Woodbridge Health Inspections: 25 Violations At 1 Facility

The Prince William Health District recorded violations at several Woodbridge restaurants during recent inspections.

WOODBRIDGE, VA — Several restaurants around Woodbridge recorded violations during health inspections from Dec. 6 to Dec. 13, according to the Prince William Health District.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Hector's of Lake Ridge, 2233 Tackett’s Mill Drive Lake Ridge, VA 22192
    • Date inspected: Dec. 7
    • Total violations: 25
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • The cook was observed eating along the cookline while preparing/handling food and/or contacting clean food contact surfaces.
      • A food employee failed to wash his hands prior to donning clean disposable gloves for the handling of ready to eat (RTE) foods.
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink in the cafe area, handwashing sink in the bar).
      • The handwashing sink near the 3 compartment sink was being used as a dump sink (vegetable leaves found in sink basin).
      • No hand towels available at the hand sink in the cafe area or at the one in the bar area.
      • There was no soap available at the handwashing sink in the bar area.
      • Bag of ready to eat (RTE) shredded cheese stored below and adjacent to raw chicken and other raw meats in True 2 door reach-in along cookline.
      • Ice cream in WIF stored uncovered; cinnamon & sugar used at dessert station on counter uncovered.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: raw shrimp @ 54 degrees F on the True 2 door reach-in refrigerator along the cookline; sliced tomato and guacamole and Chilles Rellenos in the salad prep unit at 47, 48 and 43 degrees F.
      • Cooked ground beef mixture was found cooling COVERED, IN DEEP CONTAINERS, IN LARGE PORTIONS on the counter at 162 degrees F. Per staff, it was cooked 2 hours prior to taking the temperature.
      • Observed the following TCS food thawing using an improper method: Containers of raw fish thawing on the counter.
      • There were no ambient air temperature measuring devices (thermometers) in many of the refrigeration units in the facility including the True 2 door reach-in refrigerator along the cookline, the 2 door salad reach-in refrigerator along the cookline, True 1 door prep refrigerator, the Turbo Air display
      • Various foods stored in containers throughout the facility were not labeled properly with the common name of the food (in English).
      • Employees handling food were observed to not be wearing proper hair restraints.
      • The wet wiping cloths along the cook line were not properly stored in the sanitizer bucket in between use.
      • Single service containers being used to scoop bulk food ingredients.
      • The following pieces of refrigeration equipment were in disrepair and empty: Arctic Air 2 door display refrigerator and True 2 door upright freezer; the 2 door salad prep refrigerator along the cookline doesn't appear to be able to hold foods at 41 degrees F and below; the Whirlpool white chest freezer had a large amount of ice build up inside it;.
      • Aluminum foil was used to line various pieces of equipment; raw wood shelving is being used in the drink station.
      • The Winholt heater/proofer was in disrepair; Ecolab warewashing machine in the bar is leaking.
      • Milkcrates and soda bottle racks were being used as shelving and for storage of single service items.
      • Flour and other bulk foods throughout the facility are being stored in containers designed for the storage of Christmas ornaments and similar items.
      • The waste lines from the 3 compartment sink in the kitchen, the 3 compartment sink in the bar and the 2 compartment (prep) sink in the kitchen extend into the floor drains.
      • Waste receptacles in the women's restroom used by the food service staff and customers were not covered.

      • There is no covered receptacle for sanitary napkins in the toilet room used by females.

    • Total violations: 8

      • There is a huge deposit of ice on the shelving below the condenser in the walk-in freezer.

Single service items on a dunnage rack adjacent to the washing machine in the basement are stored under an unshielded wastewater pipe. *Note: Shielding for the wastewater lines exists where the single service shelving is on rollers.
      • There is a "4 outlet" adapter on the faucet at the mop sink that allows 4 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the adapter/hoses.

      • Raw beef patties in open container stored above ready to eat (RTE) biscuit in open package.
      • Single service sandwich boxes stored with food contact surface up and exposed to potential contamination; cup dispensers up front are overstocked with cups that can be contaminated by handling them; coffee supplies stored adjacent to handwashing sink up front exposed to splash from sink during handwashing.
      • Cabinet freezer (2 door) in disrepair and marked with "out of service" sign; door to walk-in freezer is in disrepair and there is a build up of ice at the doorway.
      • The faucet at the handwashing sink in the bakery area is in disrepair with the water spraying.
      • The sprayer arm at the 3 compartment sink extends down into the basins.
      • The waste lines from the 3 compartment sink in the kitchen extend into the floor drain.

    • Date inspected: Dec. 13
    • Total violations: 4
      • Observed Food item (Raw seasoned chicken) was stored on the floor of the walk-in cooler Or Food stored less than 6"" above the floor.
      • Observed employee were working in the food service area without proper hair restraints.
      • Observed the cleaning or sanitizing product used to clean or sanitize equipment/utensils is not available.
      • There is no chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration.
