Woodbridge Health Inspections: Hibachi Grill, Great Wall Restaurant

The Prince William Health District recorded violations at several Woodbridge facilities during recent restaurant inspections.

WOODBRIDGE, VA — Several facilities around Woodbridge recorded violations during the most recent round of health department inspections from Jan. 18 to Jan. 23.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

    • Date inspected: Jan. 18
    • Total violations: 9
      • Employees are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Cooked teriyaki chicken and cooked shrimp were found at hot holding at improper temperatures.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Diced tomatoes, cut melons and leaf salad.
      • The prepared ready-to-eat cooked beef and cooked chicken teriyaki in the refrigeration unit is not labeled with a ""consume by"" date.
      • The Person in Charge could not provide a food temperature measuring device.
      • Wiping cloths improperly stored between use.
      • The cutting board(s) along the kitchen on the prep cooler and the prep table are heavily scratched and scored. The food contact surfaces are no longer easily cleaned and sanitized due to condition.
      • The walls and floors are not cleaned as often as necessary to keep them clean.

    • Total violations: 1
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • Kentucky Fried Chicken, 17450 Jefferson Davis Hwy Dumfries, VA 22026
    • Date inspected: Jan. 18
    • Total violations: 1
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissable through food.
  • Hope Aglow Christian Academy, 13550 Jefferson Davis Hwy Woodbridge, VA 22191
    • Date inspected: Jan. 23
    • Total violations: 11
      • There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications.
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink in women's restroom).
      • The handwashing sink in the kitchen was being used to rinse equipment such as the baby bottle found there.
      • Raw shell eggs stored over ready to eat (RTE) cheeses/milk in white homestyle refrigerator; open container of raw ground beef stored above commercial ready to eat (RTE) foods in the white homestyle freezer.
      • There was no ambient air temperature measuring device (thermometer) in the white homestyle refrigerator.
      • The person in charge (PIC) could not provide a metal-stemmed probe food thermometer that was capable of measuring food temperatures.
      • The following were observed during the inspection: The cabinets around the sinks in the kitchen were in disrepair with portions missing.
      • The following non-food contact surfaces were visible soiled: 1. The interior of the white homestyle refrigerator 2. The interior of the white homestyle freezer.
      • The wastewater pipes of the 2 sinks used as the 3 compartment sink are directly plumbed with no air gaps.
  • Woodbridge High School, 3001 Old Bridge Rd Woodbridge, VA 22192
    • Date inspected: Jan. 23
    • Total violations: 2
      • The back-up milk cooler, line 5/6, upstairs is still in disrepair and empty.
      • Observed that inadequate lighting was provided in the walk-in freezer due to one of three light fixtures being in disrepair.
  • Brittany's Sports Bar, 12449 Dillingham Sq Woodbridge, VA 22192

    • Total violations: 4
      • The following handwashing sinks were found to have hot water at less than 100 degrees F: women's restroom 73 degrees F; men's restroom 65 degrees F.
      • Employees handling ready to eat (RTE) foods were observed to not be wearing proper hair restraints.
      • Dumpster or outside refuse container was open or uncovered and pushed back so that it could not be closed properly. The lids also appear to be bent and may not close properly.
      • The floor in the kitchen and in the walk-in cooler (WIC) is in disrepair with missing tiles that allow dirty water to stand in the depressions. Identified 1 tile near drain along the cookline and a series of 4 tiles at the entrance of the walk-in cooler.

    • Total violations: 2
      • This food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed the dumpster outside was not covered and had littering's around it. Corrected during inspection: The littering's around the dumpster was cleared and the dumpster was properly covered.

      • Observed an Employee was working in the food service area without proper hair restraints. Corrected during inspection :Employee was seen wearing proper hair restraint.
  • Great Wall Restaurant, 16708 Jefferson Davis Hwy, Suite 113 Dumfries, VA 22026
    • Date inspected: Jan. 24
    • Total violations: 15
      • There is no person in charge with a valid Food Protection Manager Certification from an accreditied agency recognized by the Conference for Food Protection.
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissable through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • No paper towel at hand sinks.
      • Observed that hand soap was not provided at each handwashing sink.
      • Raw animal food stored over ready-to-eat food in the refrigeration unit.
      • Can opener had heavily accumulation of debris.
      • Poisonous or toxic materials found stored in hand sank and corrected on site.
      • The food thermometer is not available as necessary to measure the hot holding and cold holding accuracy.
      • Food containers (sugar, flour) were not labeled.
      • The exit door was open to insects/rodents.
      • Food not stored in freezer, food was placed on floor.
      • Single service items stored in restroom inside the kichen.
      • No test strip at 3 compartment sink.
      • Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.

      • No disposable towels were provided at the hand washing lavatory in the kitchen beside the reach in freezer.
      • The prepared ready-to-eat (RTE) cooked Crocker, cooked curry chicken in sauce in the refrigeration unit is not properly dated for disposition (The person in charge repaired the date system).
      • Unlabeled food containers with the following food items that are not easily identified by appearance: Chicken curry and various sauces in the 3 door reach in cooler.
      • The 3-door reach in cooler is linking is not maintained in good repair.
