Fredericksburg Restaurant Inspections: Violations at 31 Locations

The Rappahannock Area Health District cited violations at more than 30 locations around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — More than 30 restaurants and other food service locations accumulated violations during recent inspections, according to the Rappahannock Area Health District. The cited locations included schools, restaurants, and healthcare facilities in Stafford, Fredericksburg, and Spotsylvania.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Great Steak & Potato, Co./Surf City Squeeze, 137 Spotsylvania Mall Drive Suite #F14 Fredericksburg, VA 22407
    • Date inspected: Nov. 3
    • Total violations: 5
      • The glass door reach in cooler (RIC) is not holding temperature.
      • Observed boxes of straws under the prep sink and handwashing sink. During the inspection, the PIC relocated the items.
      • Observed several of the plastic knives in the self service counter displayed with the food contact surface facing up. During the inspection, the PIC inverted the plastic knives.
      • Observed a spray bottle of Windex that was not labeled. During the inspection, the PIC labeled the spray bottle.
      • The following were at improper temperatures in the glass door RIC (60: One container of shredded cheese (57 -59 F), one box of creamer (57 F), one container of spicy ranch dressing (56 F), one bottle of ranch dressing (58 F), seven individually packaged slices of white american cheese (58 F), six individually packaged slices of american cheese (57 - 58 F). During the inspection, the PIC discarded the food.

    • Total violations: 1
      • Observed the seals of 3 reach in coolers and 3 freezers are loose in some spots. Coolers and freezers are maintaining temperature at this time.
  • Mama’s Pizza & Subs, 5028 Plank Road Fredericksburg, VA 22407
    • Date inspected: Nov. 3
    • Total violations: 13
      • Observed food employee lather their hands under running water at the cook line hand sink. Hands were not effectively washed due to hand soap being flushed down the drain.
      • Absence of hand soap at the cook line handsink. The person in charge place hand soap at the sink during the inspection.
      • Observed food employee touch their facemask with single use disposable gloves on their hands and then proceed to handle dispensing utensil and a single use article. Instructed the food employee to remove their gloves and wash their hands.
      • Through discussion with the person in charge, the pizza dough roller is disassembled for cleaning once a week. Instructed the person in charge equipment in contact with Non-TCS foods shall be cleaned and sanitized at least once every 24 hours.
      • Observed interior surfaces of the microwave oven soiled.
      • Observed the following TCS foods hot holding at improper temperatures on the flattop grill: Two pans of fully cooked thinly sliced steak (100 degrees F, 109 degrees F) and fully cooked chicken (109 degrees F). Observed food stored in stainless steel containers on the flattop grill. The person in charge reported the foods were cooked at 11:00 a.m. (temperature taken at 12:42 p.m.) and the flattop grill was turned onto a higher temperature setting. One pan of sliced steak and the pan of chicken was observed above 165 degrees F by 1:00 p.m. The remaining pan of sliced steak (top portions, the container was overfilled) were observed at 150 degrees F at 1:00 p.m. and were voluntarily discarded.
      • Observed chemical spray bottle stored on shelf under the pizza prep table was not labeled to identify the contents. The person in charge reported the spray bottle contained bleach sanitizer and was discarded because it was broken.
      • The food establishment is reusing manufacturer containers of sliced deli meat and feta cheese for food storage of parmesan cheese (feta cheese container), feta cheese and banana peppers (deli meat containers). Instructed the person in charge to discontinue use of manufacturer containers for food storage.
      • Observed two cucumbers stored in a plastic shopping bag. The person in charge reported the cucumbers are not peeled prior to service and instructed a food employee to remove the cucumbers and place in a food grade container.
      • Accumulation of debris was noted on the following nonfood contact surfaces: Inside fryer doors, metal sheeting behind the fryers, base of the 3 door upright freezer #1 and shelving inside the walk in cooler.
      • Observed the ceiling tiles inside the male and female toilet rooms are absorbant and soiled with stains. Discussed replacing ceiling tiles with tiles that are smooth and easily cleanable.
      • The following are noted in need of cleaning: Walls throughout the food preparation areas and foors under and behind the cook line.
      • Observed 5 foot candles of light inside the dry storage area, observed blown out ceiling lights.
