Kaye went to a Waukesha Walgreens Monday and filled a prescription for 120 tablets of Amphetaime. The pharmacist filled the prescription, but the next day thought it was strange and contacted the doctor who wrote it.
He discovered Kaye was the former patient of the doctor and the prescription was a forgery. He also discovered Kay had passed similar forgeries at Walgreens locations in the Town of Brookfield and Sussex.
