Woodbridge Health Inspections: Roaches + 33 Violations At 1 Facility
The Prince William Health District inspected several facilities around Woodbridge and Dumfries during recent restaurant visits.
WOODBRIDGE, VA — Several facilities around Woodbridge recorded violations during the most recent round of health department inspections from March 29 to April 4.
Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.

- Total violations: 8
- Employees and conditional employees are not aware of the reporting requirements concerning information about their health and activities as they relate to diseases that are transmissible through food.
- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Raw animal food holding in a manner that may cause cross contamination to ready-to-eat food (RTE). Raw cut beef slices were stored next to cooked chili beans. The person in charge discarded the chili beans voluntarily.
- The prepared ready-to-eat (RTE) @ chili beans and cooked beef with carrots in the refrigeration unit is not properly dated for disposition.
- Employees observed working in the food service area without proper hair restraints.
- Wiping cloths improperly stored between use.
- The cutting board(s) along the prep cooler and on the shelves are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
- The door of the walk-in cooler is not maintained in good repair.

- Total violations: 12
- Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (men's restroom).
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pupusa mix @ 52 degrees F (made yesterday per date marking) in True prep reach-in refrigerator closest to wall.
- Containers of food were labeled improperly with pre-printed "use by" stickers BUT with the date the food item was actually "made."
- There was no ambient air temperature measuring device (thermometer) in the following refrigeration units: 1. glass front display refrigerator 2. 2 door prep refrigerator by steam table along cookline 3. True prep reach-in refrigerator along cookline 4. Prep reach-in #2 along cookline 5. True 2 door reach-in near the 2 compartment sink 6. Small white chest freezer in the back.
- The following refrigeration units in the kitchen were not holding temperatures of 41 degrees F and below: True prep reach-in along cookline (closest to wall) @ 55 degrees F.
- Shelving in walk-in cooler is deteriorated and has a build up of rust and is no longer easily cleanable.
- There is no data plate on the warewashing machine.
- The wastewater pipes of the 3 compartment sink extend down into the floor drain, eliminating the air gaps; the wastewater pipes of the 2 compartment sink extend down into the floor drain, eliminating the air gaps; there is a leak in the wastewater pipe at the handwashing sink allowing water to drain to the floor.
- Observed exposed wiring, due to missing electrical box covering, along the cookline.
- Light fixtures throughout the kitchen are in disrepair or missing all together. Lighting fixtures in the men's restroom are in disrepair.
- Observed holes in the walls above the warewashing machine and in other places throughout the kitchen (at end of cookline).

- There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have expired certifications.
- The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
- An open drinking container was stored on the dry goods shelving in the back in a manner that may contaminate food, food contact surfaces or utensils; employee observed to walk into kitchen with drink at the end of the inspection.
- There was no soap available at the handwashing sink in the unisex restroom.
- The following handwashing sinks were found to have hot water at less than 100 degrees F: *handwashing sink in back of kitchen
- Container of lo mein stored on top of other foods in top of Fagur prep reach-in refrigerator; two small containers of General Tso chicken stored in larger container of General Tso chicken and then in/on top of container of fried chicken.
- Raw chicken in secondary packages (not original packaging) stored above raw beef and raw fish in secondary packages (not original packaging) in the True 2 door upright freezer.
- Raw shrimp stored over ready to eat (RTE) shredded cabbage in walk-in cooler (WIC).
- Can opener blade unclean.
- The 3 compartment sink was found to be unclean and not properly set up; no stoppers/plugs could be found to allow the compartments to hold water; and when asked, the PIC did not understand the proper set up of the 3 compartment sink, nor did he understand the use of sanitizer. Equipment was not being washed/rinsed/sanitized properly.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked lo mein on top of other foods in top of Fagur 3 door prep reach-in refrigerator.
- Large containers of food, in volumes too great to be used in a single day, were not date marked.
- Large containers of food, in volumes too great to be used in a single day, were not marked with the date to be discarded.
- General Tso Chicken, just cooked at 11am per PIC, found sitting out near deep fat fryer and in large deep container in walk-in cooler, @ 84 degrees F @ 12pm and @ 12:45pm respectively (less than 2 hours); fried chicken, cooked at 11:30am per PIC, found sitting out near deep fat fryer @109 degrees F at 12pm.
- There was no ambient air temperature measuring device (thermometer) in the Fagur prep reach-in refrigerator.
- Various foods along cookline and other foods in white bulk storage containers were not labeled properly with the common name of the food.
- Observed a dead cockroach in a large pot; observed a glue board with many dead cockroaches on it.
- Food (boxes of poultry, broccoli) stored on the floor in the walk-in cooler and oil stored on the floor under the 3 compartment sink in the kitchen area.
- Employees handling food were observed to not be wearing proper hair restraints.
- Wet wiping cloths throughout the facility were not properly stored in the sanitizer buckets in between use.
- Utensils used to dispense ingredients along cookline and those in bulk containers were observed to be improperly stored in between usage with the handles down in the foods.
- Various single use metal cans and containers being reused to store other foods in throughout the kitchen.
- Observed soda crates being used as shelving throughout the kitchen and in the walk-in cooler.
- Some bulk foods in the facility are being stored in containers designed for the storage of home goods (Sterilite containers).
- Cardboard being used to line shelving; wooden shelving observed in back of kitchen.
- The ice machine is in disrepair and currently not in use.
- The following were observed during the inspection: 1. Cardboard containers were used to hold foods (not original food), in the Turbo Air freezer ; 2. Grocery bags were used to store ready to eat foods in.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine).
- The interior of the True 2 door upright freezer was visibly soiled with large accumulation of food debris.
- The waste lines from the the 3 compartment sink and the 1 compartment prep sink in the kitchen extend into the floor drains.
- Observed a lounge chair and several camp chairs in the kitchen.
- The floor under the prep sink in the kitchen was unclean with visible soil.
- Personal items stored throughout the kitchen with items near single service utensils (forks, napkins) up front and glasses, lighter, coats, etc stored on dry goods shelving in back (near camp chairs).