  • Falmouth Elementary School, 1000 Forbes St Fredericksburg, VA 22405
    • Date inspected: Nov. 3
    • Total violations: 1
      • Observed long hose with a spray nozzle attached to the water supply under the pre-wash sink at the dishwasher area, without backflow prevention attached to the water supply. The water was not turned on during the inspection. The person in charge disconnected the hose during the inspection. Instructed the person in charge to contact maintenance to install a proper backflow prevention device.
  • Soup & Taco Etc, 813 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Nov. 3
    • Total violations: 4
      • Observed the following are in need of cleaning throughout the facility: Walls, Floors, Vent next to reach in cooler in the back of the kitchen.
      • Observed the inside of all the cooling and freezing units of the facility are in need of cleaning.
      • Observed two containers of shredded birria beef at 86-85 degrees F. Per discussion with the person in charge, the beef was made the night prior and was left sitting on the range to cool down overnight. EHS discussed proper cooling methods with the person in charge and the two containers of beef were discarded for safety.
      • Observed the following: Two stacked containers of raw shrimp covered with plastic wrap stored above other containers of covered foods and bundles of cilantro. One container of raw mahi mahi and one container of raw salmon stored above an uncovered container of sliced cheese. Containers of raw shrimp, mahi mahi and salmon were re-arranged to the bottom shelves during the inspection.
  • Kentucky Fried Chicken #0160, 5412 Jefferson Davis Highway Fredericksburg, VA 22407
    • Date inspected: Nov. 3
    • Total violations: 9
      • The following equipment food contact surfaces were observed soiled to sight and touch: 1) several containers, 2) lids, 3) four ladles. Items were removed to be washed, rinsed and sanitized.
      • Observed insect control device located over the prep area in the back of the house and over single service articles in the front of the house.
      • The three compartment sink by the frying station is not sealed to the wall.
      • Observed a disposable air conditioner filter being used as a hood filter.
      • Observed a hose, under pressure and with a spray nozzle, attached to a faucet at the mop sink by the dry storage area. Water was turned off and nozzle removed during inspection.
      • Observed a leak in the three compartment sink faucet.
      • Observed the following in need of repair: 1) broken tiles near the walk-in-cooler, 2) the tiles need to re-grouted throughout the facility, 3) several ceiling tiles throughout the facility need to be re-adjusted, 4) duct tape and expandable foam were observed in the walk-in-cooler that may prevent necessary maintenance and easy cleaning, 5) expandable foam was observed on the wall under the cabinet in the front of the house.
      • Observed the following areas in need of cleaning: ceiling vents, ceiling tiles, walls, and floors throughout the facility.

      • The person in charge was unable to produce a test kit for the Dodecylbenzenesulfonic Acid sanitizer being used in the 3 compartment sink.

      • Observed food employee working without a hair restraint. During inspection food employee applied hair restraint.
      • The 2 new chest freezers in the back prep area are sitting in the original cardboard/styrofoam packaging.
      • The following are in need of cleaning: 1. The mop sink. 2. Under the equipment in the back prep area.
      • Observed 2 cell phones on the prep counter next to a measuring cup of frosting and a spatula. During the inspection, the cell phones were removed.
  • Ichiban Ramen & Sushi, 1632 Carl D. Silver Pkwy Fredericksburg, VA 22401

      • Observed food employee drinking coffee from an open mug while prepping food. During the inspection, the food employee discarded the drink and washed his hands before returning to work.
      • Observed a Pur filter at the handwashing faucet in the main kitchen. During the inspection, the PIC removed the filter.
      • Observed a bottle of burn gel and a first aid kit on a shelf over the prep table and prep unit. During the inspection, the food employee relocated the items.
      • Observed 2 spray bottles of BenzRid in the server area that were not labeled. During the inspection, a food employee labeled the spray bottles.
      • Observed 2 spray bottles of BenzaRid next to the chopsticks and napkins. During the inspection, a food employee relocated the spray bottles.
      • Observed a container of sliced lemons being stored in the ice of the ice machine. During the inspection, the container was removed and contaminated ice discarded.
      • Observed several food employees working without hair restraints.
      • Observed several wet wiping cloths on cutting boards throughout the kitchen and sushi bar. During the inspection, the wiping cloths were placed in sanitizer.
