On Saturday, December 12, 2015, from 10am - 3pm, ChapCare will provide FREE health insurance enrollment assistance to residents of Monrovia and Duarte.

ChapCare’s health insurance enrollment event will take place at 513 E. Lime Avenue, Monrovia, CA 91016, just 4 blocks from downtown Monrovia.

Please bring the following with you to the event:

· CA Driver’s License, CA I.D., passport, utility bill

· Recent paystub’s, last year’s tax return

· Social Security Card, Naturalization Certificate, Employment Authorization Card, Residency Cards
