Woodbridge Health Inspections: 10 Violations At 1 Place

WOODBRIDGE, VA — Several facilities in the Woodbridge area recorded violations during recent health department inspections between Feb. 8 and 15.

Health inspectors conduct most restaurant, school, child care center and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

• The handwashing sink near the upright refrigerator was being used to store clean equipment while drying.
• Raw frozen hamburger patties in a plastic bag were stored adjacent to cooked chicken slices in a plastic bag in the upright freezer.
• The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cheese and whole milk in the True upright refrigerator at 44 and 45 degree F, respectively.
• Spray bottles of chemicals (bleach water) stored on counter adjacent to can opener and bananas.
• The thermometer for the True upright refrigerator does not appear to be working properly.
• Boxes of canned goods stored on the floor in the kitchen.
• The employee handling food in the kitchen was observed to not be wearing a proper hair restraint.
• Even though the temperature gauge for the True upright refrigerator indicated that the unit was holding a temperature of 40 degrees F, all of the foods measured were found to be at 44 degrees F or higher. This is the second time this has been observed during an inspection.

• There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications.
• The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
• This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
• When asked, the Person In Charge (PIC) advised me that the commercially prepared meatballs and sauce were not reheated in the microwave to 135°F, but were placed on the steam table for reheating.
• The following food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: meatballs and sauce at 128°F.
• Spray bottles of chemicals stored hanging/aimed at or near clean equipment above and adjacent to the 3 compartment sink.
• Foods received earlier this morning, before opening, were still stored on the floor in the back.
• Observed employees working in the food service areas, up front and in the back, without proper hair restraints.
• Meat slicer (new to Subways) stored on counter (clean and uncovered), adjacent to handwashing sink, exposed to contamination of wastewater from handwashing sink.
• Single service items (cookie wrappers, condiment containers) were found stored under the waste lines for the handwashing sink up front, exposed to potential contamination.

Occoquan Elementary School: 12915 Occoquan Rd. Woodbridge, VA 22191

• Cut lettuce salad, prepared this morning found in the Traulsen 4 door refrigerator at 52 degrees F in the cooling process.
• The waste lines from the 2 compartment prep sink are directly connected into the sewage system.