      • The following in-use utensils were observed in metal containers of water: 1. One spatula next to the rice cooker at 68°F. 2. One whisk next to the gas range at 69°F. During the inspection, the food employee discarded the water and placed the utensils at the dishwasher.
      • The caulking at the pre-rinse sink is heavily soiled.
      • The chicken and pork bones are being stored directly into carry out plastic bags. Observed 4 bags of chicken bones and 8 bags of pork bones in the 2 door reach in freezer in the back hallway. During the inspection, the food employees discarded the bones.
      • The handwashing sink next to the rice cooker is leaking and slow to drain.
      • Observed the following: 1. One personal bag inside a pot under the prep table. 2. One cup of coffee on a shelf over the prep table. 3. Two boxes of granola bars and 1 cell phone over the prep table. During the inspection, the food employees relocated the items.
  • J Brian's Tap Room, 200 Hanover Street Fredericksburg, VA 2240
    • Date inspected: Nov. 4
    • Total violations: 8
      • The facility does not have an employee that is a certified food protection manager.
      • Observed no hand soap available at the handwashing sink of the bar area. During the inspection the person in charge provided soap at the handwashing sink.
      • Observed the interior surfaces of the two microwave ovens in the kitchen were soiled.
      • Observed the bar dish machine is not sanitizing at the concentration needed for proper sanitizing of the dishes (50 ppm), EHS observed a concentration of 25 ppm and instructed person in charge to discontinue the use of the bar dish machine until a technician comes to do an adjustment. Person in charge will wash and sanitize bar dishes at the kitchen dish machine.
      • Observed cardboard boxes lining the back of the floor of the walk in cooler.
      • Observed the following: an accumulation of ice on all 3 of the deep freezers and rusted shelves inside the walk in cooler.
      • The following non food contact surfaces were observed soiled: The interior surfaces of the deep fryer cabinet, the interior and exterior surfaces of the cooling units in the kitchen and basement, food and equipment shelving throughout the kitchen were observed with a heavy build of grime and food debris.
      • Observed the following: The walls and floors of the kitchen are in need of a deep and thorough cleaning.
  • Jimmy the Greek Family Restaurant, 3401 Lafayette Blvd Fredericksburg, VA 22408
    • Date inspected: Nov. 4
    • Total violations: 4
      • The light intensity at the bar 3 compartment sink is 11 foot candles.
      • Observed cracked tiles, missing flooring, and the floor paint peeling in the kitchen.
      • The drainboards at the dishwasher are not sealed to the adjoining wall.
      • The toaster is exposed to splash when using handwashing sink at the end of the sandwich prep unit.
  • Fredericksburg Health & Rehab, 3900 Plank Road Fredericksburg, VA 22407
    • Date inspected: Nov. 4
    • Total violations: 15
      • The person in charge could not produce documents to verify employee health training.
      • Observed an employee wash their hands for less than 20 seconds. One employee only washed for 10 seconds and another employee washed for 5 seconds. The person in charge spoke to the employees.
      • Observed that the hand sink is being used as a dump sink. Observed ice inside of the hand sink.
      • Observed the following item(s) require cleaning: nozzles for the juice machine (corrected), coffee machine, can opener (corrected), and the inside of the ice machine.
      • Observed that the inside of several stainless steel pans and the knives were soiled to sight and touch. The person in charge moved the items to the dish machine area for cleaning.
      • Observed that the microwave and baking pans are in need of cleaning.
      • Observed that the digital thermometer was not calibrated. EHS discussed this with the person in charge.
      • Observed that the sanitizer in the wiping cloth bucket was low (50 ppm). Observed that wet wiping cloths were stored in soap without sanitizer. The person in charge placed the cloths in sanitizer.
      • Observed ceramic plates, ceramic bowls, and silverware that were stored with the food-contact surface facing upwards.
      • Observed coffee filters stored with the food-contact surface facing upwards. The person in charge inverted the filters.
      • Observed that the hand sinks were not sealed to the walls.
      • Observed that the hand sink was cracked in 3 places and the handle was broken on the microwave.
      • Observed a build up of grime and debris inside of the dish machine and on the doors to the dish machine.
      • The following item(s) are in need of detailed cleaning: storage bins, interior of the reach in freezer, and the gaskets on the reach in freezer.
