Woodbridge Health Inspections: 38 Violations At 1 Restaurant
The Prince William Health District inspected several facilities around eastern Prince William County during recent visits.
WOODBRIDGE, VA — Several facilities from eastern Prince William County recorded violations during the most recent round of health department inspections from June 28 to July 4.
COVID-19 restrictions have relaxed around the state, and health inspectors conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- Date inspected: June 28
- Total violations: 10
- Employees and conditional employees are not aware of the reporting requirements concerning information about their health and activities as they relate to diseases that are transmissible through food.
- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- The raw animal food on display is stored in a manner that may cause cross contamination of ready-to-eat food as follows: raw eggs and raw cut slices of beef stored next to a container filled with ready to consume salad dressing in the walk in cooler.
- The can opener was observed soiled with accumulations of grime and debris.
- The date marking system for prepared ready-to-eat (RTE) vegetables mixed rice and cooked brown rice in the refrigeration unit is not in proper use.
- Observed big buckets of soy sauce and unopened boxes stored on the floor in the walk in cooler.
- Employees observed working in the food service area without proper hair restraints.
- Wiping cloths improperly stored between use.
- Observed that mops are improperly stored between use.
- The floor and the walls are not cleaned as often as necessary to keep them clean.

- Food employees wearing jewelry (watch) on their arms.
- An irreversible registering temperature indicator was not provided for the hot water mechanical warewashing operation.
- There is no chemical sanitizing (QUAT) test kit present to ensure the chemical sanitizing solution used in the 3-compartments sink is at the proper concentration.

- The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
- This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
- There was no soap available at the handwashing sink in the bar, dining side.
- Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink in service area; handwashing sink in women's restroom; handwashing sink in bar, dining side, handwashing sink in bar, pool side).
- No hand towels available at the handwashing sinks in the bar, dining side and bar, pool side.
- The tags for shell stock are not maintained for 90 days after the last sale, nor are they kept in chronological order. In addition, it was noted that the last date of service for that container was not written on the shell stock tags.
- Observed a large cardboard box of shellstock (oysters) in the WIC that did not bear a tag or label as specified by law.
- Observed raw shelled eggs being held at room temperature; practice confirmed by PIC.
- Acetaminophen, rubbing alcohol, etc were stored on shelving above food and container of foil.
- A spray bottle of chemical was stored above food; soap was stored above/near food/single service items on the shelf.
- Spray bottles of chemicals in kitchen unlabeled.
- Rice in the white upright Frigidaire refrigerator found to be cooled improperly using deep and/or covered containers.
- Observed the following TCS food thawing using an improper method: Containers of raw beef thawing at room temperature on the prep table while other foods are being prepared.
- There were no ambient air temperature measuring devices (thermometers) in either the chest freezer or the white, upright Frigidaire refrigerator.
- The person in charge (PIC) could not provide a metal-stemmed probe food thermometer that was capable of measuring food temperatures.
- White bulk storage containers/buckets were not labeled properly with the common name of the food stored within them.
- Observed fruit flies in the bar area, dining side; fly catcher observed hanging in kitchen area.
- Observed that the rear door is not properly sealed and tight fitting to prevent entry of vermin (light visible at side and bottom of door); window in kitchen in disrepair with broken pane of glass.
- Various foods in walk-in cooler stored on shelving where bottom shelf is in contact with floor.
- Foods being prepared on a prep table located in between a mop sink and a handwashing sink where splash from wastewater on both sides can contaminate foods.
- Scoops used to dispense ice at the bars were improperly stored in between usage with the handles down in the ice.
- Clean utensils were observed to be improperly stored with the food contact surfaces up.
- Various single use items such as Lawry salt containers and sour cream containers being reused to store other foods in throughout the kitchen.
- Observed soda containers being used as shelving.
- Wooden utensils being used for cooking foods in kitchen.
- Foil and cloths being used to line shelving throughout the kitchen; cardboard being used to line shelving in walk-in cooler; raw wood shelving used to store clean equipment upon.
- The Serv-Ware 3 door prep refrigerator is in disrepair with a leak pooling on the bottom shelf.
- The following equipment is in disrepair: 1. cutting boards are discolored and scored
- Grocery bags were used to store ready to eat foods in.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine).
- The 3 plugs/stoppers for the 3 compartment sink are missing.
- The wastewater pipes of the 3 compartment sink extend down into the floor drain, eliminating the air gaps; the wastewater pipe for the warewashing machine does not appear to be air gapped either; the common wastewater pipe, for the 3 compartment sink at the bar on the dining side, that ends at the drain in the cabinet, is not air gapped; there are leaks in the wastewater pipes as well.
- There was no covered waste receptacle in the women's restroom.
- Walls, floors, etc in the kitchen are unclean with visible soil.
- The wall above the 3 compartment sink is in disrepair and not easily cleanable.
- Observed that there were several pieces of equipment in disrepair, as well as other unnecessary items throughout the facility.
- Observed that inadequate lighting (measured at 3 foot candles) was provided in the kitchen at the prep table where an employee was using a knife to cut vegetables.

- The service window and the door entrance into the vehicle was open and not protected from flying insects and rodents.'

- There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications.
- The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
- Interior of refrigeration units, prep tables, etc throughout kitchen unclean with visible accumulation of food debris.
- The Conserver XL stationary, low temperature warewashing machine was being used as the sanitization step for washing/rinsing/sanitizing all of the equipment and dishes. Unfortunately, the PIC was unaware that the unit ONLY uses chemical to sanitize and is not approved as a sanitizer by heat. No Ecolab ultra san on site.
- Clean equipment including pots and pans were stored on the shelving with the food contact surface exposed to dust and other contamination.
- Observed foil being used to line shelving, etc throughout the kitchen.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine) in the warewashing machine. *test kits for quaternary ammonium used at the 3 compartment sink are available.
- Non-food contact surfaces such as exterior of refrigeration units and carts unclean with visible accumulation of food, soil.
- The faucet at the 3 compartment sink is in disrepair resulting in cold water not being available.
- The wastewater pipe of the 1 compartment prep sink extends down into the floor drain, eliminating the air gap; the wastewater pipe from the warewashing machine extends into the floor drain and is not properly air gapped; there are leaks in the wastewater pipes, especially at the wash basin of the 3 compartment sink.
- Walls, floors, etc throughout the kitchen are unclean with visible accumulated food debris and soil.
- Observed that the light cover/shield in the food prep area/warewashing area is missing.
