A free, community-driven wellness and transition experience is coming to Woodbridge to support adolescents and young adults living with sickle cell disease. This population is among the most underserved during the critical transition from pediatric to adult care, a challenging phase that can significantly affect long-term health outcomes.
Hosted by The Heart of Gold Sickle Cell Foundation of Northern Virginia, this impactful event brings together leading healthcare providers, community advocates, and support organizations to address gaps in care, access, and education.
Participants will engage in expert-led discussions, hear directly from individuals navigating the transition journey, and connect with resources focused on whole-person wellness—including mental health, insurance navigation, and preventive care.