      • Observed that the floors, walls, and ceiling are in need of detailed cleaning to include air vent covers in the walk in cooler.
  • Sam's Pizza & Subs, 6072 Riverview Drive King George, VA 22485
    • Date inspected: Nov. 4
    • Total violations: 4
      • The food contact surface of the cutting board along the grill prep unit was observed heavily stained. Cutting board was taked to the 3 compartment sink to be cleaned.
      • The food contact surfaces of the equipment being cleaned in the dish machine were not observed sanitized. There was not measurable sanitizer being dispensed into the machine. A service call was made and the 3 compartment sink was set up to sanitize equipment.
      • Employees observed working in the food service area without proper hair restraints. Hats were donned during inspection.
      • The following non-food contact surfaces were observed soiled: 1-the top of the dish machine, 2-the clean pan shelf above dish machine, 3-shevling throughout the kitchen, 4-the base of the mixer, 5-the wall fans throughout the kitchen, 6-the cook top.

      • Observed several of the cooling and freezing units did not have internal thermometers.

      • The sanitizer at the 3 compartment sink is not being pulled through the tubing. During the inspection, an employee replaced the bottle and primed the dispenser.

    • Date inspected: Nov. 5
    • Total violations: 3
      • Observed the following: One bottle of sanitizer stored on top of the ice machine, Two bottles of sanitizer being stored near a food prep sink. During the inspection the person in charge relocated the items.
      • Observed no internal thermometers inside any of the cooling or freezing units in the facility.
      • Observed a leaking pipe under the 3 compartment sink.
  • Shawn’s Smokehouse & BBQ, 7019 Harrison Rd 107 & 109 Fredericksburg, VA 22407
    • Date inspected: Nov. 5
    • Total violations: 15
      • The person in charge could not provide documentation of employee health training but was able to communicate symptoms and illnesses requiring reporting. Provided sample employee health policy during the inspection.
      • Observed a food employee lather their hands under running water, hands were not effectively cleaned due to hand soap being flushed down the drain. Instructed the food employee on correct hand washing procedures and the food employee washed their hands correctly.
      • Hot water does not flow from the warewashing handsink, dining room handsink for guests, and handsink by the Auto Shams and the handsink by the back prep sink. Only one sink in the kitchen had adequate hot water. Instructed the person in charge to contact a plumber to restore hot water. Instruct employees to wash their hands in the front hand sink which has adequate hot water.
      • Observed the can opener and potato slicer soiled. The can opener was removed to the three compartment sink for cleaning. Instructed the person in charge to remove the potato slicer for cleaning.
      • Observed the interior of the microwave oven soiled.
      • Observed package containers of chocolate cheesecake at 51 degrees F on the back prep table. The person in charge reported they cheesecakes were prepared at their location in Culpepper and were held in an employees car overnight. The person in charge decided to take the food home, the cheesecakes were labeled employee use only and placed in the walk in cooler. Stongly encouraged the person in charge to prepare the cheescake in the facility to prevent temperature abuse during transit.
      • Observed two wiping cloth buckets held in lactic acid sanitizing solution at 848 ppm, which exceeds the 700 ppm requirements based on the information poster located on the wall behind the 3 vat sink. New wiping cloth buckets were made at the 3 vat sink but the concentration was observed at 848 ppm. Instructed the person in charge to discontinue use of the sanitizer dispenser and use Chlorine as the food grade sanitizer until repairs are completed. Worked with the person in charge to set up wiping cloth buckets using Chlorine.
      • Observed the facility is conducting reduced oxygen packaging processes for cooked pork and brisket in absence of a HACCP Plan. The facility is also holding the TCS foods in the reduced oxygen environment for 3 days. Observed cooked pork and brisket held in a reduced oxygen environment dated 11/2/21. The person in charge voluntarily discarded the food for safety. Instructed the facility to discontinue this process and submit procedures to the health department for approval.
      • Observed the bottom of the back door of the food establishment is not sealed to the frame, observed light emitting from the bottom of the door.
      • Observed in use dispensing utensils stored in sanitizing solution in the heated well which is stored next to the covered pans of food. Instructed the person in charge to discontinue this process. The person in charge reported they will hold dispensing utensils in water held at 135 degrees F or higher.
      • The 3 vat and prep sink are no longer sealed to the adjacent walls.
