Woodbridge Health Inspections: The Merit School, Ebenezer Academy

The Prince William Health District recorded violations at several child care facilities around Woodbridge during recent inspections.

WOODBRIDGE, VA — The Prince William Health District found violations at several child care facilities during recent inspections. Inspectors conducted the most recently published round of inspections between June 28 and July 5.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Merit School of Prince William, The, 14308 Spriggs Rd Woodbridge, VA 22193
    • Date inspected: June 29
    • Total violations: 9
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • The integrity of the food packaging for several cans of peaches were found to be compromised (severely dented along the end seam) and therefore exposing the food to potential contamination.
      • Spray bottles of chemicals stored hanging near clean equipment.
      • Observed the following TCS food thawing using an improper method: Commercially prepared frozen chicken slices thawing at room temperature on counter (whole bag) and portioned into open bowls over microwave.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Clean equipment including pots and pans were stored on the shelving with the food contact surface exposed to dust and other contamination.
      • Single service cup being continuously used to scoop cereal; stored in product in bag.
      • A corrugated pipe extension has been placed in the original floor drain making it higher so that the wastewater pipes of the 3 compartment sink now extend down into the floor drain, eliminating the air gaps.
      • There is no covered receptacle in the restroom for the proper disposal of feminine articles.

    • Total violations: 10
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • No hand towels available at handwashing sink in the kitchen restroom.
      • Spray bottles of chemicals near warewashing machine unlabeled.
      • Clean igloo cold beverage dispensers stored on the kitchen floor.
      • Milk crates are being used for clean equipment storage.
      • The following non-food contact surfaces were visible soiled: 1. The interior of equipment storage drawers
      • The wastewater pipes of the 2 compartment sink extend down into the floor drain, eliminating the air gaps.
      • The toilet in restroom in kitchen was not operational as the water supply had been turned off. However, the toilet had been used at some time after the water had been turned off resulting in it being unclean for use.
      • Various light bulbs in hood system in kitchen burnt out and/or missing.

      • Facility is storing unnecessary items to the operation or maintenance of the establishment. Observed inoperable dishwasher in kitchen area.
      • Observed HVAC intake in kitchen area covered in dust.
  • Holy Family Catholic School, 14160 Ferndale Rd Woodbridge, VA 22193
    • Date inspected: July 1
    • Total violations: 6
      • There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications.
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees.
      • Spray bottles of chemicals in kitchen unlabeled.
      • The original wastewater pipes of the 3 compartment sink are directly plumbed with no air gaps.
