Woodbridge Health Inspections: Round 1, Harbour Grille, Papa John's

The Prince William Health District recorded violations at several Woodbridge restaurants, including a facility with 24 violations.

WOODBRIDGE, VA — Inspectors with the Prince William Health District visited several restaurants around Woodbridge during routine inspections between July 6 and July 12. They recorded violations at several facilities, including one restaurant with two dozen violations.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Round 1 Bowling & Amusement, 2700 Potomac Mills Circle Suite 882 Woodbridge, VA 22192
    • Date inspected: July 7
    • Total violations: 8
      • There is no Certified Food Manager present in the establishment.
      • Employees and conditional employees are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • The hand washing facility located next to the dishwasher is blocked with a black cart, preventing access by employees for easy hand washing.
      • The prepared ready-to-eat (RTE) cheese and marinara sauces in the refrigeration unit are not properly date marked.
      • The can opener blade is rusted dull and needs cleaning.
      • Maintenance tools are not being stored in an orderly manner.
      • Observed an accumulation of litter on the premises.
  • Papa John's Pizza #1682, 4814 Dale Blvd Woodbridge, VA 22193
    • Date inspected: July 11
    • Total violations: 17
      • There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications.
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • A food employee was drinking from an disposable water bottle in the back prep area while preparing/handling food and/or contacting clean food contact surfaces.
      • Observed a food employee wash his hands in the 3 compartment sink.
      • The integrity of the food packaging for many cans of pizza sauce were found to be compromised (severely dented along the end and side seams) and therefore exposing the food to potential contamination.
      • Unlabeled spray bottles of chemical (clear liquid, sanitizer per PIC) stored above pizza make station #1 and at 2 door refrigerator; other spray bottles in back prep area were unlabeled.
      • Chemicals (cleaning supplies) were stored under the prep station in the back along with single service pizza sauce containers; spray bottles of chemicals were stored above the pizza make station #1 (left) and on the 2 door refrigerator.
      • There was no ambient air temperature measuring device (thermometer) in the True 2 door prep refrigerator or the Continental make station refrigerator #1 (left) and the thermometer in the walk-in cooler was in disrepair.
      • White powder food stored in container was not labeled properly with the common name of the food.
      • Scoop in flour container improperly stored in between use in container with handle down in product.
      • Single service plates were stored on the floor in the back prep area.
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. One hose was observed to be fully extended into the basin. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • Extensions have been placed on the original wastewater pipes of the 3 compartment sink so that they now extend down into the floor drain, eliminating the air gaps.
      • The door to the employee toilet room that opens in to the prep area of the kitchen has no self closing device on it.
      • Mop found in walk-in cooler. Maintenance tools are not being stored in a way that prevents the contamination of food, equipment, utensils, linens, and single-service items.
      • Floor tiles throughout kitchen broken and not fixed to the wall/floor surface.
  • The Harbour Grille, 13188 Marina Way Woodbridge, VA 22191
    • Date inspected: July 11
    • Total violations: 24
      • The person in charge did not perform the following duties: 1. Ensure that employees are washing hands properly and wearing gloves when handling ready to eat foods; 2. Ensure that employees are properly maintaining the temperatures of TCS foods during cold holding; 3. Ensure that employees store foods so that they are protected from cross-contamination.
      • There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities were unable to provide documentation of current certification.
      • Observed an employee directly contacting ready-to-eat foods with his bare hands while plating in a manner that could contaminate the food.
      • Signage that notifies food employees to wash their hands was not provided at the handwashing sink used by food employees near the warewashing machine.
      • Observed a large metal container of shellstock (mussels) in the walk-in refrigerator that did not bear a tag or label as specified by law. When questioned, PIC indicated they were fresh product.
      • Chicken wings (cooked 7/10 at 3pm), mussels, etc stored uncovered in walk-in refrigerator
      • Raw chicken stored over large open stainless steel container of mussels in walk-in cooler (WIC); raw products such as scallops, cod, chicken stored adjacent to ready-to-eat (RTE) foods such as sliced tomatoes and cheeses in the prep refrigerator.
      • Bowls used for mixing salads stored on top of uncovered containers of foods such as tomatoes, in salad prep refrigerator.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: 2 trays of chicken wings: 44 degrees F(date marked 7/10/22, 3pm), pre-cooked ribs: 46 degrees F(cooked on Saturday, 7/9/22 per date marking), cut cherry tomatoes: 46 degrees F(date marked 7/9/22), quartered tomatoes: 47 degrees F (date marked 7/9/2022), sliced cheese: 45 degrees F, raw hamburger patties: 46 degrees F (all in the walk-in cooler); sliced tomatoes: 51 degrees F, raw scallops: 51 degrees F, raw cod: 49 degrees F, raw chicken: 56 degrees F (on the 2 door prep reach-in refrigerator); feta cheese: 48 degrees F and quartered tomatoes: 46 degrees F on the salad prep refrigeration unit.
      • Per review of the menu, the following items are marked with an asterisk (*) indicating that they can be cooked to order (the disclosure statement): steak on steak salad riverside burger epic smoke burger ny strip steak Per wait staff, the epic smoke burger can NOT be cooked to order but the following additional items can be: angus sirloin tuscan wrap (with sirloin) Per wait staff, the salmon can no longer be cooked to order. A reminder statement advising the customer about the increased risk of foodborne illness due to consuming undercooked or raw food is included on the menu.
      • There were no ambient air temperature measuring devices (thermometers) in the 2 door prep refrigerator, the salad prep refrigerator or the dessert station refrigeration unit.
      • White powder food stored in container on front line that was not labeled properly with the common name of the food.
      • The outer opening door of the food establishment (near the 3 compartment sink) does not close properly/completely and is not protected against the entry of insects and rodents.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Clean equipment (large metal pans) were stored stacked and wet on the clean equipment storage shelf.
      • Clean equipment including pots and pans were stored on the shelving near the cookline with the food contact surfaces exposed to dust and other contamination.
      • The following refrigeration units were not holding temperatures of 41 degrees F and below: 1. 2 door prep refrigerator 2. salad prep refrigerator 3. walk-in refrigerator 4. refrigerated drawers under grill
      • There are no test kits available in the facility for monitoring the concentrations of the chemical sanitizing solutions (quaternary ammonium in the 3 compartment sink or chlorine in the warewashing machine).
      • The following items were observed to be unclean with an heavy accumulation of grease and soil residue: 1. The fans on the cookline
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • An extension has been placed in the original floor drain eliminating the air gap between the wastewater pipes of the 3 compartment sink and the floor drain.
      • The high traffic areas of the floors in the kitchen are in disrepair with chipped/missing paint/sealant.
      • Observed that inadequate lighting was provided in the walk-in refrigerator.
      • The filters and light shields over the bulbs in the hood system over the cookline (on the grill side) are severely soiled with grease and debris build up.

    • Total violations: 1
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
