City Of New Berlin: Press Release - $100 Incentive Extended For COVID-19 Vaccinations

Wisconsinβs $100 reward program for those receiving the COVID-19 vaccine was extended until September 19th by Governor Tony Evers. If you get your first dose of any COVID-19 vaccine from a vaccine provider located in Wisconsin, Michigan or Minnesota beginning August 20th through September 19th, you are eligible to get $100. Proof of insurance, ID or citizenship are not required.

A registration form is required to be filed so the WI Department of Health can verify you received your first vaccine during the designated time frame and so they know where to send the $100 Visa Gift Card. Β Parents and guardians will need to fill out the form for children ages 12-17. Visit the Vaccine Reward Signup at https://100.wisconsin.gov/. Β Β
