floor of the walk in freezer.
- Observed a food employee with gloved hands carry out different tasks throughout the kitchen (touch equipment, prepare different foods, touch face mask) with the same pair of gloves. EHS informed the food employee of the proper usage of gloves. The food employee washed their hands and donned a new pair of gloves.
- Observed a cutting board warped and another cutting board stained. During the inspection the person in charge discarded the warped cutting boards.
- Observed the following in need of cleaning: the racks used to hold equipment in the kitchen, the racks of the walk in cooler and freezer, the outside of equipment in the kitchen.
- Observed no air break at the 3 compartment sink or the food prep sink.
- Observed the trash can of the women's bathroom did not have a cover on it.
- The following items were observed in need of cleaning: Floors underneath equipment, walls throughout the kitchen.
- Observed employees personal belongings (keys, hat) and beverages near food prep areas. During the inspection the person in charge relocated the items.