      • After four cycles, observed the low temperature dishwasher is reaching a temperature of 115 degrees F. Contact technician to evaluate the the equipment. Dishwasher is not meeting data plate specificiations.
      • Observed accumulation of debris on the following non food contact surfaces: Top of dishwasher, back of the fryers, base of convection oven and base of the 2 door upright freezer
      • The following areas are in need of cleaning: Walls and coving/moulding throughout the facility and vents in the toilet rooms.
      • Observed the accumulation of debris on the hood vent filters above the fryers and smoker.
  • ITAVIE, 7610 Heths Salient St 104 Spotsylvania Courthouse, VA 22553
    • Date inspected: Nov. 5
    • Total violations: 3
      • Observed a sanitizing bucket stored in the hand sink at the dessert counter. Bucket was removed during inspection.
      • Drink glasses were found stacked while wet after cleaning and chemical sanitization.
      • The covers of the ventilation fans in the customer restrooms were observed soiled.

      • Observed that the dish machine is not sanitizing the equipment. The person in charge set up the 3-compartment sink.
      • Observed that the ice machine and can opener (corrected) require cleaning.
      • Observed several plates and ceramic bowls that were used last night sitting on top of the sandwich prep unit. The person in charge placed the equipment in the 3-compartment sink for cleaning.
  • Bravo!, 1 Towne Centre #3500 Fredericksburg, VA 22407
    • Date inspected: Nov. 6
    • Total violations: 3
      • The Sink & Surface Cleaner Sanitizer at the 3 compartment sink is reading 452. The establishment will use a chlorine sanitizing solution at the 3 compartment sink until the Sink & Surface Cleaner Sanitizer dispensing system can be repaired.
      • Observed the interior of the fryer cabinets and casters heavily soiled.
      • The floor in front of the fryers is in need of repair.
  • Parkridge Elementary School, 2000 Parkway Blvd. Stafford, VA 22554
    • Date inspected: Nov. 8
    • Total violations: 4
      • Observed wet wiping cloths stored in soap. The bucket was disposed of and the cloths were placed in the washing machine.
      • Observed gaps in the hood system. The person in charge rearranged the hood filters. Observed that the gasket is torn on the bottom right reach in cooler door.
      • Observed that the faucet on the handsink near the dish machine is leaking.
      • Observed that the light shields and air vent covers require cleaning.
  • Mian Noodle House, 1005 Princess Anne Street Fredericksburg, VA 22401
    • Date inspected: Nov. 8
    • Total violations: 7
      • The facility's Certified Food Protection Manager certificate has expired 5/13/2021.
      • Observed a food employee preparing to go orders using his bare hand to touch the ready to eat foods. EHS instructed the food employee to toss the order he was preparing, wash his hands, and use gloves or utensils when preparing food orders.
      • Observed the dish machine of the facility is not delivering any sanitizing solution. EHS instructed the person in charge to discontinue use of the machine and wash dishes at the 3 compartment sink until the machine gets serviced.
      • Observed the interior surface of the ice machine soiled.
      • Observed the inside and outside surfaces of the reach in cooling unit and prep cooler unit soiled to sight and touch.
      • Observed no air gap of the house in the mop sink. During the inspection the person in charge cut the house to create an air gap.
      • The following were observed in need of cleaning: Walls and Floors throughout the kitchen are in need of a thorough cleaning.
  • Baba Ganoush Mediterranean Grill, 1115 Jefferson Davis Highway Fredericksburg, VA 22401
    • Date inspected: Nov. 8
    • Total violations: 21
      • No certified food protection manager.
      • The PIC could not provide an employee health policy for the food employees working in the establishment. EHS provided a sample copy of the employee health policy.
      • Observed food employee wash hands at 3 compartment sink. EHS informed PIC of observation. The food employee washed hands at handwashing sink before returning to work.
      • Observed a pumice stone used for cleaning equipment in the handwashing sink. During the inspection, the food employee removed stone.
      • Observed two dented 3 lb. cans of tomato paste being stored in drawer in shared hallway (outside of the kitchen). During the inspection, the food employee discarded the cans.
      • The following foods were not covered: 1. One container of chicken curry, 1 container of chickpea curry, and 1 container of lamb curry in the reach in cooler. During the inspection, a food employee covered the food.
