Sentara Northern Virginia Medical Center is

Sports Physicals will be held at Sentara Lorton Marketplace for the nominal fee
of $15.
Please have
students bring their Sports Physical Form with history section completed to
include parent/guardian signature. Note: students who have been previously
diagnosed with a condition such as diabetes, a heart condition, high blood
pressure or asthma will be referred to their private care physician for
athletic clearance.
