CA Healthy Kids Survey For Grades 5, 7, 9, And 11 In Novato Unified School District

Dear Parent or Guardian,

Your child is being asked to take the California Healthy Kids Survey which is administered every two years and helps our district track trends and determine areas of need. The survey includes questions about alcohol, tobacco, and other drug issues, as well as questions about school safety and the learning environment. Student participation in the survey is voluntary. We hope you will allow your child to participate in the survey so that their opinion and knowledge can be used to improve the school. Consent forms for fifth graders will be distributed when they return to school the week of November 30th. The consent form for 7th, 9th, and 11th graders found here.

The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of the school.
