During the inspection, Oak Creek environmental health staff were informed that facility staff members did not document the date, time or nature of chemical treatments that were not approved for commercial use.

"This facility has demonstrated an inability to safely and effectively treat pest infestations without posing a public health risk ... To protect the public, the City of Oak Creek will require safety assurance in the form of a written report from a licensed pest control professional or company that neither pest infestation nor chemical treatments used pose a health hazard to the public."

The specific pests in question were not revealed in the order. A call to Motel 6 was not returned.
