Live Cockroach Found In McLean-Area Restaurant Inspection

Fairfax County inspections from July 17-31 found a live cockroach, sanitizer failures, unsafe food storage and missing plans.

MCLEAN, VA — A live cockroach, dish machines that failed to provide adequate sanitizer and reduced-oxygen food packaging without an approved safety plan were among violations Fairfax County inspectors reported at nine McLean-area establishments from July 17 through July 31.

Other findings included raw beef stored above cooked rice, food held at unsafe temperatures, a pesticide not approved for use in a food establishment and an ice chest that needed cleaning.

July 17-21 Inspections

Inspection date: July 17

Inspectors found fruit flies in the food-preparation area and ordered urgent pest-control service. The establishment could not provide an employee health policy or a test kit for its bleach sanitizer. Its permit was not posted.

Pineapple boba at 71 degrees, peach-mango boba at 74 degrees and condensed milk at 72 degrees had been left on a preparation table for about 2½ hours. Staff said the items would be discarded within another 1½ hours and refrigerated in the future. An opened carton of coconut milk lacked a date mark; the manager was instructed to label foods requiring temperature control.

The manager was observed washing hands in the rinse basin of the three-compartment sink and was redirected to a designated hand sink. The ice chest needed cleaning and was washed, rinsed and sanitized. An ice-cream scoop stored in 79-degree standing water was removed, and an ice scoop touching ice by its handle was relocated. Inspectors also cited the reuse of a manufacturer's container for bleach, missing hair restraints and improper utensil storage.

Domino's
8133 Watson St., McLean, VA 22102
Inspection date: July 17

The restaurant could not provide its employee health policy, a repeat violation. Inspectors supplied reporting information for employees to review and sign. Two hand sinks also lacked sufficiently hot water: one measured 80 degrees and the other 87 degrees, below the required 100 degrees. Urgent service was required within 10 days.

Chesapeake Bagel Bakery
6815 Old Dominion Drive, McLean, VA 22101
Inspection date: July 20

Cooked sausage patties at 52 degrees, cooked spinach at 51 degrees, tomato sauce at 52 degrees and sliced roast beef at 45 degrees were found in a six-drawer preparation cooler. All were discarded. The repeat cold-holding violation carried a warning that further noncompliance could result in enforcement action.

The cooler was operating at 50 degrees. The person in charge removed its contents and contacted a repair technician during the inspection. A follow-up was planned within 10 days.

Clarity
442 Maple Ave. East, Vienna, VA 22180
Inspection date: July 21

Inspectors reported that the restaurant could not provide documentation showing that salmon served raw or undercooked in ceviche had been frozen for parasite destruction. It also lacked an approved hazard analysis and critical control point, or HACCP, plan and monitoring logs for chicken and beef packaged using reduced-oxygen and sous vide methods.

The chlorine dish machine produced a sanitizer reading of zero parts per million while equipment and utensils were being cleaned. Staff stopped using the machine and switched to the three-compartment sink until the machine could be repaired and provide at least 100 parts per million of chlorine sanitizer.

The restaurant could not provide a food thermometer and had not retained fresh-mussel shellstock tags for 90 days or preserved source information after removing shellfish from tagged containers. Inspectors cited the manager's inability to demonstrate food-safety knowledge based on those findings and scheduled a follow-up.

An incomplete employee health policy and missing procedures for responding to vomiting and diarrhea were corrected when inspectors provided documents. A cleaned ice bin was inverted to protect it from contamination. Cooked rice at 48 degrees was moved to the walk-in refrigerator to continue cooling properly.

July 22-23 Inspections

East West Cafe Tysons
1992 Chain Bridge Road, Tysons, VA 22102
Inspection date: July 22

A repeat violation was cited after the low-temperature dishwasher produced no detectable chlorine sanitizer. Staff switched to the three-compartment sink, and the dishwasher required urgent service.

Fettuccine at 47 degrees, sausage at 47 degrees, tzatziki at 43 degrees, hummus at 44 degrees, diced tomatoes at 46 degrees, rice at 57 degrees, acuka at 49 degrees and milk at 57 degrees had failed to cool properly. The foods were discarded.

Inspectors found five coolers operating between 42 and 57 degrees. The units required urgent service, and affected food was to be discarded. The report also cited direct connections between the three-compartment sink's drain lines and the grease trap, along with an exterior gap near the restroom door that could allow pests to enter. The sink required air gaps within three days.

Pho Hai Duong Tysons
8411 Old Courthouse Road, Vienna, VA 22182
Inspection date: July 23

Inspectors found raw sliced beef stored above cooked rice. The rice was moved above the beef. Beef soup held at 120 degrees was reheated to 212 degrees.

Pork-and-shrimp spring rolls at 79 degrees, unsalted butter at 78 degrees and garlic in oil at 77 degrees had been left on a preparation counter. Cooked pork measured 44 degrees and lettuce measured 47 degrees. The foods were moved to refrigeration, and excess lettuce was removed from an overfilled bag so it could be properly chilled.

Employee medicine stored near dry food and tools stored on or against food supplies were relocated. A blocked hand sink was cleared and supplied with paper towels. Inspectors also found insect-repellent spray that was not approved for use in the establishment and directed staff to remove it.

Other findings included a missing air gap at a preparation sink, missing procedures for cleaning vomit and diarrhea, inadequate monitoring of food temperatures, unprotected utensils, non-food-grade containers and trash bags used on food-contact surfaces, food stored on the floor, milk crates used for storage, missing hair restraints, a watch worn during food preparation, improperly stored knives and unlabeled bulk ingredients. Several violations were corrected during the inspection, while the sink, containers, shelving and temperature-monitoring system required follow-up action.

July 27-31 Inspections

Santini's New York Style Deli — McLean
1443 Chain Bridge Road, McLean, VA 22101
Inspection date: July 27

Inspectors observed a live cockroach on the kitchen floor outside the walk-in cooler. The person in charge contacted a pest-control technician during the inspection, and a follow-up was planned within 10 days.

Cooked chicken breast held at 94 degrees near the fryer was to be discarded at the end of four hours from cooking. The restaurant lacked test strips for its quaternary ammonia sanitizer, but the person in charge ordered them during the inspection.

Plastic food-storage trays blocking a hand sink were removed. Pizza dough stored in bags on the walk-in cooler floor was moved to elevated storage racks.

Cafe Monaco
1800 Old Meadow Road, Second Floor, McLean, VA 22102
Inspection date: July 30

Raw eggs were stored above condiments in a preparation cooler. The eggs were moved below the ready-to-eat items. The dish machine was operating with less than 10 parts per million of bleach sanitizer. The person in charge stopped using it, contacted a repair technician and switched to the three-compartment sink.
