Woodbridge Restaurant Inspections: McDonald's, Wendy's, Chili's

The Prince William Health District visited several facilities around Woodbridge during its recent restaurant inspections.

WOODBRIDGE, VA — The Prince William Health District recorded violations at several Woodbridge facilities during the most recent round of restaurant inspections. The most recently published inspections occurred between June 15 and June 21.

Many COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

    • Date inspected: June 15
    • Total violations: 2
      • A backflow prevention device or airgap not provided on a hose bibb (threaded faucet) at mop sink as required by law.
      • Urinals was not accessible to employees during all hours of operation
  • Best Western Potomac Mills, 14619 Potomac Mills Rd Woodbridge, VA 22192
    • Date inspected: June 15
    • Total violations: 4
      • Observed 3 compartment sink in kitchen directly connected- no air gap.
      • Observed boxed of apples and oranges stored directly on the floor in kitchen
      • The establishment could not demonstrate the ability to provide written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment.
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissible through food.

    • Total violations: 8
      • Observed paint flaking from wall in kitchen next to ice machine
      • Observed missing ceiling tiles in food storage area.
      • Observed boxes of apples bananas stored directly on floor
      • Sausage at breakfast buffet observed 108 degrees Fahrenheit.
      • Observed two hand sinks in kitchen without hand washing soap.
      • No hand drying provision was made available and hand washing sink for employees to dry hands after washing
      • The establishment could not demonstrate the ability to provide written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment.
      • Employees are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food.

    • Date inspected: June 16
    • Total violations: 7
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • The following non-food contact surfaces were visible soiled: 1. The interior of the cabinets holding equipment such as baking sheets
      • The hose attached to the mop sink faucet was observed to be fully extended into the basin thus creating a potential backflow issue.
      • The wastewater line on the sanitizer compartment of the 3 compartment sink extends down into the floor drain, eliminating the air gap; the wastewater lines from the other 2 compartments are directly plumbed.
      • The dumpster is missing the left lid and cannot be closed properly.
      • The dumpster or outside refuse container was open or uncovered and overflowing with refuse.

    • Total violations: 11
      • Personal items such as purses, jackets, etc store in the salad prep items adjacent to single service items.
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • The wastewater lines from the 2 compartment/ veggie prep sink are in disrepair and leaking and causing a build up of debris on the floor and drain area.
      • Single service items were stored in the cabinet under the wastewater lines of the handwashing sink in the drive through window area.
      • Covered containers of cut lettuce prepared this morning (1 hour ago) found stored in the walk in cooler on the lower shelves.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: slice tomatoes date marked 6-15-2022 in the 3 door reach in refrigerator.
      • Raw eggs stored adjacent to ready to eat (RTE) cut, washed lettuce leaves.
      • No hand towels available at the small handwashing sink up front in the drive through window area.
      • The handwashing sink up front in the drive through window area is completely blocked by a large box of single service condiments and the lemon-aide dispenser has encroached upon the sink and blocked access to the soap dispenser.
      • There was no soap available at the small handwashing sink up front in the drive through window area.
      • A food employee failed to wash her hands prior to donning clean disposable gloves when changing tasks.
  • Chili's Grill & Bar, 14432 Gideon Dr Woodbridge, VA 22192
    • Date inspected: June 21
    • Total violations: 3
      • Refrigeration unit found to be capable of holding potentially hazardous food items in cold hold in a temperature range of 41-45°F. The following unit is not able to maintain internal items at a temperature of 41°F or less: The prep cooler and draw coolers in the kitchen next to the grill.
      • The premises are not free of litter.
      • Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
  • Subway #24330, 5828 Mapledale Plz Dale City, VA 22193
    • Date inspected: June 21
    • Total violations: 3
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed a hose attached to a faucet that extended below the flood rim level of the mop sink

      • Employees observed working in the food service area without proper hair restraints.
