North County Health Services (NCHS) will provide FREE Health Care Reform Enrollment application assistance from 3:30 to 7:30 p.m. Monday, March 24 at the Encinitas Library.

For the best assistance, please bring the following documents with you:

  1. Birth Certificate (or USA Passport, Naturalization Certificate or Permanent Resident Card)
  2. Social Security Cards
  3. Income Verification - Income tax return, 3 most recent pay stubs or letter from employer (stating how much the applicant/spouse earns and how often and employer name, address, date and signature)
  4. Proof of child support received (letter or statement)
  5. Proof of California residency (water/electric bill, CA ID or driver's license)