      • Observed raw chicken being stored over ready-to-eat (RTE) gyro (lamb) in the prep unit. During the inspection, the food employee moved the raw chicken under the RTE lamb.
      • Observed 6 soiled knives in a container on the prep table. During the inspection, the knives were placed at the 3 compartment sink for cleaning and sanitizing.
      • Observed the interior of the microwave heavily soiled. During the inspection, a food employee cleaned the microwave.
      • The following were not date marked: 1. One container of hummus and two containers of tzatziki prepared on 11/5/21. 2. One container of cooked lamb prepared on 11/6/21. During the inspection, a food employee date marked all prepared foods.
      • Observed 1 bottle of Glipizide and 1 bottle of Calcium on a shelf over the naan prep table. During the inspection, the food employee relocated the medication.
      • The following were being stored outside the establishment, in the shared hallway: 7 cans of dolmas, 1 bag of fresh garlic, 3 bags of basmati rice, 5 bags of all purpose flour, 1 bag of dried chickpeas, and 4 jars of kalamata olives. During the inspection, the food employee moved the sealed food back into the kitchen.
      • Observed 2 food employees working without hair restraints. During the inspection, the food employees applied hair restraints.
      • Observed several boxes and bags of disposable cups and lids (ss articles) being stored outside the establishment in the shared hallway. During the inspection, the food employees relocated the ss articles into a secure storage unit in the shared hallway.
      • The caulking at the 3 compartment sink and the aluminum foil lining the surfaces of equipment (mini fryer, shelves throughout the kitchen) is heavily soiled.
      • Observed a Palmolive bucket being used to store sugar. During the inspection, the food employee discarded the container of sugar.
      • The following are in need of repair: 1. The handwashing sink faucet is loose. 2. No cold water at 1 of the 2 faucets at the 3 compartment sink.
      • The non food contact surfaces of all equipment in the kitchen were observed heavily soiled.
      • The wall along the 3 compartment sink and between the reach in cooler and ice cream freezer are not smooth and easy to clean.
      • The floors under all equipment are in need of cleaning.
      • Observed the following: 1. One hat, 1 cell phone, and a wallet on top of the floor mixer. 2. One bottle of Lubriderm lotion, 1 cell phone, and a phone charge on a shelf over the naan prep table. During the inspection, the personal items were relocated
  • The Battlefield Restaurant, 1018 Lafayette Blvd Fredericksburg, VA 22401
    • Date inspected: Nov. 9
    • Total violations: 11
      • No supervisors or managers are certified food protection managers.
      • Observed the food employee remove biscuits from a package and place in the microwave with bare hands. EHS informed PIC of observation. The biscuits were discarded.
      • Observed food employee dumping coffee into the handwashing sink. EHS informed food employee of handwashing sink use.
      • PIC could not produce an appropriate food temperature measuring device to verify cooking temperatures.
      • The front and back exterior doors are not self closing.
      • Observed food employees working without hair restraints.
      • Observed several wet wiping cloths on the cooler and the 3 compartment sink. During the inspection, a food employee placed all the wet wiping cloths into a sanitizer bucket.
      • The sliding doors on the beer cooler have duct tape and a piece of wood with a screw for a handle.
      • No test kit for chlorine sanitizing solution.
      • The non food contact surface of all equipment along the cook line were observed soiled.
      • The floor tile in the dry storage room and between the ice machine and back exterior door is in need of repair.
  • Rodney E. Thompson Middle School, 75 Walpole Street Stafford, VA 22554
    • Date inspected: Nov. 9
    • Total violations: 3
      • Observed a spray nozzle attached to a long hose stored under the garbage disposal with the water turned on. The hose is connected to a hose bibb backflow preventer which is not appropriate for continuous pressure. The person in charge turned the water off during the inspection. Instructed the person in charge to ensure the water is turned off when not in use.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor hot water sanitization of the high temperature dishwasher. The person in charge reported the temperature tapes were on order.
      • Observed the water pressure gauge on the high temperature dishwasher does not move during the final hot water rinse cycle. The equipment appears broken. Discussed contacting maintenance to repair or replace the equipment.
