Woodbridge Restaurant Inspections: 24 Violations At 1 Facility
The Prince William Health District recorded violations at several facilities in Woodbridge and Dumfries during recent inspections.
WOODBRIDGE, VA — Several restaurants around Woodbridge and Dumfries recorded violations during health inspections from Nov. 2 to Nov. 7, according to the Prince William Health District.
Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- The 3 compartment sink, in use upon arrival, was not set up properly. Utensils and equipment were not being washed/rinsed/sanitized. Sanitizer was not being used.
- Lettuce prepared/cut 5 hours ago, cooling at 54 degrees F.
- Spray bottles of chemicals in kitchen unlabeled.
- Chemicals stored near spices and above single service items.
- Cut lettuce in Delfield 6 drawer refrigerator and in walk-in cooler that was prepared (cut) at 9am was found cooling at 54 degree F at 2pm.
- Observed an insect control device installed above the food preparation table.
- Employees handling food were observed to not be wearing proper hair restraints.
- Scoop for egg roll filling stored down in product.
- Clean equipment was stored stacked and wet on the clean equipment storage shelf.
- Single service sauce cups are being used to scoop seasonings out of storage containers.
- The waste line from the 1 compartment prep sink in the kitchen extends into the floor drain.

- Cooked sausage in the walk-in cooler (WIC) found to be cooling improperly using deep and/or covered containers.
- Boxes of fries stored on the floor in the walk-in freezer (WIF).
- There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
- Observed that inadequate lighting was provided in the walk-in freezer outside.

- Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink near 3 compartment sink)
- No hand towels available at handwashing sink near the coffee station and the handwashing sink near warewashing machine.
- Previously cooked and cooled rice, tomato and potato soups were placed into the steam table to reheat.
- Cut melon stored in Traulsen 2 door and 1 door refrigerators in the staging area was found at 50 and 52 degrees F, respectively.
- Clean equipment was stored stacked and wet on the clean equipment storage shelf near the 3 compartment sink.
- Clean equipment was stored on shelving adjacent to the handwashing sink near the 3 compartment sink exposed to splash from handwashing.
- The following pieces of equipment were found in disrepair and not holding an ambient air temperature of 41 degrees F or below: Traulsen 2 door refrigerator in the staging area at 54 degrees F; walk-in cooler (WIC) "C" with an ambient air temperature of 66 degrees F (per CFPM, unit needs a new compressor, no TCS foods inside only commercially prepared dressings, etc., all TCS foods moved to other WICs).
- Milkcrates were used as shelving in the produce walk-in cooler.
- The faucet at the hand sink at the entrance to the kitchen is in disrepair and won't turn on; the sensor at the handwashing sink in the cold prep area is in disrepair and won't allow the water to continuously flow.
- The waste receptacle in the staff restroom was not covered.
- Observed that inadequate lighting was provided in several walk-in units including WIC "J", WIF "A", WIF "DD".

- Observed food such as frozen steak was on the floor of the walk in freezer.
- Observed the following ready-to-eat, potentially hazardous foods such as cooked chicken, chicken kebab and grounded beef which was cooked last night were being held in a container or package that does not bear a date or day in the walk-in refrigerator.
- Observed employees were working in the food service area without proper hair restraints.

- The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.

- Drinking containers were stored on the pizza prep table in a manner that may contaminate food, food contact surfaces or utensils.
- A food employee failed to wash his hands prior to donning clean disposable gloves for the handling of ready to eat (RTE) foods.
- The handwashing sink near the 3 compartment sink had a waste water hose from the air conditioning unit draining into it and a pot scrubber in it.
- There was no soap available at the handwashing sink along the cookline.

- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: lettuce and tomatoes on top of 2 door refrigerators #1 and #2 along the cookline; pre-cooked wings inside the 2 door refrigerator #2 along the cookline; pre-cooked pasta and shredded cheese on top of the 2 door refrigerator #3 along the cookline.
- Toxics stored on shelving adjacent to uncooked pasta and single service items.
- There was no ambient air temperature measuring device (thermometer) in the pizza prep refrigerator or in the dough refrigerator.
- Various white powder foods stored in containers that were not labeled properly with the common name of the food.
- Foods found stored on the floor in the walk-in freezer.
- Food (cooked pasta) being drained in strainer in 1 compartment of the 3 compartment sink while the other compartments have soiled equipment in them.
- Employees observed handling food and/or cooking without proper hair restraints.
- The wet wiping cloths along the cookline were not properly stored in the sanitizer bucket in between use.
- Knives for cutting subs were being stored in between the 2 door prep refrigerators along the cookline in a location that can not be seen or easily cleaned.
- Clean equipment including stainless bowls were stored on the shelving with the food contact surface exposed to dust and other contamination.
- Single service dip containers being used to scoop dry bulk foods.
- The following refrigeration units along the cookline were not holding temperatures of 41 degrees F and below: 2 door refrigerator #2 (Dukers) along the cookline at 45 degrees F.
- Cardboard (soiled) being used to line shelving and dunnage racks throughout the kitchen.
- The following items were observed to be unclean with an accumulation of dust and soil residue: 1. The 3 fans throughout the kitchen
- There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. One hose was observed to be fully extended into the mop basin. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
- Personal item (phone) stored on pizza preparation table; personal items (purse, coats, aprons, etc) stored on shelving with foods and single service items.

- Observed the integrity of the food packaging for Canned whole tomatoes which was dented and found to be compromised with the possibly of exposing the food to potential contamination was on the food rack. Corrected during inspection The dented canned whole tomatoes were removed and discarded from the food rack.
- Observed the following ready-to-eat, potentially hazardous foods Cut tomatoes, Meat balls and pasta sauce was being held in a container or package that does not bear a date or day in the walk-in refrigerator. - Walls, floors, etc at the hand sinks, 3 compartment sink, warewashing machine, mop sink, inside the walk-in cooler (WIC) and walk-in freezer (WIF) and food prep area in the kitchen are unclean with visible soil. The floor drains throughout the kitchen are unclean with visible soil. The fan near the 2 compartment sink is unclean with visible soil/dust.

- Floors and walls throughout facility are in disrepair and no longer smooth, non-absorbent and easily cleanable.

- The handwashing station located at kitchen entrance is being used to wash various kitchen equipment/medical equipment.
- Non-pasteurized egg (in the shell) being used to make scrambled egg for children 9 and under.
- Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Being stored with and touching various kitchen knives/food preparation utensils/food contact surfaces.
- 3 VAT sink has a hole in the sanitization side (located at bottom of vat - will not hold water).
