Kaiser Permanente Baldwin Park Medical Center Releases 2016 Community Health Needs Assessment
Data on the top health and social needs for the San Gabriel Valley are announced

Kaiser Permanente Baldwin Park Medical Center, in partnership with Citrus Valley Health Partners, held a breakfast for 100 community stakeholders to share the findings of the 2016 Community Health Needs Assessment (CHNA), which includes the latest data on the top health and social needs for the San Gabriel Valley. The event took place on November 4 at the Courtyard by Marriott Baldwin Park. Senator Ed Hernandez, who is the chair of the Senate Health Committee and a San Gabriel Valley native who grew up in La Puente, served as a guest speaker. Attendees also learned about the local grant application process and how to apply.

As a healthcare provider, our community benefit work is something we take great pride in since it affords us the opportunity to play a significant role in keeping the communities in which we live and work healthy.”
Numerous agencies attended the event including local community clinics, school districts, law enforcement, local, state and federal elected offices, community-based organizations, local business partners, health departments, and universities.

The CHNA offers a deeper understanding of community health care needs that enable hospitals to develop community benefit plans that complement and extend clinical services to address disease prevention and improve health status. Information on identified priority needs and applications for grant funding will be announced in January at kp.org/baldwinpark. Local grants typically range from $5,000 to $25,000. Grant requests more than $25,000 are available through Kaiser Permanente’s Southern California Regional Offices.