  • Hardee’s of Princess Anne #2920, 1700 Princess Anne Street Fredericksburg, VA 22401
    • Date inspected: Nov. 9
    • Total violations: 17
      • Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
      • Per discussion with the person in charge the prep table and the utensils get washed, rinsed, and sanitized at the end of the day. Proper cleaning procedures were discussed with the person in charge.
      • Several containers and utensils were observed soiled to sight and touch and were stored in soiled containers.
      • Observed the following TCS foods cold holding at improper temperatures in the prep unit by the frying station: 1) a container of country ham (45-47'F) that was placed there for over 48 hours, 2) a container of omelet mix (ham, sausage, and cheese) (45-46'F). The person in charge discarded items.
      • Observed several spiders in the outdoor storage area. The person in charge discarded all single service articles.
      • Observed employees in the back of the house without proper hair restraints.
      • Observed several containers on shelves near the 3 compartment sink that were stacked while wet after cleaning and sanitizing. The person in charge moved them to be dried using a paper towel.
      • Observed a non smooth surface on the beverage dispenser unit in the dining room area.
      • Observed the following: 1) the prep unit cooler is not operating properly by the hot holding station, facility is using time as a public health control. 2) a leak under the prep unit cooler by the walk-in-cooler.
      • Observed the hand washing sink in the front of the house is not sealed to the adjacent wall.
      • The following nonfood contact surfaces were noted with an accumulation of grime and debris. 1) the interior, including gaskets, of all refrigeration units and freezers, 2) Wire shelving near the dishwasher area., 3) the interior and exterior of the ovens near the frying station, 4) The sides and back counter of beverage dispenser at the drive-thru, 6) The counter between the self serve beverage dispenser and wall, 7) the shelves in the upstairs dry storage area, 8) the handwashing sink in the front of the house.
      • Observed a hose, under pressure and with a spray nozzle attached to a faucet placed at the mop sink basin. The person in charge turned the water off and hung the sprayer during inspection.
      • Observed the hand washing sink by the 3 compartment sink to be soiled.
      • The ceiling in the mop sink is not smooth and easy to clean.
      • Observed the following in need of cleaning: 1) ceiling tiles and vents throughout the facility, 2) walls, floors, and ceiling throughout the facility, 3) outside storage unit.
      • The following are in need of repair: 1) the tiles by the 3-compartment sink and around the drain by the drive-thru area need to be re-grouted, 2) the hole and chipped paint in the wall, the leak in the ceiling, the light shield cover, and the blown out light bulbs in the upstairs' dry storage area, 3) the counter near the drive-thru area, the counter near the soda machine in the dining room, the three compartment sink, and the hand washing sink near it need re-caulking, 4) several ceiling tiles throughout the facility need to be re-adjusted.
      • Observed the following employees' personal belongings stored next to food and equipment. 1) a bag and a coat near the walk-in freezer, 2) two jackets near the drive-thru area.
  • Caroline Culinary Academy, Caroline County High School, 19155 Rogers Clark Blvd Milford, VA 22514
    • Date inspected: Nov. 9
    • Total violations: 2
      • The interior surfaces of the microwave oven were observed soiled. Microwave was cleaned during inspection.
      • Cutting board on the top of the can storage rack was observed soiled and stained. Board was taken to the 3 compartment sink to be cleaned.

      • Less than 10 foot candles noted inside the walk in cooler.
      • Cleaning is needed on the floors and walls in the warewashing area and the walls behind the cook line. Observed oily debris on the walls behind the cook line.
      • Observed the following in poor repair: Burned out ceiling lights by the walk in cooler door and floor tiles throughout the kitchen are in need of grouting. The existing grout between the floor tiles has become erroded.
      • Observed accumulation of debris on the interior of the fryers, metal backsplash of the fryers and non foodcontact surfaces of the cooking equipment.
      • Observed the interior surfaces of the microwave oven soiled.
      • Observed covered container of raw shrimp stored on shelf over a covered container of cooked noodles in the large prep cooler. The person in charge relocated th noodles to prevent contamination.

      • Observed the interior surfaces of the microwave are soiled.
      • Observed the handle of the ice scoop inside the ice machine was making direct contact with the ice.
      • Observed the metal shelves of the dry food storage area soiled to sight and touch.
      • Observed the floors of the walk in cooler, walk in freezer and floors underneath equipment in need of cleaning.
